Abstract

Background: Perioperative hypothermia (core temperature < 36 °C) is a frequent complication during surgery and is associated with adverse outcomes such as myocardial ischemia, coagulopathy, surgical site infections, and delayed recovery. One of the contributing factors is the use of large volumes of room-temperature irrigation fluids. Warming irrigation fluids is a simple and effective strategy to maintain perioperative normothermia, but its implementation requires a thorough understanding of patient safety profiles to ensure safe and optimal practice. Purpose: To analyze the patient safety profile in the use of perioperative warm irrigation. Method: A literature review was conducted on relevant literature to analyze the effectiveness and safety profile of perioperative warmed irrigation fluids. The reviewed literature included clinical studies and research discussing the use of warmed irrigation fluids in various surgical procedures, focusing on hypothermia prevention, patient thermal comfort, and safety aspects. Data were analyzed descriptively to identify key findings regarding effectiveness, safety profile, and clinical practice recommendations. Results: Evidence from various clinical studies shows that warmed irrigation fluids improve thermal comfort, reduce the incidence of hypothermia and shivering, and support patient safety. The effectiveness is particularly notable in endoscopic and urologic procedures, and can be enhanced when combined with warmed intravenous fluids. However, the added value of combining multiple warming strategies in short-duration surgeries remains unclear. Importantly, the use of warmed irrigation fluids has not been linked to increased postoperative complications. Conclusion: Warmed irrigation fluids demonstrate an excellent safety profile when implemented optimally. Optimal implementation requires consistent temperature control, reliable warming equipment, and adherence to safety protocols. Further standardization of safety protocols and healthcare professional training in intraoperative fluid warming practices is needed to optimize patient safety outcomes and ensure safe implementation across various clinical settings.   Keywords: Hypothermia; Patient Safety; Perioperative Warm Irrigation.   Pendahuluan: Hipotermia perioperatif (suhu inti tubuh < 36 °C) merupakan komplikasi yang sering terjadi selama pembedahan dan berhubungan dengan berbagai efek merugikan, seperti iskemia miokard, gangguan koagulasi, infeksi luka operasi, serta pemulihan yang tertunda. Salah satu faktor yang berkontribusi adalah penggunaan cairan irigasi dalam volume besar dengan suhu ruang. Pemanasan cairan irigasi merupakan strategi yang sederhana dan efektif untuk mempertahankan normotermia selama perioperatif. Tujuan: Untuk menganalisis profil keselamatan pasien dalam penggunaan irigasi hangat perioperative. Metode: Tinjauan naratif dilakukan terhadap literatur yang relevan untuk menganalisis efektivitas dan profil keselamatan penggunaan cairan irigasi hangat perioperatif. Literatur yang dikaji mencakup studi klinis dan penelitian yang membahas penggunaan cairan irigasi hangat dalam berbagai prosedur pembedahan, dengan fokus pada pencegahan hipotermia, kenyamanan termal pasien, dan aspek keselamatan. Data dianalisis secara deskriptif untuk mengidentifikasi temuan utama terkait efektivitas, profil keselamatan, dan rekomendasi praktik klinis. Hasil: Bukti dari berbagai studi klinis menunjukkan bahwa cairan irigasi hangat meningkatkan kenyamanan termal, menurunkan kejadian hipotermia dan menggigil, serta mendukung keselamatan pasien. Efektivitasnya sangat menonjol pada prosedur endoskopi dan urologi, dan dapat ditingkatkan dengan kombinasi infus intravena hangat. Namun, manfaat tambahan dari penggabungan beberapa metode pemanasan pada pembedahan berdurasi pendek masih belum jelas. Penting untuk dicatat bahwa penggunaan cairan irigasi hangat tidak dikaitkan dengan peningkatan komplikasi pascaoperasi. Simpulan: Cairan irigasi hangat menunjukkan profil keselamatan yang sangat baik. Ketika diimplementasikan secara optimal membutuhkan kontrol suhu yang konsisten, peralatan pemanas yang andal, serta kepatuhan terhadap protokol. Standarisasi lebih lanjut dalam praktik pemanasan cairan intraoperatif diperlukan untuk mengoptimalkan luaran pasien.   Kata Kunci: Hipotermia; Irigasi Hangat Perioperative; Keselamatan Pasien.

