Abstract

ABSTRAK Faktor utama penyebab kegagalan implementasi telemedicine adalah kurangnya kesiapan dalam menggunakan teknologi yang ada. Implementasi telemedicine tidak akan efektif apabila penyedia pelayanan kesehatan tidak siap dengan perubahan yang terjadi. Oleh karena itu penelitian ini bertujuan untuk menggambarkan kesiapan penerapan layanan telemedicine di RSUD Sanjiwani Kabupaten Gianyar di masa depan. Penelitian ini menggunakan desain penelitian kualitatif. Pemilihan informan dilakukan dengan teknik purposive sampling. Informan dalam penelitian ini berjumlah sebanyak 14 orang. Dalam penelitian ini menggunakan teknik analisis data interaktif. Hasil penelitian menunjukkan bahwa dari konteks teknologi, organisasi dan lingkungan sudah menunjukkan pelaksanaan layanan telemedicine selama ini sudah berjalan dengan baik dan sudah menggambarkan kesiapan RSUD Sanjiwani Kabupaten Gianyar untuk menerapkan layanan telemedicine di masa depan. Namun tantangan kedepan dalam pelaksanaan layanan telemedicine di RSUD Sanjiwani Kabupaten Gianyar adalah belum terpenuhinya aspek kepuasan pasien. Oleh sebab itu, RSUD Sanjiwani Kabupaten Gianyar harus memenuhi aspek kepuasan pasien pengguna layanan telemedicine agar pengguna layanan telemedicine dapat memberikan umpan balik terhadap pelayanan yang diberikan. Kata Kunci : Kesiapan, Rumah Sakit, Telemedicine ABSTRACT The main factor that causing the failure implementation of the telemedicine is due lack of radiness to use the existing technology. The implementation of the telemedicine will not be effective if health care providers are not ready for changes. Therefore, this study aims to describe the readiness to implementation telemedicine at Sanjiwani Hospital of Gianyar Regency in the future. The study uses a qualitative research design. The selection of informants was carried out by purposive sampling technique. There were 14 informants in this study. In this study using interactive data analysis techniques. The results show that from the context of technology, organization and environment, the implementation of telemedicine has been going well and has illustrated the readiness of Sanjiwani Hospital of Gianyar Regency to provide telemedicine in the future. However, the future challenge in implementing telemedicine at Sanjiwani Hospital of Gianyar Regency is that the patient satisfaction aspect has not been met. Therefore, the Sanjiwani Hospital of Gianyar Regency must meet the aspects of patient satisfaction using telemedicine so that telemedicine service users can provide feedback on the services provided. Abstract

Background and purpose: An important component of healthcare access is transportation. Lack of transportation has been cited as an important barrier to healthcare access in remote islands region. Here we conduct a feasibility study on the implementation of water ambulance service to improve healthcare access in the region of Nusa Penida and its surrounding islands in Bali Province, Indonesia. Methods:  We employed a qualitative approach with interviews and focused group discussions with stakeholders. Data collection was conducted to assess the feasibility of operating water ambulance services, based on a framework that included technical, market, organizational, and financial feasibilities. Inductive coding was conducted and data was analyzed to evaluate the feasibility based on local capacity against pre-determined framework of assessment based in literature. Results: Qualitative interviews revealed perceived urgency and need for water ambulance service among stakeholders. They also perceive the need of water ambulance service was not mutually exclusive to other alternatives, such as expanding healthcare capacity on the islands. There was also readiness and adequate capacity from stakeholders which include a dedicated health emergency services under the health office that coordinate ambulance service in the region, readiness to create dedicated medical team and boat crew to operate water ambulance services, and readiness to cover the cost of initial procurement followed by operational costs which may include public-private partnerships. Conclusion: There was a strong perceived need for water ambulance service in Nusa Penida Islands. At the same time, local stakeholders revealed adequate basic capacity to feasibly operate water ambulance services according to benchmarks identified in literature review.

Abstract

Background and purpose: Hospital accreditation is carried out to evaluate hospitals’ performance in providing patients with excellent and safe health services. One aspect of service quality is patient safety culture. This study aims to determine the impact of hospital accreditation on patient safety culture in health service implementation at the Sanglah General Hospital in Denpasar City, Bali Province. Methods:This study used a qualitative research design. In-depth interviews were conducted to 11 informants consisting of hospital management, clinicians and nurses which were chosen purposively since they were directly related to the assessment and supervision of health service providers at Sanglah Hospital. The interview guide was based on the Modified Manchester Patient Safety Framework (MaPSaF). Data were analyzed with a thematic approach. Results: The interview results show that the ten dimensions of patient safety culture had been implemented well but still need to be improved in the dimensions of giving priority to safety, system errors and responsibilities, recording incidents and best practices, learning and influencing change, also education and training of staff and teamwork. The maturity level of patient safety culture at Sanglah Hospital is at generative culture in four dimensions and at proactive culture on the six dimensions of patient safety culture. Conclusion: Accreditation had a positive impact on the maturity level of patient safety culture at a proactive level, mainly changes in knowledge, attitudes, and behavior in health service providers. However, in its implementation, there were still constrained on infrastructure, time, and energy.

