Abstract

Background:  Early childhood development is a critical period in children's lives. The develop­ment index in early childhood in Indonesia is rela­­­tive­ly low compared to other developing coun­­­tries, which is 88.3%. Various factors that can influence early childhood development were the nutritional status of children, stimulation by pa­rents, exclusive breastfeeding, dietary patterns, and parental characteristics. This study aimed to analy­ze the contextual influence of early child­hood education on child development in Tanjung Jabung Timur, Jambi.Subjects and Method: This was a cross sec­tional study conducted at 25 early childhood schools in Tanjung Jabung Timur, Jambi, from Sep­tember to October 2019. A sample of 200 child­ren aged 3-6 years old was selected by stra­tified random sampling. The dependent variable was child development. The independent variables were nutritional status, dietary pattern, ex­clu­sive breastfeeding, maternal education, mater­nal occupation, and family income. Child deve­lop­ment was measured by The Indonesian child deve­lopment prescreening questionnaire. The other data were collected by questionnaire. The data were analyzed by a multilevel multiple logis­tic regression run on Stata 13.Results: Early childhood development increased with good nutritional status (b= 1.88; 95% CI= 0.48 to 3.28; p= 0.008), good dietary pattern (b= 1.58; 95% CI= 0.26 to 2.91; p= 0.019), exclusive breastfeeding (b= 0.64; 95% CI= -0.69 to 1.99; p= 0.346), maternal education ≥High school (b=  2.45; 95% CI= 0.97 to 3.92; p= 0.001), and family income ≥Rp 2,840,000 (b= 1.93; 95% CI= 0.32 to 0.53; p= 0.018). Early childhood development decreased with maternal working outside the house (b= -1.42; 95% CI= -2.66 to -0.17; p= 0.025). Early childhood school had low effect on child development with intra-class correlation= 10.09%.Conclusion: Early childhood development was influenced by nutritional status, dietary patterns, ex­clusive breastfeeding, maternal education, maternal employment, and family income. Abstract

Background:The development of children in the first 1000 days is influential in the long term. Therefore, it requires education at an early age to achieve optimal development. This study aimed to analyze the contextual effect of early childhood education on the development of children under five at Surabaya.Subjects and Method:This was a cross-sectional study conducted 25 preschools in Wonokromo, Surabaya, East Java, Indonesia. The study population was all children aged 2-5 years. A sample of 200 children aged 2-5 years from 25 preschools was selected randomly. The dependent variable was child development. The independent variables were the history of infectious disease, exclusive breastfeeding, number of children, maternal education, maternal occupation, family income, nutritional status, mother tongue, birth weight, and preschool level. Abstract

Background: One indicator of a country's progress is the infant mortality rate. Infant mortality is affected by many factors, both exogenous and endogenous. This study aimed to determine the factors causing significant infant mortality to reduce infant mortality in Karanganyar Regency.Subjects and Method: This study was an observational study with a cross-sectional design. This study was conducted in Karanganyar Regency from February to May 2020. The sample was 200 study subjects. The sample was selected by fixed disease sampling. The criteria were babies aged 1 month to fewer than 2 years. The variables observed for the effect were infant mortality, clean and healthy life behavior, exclusive breastfeeding, parental income, maternal education, infant nutritional intake, immunization status, birth weight of infants, illness history of infants, health services, cleanliness of the home environment. The data were collected by a set of questionnaires and analyzed using path analysis with Stata version 13.Results: The risk of infant mortality increased by the cleanliness of the home environment (b=3.84; 95%CI=1.76 to 5.92; p<0.001), infant nutrition intake (b=4.07; 95 CI=2.25 to 5.89; p<0.001), illness history of infants (b=3.03; 95%CI=1.34 to 4.72; p<0.001). The risk of infant mortality decreased by clean and healthy life behavior (b=-5.34; 95%CI=-7.75 to -2.94; p<0.001). Infant mortality was affected indirectly by exclusive breastfeeding, parental income, maternal education, birth weight of infants, immunization status, and health care facilities.Conclusion: Infant mortality is directly affected by the cleanliness of the home environment, clean and healthy life behavior, nutritional intake of infants, and illness history of infants. Abstract