Abstract

Recurrent ischemic stroke (RIS) is a major global health issue due to its high morbidity, mortality, and economic burden. While numerous studies have explored the timing and pattern of RIS globally, there is a limited understanding of this issue within the Indonesian context. In the absence of sufficient local data, synthesizing global evidence becomes crucial to inform clinical practice and policy development in regions like Indonesia. This review aimed to explore timing patterns of RIS especially early and late phases and estimate risks based on follow-up, to inform evidence-based interventions. A scoping review based on Arksey & O’Malley framework and PRISMA-ScR guidelines. Literature search (2014–2024) in PubMed, Scopus, ScienceDirect, and CINAHL. Nine studies were included and analyzed thematically. From 2,987 articles, 9 met inclusion criteria. RIS risk ranged 4.3–23.4%, with median recurrence time 21–25 days. Early RIS (<90 days) had distinct risk factors (e.g. hypertension, prior stroke, heart disease) compared to late RIS. Modified Rankin Scale (mRS) and National Institutes of Health Stroke Scale (NIHSS) were key instruments; follow-ups done via clinics, registries, or surveys. Early (<90 days) and late recurrent ischemic strokes differ in both risk factors and clinical implications. Early post-stroke monitoring is critical to reducing recurrence. To address long-term prevention and support adherence to secondary prevention strategies, technology-assisted monitoring or broader digital health solutions should be considered as potential interventions, particularly in overcoming challenges related to long-term follow-up and patient engagement.

Abstract

Penyakit kardiovaskular masih menjadi penyebab utama kematian di seluruh dunia, dengan gagal jantung menjadi salah satu manifestasi penyakit yang paling serius. Gagal jantung berdampak signifikan terhadap kualitas hidup pasien, sering kali menimbulkan gejala psikologis dan fisik seperti kecemasan, kualitas tidur buruk, dan nyeri. Penatalaksanaan gagal jantung mencakup pendekatan farmakologis dan nonfarmakologis. Di antara intervensi non-farmakologis, terapi musik telah diidentifikasi sebagai metode potensial untuk meringankan gejala fisik dan psikologis. Penelitian ini bertujuan untuk memetakan intervensi terapi musik yang ada yang bertujuan untuk mengurangi tingkat kecemasan pada pasien gangguan jantung. Penelitian ini menggunakan desain Scoping Review. Pencarian literatur yang komprehensif dilakukan dengan menggunakan database PubMed, CINAHL (via EBSCO), dan Scopus. Kriteria inklusi mencakup ketersediaan teks lengkap gratis, publikasi antara tahun 2014-2024, studi yang berfokus pada terapi musik untuk mengurangi kecemasan pada pasien dengan penyakit kardiovaskular, dan penelitian primer yang menggunakan uji Randomized Trial Control (RCT), uji klinis, atau desain kuasi-eksperimental. Pencarian awal menghasilkan 1.422 artikel, setelah dilakukan proses penyaringan, tersisa 7 artikel eligible untuk ditelaah menggunakan JBI aprasial tool. Temuan menunjukkan bahwa intervensi terapi musik berpengaruh dalam mengurangi kecemasan dan depresi, mengurangi rasa nyeri, meningkatkan kualitas tidur, dan meningkatkan kualitas hidup secara keseluruhan pada pasien dengan kondisi jantung. Terapi musik efektif mengurangi kecemasan, nyeri, gangguan tidur dan meningkatkan kualitas hidup pasien jantung. Temuan ini menunjukkan intervensi ini aman dan dapat mendukung perawatan holistik. Penelitian lanjutan dengan metode yang lebih kuat, protokol yang lebih seragam, serta evaluasi efek jangka panjang dan standar diperlukan, termasuk pemanfaatan teknologi inovatif untuk memperluas akses.

