Abstract

Background: Nowadays, the transmission of HIV-AIDS is mostly due to risky sexual activity and found in the heterosexual group, around 76.5%. The high rate may be due to stigma and discrimination in society, so risk people are reluctant to come for a check-up or treatment. This study aims to evaluate the comprehensive approach from the upstream to downstream and culture based in dealing with HIV-AIDS in the form of Mobile Voluntary Counseling Testing (VCT) that has been carried out by the Indonesian Planned Parenthood Association (IPPA) Bali, Indonesia.Methods: This descriptive cross sectional study was conducted in the 2016-2018 period in all regions of Bali, Indonesia, by convenience sampling technique. There are differences in the total number of services that have been performed which include Post-Test (+) counselling, Post-Test Counseling (-), rapid test evaluation, HIV counselling and prevention measures, as well as VCT Pre-Test counselling. The medical team and counsellor came together to meet directly with groups at risk of HIV-AIDS infection called key populations, such as commercial sex workers, transvestites, drug users, and homosexuals. The data obtained were analyzed descriptively using Microsoft Excel for Windows.Results: The results of this descriptive empirical study show that the overall Post-Test (+) counselling was conducted 9 times in 2016 (2 times), 2017 (3 times), and 2018 (4 times). Whereas in Post-Test (-) counselling there were 161 examinations in 2016, 105 times (2017), and 125 (2018). In the last 3 years, 1,472 investigations have been carried out, of which 652 times were in 2016, 367 times in 2017, and 453 times in 2018.Conclusion: Mobile VCT of IPPA Bali may be one of the effective efforts in responding to the challenges of people who tend to be taboo and less concerned about sexual and reproductive health issues.   Latar Belakang: Saat ini penularan HIV-AIDS sebagian besar disebabkan oleh aktivitas seksual yang berisiko dan ditemukan pada kelompok heteroseksual sekitar 76,5%. Tingginya angka tersebut mungkin disebabkan oleh stigma dan diskriminasi di masyarakat, sehingga orang yang berisiko enggan datang untuk melakukan pemeriksaan atau perawatan. Penelitian ini bertujuan untuk mengevaluasi pendekatan komprehensif dari hulu ke hilir dan budaya berbasis dalam berurusan dengan HIV-AIDS dalam bentuk Pengujian Konseling Sukarela Seluler (VCT) yang telah dilakukan oleh PKBI Bali, Indonesia.Metode: Penelitian deskriptif dengan pendekatan potong lintang ini dilakukan dalam rentang periode 2016-2018 di seluruh daerah Bali, Indonesia dengan menggunakan teknik convenience sampling. Terdapat perbedaan jumlah total layanan yang telah dilakukan dimana meliputi konseling Post-Test (+), Konseling Post-Test (-), evaluasi rapid test, konseling HIV dan tindakan pencegahan, maupun konseling VCT Pre-Test. Tim medis dan konselor datang bersama untuk bertemu langsung dengan kelompok yang berisiko terinfeksi HIV-AIDS yang disebut populasi kunci, seperti pekerja seks komersial, waria, pengguna narkoba, dan homoseksual. Data yang diperoleh dianalisis secara deskriptif menggunakan Microsoft Excel untuk Windows.Hasil: Hasil studi empiris deskriptif ini menunjukkan bahwa telah dilakukan 9 kali konseling Post-Test (+) baik pada tahun 2016 (2 kali), 2017 (3 kali), dan 2018 (4 kali). Sedangkan pada konseling Post-Test (-) terdapat 161 kali pemeriksaan pada tahun 2016, 105 kali (2017), dan 125 (2018). Secara keseluruhan dalam 3 tahun terakhir telah dilakukan sebanyak 1.472 pemeriksaan dimana sebanyak 652 kali pada tahun 2016, 367 kali pada tahun 2017, dan 453 kali pada tahun 2018.Kesimpulan: Mobile VCT PKBI Bali dapat menjadi salah satu upaya efektif dalam menanggapi tantangan orang-orang yang cenderung tabu dan kurang peduli tentang masalah kesehatan seksual dan reproduksi.

