Abstract

Background: Diabetes management has not been optimally implemented by patients, considering the high incidence of diabetes complications. Therefore, family empowerment is important for diabetes management. Objective: This study aims to determine factors related to diabetes management, inhibiting, and supporting factors for diabetes management to develop a family-based educational model in diabetes management through expert testing.Methods: This research used Mixed Methods design with an Explanatory Sequential Design approach. A cross-sectional approach to 207 people with DM selection using a multistage sampling technique. Data was collected by questionnaire, in-depth interview, and Delphi study. Data analysis using chi-square and logistic regression. The development of family empowerment model is based on the theory Ladder of Empowerment, Social Cohesion, and Family Support. Results: Research results showed that good knowledge of patients, good family empowerment, and absence of DM risk factors are related to diabetes management. Model development and expert testing (Delphi Study) of the model find that a family-based diabetes management module is needed. Conclusion: A good family-based diabetes management module includes aspects of Self-Instructional, Self-Contained, Stand Alone, Adaptive, and User-Friendly aspects. This module is expected to improve diabetes management for patients and their families so that diabetes complications can be prevented and the patient's quality of life increases.

Abstract

The School Health Program policy was initiated by the Indonesian government in 2014. However, by 2021, student health service coverage was still suboptimal. The Healthy Schools Campaign Program was introduced in 2022 as an alternative policy to revitalize the School Health Programme. The study aimed to analyze the Healthy School Campaign Program using the health policy triangle framework, which includes actors, content, process, and context. This research uses qualitative methods with secondary data sourced from electronic document searches. The main actors in this policy involve official government institutions, supported by collaboration with non-government partners. Policy selection is based on situational, structural, cultural and international contextual factors. The program content focuses on physical health, healthy nutrition, and student immunization. The conclusion is that the selection of content is influenced by how actors identify health problems in the early stages of policy preparation. Implementation of this program requires a clear collaboration system as a reference for its implementation in educational units. Teacher and parent involvement is also very important to increase the effectiveness and sustainability of the program.

Abstract

Background: HATI is an implementation trial aimed at improving HIV care and treatment, including through SMS reminders. Aims: This study aimed to estimate the cost and analyze the outcomes of providing SMS Reminders within the HATI program in different settings. Methods: Data were analyzed using a micro-costing approach from providers' and health systems' perspectives. Subjects were divided into intervention and control groups for outcome analysis, with adherence as the outcome indicator. Results: From a provider's perspective, the highest cost was incurred in primary health care (PHC) clinics, while the lowest was in hospital clinics, most likely due to a much higher volume of SMS sent from hospitals. Costs from the health system perspective were more than two times higher than those from the provider perspective, as they included costs borne by HATI management and intervention setup. In one year, the number of patients with >95% adherent visits was higher in the intervention group than the control group, although visits decreased over time in both groups. The highest number of patients with >95% adherent visits was found in PHC clinics, while the lowest was in hospital clinics. Conclusion: SMS reminders can be expensive initially due to setup and program costs. However, they become cheaper once they are embedded into the existing system. Further studies are necessary to determine better site options for scaling up SMS reminder intervention and to study the declining adherence rate in all clinics. Abstract

Improving knowledge and skills are crucial factors in increasing the commitment of health facility staff to support exclusive breastfeeding. This study presents the steps to formulate the modules using Delphi technique and implemented those modules in the training. This study used a mix-methods method consisting of four stages, namely literature review, qualitative study, module validity and module implementation. The modules were validated by 14 lactation experts to assess reliability, feasibility, and linguistics.  The study was conducted from 2020-2021 in 5 regions in East Java Province. A pre-post-test measurement with 493 health works and non-health workers was conducted to assess the effectiveness of the modules. The first round of Delphi showed agreement proportion between 80-100% on the three aspects. In the second round of Delphi, the strong consensus stated that the modules can be implemented in training was achieved with percentage of feasibility (79.4%), reliability (44.4%) and linguistics (44.4%). The results of the training using the modules showed a significant improvement in knowledge between 6-26 points in 5 areas (p-value0.000). In conclusion, three modules have been developed and validated successfully. The modules were effectively used to improve the breastfeeding knowledge and practice among health and non-health workers.

