Abstract

Mastoidectomy with tympanoplasty commonly requires a postauricular incision and may be complicated by retroauricular wound dehiscence, particularly in the presence of infection, tissue tension, or impaired local perfusion. Persistent wound dehiscence can delay healing, compromise cosmetic outcomes, and necessitate secondary reconstructive procedures. Local advancement flaps and skin grafting are among the reconstructive options described. The purpose to report the clinical outcome of post-mastoidectomy retroauricular wound dehiscence managed using an advancement flap and review supporting evidence for this technique. Case report: A 52-year-old man with chronic suppurative otitis media with cholesteatoma underwent canal wall up mastoidectomy with tympanoplasty. During postoperative follow-up, retroauricular wound dehiscence complicated by local infection was identified. Surgical reconstruction was performed using a retroauricular advancement flap under general anesthesia. Postoperative follow-up demonstrated satisfactory wound closure, progressive epithelialization, and acceptable cosmetic outcomes, with no recurrence or major complications. Clinical question: Does advancement flap reconstruction offer better outcomes compared to skin grafting in post-mastoidectomy retroauricular wound dehiscence? Method: A literature search was conducted in PubMed, ScienceDirect, and Google Scholar to identify relevant studies on the management of post-mastoidectomy retroauricular wound dehiscence using advancement flaps or skin grafts. Seven relevant articles were identified. Case reports and small case series demonstrated high primary wound closure rates and low recurrence following advancement flap reconstruction. Skin grafting facilitated epithelialization in selected cases but showed variable outcomes in compromised wound beds. Advancement flap reconstruction is an effective and reliable option for managing retroauricular wound dehiscence following mastoidectomy, particularly in chronic or infected wounds.

Abstract

The implementation of trimatra art learning in Early Childhood Education (ECE) has significant benefits for supporting child development, including stimulating motor skills, enhancing focus, and optimizing creativity. However, despite its advantages, the implementation of art education also encounters challenges that impact the achievement of learning objectives. This study explores the effectiveness of Project-Based Learning (PBL) methods in trimatra art for young children in Groups A and B at KB Inklusi Talenta, Kudus. This research employs a descriptive qualitative methodology, with a sample consisting of 20 children and 5 teachers from diverse backgrounds and varying special conditions. Data were collected through participatory observation, interviews with teachers, and documentation. The focus of the study is on how this method can be adapted to meet the needs of children with differing abilities and how it influences their engagement and learning outcomes. Challenges encountered include classroom management, project planning, and limited resources. The aim of this study is to identify the inhibiting factors in the implementation of project-based art learning and to provide recommendations for addressing these issues, facilitating broader adoption in inclusive ECE settings. Project-based learning in trimatra art activities has been shown to enhance children’s engagement and provide more meaningful learning experiences, making it an appealing approach in the context of inclusive education.

Abstract

Abstract

Background: Primary acquired cholesteatoma is a benign yet locally destructive middle ear lesion with an incompletely understood pathogenesis. While Eustachian tube dysfunction is commonly implicated, it does not explain cases with normal middle ear pressure. Selective epitympanic dysventilation syndrome (SEDS) has been proposed as an alternative mechanism involving localized ventilation impairment within the epitympanum. This study aims to provide a clearer understanding of the clinical features of cholesteatoma associated with SEDS, which may help clarify its etiopathogenesis, support earlier detection, limit disease progression, and guide more appropriate, individualized clinical decision-making. Case presentation A 55-year-old female presented with one year of persistent, malodorous left otorrhea and progressive hearing loss over three months. Otoscopy revealed mucopurulent discharge with an attic perforation. Nasoendoscopy showed a patent Eustachian tube. Audiometry demonstrated mild conductive hearing loss (31.25 dB), and tympanometry showed a Type A curve, indicating normal middle ear pressure. Mastoid MSCT revealed opacification of mastoid air cells and antrum with intact ossicles and no mesotympanic involvement, suggesting a localized ventilation disorder consistent with SEDS. Conclusion: SEDS is an important and underrecognized mechanism in primary acquired cholesteatoma. Its recognition is essential for early diagnosis and for guiding surgical strategies that restore epitympanic ventilation to prevent recurrence.