Abstract

Background and purpose: The incidence of Dengue Hemorrhagic Fever (DHF) in Bali, especially in Denpasar, remains high. Efforts to eradicate mosquito nests have been carried out but did not give a significant reduction in the incidence number. The dengue vaccine is now available and can be a way of prevention. The dengue vaccine has not been included in the mandatory immunization program by the government so people who want to get vaccinated have to pay for it by themselves. This study aims to determine the relationship between socio-demographic factors and perceptions with the willingness to pay for the dengue vaccine among parents of elementary school students in Denpasar. Methods: This study used a cross sectional design involving 100 mothers of students from grade 3 to grade 6 at Dauh Puri 5 Elementary School and Dauh Puri 6 Elementary School who were selected with systematic random sampling. Data collected included education level, income, respondents’ perceptions and willingness to pay for dengue vaccine. Data were collected by interview using a questionnaire, the results were analyzed using a multivariate logistic regression. Results: The average willingness of parents to pay for the dengue vaccine per dose was IDR 131,170. Percentage of respondents who were willing to pay for the dengue vaccine was 31%. The results of analysis showed that variables associated with parents’ willingness to pay for the dengue vaccine were education level (OR=4.06; 95%CI: 1.19-13.76), perceived susceptibility (OR=1.59; 95%CI: 0.58-4.37) and perceived benefits (OR=2.60; 95%CI: 1.00-6.81). Conclusion: Parents’ willingness to pay for the dengue vaccine remains very low compared to the current price of vaccine per dose. The government needs to provide subsidies so that the dengue vaccine can be included in the national immunization program and be accessed by all people. Further research is needed with a broader scope and more varied population’s characteristics to obtain a willingness to pay value that can represent society in general.

Abstract

Background and purpose: Cervical cancer is the second most prevalent cancer in women after breast cancer. Nationally, the coverage of cervical cancer screening among reproductive age women from 2014 to 2018 only reached 7.34%, while in Badung District was 10.3% in 2018. In the era of the Universal Health Coverage (UHC) schemes, cervical cancer screening has been covered. This study aims to explore factors associated with the utilization of cervical cancer screening programs in the UHC era in Badung District. Methods: This cross-sectional study was conducted from January-February 2020, involving 195 women aged 20-50 years who were selected through multistage random sampling from two villages in the Badung District. Characteristics of the women, perceptions of seriousness, vulnerability, barriers and benefits, self-efficacy, cues to action and knowledge on cervical cancer screening programs were assessed. Data was analyzed descriptively, and association was assessed with Chi square and multiple logistic regression. Results: The proportion of reproductive age women who utilized cervical cancer screening program in the last 5 years was 44.1%. The utilization of cervical cancer screening program was associated with age (AOR=0.03; 95%CI: 0.03-0.300), perceived vulnerability (AOR=28.77; 95%CI: 3.745-221.68), perceived barriers (AOR=55.70; 95%CI: 7.12-435.72), selfefficacy (AOR=34.44; 95%CI: 4.34-273.66), cues to action (AOR=19.56; 95%CI: 3.32-115.24). Conclusion: The perception of barriers was found to be significantly associated with the use of cervical cancer screening programs along with other variables related to the Health Belief Model (HBM). Structured campaigns and improvement of the quality of services should be carried out in order to increase access of women to cervical cancer screening.

Abstract

Background and purpose: The pandemic of COVID-19 is an unprecedented medical and economic challenge for healthcare system. The number of patients during pandemic of COVID-19 had a rapid growth which led to changes in services aspects that threaten financial viability of hospitals, including private hospitals. This study aims to determine the efforts of private hospitals in maintaining financial stability during the pandemic of COVID-19. Methods:  A qualitative study with a multi-case design carried out in 7 (seven) private hospitals in Denpasar. Informants were obtained based on a purposive sampling approach. This study recruited 19 informants from 7 private hospitals in Denpasar. Data was collected through in-depth interviews conducted face-to-face, using the Zoom and Whatsapp applications. The data were analysed using a content analysis approach. Results: Almost all hospitals stated that their finances disrupted. Financial mitigation efforts were difficult to do, hospital preparation and responses were dependent on the hospital's financial condition and regulations that changing, and recovery efforts were not easy either because in general the community's economy has not recovered. Conclusion: Private hospital finances were experiencing disruption. Mitigation efforts were difficult to do due to the unpredictable changing regulations and policies during the pandemic. Based on the study results, it can be recommended that the hospital management needs to develop a service strategy that is able to overcome the problem of the surge in the number of patients.

Abstract

Notable challenges and accomplishments have characterized Indonesia's efforts in combating HIV AIDS. As of 2022, it is estimated that 546,573 people are living with HIV in the country, with new infections totaling 24,223 in the same year. Recent data shows a decrease in new HIV infections and AIDS-related deaths. However, there remains a concerning steady rate of new infections within the men who have sex with men(MSM) population.1 Although there have been improvements in Indonesia's HIV care cascade, particularly in achieving the first target of the 95-95-95 goals, challenges persist. In 2022, only 42% of people living with HIV (PLHIV) knew their status and were on HIV treatment, and a mere 15% of those on treatment achieved viral suppression.2 These figures highlight the necessity for strategies that are specifically tailored to meet the unique needs of different communities within the HIV AIDS landscape in Indonesia.

