Abstract

WHO memperkirakan sekitar 40% kematian ibu di negara berkembang berkaitan dengan anemia dalam kehamilan. Deteksi kasus anemia pada kehamilan belum dilakukan sesuai dengan standar pengelolaan anemia pada kehamilan. Maka dilakukan analisis faktor- faktor yang mempengaruhi kinerja bidan dalam pengelolaan anemia pada kehamilan di Kota Singkawang.Jenis penelitian survei analitik dengan pendekatan Retrospektif. Jumlah sampel 52 bidan Puskesmas wilayah Kota Singkawang dipilih secara proportional stratified random sampling dengan kriteria inklusi. Pengumpulan data dengan kuesioner terstruktur dan observasi langsung. Data dianalisis dengan uji korelasi Spearman Rank dan Regresi Logistik.Hasil penelitian mayoritas responden berusia antara 20-35 tahun (63,5%), pendidikan DIII/DIV/S1 Kebidanan (90,4%), masa kerja ≥10 tahun (55,8%). Kinerja bidan mempunyai kategori baik (59,6%), pengetahuan baik (51,9%), sarana prasarana lengkap (53,8%), kepemimpinan baik (63,5%), motivasi baik (55,8%), komunikasi baik (53,8%). Semua variabel bebas (pengetahuan, sarana prasarana, kepemimpinan, motivasi dan komunikasi) hubungan dengan kinerja bidan dalam pengelolaan anemia pada kehamilan (p < 0,05). Variabel yang berpengaruh terhadap kinerja bidan dalam pengelolaan anemia pada kehamilan adalah pengetahuan (p=0,002) dan kepemimpinan (p=0,006). Disarankan untuk Dinas Kesehatan dan Puskesmas memfasilitasi bidan dalam meningkatkan pengetahuan dan ketrampilan melalui pendidikan formal dan pelatihan-pelatihan, meningkatkan supervisi dan rapat koordinasi, menyediakan sarana prasarana pendukung dalam pengelolaan anemia. World Health Organization estimated that about 40% maternal mortality in developing countries was related to anemia during pregnancy. Detection of anemia cases during pregnancy was not conducted according to standard of management for anemia during pregnancy. Therefore, analysis factors affecting work performance of midwives in the management of anemia during pregnancy in Singkawang city. This was an analytical survey with retrospective approach. The number of samples was 52 midwives of primary healthcare centers in Singkawang city. They were selected using proportional stratified random sampling method, and they fulfilled inclusion criteria. Data were collected using structured questionnaire and direct observation. Spearman Rank correlation test and logistic regression were applied in the data analysis. Results of the study showed that majority of respondents’ age was 20-35 years old (63.5%), highest level of education was DIII/DIV/S1 midwifery (90.4%), work period was 10 years or more (55.8%). Work performance of midwives was in a good category (59.6%), respondents’ knowledge were good (51.9%), facilities were complete (53.8%), leadership was good (63.5%), motivation was good (55.8%), communication was good (53.8%). All independent variables (knowledge, facilities, leadership, motivation, and communication) were associated with work performance of midwives in the management of anemia during pregnancy (p< 0.05). Variables affecting the work performance of midwives in the management of anemia during pregnancy were knowledge (p= 0.002) and leadership (p= 0.006). Suggestions for district health office and primary healthcare centers were to facilitate midwives to improve their knowledge and skills through formal education and trainings, to improve supervision and coordination meetings, to provide supporting facilities in the management of anemia.