Abstract

Background and purpose: Adolescents with hearing impairment face a number of exclusion due to their limitation, including lack of access to information on reproductive health. This study aims to explore needs for sexual and reproductive health education for students with hearing impairment at Singaraja. Methods: A qualitative study was conducted at Buleleng District. Data were collected through in-depth interviews to 19 informants. Informants were purposively selected based on their specific roles. Our informants included students with hearing impairment, head of school, teachers and parents or carers. Data were analysed using a thematic approach. Results: The majority of students with hearing impairment had low level of knowledge associated to sexual and reproductive health. However, sexual and reproductive health practices between adolescents with and without hearing impairment were generally comparable. Needs for sexual and reproductive health education must cover several elements: (1) education material must be tailored based on age, (2) learning methods should be tailored based on their intellectual ability, and (3) learning methods must use contextual and real model approach to allow better transfer of information. Teachers faced difficulty to provide sexual and reproductive health education for students with hearing impairment even though some modules are already available. Conclusions: The provision of sexual and reproductive health education for students with hearing impairment requires tailored material, simple and attractive learning methods, and visual media. Further analysis of the existing reproductive health education curricula for students with hearing impairment is required in order to ensure fulfilment of reproductive rights of adolescent with special needs.

Abstract

Background and purpose: Pre-eclampsia is a major contributing factor of maternal mortality. Dompu District has a high rate of maternal mortality relative to the rest of districts in West Nusa Tenggara Province. Pre-eclampsia is the major cause of maternal mortality in Dompu District. This study aims to examine risk factors of pre-eclampsia among pregnant women. Methods: A case control study was conducted in five public health centres in Dompu District. A total of 104 women who gave birth and have diagnosed with pre-eclampsia during the pregnancy were recruited as cases. A total of 104 women who gave girth and did not have pre-eclampsia were selected as controls. All cases and controls were selected using a systematic random sampling technique from the 2016 pregnant women cohort at five public health centres. Data were collected through interview and extraction from the maternal and child health book. Data were analysed using bivariate and multivariate techniques. Results: Our multivariate analysis found that pre-eclampsia was associated with parity of 1 or >3 (AOR=3.64; 95%CI: 1.85-7.17), body mass index of >27.0 kg/m2 (AOR=3.01; 95%CI: 1.39-6.48), history of chronic diseases (AOR=16.43; 95%CI: 3.32-81.22) and smoke exposure (AOR=3.71; 95%CI: 1.95-7.05). Conclusions: Parity of 1 and >3, BMI of >27.0 kg/m2, history of chronic illnesses and smoke exposure are risk factors for pre-eclampsia in Dompu District, West Nusa Tenggara.

Abstract

Background and purpose: The 2016 Report of Performance Accountability of Government Agencies, Ministry of Health of Indonesia showed only a small proportion of public health centres (PHCs) are accredited. In West Lombok District, some PHCs are not accredited. This study aims to examine factors associated with PHC’s staff readiness for accreditation. Methods: A cross-sectional survey was employed involving seven non-accredited PHCs. A total of 165 out of 310 PHC’s staff were recruited using a systematic random sampling. Data was collected from February to March 2017. Self-administered questionnaire was used to collect data on socio-demographic characteristics, duration of service, content, process, and context changes, individual attributes, and readiness to change. Logistic regression was applied to examine the association between readiness to change with independent variables. Results: As many as 72.1% of respondents are ready to change. From the change efficacy and appropriateness dimensions, as many as 46.1% and 97.0% of respondents are ready to change. Multivariate analysis shows an association between readiness to change with administrative systems (AOR=4.47;95%CI:2.05-9.74) and working procedure (AOR=2.95; 95%CI: 1.19-7.30). There is no significant association between readiness to change with technological improvement, promotional strategy, staff engagement, organisational commitment and managerial support from health offices. Conclusions: The readiness to change among PHC’s staff is high. The availability of working procedure and administrative systems improves PHC’s staff readiness for the accreditation. These findings suggest the importance of inclusion of all PHC’s staff during the accreditation processes.