Abstract

Online Registration Application (APDOL) as an alternative way of Wangaya Hospital Denpasar City in overcoming the problem of handling patient queues. Which began to be applied at the Wangaya Hospital, Denpasar City in 2018. This study aims to describe the patient's perception of the use of APDOL use at the Wangaya Hospital, Denpasar City. This study is a qualitative descriptive study conducted in February-June 2022. The informants in this study consisted of 20 visiting patients at Wangaya Hospital. Data were collected by semi-structured interviews. The data analysis technique was carried out with three activities, namely data reduction, data presentation, and drawing conclusions. The results of this study indicate that patients who use APDOL give a good response to the presence of APDOL in dealing with queuing problems at Wangaya Hospital. The inhibiting factor of the use of APDOL is the lack of socialization by the hospital. While the supporting factors for the existence of APDOL are the ease of using APDOL, increasing effectiveness, and flexible usage time. It is recommended that Wangaya Hospital conduct on going socialization to introduce APDOL to patients.Keywords: Online Registration Application (APDOL), Application Usage Utilization, Perception, Technology Acceptance Model (TAM).

Abstract

Aplikasi Jamsostek Mobile (JMO) merupakan suatu aplikasi yang diluncurkan oleh BPJS Ketenagakerjaan yang bertujuan untuk memberikan kemudahan pelayanan administratif bagi peserta BP Jamsostek. Pemanfaatan terhadap aplikasi JMO per tahun 2022 yaitu baru mencapai 57,65%, hal ini masih belum sesuai dengan target yang ditentukan oleh BPJS Ketenagakerjaan cabang Bali Denpasar yaitu sebesar 100%. Penelitian ini bertujuan untuk mengetahui faktor yang berhubungan dengan minat pemanfaatan aplikasi JMO di BPJS Ketenagakerjaan Cabang Bali Denpasar dengan pendekatan Technology Acceptance Model. Desain penelitian yang digunakan adalah kuantitatif dengan metode cross-sectional. Sampel dari penelitian ini adalah peserta BPJS Ketenagakerjaan Cabang Bali Denpasar sebanyak 137 orang yang terpilih menggunakan metode Consecutive sampling. Penelitian dilakukan selama bulan Maret-Mei 2023. Instrumen pengumpulan data menggunakan kuesioner. Analisis data dilakukan secara univariabel dan bivariabel. Hasil penelitian menunjukkan sebanyak 72,67% responden berminat untuk terus memanfaatkan aplikasi JMO. Terdapat hubungan yang signifikan antara persepsi kebermanfaatan, persepsi kemudahan, persepsi kredibilitas dan jumlah informasi dengan minat untuk terus memanfaatkan aplikasi JMO (p<0,05). Sebagai upaya dalam meningkatkan minat peserta BP Jamsostek untuk terus memanfaatkan aplikasi JMO, BPJS Ketenagakerjaan agar terus melakukan sosialiasi dan dukungan kepada peserta untuk menggunakan aplikasi JMO. Abstract

ABSTRAKSI-DOLIR merupakan pendaftaran online yang dimiliki Puskesmas I Denpasar Timur untuk pasien yang hendak mencari surat rujukan rutin agar pasien mendapatkan surat rujukan lebih cepat dan mudah. Akan tetapi sejak Bulan April-September 2022, target SI-DOLIR tidak pernah tercapai dan evaluasi program yang tidak sesuai. Salah satu cara untuk mengetahui seberapa besar minat penggunaan seseorang terhadap teknologi adalah dengan teori UTAUT. Tujuan penelitian ini untuk mengetahui bagaimana pengaruh faktor UTAUT dan faktor sosiodemografi terhadap minat penggunaan SI-DOLIR ke depannya pada pasien BPJS kesehatan yang belum pernah menggunakan SI-DOLIR menggunakan metode UTAUT. Penelitian ini merupakan penelitian kuantitatif dengan desain cross-sectional. Teknik pengambilan sampel dalam penelitian ini adalah consecutive sampling dengan sampel berjumlah 110 responden. Data akan dianalisis menggunakan analisis regresi logistik. Hasil analisis menunjukan jika faktor ekspektasi kinerja, ekspektasi usaha, dan tingkat pendidikan memiliki pengaruh terhadap minat penggunaan SI-DOLIR ke depannya. Sedangkan pengaruh sosial, kondisi yang memfasilitasi, usia, dan jenis kelamin tidak berpengaruh secara signifikan terhadap minat penggunaan SI-DOLIR ke depannya saat mencari surat rujukan rutin. Kesimpulan yang di dapat jika faktor UTAUT ekspektasi kinerja dan ekspektasi usaha serta faktor sosiodemografi tingkat pendidikan berpengaruh terhadap minat penggunaan SI-DOLIR pada pasien BPJS Kesehatan untuk mencari surat rujukan rutinKata Kunci : Minat Penggunaan, Faktor-Faktor, UTAUT, Pendaftaran Online
