Abstract

The use of natural products has been widely used as a resource of new bioactive chemical compounds. One of them is the Paku Atai Merah (Angiopteris ferox Copel) tuber which has long been used empirically by the Dayak tribe of East Kalimantan as an anti-cancer. The purpose of this study was to determine the anticancer cytotoxic activity of the extract and fractions of Paku Atai Merah tuber against T47D breast cancer cells in vitro. Extract of Paku Atai Merah tubers was obtained by maceration method using ethanol solvent until obtained the ethanolic extract then fractionated using various solvents to obtain n-hexane, ethyl acetate, and aqueous-ethanol fractions. The cytotoxic effect was carried out based on the MTT assay. Phytochemical screening tests showed positive results for the presence of flavonoid, phenolic, tannin, saponin and steroid compounds. The results of the cytotoxic activity study showed that the ethyl acetate fraction had moderate cytotoxic activity in T47D cancer cells with an IC50 value of 84.8 µg/ml. Ethanol extract (513.06 µg/ml) and n-Hexane frsction (881.97 µg/ ml) were also included in the weak category. This study indicates that ethyl acetate fraction can be developed as a supportive therapy for breast cancer treatment.

Abstract

Kratom (M. speciosa), has a long history of traditional use for various ailments as well as for recreational purposes due to its opioid and psychoactive effects. Nowadays kratom is easily accessible via online markets, with leaf powders commonly categorized by vein color, suggesting different effects despite minimal variations in alkaloid content. To improve the identification and characterization of kratom samples, fingerprinting methods using chemometric tools are increasingly applied in food and pharmaceutical analysis. This study explores a combination of Fourier Transform Infrared Spectroscopy (FTIR) and Thin-Layer Chromatography (TLC) densitometry data, analyzed with Principal Component Analysis (PCA), to develop a model for distinguishing kratom venation and other alkaloid-containing plants.The TLC chromatogram revealed six consistent peaks (Rf values of 0.17, 0.27, 0.42, 0.73, 0.8, and 0.9), correlating with alkaloids found in kratom. Using PCA, we combined FTIR absorbance values at selected wavenumbers with TLC chromatogram data, resulting in four principal components (PC1, PC2, PC3, and PC4) that explained 84.1%, 9.7%, 2.7%, and 2.5% of the variance, respectively. The resulting score plot demonstrated distinct clustering of samples, which was then verified with cluster analysis. The resulting analysis indicated effective differentiation between kratom vein colors and plant species. The developed FTIR-TLC-PCA model offers a promising approach for sample classification, potentially aiding quality control and authenticity verification in pharmaceutical applications.

Abstract

Administering multiple intravenous (IV) preparations through a single catheter line can lead to incompatibility issues, such as precipitation. Precipitation can be avoided with sufficient knowledge by nurses during the administration of IV preparations to patients. This research aims to assess nurses' knowledge and management of incompatibility issues in IV pharmaceutical preparations. This non-experimental research collected data through interviews with healthcare professionals (nurses). The research was conducted at Sultan Agung Islamic Hospital, Semarang. A total of 14 healthcare professionals (nurses) who reconstituted and administered IV pharmaceutical preparations to patients in the Intensive Care Unit (ICU) participated in the research. The results revealed that 13 nurses (92.9%) encountered incompatibility incidents with seven types of drugs. Five nurses (29.4%) reported incompatibility issues with cefoperazone. The most frequently observed incompatibility was color change, reported by seven nurses (53.8%), such as white spots appearing during phenytoin reconstitution and discoloration in IV pharmaceutical preparations of cefoperazone and ceftriaxone. The most common approach to addressing incompatibility was replacing the preparation, reported by six nurses (46.2%). The management of incompatibility issues varied among nurses, even when dealing with the same pharmaceutical preparation. Therefore, training on sterile preparation incompatibility should be provided to all healthcare professionals to establish a standardized operational procedure (SOP) for handling incompatibility in IV pharmaceutical preparations.

Abstract

Compartmental modeling analysis was used to understand the transport mechanism of drugs in biological systems by computation presented as intercompartmental flows or material. This study was aimed to implement compartmental modeling analysis for in vitro permeation studies in transdermal delivery. The cumulative drug transported versus time were obtained based on the previous report and implemented the two structural different proposed models. WinSAAM software (Windows-based Simulation Analysis and Modeling-WinSAAM Project Group, University of Pennsylvania) was used to analyze data with compartmental analysis. The chosen models were selected through visual and numeric evaluation. The best model had been chosen and could figure out the drug transport kinetics in the biological system. The compartmental modeling approach was helpful used in understanding drug transport mechanisms in transdermal delivery and effectively estimate the drug transport parameter.