Abstract

Penerapan 10  langkah menuju keberhasilan menyusui (LMKM) merupakan upaya pemerintah untuk meningkatkan program  ASI eksklusif. Di lingkungan   RSAL dr. Ramelan Surabaya sudah tertempel  tentang 10 LMKM namun belum ada kebijakan khusus tentang ASI eksklusif sehingga  bidan masih memberikan  susu formula (PASI) pada bayi baru lahir rawat gabung . Secara umum penelitian ini bertujuan untuk mengevaluasi pelaksanaan 10 langkah menuju keberhasilan menyusi (LMKM) oleh bidan di RSAL dr. Ramelan Surabaya    Metode penelitian yang digunakan adalah kualitatif yang disajikan secara  deskriptif eksploratif. Informan utama adalah 7 bidan pelaksana di poli hamil, kamar bersalin dan ruang rawat gabung, sedangkan  3 bidan kepala ruangan dan 1 bidan supervisor, serta 3 pasien sebagai informan triangulasi. Pengumpulan data dilakukan melalui wawancara mendalam dan studi dokumentasi Analisis data hasil wawancara dengan menggunakan metode interactive model melalui beberapa tahapan seperti: data collection,  dan data reduction,  data display Conclusion drawingHasil penelitian menunjukkan bahwa pelaksanaan 10 LMKM belum terlaksana dengan baik antara lain : belum semua bidan memberikan penjelasan tentang manfaat dan tehnik menyusui yang benar, bidan masih memberikan susu formula pada bayi rawat gabung, belum terbentuk kelompok pendukung ASI (KP-ASI). Hal tersebut disebabkan masih sebagian kecil bidan yang sudah mengikuti pelatihan manajemen laktasi, sehingga motivasi diri kurangPerlu adanya kebijakan tertulis tentang pemberian ASI Eksklusif, serta diharapkan semua bidan dapat mengikuti pelatihan manajemen laktasi sehingga pelaksanaan 10 langkah menuju keberhaslan menyusui (LMKM) lebih optimal, dan perlu juga adanya kebijakan yang tegas tentang keberadaan susu formula Establishment of the 10 steps to be success in breastfeeding (LMKM) was a government effort to improve exclusive breastfeeding program. Ten LMKM information had been displayed in dr. Ramelan navy hospital (RSAL) Surabaya; however, there was no special policy regarding exclusive breastfeeding. Therefore, midwives were still giving formula milk (PASI) to infants in the integrated care room. In general, the objective of this study was to evaluate the implementation of 10 steps to be success in breastfeeding (LMKM) by midwives in RSAL dr. Ramelan Surabaya.  This was a qualitative study with descriptive explorative approach. Main informants were 7 midwives in the pregnancy policlinic, delivery room, and integrated care room. Triangulation informants were 3 midwives as chief of the room, one supervisor midwives, and 3 patients. Data were collected through in-depth interview and documentation study. Interactive model was applied in the data analysis. Steps on the interactive model included data collection, data reduction, data display, and conclusion drawing.Results of the study showed that ten LMKM had not been implemented properly. Not all midwives explained correctly breastfeeding benefits and breastfeeding technique; midwives still gave formula milk to infants in the integrated care room; breastfeeding support group (KP-ASI) had not been formed yet. Only a small portion of midwives who had attended lactation management training; and it caused an insufficient self-motivation among midwives.  Written policy regarding exclusive breastfeeding is needed. All midwives are expected to attend in lactation management training; it will optimize the implementation of 10 LMKM. Strong policy regarding the existence of formula milk is needed.

Abstract

 Breastfeeding improvement (PPASI) program was an effort to increase the coverage of exclusive breastfeeding. Successfulness of the exclusive breastfeeding required support from midwives. Indonesian Midwifery Association (IBI), as a midwives professional organization, had declared to implement PPASI program step by step and continually by all IBI board members. Although IBI boards at Pontianak branch had conducted several seminar activities regarding breastfeeding and lactation management training for IBI members but private practice midwives were still giving formula milk to the newborns. Based on that problem, this study was conducted with the objective to explain managerial role of IBI boards in implementing PPASI program in the area of Pontianak city. This was a qualitative study using phenomenology approach. Study informants were IBI branch and sub branch board members in the area of Pontianak city. Data were collected by in-depth interview and analyzed using content analysis method. Results of the study showed the roles of IBI boards as leader in implementing PPASI program in the motivational activities. IBI boards reminded IBI members to do lactation management in every informal small group social gathering (arisan). Evaluation of lactation management was conducted by IBI boards because they were on duty as midwives coordinators at Puskesmas and as room chiefs in hospitals. Guidance to the members was conducted by the boards through ‘arisan’ and there was no rewards given to members who had implemented lactation management. The role of boards as information resource was done by conducting lactation seminars for IBI members and in collaboration with district health office to implement lactation management training. There was no special standard for lactation management and socialization was only given to the members who wanted to apply for private practice midwives (BPS) permit. In term of IBI board role as policy makers, the board had not made planning for PPASI program. Planning and organizing were only done for seminar activities about breastfeeding. No written regulation issued by IBI board and no sanction was assigned to the members in implementing PPASI program. Based on the study results, it was suggested to IBI board to formulate evaluation format and to give rewards to IBI members in order to motivate them to implement lactation management, to make specific standard operating procedure for lactation management and to make understandable and details regulations regarding PPASI program implementation, to assign sanction to members who broke the rules.

