Abstract

ABSTRAKPuskesmas sebagai salah satu penyedia layanan kesehatan bagi masyarakat perlu memiliki mutu pelayanan yang berkualitas sesuai dengan harapan masyarakat untuk dapat menjaga kepuasan serta kepercayaan pasien khususnya di masa pandemi COVID-19 seperti saat ini. Tujuan penelitian ini untuk mengetahui strategi dalam mempertahankan dan meningkatkan mutu pelayanan kesehatan pada masa pandemi COVID-19 di Puskesmas Kuta II Kabupaten Badung. Penelitian ini menggunakan metode kualitatif. Informan dari penelitian ini merupakan petugas dari Puskesmas Kuta II sejumlah 17 orang yang dipilih melalui teknik purposive sampling. Instrumen pengumpulan data menggunakan pedoman wawancara mendalam dan lembar observasi. Analisis data dilakukan dengan reduksi data, penyajian data, serta penarikan kesimpulan atau verifikasi. Hasil penelitian menunjukkan bahwa strategi yang ditetapkan oleh Puskesmas Kuta II dalam mempertahankan dan meningkatkan mutu pelayanan kesehatan pada masa pandemi COVID-19 telah mencakup seluruh aspek yang terdapat pada 4 prinsip penjaminan mutu dan 5 dimensi mutu dengan lebih menekankan pada selalu memenuhi kebutuhan atau harapan pelanggan dengan berusaha memberikan fasilitas yang baik serta memberikan pelayanan yang cepat, tepat, dan aman. Namun, dalam pelaksanaan strategi tersebut terdapat beberapa kendala seperti terbatasnya tempat yang ada dan SDM yang belum memadai. Oleh sebab itu, diperlukan adanya peningkatan manajemen SDM dan peningkatan penataan ruangan di puskesmas.Kata Kunci: Strategi, Mutu Pelayanan Kesehatan, Puskesmas

Abstract

ABSTRAKRSUD Kabupaten Klungkung merupakan salah satu rumah sakit yang belum dapat mencapai indikator mutu kelengkapan rekam medis 24 jam setelah selesai pelayanan. Kelengkapan, keakuratan, dan ketepatan rekam medis merupakan tanggungjawab utama dokter. Perilaku dokter dalam penyelesaian pengisian rekam medis salah satunya dapat dipengaruhi oleh kondisi kerja tempatnya bertugas. Penelitian ini bertujuan untuk mengetahui faktor-faktor kondisi kerja yang melatarbelakangi keterlambatan penyelesaian pengisian dokumen rekam medis oleh dokter di Instalasi Rawat Inap RSUD Kabupaten Klungkung. Penelitian ini menggunakan metode deskriptif kualitatif. Pengumpulan data menggunakan metode wawancara mendalam dan studi dokumentasi yang dilakukan di instalasi rawat inap RSUD Kabupaten Klungkung pada bulan April-Mei 2017. Informan pberjumlah delapan orang yang terdiri dari dokter spesialis dan manajemen rumah sakit. Hasil penelitian ini menunjukan bahwa beban kerja yang cukup tinggi, belum efektifnya supervisi, belum maksimal pengetahuan dokter terkait rekam medis, belum terbiasanya dokter menggunakan formulir rekam medis yang baru, serta belum efektifnya pelaksanaan sosialisasi melatarbelakangi keterlambatan penyelesaian pengisian rekam medis oleh dokter. Selain itu, faktor diluar kondisi kerja yang melatarbelakangi keterlambatan penyelesaian pengisian yaitu kebijakan hari libur dan kepulangan pasien diluar jam kerja dokter spesialis. Simpulan penelitian ini diketahui bahwa faktor kondisi kerja yang belum optimal di RSUD Kabupaten Klungkung melatarbelakangi terjadinya keterlambatan penyelesaian pengisian dokumen rekam medis oleh dokter spesialis di instalasi rawat inap. Maka perlu adanya perbaikan pada faktor kondisi kerja yang ada di RSUD Kabupaten Klungkung.Kata Kunci : Dokter, Kondisi Kerja, Rekam Medis. ABSTRACTKlungkung Public General Hospital has not able to achieve one of the quality indicators namely completeness of patients’ medical records after 24 hours of the patient care is finalized. The completeness and validity of the medical record is the responsibility of the physician. This study aimed at identifying factors related to the physicians’ performance in filling up the medical records. This study used a qualitative method with in-depth interviews conducted among eight specialized physician and administrative staff working at in-patient wards of Klungkung Public General Hospital from April to May 2017. The study results showed two groups of factors influencing the performance of physicians in filling up the medical records. The first was internal working condition factors consist of a high work-load, lack of socialization and supervision, lack of knowledge related to the medical records management, and problems with the new forms. The second was external factors such as off-day policy as well as the time difference between patients’ discharge and physician working hours. The study conclusion is that suboptimal working conditions influence the completeness of patients’ medical records. A human resources strategy to optimize working condition particularly for physicians, is needed.Keywords: Doctor, Work Condition, Medical Record

