Abstract

Henoch-Schönlein purpura (HSP) is an acute immunoglobulin A (IgA)–mediated vasculitis involving the small vessels. This disease cause systemic involvement of many organ especially the skin, the gastrointestinal (GI) tract and the kidneys. The etiology of HSP is not clear. Precipitating factors include drugs, chemicals, viruses and bacteria. Pathogenesis of HSP involve Ig A–mediated immune complex that are circulated in blood vessel and deposited in many organs. The presence of IgA-mediated immune complex deposits will activate the complement system, causing inflammation in the form of vasculitis that damage the small blood vessels in many organs. Hsp is a self-limiting disease that require supportive therapy. The problem that arises is how to distinguish HSP with other vasculitis diseases. A skin biopsy in patients with hsp will reveal leukocytoclastic vasculitis in small vessel. That kind of vasculitis are also found in urticarial vasculitis, hypersensitivity vasculitis and vasculitis due to cryoglobulinemia. Direct Immunofluorescence test from skin biopsy tissue will help to make the diagnosis of HSP. The presence of IgA deposits in small blood vessels wall can distinguish HSP from other vasculitis.

Abstract

Intravascular lymphoma is a rare type of lymphoma with some fundamental differences compared to other types of non–Hodgkin lymphoma in general. Its clinical manifestations vary greatly and depend on the affected organs. There is no pathognomonic signs of intravascular lymphoma, either in clinical, laboratory or radiological findings. This case is challenging to detect and often misdiagnosed because it does not show symptoms that are commonly found in other types of lymphoma, such as mass or nodules formation. The treatment is relatively similar to other lymphoma in general, but the prognosis is worse because it was mostly diagnosed in the advanced stage. These specific conditions become the main reason to separate this case as a special entity of lymphoma.

Abstract

Introduction: Data from Indonesian Health Ministry stated that there was an increase in the prevalence of breast tumors in 2015 – 2017 by 1.8 per 100 to 21.3 per 100,000 women aged 30-50 years. The most common types of benign breast lesions found in women worldwide are fibrocystic changes of breast and fibroadenoma. Aim: This research aims to analyze the factors of age, age at menarche, parity, use of hormonal drugs, and family history with the incidence of benign breast lesions in RSUD Dr. Soetomo Surabaya. Methods: This research method is observational analytic with a case-control approach, consisting of a case group and a control group of 41 respondents in each group. The statistical test of the study used the Chi-Square Test, Independent t-test, Mann Whitney, and Odds Ratio (OR). Results: The results of statistical tests obtained age factor p(sig)=0.002, hormonal use p(sig) 0.000, family history p(sig) 0.016, age at menarche p(sig) 0,282, and parity p(sig) 0.568. Conclusion: Conclusion there is a relationship between age, use of hormonal drugs, and family history and there is no relationship between age at menarche and parity with the incidence of benign breast lesions in RSUD Dr. Soetomo Surabaya. Family history has an Odds Ratio (OR) of 4.796. Keywords: Breast Lesions, Reproductive Health

Abstract

Indonesia was declared the country with the highest number of COVID-19 cases in ASEAN, and East Java was the province with the highest confirmed case fatality rate (7.3%) in 2021. In the case of COVID-19, there was a drastic elevation of the anti-inflammatory cytokine IL-10. This study aimed to ensure the role of IL-10 against pulmonary fibrosis in COVID-19 and non-COVID-19 patients. This study is a cross-sectional (non-experimental) study. The study sample consisted of 40 lung tissue samples from COVID-19 and non-COVID-19 patients at Dr. Soetomo General Academic Hospital, Surabaya. The IL-10 antibody reagent (GTX632359, GeneTex) was used to assess IL-10 expression, and hematoxylin and eosin (H&E) were used to determine the degree of pulmonary fibrosis. For Spearman's correlation, the data were analyzed using Statistical Package for the Social Sciences (SPSS). The mean percentage of IL-10 expression in COVID-19 patients (40.07%) was higher than in non-COVID-19 patients (26.60%). However, the mean for the degree of pulmonary fibrosis score in non-COVID-19 patients (4.43) was higher than in COVID-19 patients (3.67). Furthermore, a negative correlation (p=0.000; r=-0.281) was found between the percentage of IL-10 expression and the degree of pulmonary fibrosis. The percentage of IL-10 expression in macrophage cells had a significant negative correlation with the degree of pulmonary fibrosis score.

Abstract

Kegiatan pengabdia masyarakat ini bertujuan untuk meningkatkan pemahaman tenaga kesehatan dan masyarakat mengenai deteksi dini serta pencegahan kanker serviks. Pelatihan tenaga kesehatan dilakukan agar mereka dapat melakukan prosedur Pap smear dengan benar dan sesuai standar, sementara penyuluhan masyarakat bertujuan untuk meningkatkan kesadaran akan pentingnya deteksi dini dan pencegahan kanker serviks. Studi ini menggunakan metode deskriptif observasional dengan pendekatan cross-sectional, mencakup dua kegiatan utama: pelatihan tenaga medis secara daring dan penyuluhan masyarakat secara langsung. Pelatihan Pap smear diikuti oleh 51 peserta yang terdiri dari perawat, bidan, dan dokter. Evaluasi sebelum dan sesudah pelatihan menunjukkan peningkatan pemahaman yang signifikan, di mana peserta dengan skor 80-100 meningkat dari 58,82% menjadi 82,35%. Penyuluhan kanker serviks diikuti oleh 74 peserta. Sebelum penyuluhan, hanya 12,16% peserta yang memperoleh nilai 80-100, namun setelahnya meningkat menjadi 100%. Sebanyak 62,2% peserta telah menikah, 8,1% memiliki riwayat hubungan seksual sebelum usia 17 tahun, dan 9,5% memiliki lebih dari satu pasangan. Selain itu, 98,6% belum pernah menerima vaksin HPV, dan 78,4% jarang melakukan Pap smear, menunjukkan rendahnya kesadaran pencegahan kanker serviks. Setelah penyuluhan, 60 peserta menjalani skrining kanker serviks. Mayoritas (41,7%) berusia 31-40 tahun. Hasil skrining dengan sistem Bethesda menunjukkan 100% NILM. Berdasarkan klasifikasi Papanicolaou, 88,1% termasuk kelas II (96,2% inflamasi, 1,9% bacterial vaginosis, 1,9% atrofi), dan 11,1% kelas I. Program ini terbukti efektif dalam meningkatkan pemahaman tenaga kesehatan dan masyarakat, serta keterampilan tenaga kesehatan dalam prosedur Pap smear, yang berkontribusi pada peningkatan kualitas layanan deteksi dini kanker serviks di RSIA Hikmah Sawi Bangkalan.