Abstract

AbstrakDi Kabupaten Grobogan, kejadian penyakit diare, campak, frambusia, dan difteri cukup tinggi. Kejadian penyakit – penyakit ini merupakan indikator PHBS masyarakat. STBM bertujuan untuk menurunkan penyakit – penyakit tersebut, namun pelaksanaan program STBM dengan indikator buang air besar sembarangan, cuci tangan pakai sabun, pengolahan air bersih, pengolahan sampah dan pengolahan limbah cair masih rendah. Penelitian ini bertujuan untuk menggambarkan pelaksanaan program STBM di Kabupaten Grobogan.Jenis penelitian adalah penelitian kualitatif dengan pendekatan fenomena. Pengumpulan data dilakukan dengan wawancara mendalam kepada 8 informan utama petugas promkes dan kesling, Focus Group Discussion dengan informan triangulasi yaitu kader, tokoh agama, tokoh masyarakat dan anggota masyarakat. Analisis data dengan content analysis.Hasil penelitian menunjukkan bahwa : sebagian besar petugas melaksanakan program STBM dengan baik. Hal yang masih kurang baik adalah kegiatan monitoring. Sebagian besar pengetahuan petugas tentang pelaksanaan program STBM sudah baik. Belum semua petugas bersikap baik dalam melaksanakan program STBM. Sebagian besar petugas belum mengetahui peraturan dalam melaksanakan program STBM tetapi kader sudah mengetahui. Semua petugas telah mendapatkan pelatihan program STBM. Semua kepala puskesmas telah melaksanakan pengawasan rutin. Sarana dan prasarana yang berupa alat pembuat jamban, sarana cuci tangan, alat pengolah sampah, lembar balik.Disarankan Dinas Kesehatan untuk meningkatkan kerjasama lintas program, lintas sektoral dalam pelaksanaan program STBM. Selain itu juga meningkatkan peran kader dalam pelaksanaan program STBM. AbstractIn Grobogan district, the occurrence of diarrhea, morbilli, framboesia, and diphtheria was relatively high. The presence of these diseases was an indicator of hygienic and healthy live behavior (PHBS) of a community. Objective of community based total sanitation (STBM) was to reduce those diseases. However, the implementation of STBM program was still inadequate. Indicators of STBM program were improper defecation, washing hand using soap, clean water management, waste management, and liquid waste management. Objective of this study was to describe the implementation of STBM program in Grobogan district.This was a qualitative study with phenomenon approach. Data collection was done by conducting in-depth interview to 8 main informants: health promotion and environmental health workers. Focus group discussion was conducted to triangulation informants; triangulation informants consisted of cadres, local religious leaders, local community leaders, and local people. Data were analyzed using content analysis method.Results of the study showed that the majority of workers implemented STBM program well. A thing that was still insufficient or inadequate was monitoring. The majority of worker’s knowledge regarding the implementation of STBM program was sufficient. Not all workers had good attitude in the implementation of STBM program. The majority of workers did not know regulations in implementing STBM program; however, cadres knew the regulation. All workers had received STBM program training. All heads of primary healthcare centers (puskesmas) had conducted routine supervision. Facilities were in the form of water closet maker instruments, hand washing facility, waste management instruments, and flip charts.Suggestions for District Health Office were to improve cross program and sector collaborations in the implementation of STBM program, to increase the role of cadres in the implementation of STBM program.