Abstract

Pengabdian masyarakat berupa pelatihan ini bertujuan untuk meningkatkan pengetahuan danketerampilan staf Dinkes dan Puskesmas di Gianyar dalam merencanakan kebutuhan SDM denganmetode WISN. Pelatihan WISN diselenggarakan pada tanggal 12 Juni 2015 di Gedung PSKM FKUnud. Sebelum pelatihan dimulai, dilakukan instalasi software WISN pada seluruh laptop peserta.Kegiatan pelatihan terdiri dari pemberian materi, diskusi, serta praktik penggunaan software WISN.Untuk evaluasi, seluruh peserta diminta untuk mengisi kuesioner pre dan post test. Nilai pre dan posttest dianalisis secara statistik dengan uji t dua sampel berpasangan untuk melihat ada tidaknyaperbedaan pengetahuan. Pelatihan dihadiri oleh 25 orang peserta (89,3%) dari Dinkes Gianyar dan13 Puskesmas di Kabupaten Gianyar. Saat pelatihan, seluruh peserta telah mendapatkan danmengetahui cara instalasi software WISN. Terdapat peningkatan pengetahuan peserta pelatihansebelum dan sesudah mengikuti pelatihan WISN sebesar 5,08 dengan nilai p <0.001. Seluruh pesertadapat melakukan analisis dengan software WISN dengan panduan tim pelaksana dan menyatakanakan menerapkan metode WISN di institusi mereka. Pelatihan telah berjalan dengan baik danberhasil meningkatkan pengetahuan dan keterampilan petugas Dinkes dan Puskesmas di Gianyardalam menggunakan software WISN. Bimbingan teknis dan follow up perlu dilakukan untukmendorong implementasi metode WISN untuk perencanaan kebutuhan SDM di Gianyar.

Abstract

Environment, a variable that often gets special attention in assessing public health and environmental health, is an effort to prevent disease. Environmental Health Risk Assessment (EHRA) is a comprehensive study about sanitation facilities' conditions and risky behaviors on public health. Abiansemal District is one of the tourist destinations that has a very complex impact, especially on environmental health aspects. Behavior is a highly potential factor for health status, clean and healthy behavior is strongly influenced by the availability of sanitation facilities in order to control physical environmental factors that can harm health. This study determines the Health Risk Index and obtains an overview of the sanitation facilities condition and community behavior that has a risk for environmental health in Abiansemal District, Badung Regency. This study used stratified random sampling using a sample in the Abiansemal sub-district which has been determined as the target study area. Households to be visited in each village were selected randomly by stratified random sampling. After finding the first household visited, an interview was conducted according to the available questionnaire and after it was completed it was forwarded to neighbors until it found 40 houses per village. SPSS 20.0 is used for the data analysis. The description of sanitation facilities and community behavior that poses a risk to health in strata 1 has the main problem in domestic waste with the highest score of 36, while strata 2 has the main problem in solid waste with a score of 43 and strata 3 has the main problem in domestic wastewater with a score of 45. Analysis results of the sanitation risk index for the category of low risk/less risky area for strata 1 (1 village), the category of high-risk area for Strata 2 (11 villages), and the category of moderate risk area is in Strata 3 (6 villages). Abstract

