Abstract

Background and purpose: Increasing of HIV+ cases resulted in the increasing demand for antiretroviral (ARV) and access to ART services. This study aims to explore barriers of integrating ART services into primary health centres in Badung District. Methods: Explorative interviews were conducted with CST officers at Badung District Hospital, puskesmas (health centres), NGOs, policy makers, community leaders and PLWHA. Data were collected by conducting indepth interviews, focus group discussion and observation. Data were thematically analysed. Results: The study found that there were variations in internal barriers across different puskesmas in Badung District to be a satellite site. The main internal barriers perceived by health officers particularly in relation to issues around the availability of human resources, lack of competencies and increase of workload. Internal capacity of puskesmas which also perceived as barriers including the availability of laboratory facilities, absent of operational procedures, limited information system and source of funds that predominantly still from donor agencies. External barriers perceived by respondent were stigma and discrimination. Conclusions: Barriers to integrate ART services into puskesmas consist of structural barriers from internal puskesmas as well as external barriers such as stigma and discrimination towards PLWHA.

Abstract

The public health centre reports showed that the level of parent’s participation in children dental health care in Tegallalang I Public Health Centre was low (31.7%-35.1%). This study aims to determine the relationship between parent’s knowledge, attitudes, motivation and transportation accessibility to parent’s participation in child dental health care in Tegallalang I Public Health Centre. A cross-sectional study was conducted to 100 parents whose children were referred to undergo dental health examinations in Tegallalang I Public Health Centre using a systematic random sampling method. Data of knowledge, attitudes, motivation and transportation accessibility were collected using an interview guide, while parent’s participation was obtained from dental care registration. The data were analised by logistic regression. The results showed that level of parent’s participation in child dental health care was low (41%). Variables that were significantly associated with level of parent’s participation in child dental health care were knowledge [OR=7.35; p=0.001], motivation [OR=4.89; p=0.028] and attitudes [OR=4.19; p=0.039). Transport accessibility was not associated with parent’s participation [OR=2.35; p=0.123]. The conclusion of this study is that parent’s participation in child dental health care was influenced by parent’s knowledge about oral health, motivation and attitudes towards child dental health care. It is recommended to upscale health promotion related to dental health and the benefits of treatment in order to encourage parent’s participation in children dental health care.

Abstract

Breast cancer is the most common cancer that found in women. More than 800,000 newly identified breast cancer cases are diagnosed in the whole world annually. In Indonesia, breast cancer is the second common cancer among women after servical cancer. Risk factors of breast cancer include pregnancy history, age at first pregnancy, breast feeding history, utilization of hormonal birth control, menarche history, genetic tendencies and past breast infections. This study aimed to assess the risk factors that contribute to increasing breast cancer incidence in cancer patients at Sanglah Hospital. Research used a matched paired case-control study with a sample of 38 female breast cancer patients and 38 female non-cancer patients with similar backgrounds, age and address. Data was collected through questionnaire. Data analysis was conducted using bivariate with McNemar test and multivariate analysis using logistic regression. The result of bivariate analysis showed that risk factors that increased breast cancer included breast disease history (OR=13.5; 95%CI: 3.21-56.77) and genetic tendencies (OR=8; 95%CI: 1.84–34.79). The result of multivariate analysis showed that the only significant risk factor was breast infection history (OR=43.19; 95%CI: 8.79-212.27). Future recommendations include increased health promotion about the importance of early detection and screening, as well as information related to the dangers of breast infection/disease. National policy regarding access to mammography facilities should also be prioritised.

Abstract

Background and purpose: The study aims to determine factors influencing the utilization of accredited government-run ANC facilities at the Kawangu Health Centre. Methods: Research was a cross-sectional study, with a total of 85 samples taken by consecutive sampling from the Kawangu Health Centre. The dependent variable was the use government-run ANC facility at the Kawangu Health Centre. The independent variables were knowledge, awareness, attitudes, access to health services, frequency of receiving information and family support. The data were collected by interview using questionnaires. The data analysis includes univariate, bivariate (chi-square) and multivariate analysis with logistic regression. Results: The results of the study indicated that there was a significant relationship between the use of accredited government-run ANC facilities with maternal knowledge/awareness (p=0.001), attitude (p<0.001), ability to access to health services (p<0.001), frequency of information (p=0.039), and family support (p<0.001). Multivariate analysis indicated that the only significant independent variable related to the use of adequate delivery was the ability to access health services with OR =11.68 (95%CI: 1.37 to 99.89). Conclusion: An inability to access either due to distance to travel, lack of vehicle and/or infrastructure concerns such as poor quality/lack of roads was the dominant variable in the utilization of accredited government-run ANC facility.

Abstract

Background and purpose: The objective of this study was to explore the supporting and the inhibiting factors influencing the use of implant contraceptive use by women of childbearing age. Methods: The study was qualitative using a phenomenological approach. Data collection involved a purposively selected sample, conducting two FGD (10 implant users and 10 non-users) and 11 in-depth interviews among private midwives, government midwives, husbands, in-laws, and family planning staff. Results: FGD found that perceptions and attitudes towards implants of those already using the contraceptive were positive, while the perceptions and attitudes of non-users were less so. Some respondents intimated that they experienced side effects though these were not a deterring factor in usage. Enabling factors for the use of implant contraceptives were the availability and accessibility to the healthcare facilities and the support of their husbands. Access to facilities and infrastructure were not inhibiting factors either for the users or the non-users. Findings from in-depth interviews indicated that inhibiting factors included were the desire to have more children and the lack of promotion of the implant contraceptives. Healthcare provider informants stated that the lack of training was an inhibiting factor. Conclusion: Enabling factors for the use of implant contraceptives were the availability and accessibility to the healthcare facilities and the support of their husbands. Inhibiting factors were the desire to have more children, the lack of proper training for the health workers, as well as the lack of promotion of the implant contraceptive in the community

