Abstract

Breast milk has an ideal nutritional content for babies according to the needs to grow and develop optimally. Indonesia has a very rich diversity of foodstuffs, one of the habits of the Indonesian people is to regularly consume certain vegetables during breastfeeding. However, some vegetables are not yet popular for breastfeeding mothers to consume even though they contain many benefits such as banana heart, young papaya and spinach. The purpose of this literature review is to provide an evidence-based study on the potential of local vegetables that are commonly consumed during breastfeeding in Indonesia but the content of galaktogogum is not widely known, namely: papaya, spinach, and banana heart. This article is a literature review that discusses the potential of local plants as a source of galactogues. Article search involves 2 databases, namely PUBMED and Google Scholar. The keywords used are: ("Milk, Human"[Mesh]) AND "Galactogogues"[Mesh] AND PAPAYA". ("Milk, Human"[Mesh]) AND "Galactogogues"[Mesh] AND banana flower". ("Milk, Human"[Mesh]) AND "Galactogogues"[Mesh] AND Spinach". ("Milk, Human"[Mesh]) AND "Galactogogues"[Mesh] AND Spinach. Banana hearts, young papayas, and spinach are vegetables that are able to increase breast milk production through galactogue compounds and their derivatives. The saponins contained in the heart of bananas and young papaya can increase the activity of the hormone oxytocin in the myoepithelial cells surrounding the alveolus and ductus. Meanwhile, the phytoestrogen content in spinach can cause biological effects similar to estrogen that are able to increase breast milk production.

Abstract

The purpose of this study is to determine prevalence of at risk inadequate intake of dietary zinc, and to analyze differences of zinc serum level between dietary zinc quintile and stunting status. A cross sectional study was conducted to 70 school children (9-12 years) selected by simple random sampling. Height was measured by stadiometer and weight was measured using calibrated digital weight-scale. Zinc serum level was analyzed by Atomic Absorbance Spectrophotometry (AAS) method, and dietary intake of zinc and iron was obtained using semi-quantitative Food Frequency Questionnaire (FFQ). Data were analyzed using Anova test. Seventy percent subjects were at risk inadequate intake of dietary zinc based on Estimated Average Requirement (EAR) cut off and zinc serum levels among all subjects in this study were <65 μg/dl. There were 47.1% stunting children found in this study. Mean zinc serum levels significantly differences between dietary zinc quintile (p=0.000) and between stunting status (p=0.000). At risk inadequate dietary zinc still contributed to zinc deficiency in coastal school children population. Zinc status was significantly different between stunted and normal children. Keywords: coastal area, dietary zinc, EAR, stunting, zinc serum levels

Abstract

The prevalence of obesity in Indonesia among adults in 2007 – 2023 has continued to increase from 10,5% to 23,4%. The causes of obesity are multifactorial, but those that have an essential role are diet and lifestyle. Weight gain is associated with unhealthy food intake, a lack of physical activity, and poor sleep quality. This study aimed to analyze the associations of AHEI (Alternative Healthy Eating Index), sedentary lifestyle, and sleep quality with the incidence of obesity. This research used a cross-sectional study among subjects aged more than or equal to 40 years, involving 153 subjects in Semarang in February 2024 and using purposive sampling. AHEI data were collected through an interview process using the SQ-FFQ in days to years, sedentary lifestyle using the SBQ in a week, and sleep quality using the PSQI in a month. Fisher’s exact and chi-square tests were used for bivariate analysis, whereas logistic regression was used for multivariate analysis. The results showed that a sedentary lifestyle on weekdays and weekends was associated with the incidence of obesity (p=0,000; OR=8,014) and (p=0,000; OR=18,516, respectively). In conclusion, a sedentary lifestyle is associated with incidence of obesity, while AHEI and sleep quality are not associated with incidence of obesity.

Abstract

Infants who are not exclusively breastfed are at a risk of iron deficiency. Iron deficiency affects motor development, intelligence, and endurance. Adequate nutrient intake can increase breast milk iron levels, because maternal food intake greatly affects the amount of iron in breast milk. This study aimed to determine the relationship between maternal nutrient intake and iron levels in the breast milk. This study used a cross-sectional approach from October to December 2024. The subjects were 90 mothers who breastfed babies aged 2-24 weeks, with a probability proportional to size. Nutrient intake and breast milk iron stage were the variables that were studied. Data were analyzed using the Pearson’s correlation test and multiple linear regression analysis. Results showed that foremilk iron levels ranged from 60,62 µg/dL to 632,94 µg/dL, while hindmilk iron levels varied from 127,44 µg/dL to 680,38 µg/dL. Iron intake was measured between 13,17 g and 115,6 g, showing a significant correlation between nutrient intake and breast milk (p < 0,05). In addition, it was found that breastfeeding mothers' iron intake was the most influential factor on the breast milk iron stage. In conclusion, maternal iron intake significantly affects breast milk iron levels. Ensuring adequate maternal iron intake, supported by vitamin C consumption, can help increase the availability of iron in breast milk, thereby benefiting infant growth and development.