Abstract

Introduction: Parenting a child with cancer presents significant challenges that can increase parental stress. Without appropriate interventions, this stress may impair parents' ability to provide the best possible care for their children. To implement effective care strategies, healthcare professionals, particularly nurses, must assess the stress levels experienced by parents. One objective indicator of stress is salivary Alpha-Amylase (sAA), which can be measured using a cocorometer through saliva samples. This study aimed to assess sAA levels among parents of children with cancer. Methods: The research employed a descriptive, quantitative, cross-sectional design with a sample of 30 parents from Rumah Singgah Yayasan Peduli Kanker Anak Bali, selected through purposive sampling. The research instrument used was a NIPRO-brand cocorometer. Results: Findings revealed an average sAA level of 34.93 kIU/L, with a range from 5 kIU/L to 60 kIU/L. Higher sAA levels were observed among female respondents, those residing outside of Bali, and those caring for children under two years old. Conclusion: Continuous monitoring of parental stress using objective measures such as sAA is recommended for nurses to support optimal stress management.

Abstract

Prevalence of HIV/AIDS remains high worldwide. Pre-exposure prophylaxis or PrEP is the use of an antiretroviral medication by HIV-negative people to reduce the risk of HIV acquisition. Ths systematic review aimed to analyse chalenge of hiv pre-exposure prophylactic use in low risk population. We conducted a search from the Scopus, Google Scholar, Science Directory and PubMed data bases. A total of 200 articles met the specified keywords, namely HIV, PreP, woman. In conclusion, PrEP is a highly effective HIV prevention tool for women when used consistently and as directed. Adherence, along with addressing behavioral, biological, and social factors, plays a crucial role in its effectiveness. Adherence to PrEP among women is influenced by a combination of factors related to healthcare access, support systems, individual perceptions, and daily routines. By addressing these factors and providing comprehensive support, healthcare providers can help maximize the effectiveness of PrEP in preventing HIV infection among women at risk. While HIV PrEP is highly effective when used correctly, the risk of drug resistance underscores the importance of regular monitoring, adherence support, and comprehensive counseling to maximize its effectiveness in HIV prevention efforts.

Abstract

Background and purpose: The National Health Insurance System has implemented a performance-based capitation (PBC) norm in primary health care facilities (PHCF). PBC has three indicators: contact rate, non-specialist referral ratio (NSRR), and controlled chronic diseases control program (Prolanis) participant ratio (CPPR). This study aims to explore the implementation of PBC payment at public health centers in Denpasar, Bali, Indonesia. Methods: This study utilizes an explanatory sequential mixed method design. Data collection was conducted from February to March 2023. In the first phase, quantitative data were gathered through documentation studies on PBC achievements. The second phase involved qualitative data collection through in-depth interviews with 11 informants, who were selected purposively. Thematic analysis was employed to identify internal and external factors hindering and driving PBC fulfillment. Results: Among the 11 public health centers in Denpasar, two met the PBC target in 2020, three in 2021, and seven in 2022. During 2020 to 2022, the NSRR and contact rate targets were achieved by all public health centers, except for the contact rate in 2021. The CPPR was the indicator with the lowest achievement, but shows an increasing trend. Strengths contributed to PBC's achievement including sufficient human resources and service facilities, an active Prolanis club, and a supportive Primary Care (P-Care). Weaknesses include the absence of planning documents, lack of capitation service reward, insufficient availability of medicines, and inadequate features of e-Puskesmas. Threats were suboptimal condition of Prolanis participants, the COVID-19 pandemic, non-specialist referral demands from patients, and increased number of target population. Conclusion: The achievement of PBC at public health centers in Denpasar from 2020 to 2022 was suboptimal. The emergence of various factors namely strengths, weaknesses, opportunities and threats, can be utilized in strategic formulation to optimize PBC in Denpasar public health centers.