Background: The COVID-19 pandemic has constituted concurrent public health and economic crises. An inter-correlation between economic and public health impacts due to the COVID-19 pandemic needs to be studied to improve mitigation measures.Aims: This study identified a correlation of the economic insecurity and perceived stress with adherence to recommended preventive behaviours Methods: This across-sectional analytic study was conducted to adults in the working areas of East Denpasar Primary Healthcare Center I. Respondents were selected using consecutive sampling and given a self-administered questionnaire. The research variables included demographic characteristics, economic insecurity indicators, perceived stress, and adherence to handwashing, mask-wearing, physical distancing, and limitation on the social gathering. Correlations, linear regressions, and path analyses were conducted using IBM SPSS 23.0.Results: As many as 161 respondents of which 34.2% males were involved had a mean age of 36.31 (± 7.16) years. Sex, job insecurity, income insecurity, and perceived stress were found as independent determinants in females. Female sex and job insecurity was associated with better preventive behaviours with an adjusted β value of 0.276 and 0.306, while income insecurity and perceived stress had the opposite association with a β value of -0.247 and -0.224.Conclusion: There are correlations between economic insecurity and preventive behavioural practices during COVID-19. It is suggested that public health policies against COVID-19 cover measures of economic safety nets to improve adherenceKeywords: behaviour, COVID-19, economic insecurity, perceived stress, prevention.

Abstract

The integration of cellular technology in healthcare (M-Health) has the potential in coping with the constraints of the medical service system and to become a breakthrough to support the patients’ medication adherence. This review was conducted to compare and evaluate the effectiveness of M-Healthin terms of the patients’ medication adherence.The literature review conducting on PubMed, Cochrane Library, Ebsco Health and Taylor & Francis was confined to the articles published between 2010 and 2019. Fifteen articles relating to medication adherence were reviewed, eleven of which employed RCT methods including quasi-experiment that assessed the effectiveness of M-Health to measure the adherence, while four other researches employed qualitative studies to explore patients’ perceptions.The review showed that the intervention of m-Health is effective to drive the compliance of the treatment in accordance with healthcare promotion efforts as well as the prevention, screening and the management of the diseases. Two studies have shown inconsistent results regarding the prevention of obesity and the management of hypertension patients. The development of such model abd the integration of parallel m-Health and healthcare promotion will improve the long-term medication adherence. Abstract

The Workload Indicators of Staffing Need (WISN) software application training aims toimprove the knowledge and skill of Karangasem Regional Public Hospital (RSUD Karangasem) staff in undertaking workload-based human resources planning analysis using WISN software. The training was conducted in 24 September 2016 at RSUD Karangasem. Fifty seven managerial staff of RSUD Karangasem participated in the training. The training consist of WISN software installation, presentation, discussion, and practices on how to use WISN software. The trainees were asked to fill in pretest and posttest questionaires. The test scores were analyzed statistically using paired t-test. There was knowledge improvement of 6,21 after the training with p value < 0.001. All trainees were also able to conduct analysis using WISN software under the guidance of the trainers. The training has been well performed and successfully improved the knowledge and skill of RSUD Karangasem staff in applying WISN method. They stated their willingness to apply WISN method to their institution. However, further technical guidance is required to set up the system for the implementation of WISN method on human resource planning in RSUD Karangasem.

