Abstract

Background: Bacterial Vaginosis (BV) in pregnancy is caused by several physiological changes that disrupt the vaginal ecosystem, namely hormonal changes and vaginal flora. Treatment with antibiotics for Bacterial Vaginosis (BV) cases has a risk of resistance, so alternative therapies are needed, one of which is propolis extract. This study aims to prove the effect of giving propolis extract to pregnant women with bacterial vaginosis. Methods: This study used a quasi-experimental type of clinical trial research with The Pretest-Posttest Control Group Design. The population in this study were 100 pregnant women who experienced flour albus at Anugrah Pratama Clinic Surabaya in September-December 2021. The sample of pregnant women with BV was 44 people who were divided into control and intervention groups. Independent variabel is giving propolis extract and dependent variabel is infection of bacterial vaginosis. The sampling technique in this study was consecutive sampling. Data analysis using Chi Square Test. Results: The control group did not reduce bacterial vaginosis infection, which remained with BV (+) infection by 54.5% and BV infection (-) by 45.5%, while the intervention group was proven to reduce bacterial vaginosis infection to BV infection (+) by 18 .2% and BV infection (-) of 81.8%. Analysis: The results of the Chi-Square test showed that the p-value of BV infection was 0.012 and bacterial vaginosis infection α (0.05). Conclusion: There is a significant difference between the control group and the group given propolis extract. It can be concluded that there is an effect of propolis extract administration on reducing infection in pregnant women with bacterial vaginosis.

Abstract

Scabies is a skin infection caused by Sarcoptes scabiei var. hominis. This disease may present severe clinical manifestations in immune-compromised patient, well-known as Norwegian scabies or crusted scabies.A 36-year old man with AIDS had chief complaint thick crust almost all over his body in this case. History of household member infected by scabies before was present. Clinical findings show hyperpigmented macules unsharply marginated, covered with thick scales and accompanied by papules, fissures, and erotion. T cell CD4 level was 12 cell/μL. Scraping examination showed scabies infection and so did the histopathology examination. This patient was treated by topical Permethrin 5% combined with 2-4 ointment application in between permethrin usage. Before topical scabicide was given, thick crust was previously treated by topical urea 10% and wet dressing by normal saline. On day 14 after the patient first came there was lesion improvement.

Abstract

Helicobacter pylori is a bacteria that commonly cause chronic gastritis. Identification of its infection is essential for eradication treatment. Detection of H.pylori bacteria in gastric biopsy specimen by histology method is a diagnostic tool that widely accepted because it is superior to serology examination. Although the bacteria can be seen in routinely Hematoxylin-Eosin staining, Modified Giemsa and Whartin Starry stain was commonly used to see the bacteria more clearly. Whartin Starry stain gives more contrast to the bacteria but modified Giemsa stain is preferable at many centres because it is a cheaper and simpler method. This study want to find out whether there is difference result in detection of H.pylori using these two stains. Material and methods. Paraffin blocks from gastric biopsy patients with chronic gastritis were retrieved from Anatomic Pathology Laboratory Dr.Soetomo Hospital Surabaya in the year 2017. Thirty paraffin blocks were taken randomly and were made into microscopic slides for staining with Warthin-starry and modified Giemsa stain concomitantly. Results. Specimen with Whartin starry stain show 19 out of 30 were positive for H.pylori while modified-Giemsa stain found 16 out of 30 specimen were positive for H.pylori. Whartin Starry stain use silver reagent to coat the bacteria so it become more clearly visible. Conclusion. Detection of H.pylori Warthin-starry stain give more chance to obtain positive result because it use silver technique that coat the bacteria making it is more clearly visible in microscopic examination.

Abstract

Background: Basal cell carcinoma (BCC) is the most common skin cancer and can cause local tissue damage. BCC can occur in all populations, regardless of skin color. Dermoscopy has compatibility with histopathology in determining the benign or malignant nature of a lesion. Dermoscopy can improve the diagnostic accuracy of BCC by >90%, with 7 parameters that can be found: arborizing vessels, blue-gray ovoid nests, blue-gray dots/globules, maple leaf-like areas, spoke-wheel areas, ulceration, and hiny white areas. Purpose: To assess the compatibility of each dermoscopy parameter with its corresponding histopathological parameters. Methods: Observational analytic study with a cross-sectional approach was performed. Dermoscopy parameters were obtained by reassessing all photographs, while histopathological parameters were obtained from pre-existing slide reviews. Cohen's Kappa test was performed to analyze both variables. Result: A Total of 26 samples met the inclusion criteria. Most BCCs were found in female sex (65.4%), age >50 years (80.8%), and facial location (88.5%). Cohen's Kappa test was significant in 2 out of 7 dermoscopy parameters: "blue-gray ovoid nests" (p = 0.0019; r = 0.458) and "spoke wheel areas" (p = 0.037; r = 0.371). The "shiny white areas” parameter could not be analyzed because there was no negative variation in the examination. The possibility of false positives and negatives could not be ruled out.  Conclusion: Significance between the two variables is found in 28.6% of the 7 parameters analyzed...

