Abstract

Background: Malaria remains a public health challenge in eastern Indonesia, especially in remote areas like Manusa Village, where access to healthcare is limited. The community relies on traditional treatment using eye drops made from Kenar leaves (Kleinhovia hospita L.). The purpose of this study is to explore, from an epidemiological perspective, the use of Kenar leaves as a primary component in the formulation of traditional malaria remedies within the community of Manusa Village is lacking. Methods: This study employed a quantitative descriptive design with a cross-sectional approach. Data were obtained through structured questionnaires and phytochemical analysis of Kenar leaves. A total of 102 respondents voluntarily participated in the study. Results: Among respondents from 102 respondents, 67,6% had experienced malaria, and 68,6% reported using Kenar leaf eye-drops. Use was significantly associated with prior malaria history (POR = 15,057; p < 0,001) and age group 26-45 years (POR = 17,813; p < 0,001). Conversely, respondents with current malaria symptoms were less likely to use the drops (POR = 0,042; p = 0,002). Twice-weekly use was associated with better recovery outcomes (POR = 0,168; p = 0,003). The majority (84,3%) reported mild side effects, such as transient eye irritation. Phytochemical analysis confirmed the presence of alkaloids, flavonoids, steroids, and tannins compounds with known antimalarial properties. Conclusion: This study provides new evidence linking traditional knowledge and community-based practices with malaria self-treatment, highlighting implications for health promotion strategies.

Abstract

Background: Breast milk is an important source of nutrition for the growth and development of infants. It is highly susceptible to the accumulation of lipophilic compounds such as heavy metals. Breast milk can become a route through which infants are exposed to toxic substances like heavy metals. This study aimed to identify variations of trace elements and their associated factors contributing to the existence of trace elements in breast milk. Methods: This research employed a systematic literature review methodology utilizing PubMed, Scopus, and Science Direct databases. The search strategy followed specific English keywords to align with the PRISMA Extension for Systematic Reviews guidelines. Data synthesis involved a narrative approach, summarizing the main findings and interventions outlined in the chosen studies. The included articles consisted of full-text primary research published between 2013 and 2023. To mitigate potential language bias, only publications in English were considered for inclusion. Results: Our systematic review, comprising nine articles, revealed the presence of toxic metals including lead (Pb), cadmium (Cd), aluminum (Al), copper (Cu), chromium (Cr), mercury (Hg), and arsenic in human milk. Associated factors contributing to this presence include food intake (such as seafood, fruit, and canned fruit), water and canned drink consumption, smoking and exposure to smoke, regional location, and medical conditions. Conclusion: Heavy metals, including lead, cadmium, aluminum, copper, chromium, mercury, and arsenic are present in breast milk, influenced by factors like diet, water consumption, smoking, regional zones, and medical conditions. These findings underscore the urgent need for public health policies to monitor environmental exposure in lactating mothers and to develop dietary guidelines minimizing toxic exposure.

Abstract

Background: Banyumas Regency had the second-highest number of TBC cases in Central Java Province in 2023, totaling 7,060 cases. The incidence of TBC increased by approximately 27.27% from 2022 to 2023. Despite various TB control efforts, conventional epidemiological methods often fail to capture the spatial patterns of disease transmission, making it challenging to identify localized hotspots and effectively allocate resources. Purpose: This study aimed to analyze the spatial relationship between HIV prevalence, population density, healthy living behaviors, and housing conditions in relation to TBC incidence. Additionally, it sought to identify tuberculosis hotspots in Banyumas Regency. Methods: The data were obtained from the Banyumas Tuberculosis System Information in August 2024. Total of 4,853 registered patients who sought treatment and resided in Banyumas Regency in 2023 was assessed. TBC distribution patterns were analyzed using the spatial autocorrelation method through the Moran Index and Local Indicators of Spatial Associations (LISA). Results: The analysis revealed significant clustering of TBC incidence with HIV prevalence, clean and healthy living behavior, and population density. The significant dispersion was noted with healthy houses. Notably, six 27 subdistricts—Kembaran, North Purwokerto, South Purwokerto, East Purwokerto, Sokaraja, and Sumbang—demonstrated significant local spatial autocorrelation. Conclusion: The study found TBC incidence was clustered in areas with high HIV prevalence, poor living conditions, and high population density, while healthy houses showed a dispersed pattern, highlighting the importance of prioritizing interventions, policy changes, and resource allocation.