Abstract

Tetanus, an infection caused by Clostridium tetani, remains a global health concern due to its high fatality, morbidity, and mortality rates. Although the spores of this bacterium are commonly found in soil and animal waste, contamination of the oral cavity due to poor oral hygiene and inadequate dental care also presents a significant risk. The aim of this study was to describe the role of dental infection in the incidence of tetanus in the intensive care unit. This study used descriptive study design with case report method. A 67-year-old male patient was admitted for 38 days to the intensive care unit of a government hospital in Bandung due to grade III generalized tetanus, with Ablett grade IV classification and Tetanus Severity Score (TSS) of 7. The patient had a history of stab wounds from wood and multiple plaque (+) in the oral cavity due to poor oral hygiene and had a habit of cleaning teeth using sticks that were not clean and not in accordance with recommended dental care tools.. History of tetanus vaccine with incomplete dose. During treatment, the patient required mechanical ventilator assistance with CPAP-PS mode. Conclusion of this research that the route of entry for tetanus-causing bacteria can be through wounds, including wounds in the oral cavity that often go unnoticed. Therefore, it is very important to maintain oral hygiene through the application of proper oral hygiene and eliminate the habit of cleaning teeth with dirty tools.

Abstract

ABSTRACT Indonesia is a country with a high risk of disasters, including earthquakes. Preparedness is important to reduce the impact of disasters, while anxiety can influence preparedness by fostering motivation. The purpose of this study was to determine the relationship between anxiety and earthquake disaster preparedness among SMAN 1 Cisarua students located in the Red Zone of the Lembang Fault. This research is a quantitative research with a cross-sectional approach involving 95 students as respondents. The instruments used were the Indonesian version of the General Anxiety Disorder (GAD-7) questionnaire to measure anxiety and the LIPI-UNESCO/ISDR (2006) preparedness questionnaire to measure preparedness. The data of the research respondents were analyzed using Spearman rho non-parametric analysis. The results showed that most students had a moderate level of anxiety (58,9%) and a ready level of preparedness (60%). The p-value obtained is 0,013 and the correlation coefficient is 0,255 which indicates that there is a significant relationship between anxiety and earthquake disaster preparedness with a weak relationship strength and positive relationship direction. Anxiety with earthquake disaster preparedness has a positive relationship at a certain level, namely anxiety at a moderate level will contribute to increasing preparedness by fostering encouragement or motivation. Keywords: Anxiety, Disaster Preparedness, Earthquake, Student  ABSTRAK Indonesia merupakan negara yang berisiko tinggi terhadap bencana, termasuk gempa bumi. Kesiapsiagaan penting untuk mengurangi dampak bencana, sedangkan kecemasan dapat memengaruhi kesiapsiagaan dengan menumbuhkan doroangan atau motivasi. Tujuan penelitian ini adalah untuk mengetahui hubungan kecemasan dengan kesiapsiagaan bencana gempa bumi pada siswa SMAN 1 Cisarua yang berlokasi di Zona Merah Sesar Lembang. Penelitian ini merupakan penelitian kuntitatif dengan pendekatan cross-sectional yang melibatkan 95 siswa sebagai responden. Instrumen yang digunakan adalah kuesioner General Anxiety Disorder (GAD-7) versi bahasa Indonesia untuk mengukur kecemasan dan kuesioner kesiapsiagaan LIPI-UNESCO/ISDR (2006) untuk mengukur kesiapsiagaan. Data responden penelitian dianalisis menggunakan analisis non-parametrik Spearman rho. Hasil penelitian menunjukkan jika sebagian besar siswa memiliki tingkat kecemasan sedang (58,9%) dan tingkat kesiapsiagaan siap (60%). Nilai p-value yang didapatkan sebesar 0,013 dan koefisien korelasi 0,255 yang mengindikasikan bahwa terdapat hubungan yang signifikan antara kecemasan dan kesiapsiagaan bencana gempa bumi dengan kekuatan hubungan yang lemah serta arah hubungan yang positif. Kecemasan dengan kesiapsiagaan bencana gempa bumi memiliki hubungan yang positif pada tingkat tertentu, yakni kecemasan pada tingkat sedang akan berkonstribusi dalam meningkatkan kesiapsiagaan dengan menumbuhkan dorongan atau motivasi.  Kata Kunci: Kecemasan, Kesiapsiagaan, Gempa Bumi, Siswa