Abstract

HIGHLIGHTS 1. Mitotic index and grade are prognostic factors for endometrial cancer, but both are independent.2. Stage and mitotic index associated with cell proliferation affect the prognosis of endometrial cancer.   ABSTRACT Objective: This study aimed to analyze the correlation between the mitotic index and the stage and grade of endometrial cancer. Materials and Methods: We collected pathology reports of endometrial cancer from the Pathology Laboratory at Dr. Soetomo General Hospital in Surabaya, Indonesia, covering cases diagnosed between 2018 and 2020. A total of 106 cases of endometrial cancer were included in this study. For each case, detailed records of the cancer stage, grade, and mitotic index were recorded. The mitotic index, an indicator of cell proliferation, was quantified, and its correlation with cancer stage and grade was assessed. To determine the strength and direction of these relationships, we performed a Spearman rank correlation statistical analysis for non-parametric data. Results: Our findings indicated a significant positive correlation between the mitotic index and the stage of endometrial cancer. An increase in the mitotic index, reflecting a higher proliferation rate of cancer cells, was associated with a more advanced cancer stage, suggesting that the mitotic index could potentially serve as a prognostic marker for assessing tumor progression in endometrial cancer. However, our analysis revealed no significant correlation between the mitotic index and the histological grade of endometrial cancer, implying that the grade, which typically reflects the differentiation status and morphological characteristics of the tumor cells, is independent of the proliferation rate as measured by the mitotic index. Conclusion: The mitotic index is positively correlated with the stage of endometrial cancer but does not show a correlation with the histological grade. These findings highlight the potential use of the mitotic index in staging endometrial cancer.

Abstract

Kanker payudara adalah tumor ganas yang terbentuk dari sel-sel payudara yang tumbuh dan berkembang tanpa terkendali sehingga dapat menyebar ke jaringan atau organ yang berada dekat dengan payudara atau ke bagian tubuh lainnya. Annona muricata Linn atau Graviola telah digunakan sebagai obat tradisional untuk menangani berbagai jenis penyakit seperti demam, rematik, kanker dan juga sebagai sedatif, insektisida dan imunosupresif. Pada penelitian ini, cell line kanker payudara MCF-7 diberi ekstrak etanol A. muricata kemudian diinkubasi 24 jam. Setelah itu dilakukan uji sitotoksik menggunakan metode MTT assay. Parameter yang digunakan pada uji sitotoksik adalah IC50 yang menunjukkan konsentrasi inhibisi sel kanker sebanyak 50% dengan nilai IC50 sebesar 117,87 μg/ml. Ekstrak dapat dikatakan memiliki aktivitas sitotoksik jika nilai IC50 kurang dari 1000 μg/ml setelah 24 jam kontak dengan sel kanker. Dengan demikian, dapat disimpulkan bahwa ekstrak etanol daun A. muricata dapat digunakan sebagai pilihan terapi anti-kanker alami untuk kanker payudara.Kata Kunci:  Kanker payudara, Ekstrak etanol daun Annona muricata, MCF-7, MTT assay

Abstract

Perubahan klinis pada payudara sebagian besar bersifat jinak, hanya 3 – 6% yang merupakan keganasan payudara. Namun demikian, kanker payudara menjadi perhatian utama mengingat angka kejadian yang terus menerus meningkat serta morbiditas dan mortalitas yang tinggi. Pengobatan kanker hingga saat ini masih menjadi tantangan. Hal ini membuka peluang penelitian terapi kanker terutama dari bahan herbal. Daun sirsak telah banyak digunakan masyarakat secara tradisional untuk pengobatan tumor. Di samping itu berbagai studi juga telah menunjukkan efek sitotoksik tanaman ini terhadap berbagi cell line kanker. Namun demikian belum ada penelitian yang menganalisis efek ekstrak daun sirsak terhadap populasi limfosit di lingkungan mikro tumor terutama yang berfungsi pada respon imun anti tumor. Penelitian ini bertujuan untuk mengetahui infiltrasi sel limfosit T CD8 pada jaringan tumor yang payudara tikus Sparaque Dawley yang diberikan ekstrak etanol daun sirsak. Ekspresi CD8 dianalisis melalui pewarnaan imunohistokimia. Ekspresi CD8 positif ditandai dengan warna coklat pada sitoplasma sel limfosit, selanjutnya ekspresi CD8 dari setiap sampel dihitung pada 3 lapang pandang dengan high-power field, kemudian ditentukan nilai rata-ratanya. Pewarnaan HE menunjukkan variasi infiltrasi limfosit pada nodul payudara tikus SD. Rata-rata ekspresi CD8 kelompok kontrol menunjukkan hasil lebih tinggi dibandingkan kelompok perlakuan. Namun demikian, hasil uji komparasi menunjukkan tidak ada perbedaan bermakna  (p > 0,05) antara ekspresi CD8 kelompok kontrol dengan perlakuan. Hasil ini menunjukkan bahwa pemberian ekstrak daun sirsak tidak menyebabkan perbedaan bermakna ekspresi CD8 pada populasi limfosit yang menginfiltrasi tumor payudara tikus SD.