Abstract

Background and Purpose: Tuberculosis (TB) is more common in developing countries, including Indonesia. Denpasar City contributes the most on the total number of TB patients in Bali Province. There is evidence on increased burden of diabetes mellitus (DM) patients with TB comorbidity. This study aims to examine risk factors of TB among DM patients. Method: A case control study was conducted in Denpasar City. A total of 135 samples, 45 cases and 90 controls, were recruited to participate in the study. Cases were DM patients with TB comorbidity and were taken from patients register from 2013 to 2015. Controls were DM patients without TB comorbidity and were taken from Bali Province screening operational study. Data were collected using interviews, observations, measurements and document review. Data were analysed using bivariate analysis and multivariate with logistic regression model. Results: Demographic characteristics between cases and controls were comparable in sex and family income but not comparable in age groups, education, domicile, and employment. Risk factors of TB among DM patients were house density (AOR=36.11; 95%CI: 5.0-259.9), undernutrition (AOR=24.76; 95%CI: 3.7-162.3), inadequate glycemic control (AOR=12.64; 95%CI: 2.4-66.2), and being employed (AOR=10.55; 95%CI: 1.2-92.7). TB infection among DM patients was associated with aged<60 years (AOR=7.47; 95%CI: 1.2-44.3) and being male (AOR=5.42; 95%CI: 1.2-24.0). TB co-infection among DM patients was also associated with low education level (AOR=6.96; 95%CI: 1.0-48.6), contact with TB patients (AOR=5.84; 95%CI: 0.7-46.6) and inadequate house ventilation (AOR=2.92; 95%CI: 0.6-13.1). Conclusion: Socio-demographic characteristics, physical environments of the house and clinical conditions are risk factors of TB among DM patiens.

Abstract

Background and purpose: The performance of nurses in providing care to patients is an important component for patient satisfaction and healing. Nurse's performance is determined by many factors. This study aims to determine the association between nurse performance in providing nursing care with education, nurse perceptions of financial rewards, career development opportunities, and supervision of the ward head. Methods: A cross-sectional survey was conducted on all nurses (164 people) at inpatient wards of Tabanan General Hospital. Data collection was carried out using a self-administered questionnaire in April 2018. Bivariate analysis was performed using chi-square test, and multivariate analysis was conducted using multiple logistic regression to determine the adjusted odds ratios. Results: The results showed that the mean score of nursing care performance was 70.17 from a maximum score of 80. All independent variables were found to be associated with nurse performance, namely perceptions of career development opportunities (AOR=1.6; 95%CI: 1.03-1.3; p=0.01), financial rewards (AOR=1.1; 95%CI: 1.01-1.2; p=0.03) and supervision of the ward chief (AOR=1.1; 95%CI: 1.01-1.2; p=0.02). Conclusion: Nurses' perceptions of financial rewards, career development opportunities, and supervision of the ward chief are associated with nursing care performance. Provision of rewards, career development and supervision need to be considered in the efforts to improve nurse performance.

Abstract

Background and purpose: The incidence of morbidity and mortality due to tobacco use is very high. There are several strategies to help smokers quit, one of which is counseling to stop smoking in a clinic setting. The purpose of this study was to explore in depth the utilization of smoking cessation clinics at a Public Health Centre (PHC). Methods: A qualitative study through in-depth interviews was conducted with 14 informants who had been provided with a counseling at a smoking cessation clinic, consisting of five informants who had quit smoking and nine informants who were still smoking. Triangulation of data was carried out by conducting in-depth interviews with the head of PHC and one counselor on duty at the smoking cessation clinic. The information collected includes the utilization of the clinic, strategy of PHC in recruiting patients, knowledge about smoking cessation and the benefits of attending counseling. Data collection was carried out at the home of each informant during March-April 2018. The results of the interviews were analyzed thematically after a verbatim transcription being conducted. The data presented in a narrative to provide an in-depth description of the utilization of smoking cessation clinics in a PHC. Results: The results of interviews with PHC staff showed that in order to increase the utilization of the smoking cessation clinic, the services are integrated into the general polyclinic. Interviews with informants who have not stopped smoking indicate that to improve the utilization of the clinic, promotion and service variations are needed and not only focus on counseling. Interviews with informants who have stopped smoking show that counseling at the clinic can increase their willingness to stop smoking. Conclusion: To improve the utilization of smoking cessation clinics, in addition to integrating services into the general polyclinic, promotion and variations in services are also required.