Abstract

Background and purpose: The quality of hospital services during the COVID-19 pandemic tends to decline. This can affect patients’ satisfaction in carrying out treatment and care. Risk communication and caring behavior are important aspects that can affect patient satisfaction. The purpose of our study was to determine the relationship between risk communication and caring behavior of nurses with patient satisfaction on internal medicine polyclinic in the Mangusada Regional Hospital, Badung District, Bali Province, Indonesia.  Methods: Our research design is analytic observational with cross sectional approach. We conducted research at the internal medicine polyclinic of the Mangusada Regional Hospital, Badung District from March 1 to April 21, 2022. Samples of our study were 120 patients who visited the internal medicine polyclinic. Variables of our study which include patient characteristics, risk communication and caring behavior were collected using a questionnaire and analyzed with univariate, bivariate and multivariate approach using multiple logistic regression test with 95% confidence interval.  Results: The study revealed that 52.5% of patients had low satisfaction, 54.2% of patients perceived low risk communication and 54.2% of patients perceived low caring behavior. There is a relationship between nurse risk communication with patient satisfaction (aOR=5.2; 95%CI: 1.9-14,3, p=0.001) and nurse caring behavior with patient satisfaction (aOR=9.8; 95%CI: 3.7 – 26,0, p=0.000).  Conclusion: Risk communication and nurse caring behavior are related to patient satisfaction at internal medicine polyclinic in the Mangusada Regional Hospital, Badung District. This underscores the importance of focusing on improving care practices and communication strategies to enhance patient satisfaction. It also emphasizes the need to ensure the good ratio of health care workers to the number of patients in order to provide quality health services.

Abstract

The coronavirus disease 2019 (COVID-19) is the first pandemic in history where technologies and social media are used on a large scale to make people safe, informed, productive, and connected. At the same time, these technologies enabled the rise of the infodemic, which endangered pandemic control. This study aimed to assess COVID-19 information exposure in the community, the efforts to find related information in online media, and COVID-19 preventive behavior. A cross-sectional study was conducted with 909 participants in Indonesia using the consecutive sampling technique. Data were collected using a questionnaire distributed through social media (WhatsApp, Instagram, and Facebook) and analyzed using univariate analysis, bivariate analysis (Chi-square test), and multivariate analysis (multiple logistic regression). The results showed that about 838 (92.2%) participants said they often or always obtain information about COVID-19 online, 662 (72.8%) participants stated that information from online sources increased their knowledge of the disease, and 728 (80.1%) said that online information enabled them to make preventive efforts. Marital status (AOR: 1.81, p-value = 0.002) and perceived susceptibility (AOR: 1.42, p-value= 0.011) were the most influential factors for COVID-19 preventive behaviors. Information sources and channels frequently accessed by the community must be professionally managed by the government as valuable tools for mitigating an epidemic or pandemic.

Abstract

Political affiliation has been reported as a determinant of COVID-19 vaccine acceptance in some countries, although few studies have examined the Asian context. This study aims to fill this gap by employing an ecological study design using Indonesian regions as data points. Political affiliation was represented by incumbent President Jokowi’s vote share in the 2019 presidential election. Potential confounders included population density, human development index, availability of hospitals and primary health care, 2019–2020 economic growth, COVID-19 mortality rate, and proportion of Muslims in the population. The final analysis included 201 out of 501 districts and cities in Indonesia. Controlling for confounders, multivariate regression found that Jokowi’s vote share was an independent predictor of vaccination rate, with standardized β and R2 values of 0.350 and 0.734 for the first dose vaccination rates and 0.251 and 0.782 for the second dose, respectively. This association may be underpinned by differences in religiosity, public trust, and vulnerability to misinformation between Jokowi’s supporters and the opposition. Improving public trust in a politically polarizing society is crucial to improving future coverage of COVID-19 and other vaccines.