Abstract

Pelayanan antenatal adalah memberikan pelayanan kesehatan bagi ibu hamil dan janinnya sesuai standar minimal 7T serta minimal empat kali pemeriksaan selama kehamilan. Standar waktu tersebut untuk menjamin perlindungan kepada ibu hamil, berupa deteksi dini faktor risiko, pencegahan dan penanganan komplikasi. Berdasarkan cakupan Pemantauan Wilayah Setempat Kesehatan Ibu dan Anak (PWS-KIA), angka kematian ibu dan bayi serta survei awal terhadap bidan desa disimpulkan bahwa masih rendahnya kineja bidan desa.Tujuan penelitian adalah mengetahui faktor apa saja yang mempengaruhi kinerja bidan desa dalam deteksi dini resiko tinggi ibu hamil pada pelayanan antenatal. Jenis penelitian bersifat explanatory research dengan pendekatan cross sectional. Pengumpulan data menggunakan kuesioner terstruktur danobservasi. Populasi penelitian adalah bidan desa, dengan sampel 93 responden diambil denganteknik simple random sampling. Hasil penelitian menunjukkan 73,1% bidan desa pada rentangumur 25-35 tahun dan 37,6% rentang masa kerja 36-60 bulan. Analisis bivariat dengan tabulasisilang dan uji Pearson Product Moment menunjukkan bahwa variabel yang berhubungan dengankinerja yaitu pengetahuan (ρ=0,000), motivasi (ρ=0,004), persepsi supervisi bidan koordinator(ρ=0,016), persepsi beban kerja (ρ=0,047). Analisis multivariat dengan uji regresi logistikberganda menunjukkan adanya pengaruh bersama-sama variabel pengetahuan dan saranaprasarana dengan nilai signifikansi 0,000 dan R2 0,394, berarti 39,4% variabel kinerja dapatdijelaskan oleh variasi dari kedua variabel tersebut. Dinas Kesehatan Kabupaten diharapkanmemberi pelatihan, melengkapi sarana prasarana bidan desa sebelum bekerja di desa,membentuk tim atau panitia sebagai wadah konsultasi. Puskesmas agar melengkapi saranapendokumentasian, kepala puskesmas dan bidan koordinator aktif mengadakan pembinaan. Antenatal care provided health service for pregnant women and their fetuses according to theminimum standard of 7T, and minimum of four examinations during pregnancy. These standards were intended to assure protection to pregnant women, such as risk factors early detection,complication prevention and management. Based on the coverage of maternal and children local area monitoring (PWS-KIA), maternal and infant mortality rates, and preliminary survey to village midwives, it was concluded that work performance of village midwives was still inadequate. Objective of this study was to identify factors affecting work performance of village midwives in conducting early detection of high risk pregnancy in the antenatal care. This was an explanatory research with cross sectional approach. Data were collected using structuredquestionnaire and observation. Study population was village midwives. Samples consisted of 93respondents who were selected using simple random sampling technique. Results of the studyshowed that 73.1% of village midwives’ age were in the range of 25-35 years old, and 37.6% of village midwifes duration of work were in the range of 36-60 months. Bivariate analysis withcross tabulation and Pearson Product Moment test indicated that variables related to workperformance were knowledge (p= 0.000), motivation (p= 0.004), perception on supervision ofcoordinator midwives (p= 0.016), and perception on workload (p= 0.047). Multivariate analysisusing multivariate logistic regression test indicated common effect of knowledge and facilitiesvariables with significance value of 0.000 and r 2 = 0.394; it meant that 39.4% of workperformance variable could be explained by variation of those two variables. District healthoffice is expected to provide training, to complete facilities for village midwives before startingto work in the village; to form team or committee as a consultation place. Puskesmas is expected to complete documentation facilities; head of puskesmas and coordinator midwives are advised to actively doing supervision.