Abstract

Introduction: Female sex workers (FSWs) remain an important key population affected by HIV&AIDS in Indonesia. Indirect FSWs, in particular, are exposing to higher risk comparing to direct FSWs due to their characteristics and working conditions that could be barriers to safe sex or consistent condom use. This study aimed to identify association between sociodemographic characteristics, work venue, and knowledge on HIV&AIDS toward condom use among indirect FSWs. Methods: This was a cross-sectional study using secondary data from a survey on indirect FSWs conducted by Center for Public Health Innovation (CPHI), Udayana University, Indonesia during August to October 2017. About 171 indirect FSWs were eligible for this analysis. The dependent variable in this study was consistent condom use during the last month. Independent variables consisted of socio-demographic characteristics, work venue, and knowledge on HIV&AIDS. Data were analysed using univariate and multivariate analysis. Results: About half of indirect FSWs (50.17%) reported using condom consistently with their clients during the last month. Indirect FSWs were more likely to use condom consistently if they completed senior high school or higher (OR=3.49; 95%CI=1.42-8.57), but less likely to use condom if they were currently or ever married compared to single ones (OR=0.23; 95%CI=0.06-0.82). Those who worked in karaoke, cafe, and bar were less likely to use condom consistently compared to those in massage parlor, spa, and beauty salon (OR=0.09; 95%CI=0.03-0.26). Conclusions: Level of consistent condom use was low, indicating the need to intensify HIV prevention to indirect FSWs. Significant determinants of consistent condom use included marital status, educational level, and work venue. This study informed that providing HIV&AIDS information and improving negotiation skill that fit with characteristics of indirect FSWs and giving institutional support at workplace is crucial to ensure consistent condom use among indirect FSWs.

Abstract

A lot of misinformation about COVID-19 on social media possibly hinder the practice of healthy behaviors that are essential to protect individuals. This situation is exacerbated by the lack of health literacy in Indonesia. This study aimed to determine the influence of social media information exposure on the knowledge and behavior of Indonesians in compliance with COVID-19 protocols. An online survey of 909 people was conducted from July 2nd to August 10th, 2020. The data collected were knowledge and behavior of preventing COVID-19, while independent variables were socio-demographic characteristics and exposure to social media information. Inclusion criteria were defined as follows: minimum age of 18 years and domiciled in the country during the data collection period. The analysis used for the data collected were univariate and multivariate. The result showed that gender, age, marital status, and social media presence significantly affect a person's knowledge about COVID-19 with a p-value of 0.05. Enforcement behavior of health and healthy living protocols is significantly influenced by the respondent’s gender, marital status, education level, the island of residence, and exposure to online information. The frequency of exposure to information affects people's knowledge and behavior to implement health protocols and healthy living in the COVID-19 era, and it is further influenced by socio-demographic characteristics.

Abstract

Pharmacy counseling, which provides quality drug information, improves patient safety. However, this service is not optimally provided in Indonesia. A new model, modified pharmacy counseling (MPC), was developed to make it easier for service providers. We aimed to measure MPC effectiveness in improving short-term antibiotic compliance. This community trial (ISRCTN82062391) involved patients (age, ≥18 years) recruited from selected pharmacies between December 2020 and February 2021 in two Bali Province districts. At baseline, data regarding sociodemographic characteristics, knowledge levels, and attitudes toward antibiotic use were collected. MPC effectiveness was assessed using Morisky medication adherence scale-8. Of 300 recruited patients, 290 participated (response rate, 96.67%). The baseline study revealed that knowledge regarding short-term antibiotic use is lacking; however, awareness regarding antimicrobial resistance is high. MPC significantly improved short-term antibiotic compliance (risk ratio, 2.849; 95% confidence interval, 1.904-6.640; p<0.001). The mean rank significantly differed between the intervention (61.05) and control groups (37.95). The proportions of compliant patients in the intervention and control groups were 35.6% and 12.5%, respectively. The main reason for low compliance was forgetting and not being made aware by pharmacy workers in the intervention (46.8%) and control (55.6%) groups, respectively. These findings provide insights for improving pharmaceutical care.