Background: Accurate early detection of bacteriemia plays an important role in the treatment and prognosis of infected neonates, but is constrained by the limitations of spe­cific sepsis markers for detecting bacteriemia. NLCR or neutrophil to lymphocyte count ratio in adult population shows a more sen­sitive parameter in predicting infection and has the advantage of being economical and widely available. The role of NLCR in the neonatal population has not been widely studied. The purpose of this study was to analyze NLCR versus CRP as a predictor of neonatal sepsis.Subjects and Method: This study take ana­lytic observatinal with diagnostic test approach to subject with sepsis risk factor and neonatal sepsis at dr. Moewardi hospital from April until May 2017. Characteristics data as gestational age, gender, birth weight, IT ratio, leucocyte count, neutrophil and lym­phocyte count, NLCR, CRP and blood culture. Data presented descriptively and statistical analysis was performed.Results: No significant correlation between subject’s characteristics and neonatal sepsis. NLCR with cutoff point for 2.22 has sensi­ti­vity, specificity, positive and negative pre­dictive value for 87.5%, 75%, 70% and 90%, respec­ti­vely. CRP with cutoff point for 0.25 has sensi­tivity, specificity, positive and nega­tive predic­tive value for 83.3%, 55.6%, 55.6% and 83.3% respectively. While IT-rasio has sensitivity, spe­cificity, positive and negative predictive value for 29.2%, 91.7%, 70% and 66%, respectively.Conclusion: NLCR has a satisfactory pre­dictive value for neonatal sepsis that widely available and economic predictor alternative with cutoff point for 2.22.Keywords: NLCR, CRP, predictor, neonatal sepsisCorrespondence: Irfan Dzakir Nugroho. Abstract

Latar belakang. Hipertensi arteri pulmonal (HAP) merupakan komplikasi yang sering terjadi penyakit jantung bawaan (PJB). Peningkatan tekanan vaskular paru disebabkan oleh disfungsi sel endotel paru yang ditandai oleh inflamasi perivaskular. Rasio neutrofil limfosit (RNL) merupakan salah satu penanda biologis inflamasi yang murah dan mudah dan berhubungan dengan penyakit jantung.Tujuan. Untuk mengetahui hubungan RNL dengan HAP pada anak dengan PJB asianotik.Metode. Penelitian potong lintang pada pasien anak usia 1 bulan – 18 tahun dengan penyakit jantung bawaan sianotik yang dirawat dan/atau poliklinik anak RSUD dr. Moewardi Surakarta antara Januari 2018 – Januari 2019. Diagnosis HAP berdasarkan pemeriksaan ekokardiografi Doppler. Semua subyek dilakukan pemeriksaan darah rutin lengkap. Analisis data dengan uji t tidak berpasangan, uji Mann Whitney dan uji chi square/fisher exact test. Hasil. Tiga puluh pasien PJB asianotik (aliran sistemik ke paru) terdiri atas 17 anak laki-laki, 13 anak perempuan. Hipertensi arteri pulmonal terjadi pada 17 pasien dan tidakHAP sebanyak 13 pasien. Defek PJB paling banyak defek septum atrium (DSA) dan defek septum ventrikel (DSV). Rasio neutrofil limfosit pada HAP lebih tinggi daripada tidak HAP pada anak dengan PJB asianotik (3,56±1,07 vs 2,04±0,34; p<0,001). Nilai cut off point RNL sebesar 2,355 dengan nilai AUC sebesar 0,901 dengan sensitivitas 84,2% dan spesifisitas 84,6%. Kesimpulan. Peningkatan RNL berhubungan dengan HAP pada anak dengan PJB asianotik.