Abstract

The high MMR of the government issued a Decree of the Minister of Health of the Republic of Indonesia concerning the MCH Handbook, stating that the MCH Handbook is a tool for early detection of disorders or health problems for mothers and children, so that when the MCH handbook is not utilized properly it will be difficult to detect early on mothers and child. The purpose of this study was to determine factors: perception of supervision and availability of MCH books influenced the use of MCH books. This type of observational quantitative research is cross sectional. The population of all village midwives is 124 people who work in 16 puskesmas working areas in Kolaka Regency. Sampling was done by simple rondom sampling method, obtained as many as 55 people who met the inclusion and exclusion criteria. The analysis uses moment or sperman’s rho product analysis, and multivariate analysis with linear regression test. Results: perception perception variables with Pearson's R. correlation test obtained r = 0.244 and p = 0.73 where the value of p> 0.05 means that there is no significant relationship between perception of supervision with the use of the MCH handbook. Variable availability of MCH books with the Spearman correlation test, s Rho obtained r = 0.157 and p = 0.25 where p> 0.05, which means there is no significant relationship between the availability of MCH books with the use of MCH books. Conclusion: the study found that the perception of supervision and provision of MCH books did not significantly influence the use of MCH books in antenatal services by village midwives in Kolaka District. Suggestions Kolaka District Health Office needs to increase the role of periodic supervision and procurement of MCH books in accordance with the target number and availability of MCH books at health service sites.

Abstract

Pelayanan petugas gzi pada pasien rawat inap kelas III di RSUD Kajen Kabupaten belum sesuai harapan karena masih ada ketidakpuasan pasien/ keluarga pasien dalam hal pemberian makan yang tidak tepat waktu, tampilan makanan tidak menarik dan pelayanan petugas gizi yang kurang peduli, kurang tanggap dan kurang ramah. Penelitian ini bertujuan untuk mengetahui hubungan persepsi tentang mutu pelayanan petugas gizi dengan kepuasan pasien dalam pelayanan gizi di rawat inap kelas III. Jenis penelitian adalah survei analitik dengan pendekatan cross sectional. Pengumpulan data dilakukan dengan wawancara terstruktur. Variabel bebas adalah persepsi kehandalan, ketanggapan, jaminan, empati dan bukti langsung, variabel terikat adalah kepuasan pasien dalam pelayanan gizi. Jumlah sampel sebanyak 135 subjek diperoleh dengan teknik consecutive sampling. Analisis data menggunakan analisis korelasi Spearman Rank untuk analisis bivariat dan regresi linier ganda untuk analisis multivariat. Hasil penelitian menunjukan 59,3% persepsi kehandalan, baik, 61,5% ketanggapan, baik, 56,3% jaminan,, baik, 50,4% empati baik dan 65,2 % bukti langsung, baik serta 58,5% persepsi kepuasan, puas terhadap pelayanan gizi. Hasil analisis bivariat menunjukan bahwa ada hubungan yang signifikan antara kehandalan, ketanggapan, jaminan, empati dan bukti langsung terhadap kepuasan pasien dalam pelayanan gizi (p < 0,05 ). Hasil analisis mulitvariat menunjukan faktor yang berpengaruh paling kuat terhadap kepuasan pasien adalah persepsi bukti langsung (B=0,230). Disarankan kepada manajemen rumah sakit untuk meningkatkan pembinaan, pelatihan custumer service pada petugas gizi Nutritionist services to third-class inpatients in Kajen district general hospital (RSUD) of Pekalongan district were not according to what was expected. This was cause by unsatisfaction of patients or their family members regarding: meals that were not served in time; meal presentation that was not interesting; and nutritional staffs who did not care, were not responsive, and were unpolite. Objective of this study was to identify association between perception on service quality of nutritional staffs and patient satisfation toward nutritional service in the third-class inpatient rooms.  This was an analytical survey with cross sectional approach. Data were collected through interview guided by structured questionnaire. Independent variables were perception on validity, responsiveness, assurance, emphaty, and direct evidences. Dependent variable was patient satisfactory toward nutritional services. The number of study samples was 135 subjects selected using consecutive sampling. Data analysis was done by applying Spearman Rank correlation for bivariate analysis, and by applying multiple linier regression for multivariate analysis. Results of the study showed that 59.3% of respondent’s perception on validity were good; 61.5% of respondent’s perception on responsiveness were good; 56.3% respondent’s perception on assurance were good; 50.4% respondent’s perception on empathy were good, 65.2% of respondent’s perception on direct evidence were good; and 58.5% of respondent’s satisfaction were satisfactory toward nutritional service. Results of bivariate analysis showed significant associations between patient satisfaction toward nurtritional service and validity, responsiveness, assurance, empathy, and direct evidence (p< 0.05). Result of multivariate analysis showed that the most influencing factor toward patient satisfaction was perception on the direct evidence (B= 0.230). Hospital management is suggested to improve customer service assistance and training to nutritional staffs.