Abstract

Bengkuang (Pachyrhizus erosus) has been traditionally used as sun screening and skin whitening. The active compounds in bengkuang extract already published included their activities in antioxidant and skin whitening. However, standardization of bengkuang extract has not been studied. This research aims to find out the analysis procedure by High Performance Liquid Chromatography to make standardization bengkuang extract.The first step of this research was collecting bengkuang from Prembun, Central Java, Indonesia in dry season. After cleaning and peeling, bengkuang root was sliced, dried and ground to make powder. Then followed by extraction using Soxhlet in petroleum ether and subsequently in methanol. Methanol extract was evaporated and then partitioned with ethyl acetate-water. Ethyl acetate fraction was evaporated and then separated in open column chromatography using silica gel as stationary phase and a gradient mixture of chloroform-ethyl acetate-methanol as mobile phase. Bio guided fraction method was used for separation and purification to get isolated compounds. The isolated compounds obtained from this fractionation were then elucidated and analyzed their activities.A new compound (8,9-furanyl-pterocarpan-ol) has been selected as a biomarker for extract standardization. The optimum of HPLC condition for standardization consisted of a column (Zorbax SB-C18; i.d. 0.46 cm; 5 μm particle size), mobile phase (gradient elution of MeOH-water) with flow rate of 1 ml/min and detector (UV-detector at 293 nm). The obtained LOD value was 0.51 ± 0.02 µg. The potentials of this compound to absorb UV ray, antioxidant and anti-tyrosinase were 4.018 mAU*S/mml; 2.113±0.001mM (SC50); 7.19±0.11 mM (IC50), respectively.Keywords : bengkuang (Pachyrhizus erosus) extract, (8,9)-furanyl-pterocarpan-3-ol, standardization, sunscreen, skin whitening

Abstract

Reactive Oxygen Species (ROS) is one of the cancer-causing agents, one of which is lung cancer. In addition to being carcinogenic, ROS can also be used to kill cancer cells themselves, by increasing their levels to the threshold of apoptosis. Therefore, it is necessary to inhibit certain antioxidant enzymes that are highly expressed in lung cancer. One of them is AKR1C1 which plays a role in the eradication of intracellular ROS. However, AKR1C1 has a high structural similarity to AKR1C2, so it can inhibit therapy causing selectivity problems. Vetiver (Vetiveria zizanioides) has potential as an anticancer. This study was conducted to explore vetiver as a co-chemotherapeutic agent for lung cancer targeting AKR1C1 selectively. The method used is distillation, identification of vetiver compounds using GC-MS, and through bioinformatics studies. Predictive analysis with KNIME was carried out to determine the activity of the test compound. All tested vetiver compounds had a predictive value of 1 (active) on AKR1C1 and 0 (inactive) on AKR1C2. Through GC-MS obtained 354 compounds were identified. These compounds are used to filter the compounds predicted by KNIME. The molecular docking results showed that of the 10 tested vetiver compounds, there was 1 compound that had the strongest bond in interacting with AKR1C1, namely beta vetispirene compound with an S-score of -15.12 kcal/mol, and stronger than native ligand and aspirin. Based on the research data, it can be concluded that the beta vetispirene compound in vetiver can be a potential co-chemotherapy agent for lung cancer in targeting AKR1C1 selectively. Abstract

Meropenem is a broad-spectrum beta-lactam antibiotic widely used in intensive care units (ICUs) for severe bacterial infections. Therapeutic drug monitoring (TDM) is essential to optimize its dosing, ensuring effective bacterial eradication while minimizing toxicity and resistance.This study aimed to determine meropenem concentrations in human plasma using LC-MS/MS and evaluate its application in TDM for ICU patients. Meropenem concentrations in plasma samples from ICU patients were analysed using a validated liquid chromatography-tandem mass spectrometry (LC-MS/MS) method. The study included 40 plasma samples from 20 patients receiving meropenem continuous infusion. Validation followed ICH M10 (2022) guidelines, assessing specificity, accuracy, precision, stability, and dilution integrity. The developed LC-MS/MS method demonstrated high selectivity, sensitivity, and linearity (r = 0.9930.996) over the 102000 ng/mL range. Accuracy and precision met ICH M10 acceptance criteria, with %CV 15%. All ICU patients maintained %fTMinimum Inhibitory Concentration (MIC) 40%, ensuring adequate bacterial eradication. Notably, patients with renal impairment required dose adjustments, while those with high creatinine clearance needed increased dosing. The validated LC-MS/MS method is suitable for meropenem TDM in ICU patients, allowing individualized dosing adjustments to optimize therapy.