Abstract

ABSTRACT Earthquakes are unpredictable, making disaster preparedness essential. Risk perception is key, but its relationship with preparedness remains inconsistent. To analyze the relationship between risk perception and disaster preparedness. A cross-sectional study with 73 high school students selected via accidental sampling from a population of 1,509. Risk perception was measured using PRISM (S-CVI/ave = 0.9; reliability r = 0.95, p < 0.001), and preparedness using LIPI/UNESCO-ISDR (validity 0.566–0.895; reliability 0.969). Data were analyzed using descriptive statistics and Pearson correlation. Most students had low-risk perceptions (50.7%) but were categorized as prepared (64.4%). No significant correlation was found (p = 0.1, r = -0.19). Disaster knowledge significantly influenced preparedness (p < 0.05). Disaster knowledge affects preparedness more than risk perception. Schools should integrate disaster education into curricula, collaborate with BPBD, and empower extracurricular programs like PMR and Scouts. Keywords: Earthquake, Preparedness, Risk Perception, Students

Abstract

Perilaku mencuci tangan dengan sabun di Indonesia masih menjadi masalah. Penyebabnya dikaitkan dengan kurangnya kesadaran dalam mencuci tangan pakai sabun. Penelitian ini bertujuan untuk mengetahui pengaruh pemberdayaan komunitas sekolah terhadap penerapan mencuci tangan di kalangan siswa sekolah dasar di Kecamatan Dayeuhkolot, Kabupaten Bandung. Penelitian ini menggunakan desain quasi eksperimental dengan pretest dan posttest serta melakukan analisis deskriptif dan inferensial. Sampel terdiri dari 24 guru, 377 siswa di kelas 4-6, dan 24 dokter kecil. Metode pendekatan dalam penelitian ini menggunakan usaha kesehatan sekolah terpadu (gabungan model fit for school dan UKS terpilih), yang terdiri dari enam tahap. Instrumen terdiri dari kuesioner pengetahuan, lembar observasi, dan lembar checklist. Cuci tangan pakai sabun dievaluasi selama tiga bulan. Hasil menemukan bahwa terdapat skor meningkat dalam kategori baik untuk keterampilan cuci tangan pakai sabun guru dari 12,5% menjadi 100%, skor dokter kecil dalam kategori baik meningkat pada keterampilan cuci tangan pakai sabun dari 0% sampai 100%, keterampilan dari cuci tangan pakai sabun pada siswa meningkat dalam kategori baik dari 0% menjadi 87,5%. Pemberdayaan komunitas sekolah memengaruhi perilaku mencuci tangan di kalangan siswa SD. Handwashing with soap behavior in Indonesia remains a problem. The cause is associated with lack of awareness in handwashing with soap. This study aimed to determine effect of school community empowerment on handwashing implementation among elementary school students in Dayeuhkolot Subdistrict, Bandung District. This study used quasi experimental design with pretest and posttest, also descriptive and inferential analysis. Samples consisted of 24 teachers, 377 students at 4th – 6th grade and 24 little doctors. The approach method in this study was using integrated school health effort (combined model of fit for school and selected school health effort) consisting of six stages. Instruments were knowledge questionnaires, observation and checklist sheets. Handwashing with soap was evaluated for three months. Results found that score of little doctors in good category increased in skill of handwashing with soap from 0% to 100%, the skill among the students improved in good category from 0% to 87.5%. School community empowerment affects on handwashing behavior among elementary school students.