Abstract

AbstrakImplementasi P4K bisa berjalan apabila bidan pelaksana dapat memberdayakan masyarakat untuk melaksanakan P4K. Namun Implementasi P4K di Kota Kediri khususnya dalam pemberdayaan masyarakat masih dibawah target yang ditetapkan oleh Dinas Kesehatan Kota Kediri. Tujuan dari penelitian ini adalah menjelaskan implementasi kegiatan pemberdayaan masyarakat oleh bidan untuk mendukung P4K di Puskesmas Kota Kediri tahun 2011.Penelitian ini dilakukan pada bulan Mei sampai Juli 2011, dilakukan di seluruh Puskesmas Kota Kediri. Jenis penelitian ini adalah deskriptif yang bersifat kualitatif dengan pendekatan cross sectional. Data yang digunakan adalah data primer dengan menggunakan teknik indepth interview serta menggunakan data sekunder. Analisis data dalam penelitian ini menggunakan content analysis (analisis isi).Hasil penelitian menujukkan bahwa kegiatan pemberdayaan masyarakat yang tidak berjalan adalah tabulin dikarenakan banyaknya jaminan kesehatan untuk masyarakat, ukuran dasar dan tujuan kebijakan yang tidak jelas. Meskipun sumber daya manusia sudah sesuai namun tidak ada dana dan fasilitas khusus walaupun sudah ada bantuan dari gerdu sehati. Selaian itu manajeman kontrol yang tidak dijalankan oleh puskesmas dan pembinaan oleh bidan tanpa memiliki forum khusus menjadikan proses komunikasi antara kader dan masyarakat menjadi kabur. Walaupun sikap bidan positif dalam menjalankan tanggung jawabnya jika tidak ditunjang dengan dukungan dari masyarakat maka tidak akan berjalan.Saran yang dapat diberikan kepada Dinas Kesehatan adalah perlu dibuatnya ukuran dasar dan tujuan kebijakan yang jelas tentang pelaksanaan P4K serta pelatihan bidan dan kader secara berkala tentang pemberdayaan masyarakat. Fungsi kontrol yang baik harus dijalankan oleh Puskesmas serta peningkatan komunikasi yang dilakukan oleh bidan untuk menjalankan kegiatan pemberdayaan masyarakat. AbstractImplementation of delivery planning and complication prevention program (P4K) could run if midwives could empower people to do P4K. However, implementation of P4K in Kediri city particularly in community empowerment was still below the target set by Kediri city health office. The objective of this study was to explain implementation of community empowerment by midwives to support P4K at primary healthcare centers in Kediri city, in 2011. This study was conducted in May – July 2011 in all primary healthcare centers in Kediri city. This was a qualitative study with cross sectional approach. Primary data were collected through in- depth interview technique, and secondary data were also used. Content analysis method was applied in the data analysis. Results of the study showed that tubulin was the community empowerment activity that was not implemented due to many options for health insurance in the community; basic indicator, purpose of the policy were not clear. Although human resources was appropriate, and there was a support from gerdu sehati; however, there was no funding and special facilities. Additionally, control management was not performed by primary healthcare center; supervision by midwife did not provide special forum for communication between cadres and people in the community. Although attitude of midwives was positive in doing their responsibilities but there was no support from the community, as a result the program would not run. Suggestions for district health office are to make clearly basic indicator and policy objective regarding P4K implementation; to conduct routine training for midwives and cadres regarding community empowerment. Primary healthcare center has to implement good control function; to improve communication done by midwives to implement community empowerment activities.