Abstract

Background and purpose: The coverage of exclusive breastfeeding is reported to be low. The aim of this study was to explore the extent of the roles of midwives during pregnancy, childbirth and postpartum to increase exclusive breastfeeding. Methods: This study applied a qualitative approach by in-depth interviews based on interview guidelines to explore the role of midwives in increasing exclusive breastfeeding during pregnancy, childbirth and postpartum. This study was conducted in the area of Kuta I Public Health Center (PHC), Badung District, Bali Province from October 2017 to July 2018. Interviews were conducted with 12 informants chosen purposively, namely three PHC midwives, two private practice midwives, one nutrition program manager, four mothers who managed to provide exclusive breastfeeding and two who did not. Data were analyzed with a thematic approach. Results: This study shows that the roles of midwives are not optimal. The role that has been carried out during pregnancy is to provide information about breast care but not to practice breast care. During labor, the midwife facilitated the initiation of early breastfeeding except for mothers who experienced childbirth complications. During the postpartum period the midwife provides information about breastfeeding to working mothers and monitors exclusive breastfeeding at the integrated health services centers or in Indonesia called pos pelayanan terpadu (posyandu) until the infant reach the age of six months. Midwives provide information about breast care and how to overcome problems in breastfeeding, but do not facilitate the practice of both. All midwives do not conduct home visits for several reasons, such as distance to residents’ homes, high burden of tasks and lack of human resources. Midwives also provide counselling at posyandu on maternal and child nutrition but do not focus on exclusive breastfeeding. Conclusion: The roles of midwives in facilitating exclusive breastfeeding during pregnancy, childbirth and postpartum are still superficial and not in accordance with the guidelines for obstetric care. The roles of midwives need to be enhanced to increase exclusive breastfeeding.

Abstract

Background and purpose: Patient centered care in hospitals has yet to be well implemented. The role of case managers is very important in the implementation of patient centered care in hospitals. Research publications on the role of case managers in patient centered care in Indonesia remain limited. This study aims to explore the roles of case managers and problems encountered in the implementation of patient centered care in hospitals. Methods: This study used a qualitative design with a case study approach conducted at Bangli District Hospital in Bali in April 2019. Data was collected through in-depth interviews with seventeen purposively selected informants consisting of ten case managers, one deputy director of medical services, two patient’s family members, two specialist doctors and two implementing nurses at Bangli District Hospital. This study uses the Patient Centered Care Framework from Greene which consists of three dimensions in patient centered care namely interpersonal, clinical and structural dimensions. Data was analyzed thematically using deductive and inductive techniques. Data validation was conducted through triangulation of data based on different sources, member checking and peer debriefing. Results: In this study, five main themes were found regarding the role of case managers in the implementation of patient centered care in hospitals, namely interpersonal, clinical and structural dimensions, support and expectations of case managers. The identified role of the case manager in the interpersonal dimension consisted of listening to patients and families actively, coordination and collaboration in problem solving, providing accurate and easy to understand information about care. Problems encountered included lack of communication between the service provider components and between the service provider with patients and their families. Roles of case manager in the clinical dimension are to hold meetings with a professional team of care givers and continuous monitoring of services to patients, while the issue encountered is the mismatch of time of service provision by specialist doctors. Roles of case managers in the structural dimension is coordination with policy holders, use of information technology and patient reporting. Problems encountered were inappropriate recruitment of case managers, lack of training of case managers, incomplete facilities, inconvenience of services and inadequate incentives. Supports from hospitals for case managers were support of facilities and legality as case managers. Expectations of case managers include case manager training, rewarding and career development opportunities. Conclusion: The roles of case managers in implementing patient centered care in Bangli District Hospital are not optimal. Efforts to address issues in the interpersonal, clinical and structural dimensions need to be carried out by the hospital management to enhance the role of case managers in the implementation of patient centered care.

Abstract

Background and purpose: Satisfaction survey among patients of the inpatient unit at Ganesha Public Hospital indicated lack of caring behavior among nurses. This study aims to reveal level of quality of care and the factors associated with related behavior among inpatient ward at Ganesha Public Hospital. Methods: The study used cross-sectional survey design. Data were collected through self-administered questionnaires among 48 inpatient nurses during November-December 2014. Multivariate logistic regression was applied to determine factors associated with caring behavior of nurses. Results: The study found that 56.3% believed they provided a relatively high quality of care and 54,2% of nurses indicated a positive perception of the organization culture. Result of multivariate analysis indicated that only rewards system is independently associated with caring behavior of nurses (adjusted OR=23,39; 95%CI: 1.53-356.94; p=0,023). Conclusion: It was evident that nurses’ practice toward caring behavior is related to their attitude toward the rewards system of nurses’ performance.