Abstract

The coronavirus disease 2019 (COVID-19) pandemic has had a global impact on mental health and sleeps quality that is unprecedented in the 21st century.This systematic review aimed to assess the psychological impact of COVID-19 and its effect on people’s sleep. Systematic searches were conducted viaPubMed, ScienceDirect, and ProQuest from 2019 to August 2020, following PRISMA guidelines. Articles were selected based on eligibility criteria such as having a cross-sectional study design, assessing the general public’s mental health status or sleep problems, medical workers and non-medical workers duringthe COVID-19 pandemic, and using standardized and validated scales for measurement. The keywords: COVID-19 OR SARS-CoV-2 AND Mental health ORPsychological health AND General population OR General public AND sleep AND Impact of Coronavirus disease 2019. A total of 23 articles were selectedafter being assessed. During the period of the COVID-19 pandemic, medical workers were found to have psychological problems such as anxiety, depression, stress, Post-Traumatic Stress Disorder (PTSD), psychological distress, somatization symptoms, suicidal ideation, high risk of severe mental illness, worry, and insomnia. Non-medical workers experienced symptoms of depression. The general public experienced psychological problems: depression, anxiety, stress, bad mood, inability to stop thinking about COVID-19, panic, and experiencing sleep problems such as changing sleep patterns, sleep disturbances, sleep quality, difficulty with sleep initiation, and shortened sleep duration. The COVID-19 pandemic has been associated with psychological problems and sleep disorders for medical workers, non-medical workers, and the general public.

Abstract

Background: The prone position is one of the highly recommended interventions to treat hypoxaemia and lung injury in ARDS patients. However, its efficacy in non-intubated patients remains inconclusive due to predominantly being reviewed through observational and case studies. This review aims to evaluate the efficacy of prone position on the oxygenation status and mortality rate of ARDS patients through experimental studies. Methode: This review systematically searched three international databases: PubMed, Web of Science, and CINAHL from November to December 2022, without restricting the publication year. The focus was solely on experimental studies examining the effectiveness of prone positioning in adult, non-intubated ARDS patients. The quality of studies was assessed using the JBI critical appraisal tool. Results: Of the 12 included studies, most RCTs were identified as poor quality. All patients appeared to be infected with Covid-19, and the majority were on conventional oxygen therapy. Most studies showed that the prone position was effective in improving the oxygenation status for up to two hours post-session, with a mortality rate not substantially different from that of control group. Further good-quality experimental studies are needed to strengthen these results. Conclusion: The prone position is quite effective in improving the oxygenation status of patients.

Abstract

Introduction:Pain is a problem experienced by patients with burns. Pain reduction is the main focus in the management of burn patients. Non-pharmacological treatment options with music therapy and spiritual care approaches are part of nursing care. Objective: The study was conducted to identify the effectiveness of music therapy and spiritual care in reducing pain during dressing changes for burn patients. Materials and methods:The study used a quasi experiment pre-post test method on 30 burn patients with the music therapy and other with spiritual care intervention. Samples were taken using accidental sampling. Patients were measured for pain level using Numeric rating scale (NRS). Data were analysed using univariate analysis, dependent t-test and an independent t-test to compare between music therapy and spiritual care. Result: Music therapy and spiritual care have a significant effect on the measurement results before and after the intervention resulting in a value of (P=0.000) p a (0.005). While the mean difference between music therapy and spiritual care after after dressing change (P=0.000) and (t=5.875). Conclusion: There is an effect of music therapy intervention and spiritual care on reducing pain in burn patients undergoing dressing change procedures. Spiritual care had a higher average reduction than music therapy.