Abstract

Upaya pemantauan cakupan pelayanan KIA yang dikenal dengan Pemantauan Wilayah Setempat Kesehatan Ibu dan Anak (PWS-KIA) merupakan salah satu tugas bidan. Pelaksanaan PWS-KIA dengan baik dapat digunakan untuk melakukan tindak lanjut dan perbaikan pelayanan KIA. Namun berdasarkan data pelaksanaan PWS-KIA tahun 2011 hanya 40% bidan yang melakukan tahapan kegiatan PWS KIA secara lengkap. Penelitian ini bertujuan untuk menganalisis faktor-faktor yang berhubungan dengan kinerja Bidan Desa dalam Pelembagaan PWS-KIA di Wilayah KabupatenMajalengka Provinsi Jawa Barat Tahun 2011. Penelitian ini merupakan penelitian observasional dengan pengumpulan data secara cross sectional. Populasi penelitian ini adalah semua bidan desa yang ada di wilayah kerja Dinas Kesehatan Kabupaten Majalengka, yaitu 360 bidan tersebar di 31 Puskesmas dengan sampel sebanyak 190 secara simple random sampling. Pengumpulan data dilakukan dengan wawancara dan observasi menggunakan kuesioner dan lembar pengamatan serta analisis data dilakukan secara univariat, bivariat, dan multivariat. Dari hasil penelitian didapatkan bahwa kurang dari setengahnya (42,1%) Kinerja Bidan Desa dalam pelembagaan PWS-KIA kurang baik. Dari hasil analisis didapatkan variabel yang berhubungan dengan kinerja bidan dalam pelembagaan PWS-KIA di wilayah Kabupaten Majalengka Tahun 2011 adalah pengetahuan, ketersediaan sarana dan fasilitas PWS-KIA, pembinaan Kepala Puskesmas, dukungan Kepala Desa (nilai p < 0,05). Secara bersama-sama variabel yang berpengaruh paling besar adalah dukungan/ketersediaan sarana dan fasilitas PWS KIA dengan nilai exp (â)=9,784 (nilai p =0,000). Disarankan kepada Dinas Kesehatan Kab. Majalengka/Puskesmas, dalam rangka meningkatkan kinerja bidan desa dalam pelembagaan PWS-KIA adalah memberikan dukungan sarana dan fasilitas PWS-KIA berupa penyediaan buku pedoman PWS-KIA, alat tulis/perlengkapan kantor, peralatan komputer; pelatihan pengelolaan data PWS-KIA berbasis komputer; pembekalan bidan desa pada awal tugas; dan supervisi oleh Kepala Puskesmas kepada bidan desa; serta koordinasi kepada Kepala Desa maupun kepada Camat dalam rangka dalam pelembagaan PWS-KIA. Monitoring effort on maternal and child health (KIA) service coverage known as local area monitoring of maternal and child health (PWS-KIA) was one of midwives duties. Good implementation of PWS-KIA could be used to follow up and improve KIA service. However, based on PWS-KIA implementation data in 2011, only 40% midwives who performed complete steps of PWS-KIA activities. Objective of this study was to analyze factors related to work performance of midwives to institutionalize PWS-KIA in Majalengka district area of West Java province, 2011. This was an observational study, and data was collected cross sectionally. Study population was all village midwives who resided in the work area of Majalengka district health office. These 360 midwives were distributed in 31 primary healthcare centers. Samples were 190 midwives. They were selected using simple random sampling method. Data collection was done through interview and observation using questionnair and observation form. Univariate, bivariate, and multivariate analysis were applied in the data analysis. Results of the study showed that work performance of less than a half (42.1%) of village midwives who institutionalized PWS-KIA was inadequate. Variables related to work performance of midwives who institutionalized PWS-KIA in Majalengka district area in 2011 were knowledge, availability of PWS- KIA facility, supervision of the head of puskesmas, and village leader support (p< 0.05). The most influencing variables was support/ the availability of PWS-KIA facilities (exp (â)= 9.784, p= 0.000). Suggestions for Majalengka district health office or primary healthcare centers are to provide PWS-KIA facility support to improve work performance of village midwives who institutionalized PWS-KIA; the support is in the form of providing PWS-KIA guideline books, stationery, computer devices; to conduct training on computer based PWS-KIA management data; to provide training for village midwives in their first duty; to conduct supervision by the head of primary healthcare center to village midwives; .to conduct coordination with village leader and sub district leader for PWS-KIA institutionalization.