Abstract

Stunting in children increases their risk of mortality, cognitive impairment, motor development, and poor body function. This study aimed to examine the relationship between low birth weight, low birth length, sex, age, and weight growth status and the prevalence of stunting, underweight, and wasting in children aged 6–36 months in West Lombok Regency, Indonesia. Methods: A cross-sectional study was conducted between November 2023 and May 2024 with 544 children. Secondary data were obtained from two community health centers in the study area. Data analysis was performed using the Spearman correlation test and chi-square test. Results: Birth weight and length were significantly associated with nutritional status in children. Higher birth weight was positively correlated with a better weight-for-age Z-score (WAZ; p = 0,001), height-for-age Z-score (HAZ; p = 0,009), and weight-for-height Z-score (WHZ; p = 0,0001), indicating a strong influence on overall growth and nutritional outcomes. Similarly, a greater birth length was significantly associated with WAZ (p = 0,0001), HAZ (p = 0,002), and WHZ (p = 0,006). Age was also correlated with HAZ (p = 0,005) and incidence of stunting (p = 0,0001), suggesting an age-related cumulative effect on linear growth. Moreover, children with low birth weight had a significantly higher risk of stunting (p = 0,0007), whereas those with low birth length were more likely to be underweight (p = 0,050). In conclusion, LBW and LBL are significant risk factors for stunting, wasting, and underweight in children aged 6–36 months in West Lombok Regency.

Abstract

Stunting in toddlers remains a critical public health issue that affects physical growth, cognitive development, and overall well-being. This study aims to analyze the effectiveness of training using booklets on the knowledge and attitudes of Posyandu Satellite cadres in accompanying complementary feeding for stunted toddlers aged 12–59 months in the working area of Lamper Tengah Health Center, Semarang. A quantitative research design with a quasi-experimental nonequivalent control group approach was employed, involving 44 cadres divided into intervention and control groups. Data were collected through pre-test and post-test questionnaires, focusing on knowledge and attitude variables. The results showed a significant improvement in the knowledge level of the cadres after training, with respondents in the “good” knowledge category increasing from 72.7% to 86.4%. Similarly, the attitude of cadres improved, with those in the “positive” attitude category increasing from 22.7% to 68.2% after the intervention. Statistical analysis using paired t-tests and Wilcoxon tests confirmed a significant difference (p < 0.05) between pre-test and post-test scores for both knowledge and attitudes. The use of booklets proved to be an effective and practical educational tool, providing structured and easy-to-understand information that enhanced the cadres’ capacity to support complementary feeding practices for stunted toddlers.

Abstract

Background: Breast milk is an important source of nutrition for the growth and development of infants. It is highly susceptible to the accumulation of lipophilic compounds such as heavy metals. Breast milk can become a route through which infants are exposed to toxic substances like heavy metals. This study aimed to identify variations of trace elements and their associated factors contributing to the existence of trace elements in breast milk. Methods: This research employed a systematic literature review methodology utilizing PubMed, Scopus, and Science Direct databases. The search strategy followed specific English keywords to align with the PRISMA Extension for Systematic Reviews guidelines. Data synthesis involved a narrative approach, summarizing the main findings and interventions outlined in the chosen studies. The included articles consisted of full-text primary research published between 2013 and 2023. To mitigate potential language bias, only publications in English were considered for inclusion. Results: Our systematic review, comprising nine articles, revealed the presence of toxic metals including lead (Pb), cadmium (Cd), aluminum (Al), copper (Cu), chromium (Cr), mercury (Hg), and arsenic in human milk. Associated factors contributing to this presence include food intake (such as seafood, fruit, and canned fruit), water and canned drink consumption, smoking and exposure to smoke, regional location, and medical conditions. Conclusion: Heavy metals, including lead, cadmium, aluminum, copper, chromium, mercury, and arsenic are present in breast milk, influenced by factors like diet, water consumption, smoking, regional zones, and medical conditions. These findings underscore the urgent need for public health policies to monitor environmental exposure in lactating mothers and to develop dietary guidelines minimizing toxic exposure.