The findings of this study can be used as an additional reference in determining the risk of placenta accreta in pregnant women, specifically as an input in screening and antenatal care programs for pregnant women to provide early detection and appropriate management.   Abstract The incidence of placenta accreta during pregnancy constitutes a serious problem in reproductive health. This complication has increased significantly over the last few decades, especially in developed countries where cesarean deliveries have also risen. Placenta accreta spectrum can cause very severe bleeding and potentially lead to other serious complications. The purpose of this study was to analyze the risk factors associated with the incidence of placenta accreta in women giving birth at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia. This quantitative study used an analytical observational research design, specifically the case-control approach. The study sample was all mothers who gave birth at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, between 2019 and 2023. In addition to a univariate analysis, the data were subjected to bivariate analysis using a two-sample independent t-test or Mann-Whitney test for ordinal data and for nominal data using the Chi-square test as well as multivariate analysis using logistic regression (p<0.05). The results showed that parity, a history of cesarean section, body mass index, and interbirth interval were the variables identified as significantly associated with placenta accreta. According to the logistic regression analysis, parity, a history of cesarean section, and a history of curettage were identified as the primary determinants of placenta accreta incidence. The classification of parity was found to have the most significant relationship to the incidence of placenta accreta. This study concluded that parity classification, a history of cesarean section, body mass index, and interbirth interval are the significant risk factors contributing to the increased incidence of placenta accreta at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia.

Abstract

Tumor and its microenvironment can interact each other. Macrophage is part of tumor microenvironment. New drugs targeting specific superficial receptor of macrophage or cytokine of macrophage polarization have been found. Therefore, macrophage phenotype and its ratio of M1/M2 macrophage need to be identified. This identification could lead us to prognose breast cancer and monitor its therapy. Analytical observational study with cross sectional approach, conducted on paraffin block sample of breast carcinoma from Anatomical Pathology Laboratory of Dr. Soetomo General Hospital. The samples divided into four groups based on nodal metastasis staging (N0, N1, N2, and N3) and stained with antibody against CD68 and CD163. The ratio of  CD68 and CD163 were analyzed with Anova test. There were difference expression of CD68 as M1 macrophage marker in various axillary node metastasis groups (p=0.015). There were difference ratio of CD68 as M1 macrophage marker and CD163 as M2 macrophage marker in various axillary node metastasis groups (p=0.005). There were difference ratio of macrophage M1 and macrophage M2 between N0 and N3 group, N2 and N3 group also.

Abstract

Penelitian ini bertujuan untuk meningkatkan pemahaman masyarakat tentang deteksi dini kanker serviks dan evaluasi Pap smear, serta meningkatkan keterampilan pengambilan sampel Pap smear pada penyedia layanan kesehatan di Kecamatan Mawaddah, Kabupaten Mojokerto. Studi deskriptif observasional dengan desain cross sectional dilakukan pada 100 wanita berusia 20-70 tahun dari Rumah Sakit Mawaddah. Penilaian dilakukan sebelum dan sesudah pemaparan materi. 100 sampel Pap smear yang dikumpulkan oleh bidan setempat dievaluasi untuk menilai kecukupan spesimen menggunakan sistem Papanicolaou dan Bethesda. Kegiatan ini secara signifikan meningkatkan pemahaman tentang kanker serviks (8,05% menjadi 82,26%), sementara kegiatan pelatihan tenaga medis meningkatkan pengetahuan mengenai pengambilan sampel Pap smear (51,62% menjadi 100%). 99% dari sampel Pap smear yang dikumpulkan oleh tenaga medis di Kecamatan Mawaddah dinilai memadai dan memuaskan. Program pengabdian masyarakat yang berfokus pada deteksi dini kanker serviks melalui Pap smear, secara efektif meningkatkan pemahaman masyarakat, meningkatkan pengetahuan dan keterampilan pengambilan sampel oleh tenaga medis, yang dapat meningkatkan pula efektifitas layanan Pap smear di Rumah Sakit Mawaddah pada masyarakat.