Abstract

AbstrakAKI di Kabupaten Pati tahun 2011 meningkat lagi menjadi 24 kasus (116.17/100.000 KH) dengan penyebab langsung kematian ibu masih saja didominasi oleh perdarahan 3 kasus (12.5%), infeksi 2 kasus (8.33%), hipertensi 8 kasus (33.33%), eklamsi 1 kasus. Tujuan penelitian adalah untuk menganalisis beberapa faktor yang mempengaruhi pelaksanaan tindakan pencegahan infeksi pada pertolongan persalinan oleh bidan desa pasca pelatihan asuhan persalinan normal di Kabupaten Pati.Jenis penelitian observasional dengan pendekatan  cross sectional  Populasi adalah seluruh bidandesa pasca pelatihan APN. Jumlah sempel 69 bidan desa pasca pelatihan APN dipilih secara  purposive dengan kriteria inklusi. Pengumpulan data dengan kuesioner terstruktur dan observasi langsung. Data dianalisis secara kuantitatif dengan uji korelasi Chi-square dengan metode Yate Correction dan regresi logistik linear.Responden mempunyai pengetahuan baik (76,2%), responden mempunyai sikap baik (75,0%), responden mempunyai motivasi baik (71,4%), responden mempunyai persepsi terhadap supervisi baik (69,8%), responden mempunyai peralatan lengkap (70,8%), responden melaksanakan prosedur pencegahan infeksi dalam kategori baik (59,4%). Faktor yang mempengaruhi pelaksanaan prosedur pencegahan infeksi adalah pengetahuan (ρ = 0,001), sikap (ρ = 0,0001), motivasi (ρ = 0.004), supervisi (ρ = 0,046), peralatan (ρ = 0,008).Semua variabel (pengetahuan, sikap, motivasi, supervisi, dan peralatan) berpengaruh terhadap pelaksanaan prosedur pencegahan infeksi karena nilai signifikan dibawah 0.05 (p-value < 0.05) namun variabel yang paling berpengaruh adalah sikap (p-value = 0.000, Exp(B) =9.6000)  dan pengetahuan (p-value = 0.001, Exp(B) = 6.400).Kepada Dinas Kesehatan dan organisasi IBI Kabupaten Pati untuk memberikan pembinaan agar lebih meningkatkan ketrampilan dan pengetahuan bidan desa yang terkait dengan tugas dan fungsi bidan lewat pelatihan-pelatihan, supervisi yang positif terutama tentang prosedur pencegahan infeksi pada pertolongan persalinan. AbstractMaternal mortality rate (AKI) in Pati district in 2011 increased to 24 cases (116.17/100000 live- births). Direct causes of maternal death were dominated by hemorrhage (3 cases or 12.5%), infection (2 cases or 8.33%), hypertension (8 cases or 33.33%), and eclamsia (1 case). Therefore, analysis of factors affecting the implementation of infection prevention actions in the delivery assisted by village midwives with post normal-delivery care (APN) training in Pati district was done. This was an observational study with cross sectional approach. The number of samples was 69 village midwives with post APN training. They were selected purposively according to the inclusion criteria. Data were collected through interview guided by structured questionnaire and through direct observation. Data were analyzed quantitatively by applying Chi Square test, Yate Correction, and linear logistic regression. Results of the study showed that respondents had good knowledge (76.2%), good attitude (75.0%), good motivation (71.4%), good perception on supervision (69.8%), complete instruments (70.8%), and good category for the implementation of infection prevention procedure (59.4%). Factors affecting the implementation of infection prevention procedure were knowledge (p= 0.001), attitude (p= 0.0001), motivation (p= 0.004), supervision (p= 0.046), instruments (p= 0.008). All variables (knowledge, attitude, motivation, supervision, and instruments) influenced the implementation of infection prevention procedure (p-value d” 0.05). However, the most influencing variables were attitude (p= 0.000, Exp(B) = 9.60) and knowledge (p= 0.001, Exp(B) = 6.40). Suggestions for district health office and IBI organization of Pati district are to give guidance to improve skills and knowledge of village midwives related to the job and function of midwives; it can be done through trainings, positive supervision mainly on infection prevention procedure in delivery assistance.