Abstract

Background and purpose: Smoke-free area regulation has been issued recently in Bali Province and Badung District. This study aimed to identify factors associated to the compliance of local smoke-free regulation in star hotel. Methods: This study was a cross-sectional analytic study conducted in Badung District from March to May 2014. Samples were 104 hotels and the managers that selected using systematic random sampling. Compliance data collected through observation using the observation form and the data of managers through interviews using a structured questionnaire. Data were analyzed using chi-square test and poisson regression. Result: The results shows that the compliance of star hotels to local smoke-free regulation was low (15.4%). The concrete support of hotel managers was significantly associated with the compliance to smoke free regulation (PR=4.25; 95%CI: 1.03-17.58). The factors that not significantly associated to the compliance were the knowledge (PR=2.0; 95%CI: 0.8-4.9), attitude (PR=2.5; 95%CI: 0.8-8.2), and organization support (PR=1.8; 95%CI: 0.7-4.5). Furthermore the hotel compliance of managers who have smoking behavior was 0% while the managers without smoking behavior, the compliance was 19.5%. Conclusion: The compliance of star hotels in Badung District on the implementation of local smoke-free regulation was relatively low and the compliance was associated with the presence of concrete support of the hotel managers.

Abstract

Background and purpose: Targetted adolescent health services (PKPR) at the South Kuta Health Center have been offered since 2007. However, these are rarely accessed (less than 5 visits per month) whereas adolescent health incidents/concerns such as abortion, HIV/AIDS and drug use are reported to increase. This study aimed to determine the predisposition, supporting and reinforcing factors of PKPR utilization. Methods: The study was qualitative and conducted among 27 students in four sessions of focus group discussions, as well as indepth interviews with four key informants. Data collected consisted of knowledge, attitudes and views of participants on PKPR facilities. Thematic analysis was conducted to identify predisposition, supporting and reinforcing PKPR utilization factors. Results: The study participants intimated that their knowledge about PKPR was low, although they recognised the benefits of such service. They also acknowledged that the quality of facilities/infrastructure were poor, the IEC materials were interesting but the deliverance was unclear and the attitude of providers was unfriendly. Funds, inappropriate clinic hours and labor provided were minimal. Schools, health centers and the government health office strongly supported PKPR activities and the role of peer counselors. School health programs also encouraged students to take advantage of PKPR. Conclusion: Local schools, health centers and the government health office were supportive of PKPR activities, however the student’s knowledge about PKPR was low because coverage of PKPR was very limited.

Abstract

Backgrounds and purpose: Indonesia is considered of the countries in Southeast Asia with low food quality. The poor behaviors of food handlers in applying good manufacturing practice may bring about a decline in food quality which can lead to the risk of health conditions such as diarrhea, worm infection or food poisoning. This study aims to determine the factors associated with the behaviors of food handlers in the application of good manufacturing practice at the household food industry level. Methods: Cross-sectional survey was conducted among 79 food handlers, working at the household food industry in Karangasem. Data were collected through interviews and observations and analyzed using poisson regression to understand the association of knowledge, attitudes and managerial support with behavior of food handlers. Results: The knowledge and attitude of food handlers were relatively poor at respectively 48.10% and 53.16%. The behavior of food handlers classified as good was 49.37%. Behavior of food handlers was significantly associated with knowledge (PR=1.48; 95%CI: 1.01-2.15), attitude (PR=2.13; 95%CI: 1.47-3.08) and managerial support (PR=3.01; 95%CI: 1.77-5.13). Conclusion: The behavior of food handlers in the application of good manufacturing practice at the household food industry remains low and was associated with knowledge, attitude and level of managerial support.

Abstract

Background and purpose: The incidence of recurrence in patients with pulmonary tuberculosis (TB) in Denpasar in 2014 was relatively high, around 3.5% of 1082 patients. This study aims to determine the risk factors for recurrence of pulmonary TB patients in Denpasar. Methods: The study design was a case control with 46 patients with pulmonary TB recurrence aged over 15 years as cases, and 92 patients who had recovered as control. Respondents were selected using systematic random sampling from the TB register from 2013 to 2015 in public health centers in Denpasar. Data were collected by a search of documents, observations, measurements and interviews conducted using questionnaires. Bivariate and multivariate analysis were conducted using chi square test and logistic regression respectively. Results: Risk factors that associated with recurrence of pulmonary TB were co-morbidity of diabetes mellitus (AOR=9.6; 95%CI: 2.17-43.08), adherence (AOR=7.6; 95%CI: 2.85-20.17), exposure to cigarette smoke during treatment (AOR=3.6; 95%CI: 1.41-9.16), home ventilation <10% (AOR=3.4; 95%CI: 1.27-9.47), house contact with pulmonary TB patients (AOR=3.1; 95%CI: 1.31-7.46) and malnutrition (AOR=2.8; 95%CI: 1.02-7.72). Conclusion: Risk factors for recurrence of pulmonary TB among patients were co-morbidity of diabetes mellitus adherence, cigarette smoke exposure during the treatment period, home ventilation <10%, house contact with pulmonary TB patients and malnutrition.

Abstract

Background and purpose: Patient satisfaction, including on radiography services, is an important component of health care provision. Surveys on patient satisfaction have been carried out particularly in government hospitals, but a more detailed study to determine the relationship between patient satisfaction with the responsiveness of service providers has yet to be carried out. This study aims to determine the relationship between the six domains of responsiveness of service providers and outpatient satisfaction at three conventional radiography facilities in Denpasar. Methods: Study was conducted by cross-sectional survey of 158 respondents selected consecutively in three conventional radiography services, namely a government hospital, a private hospital and a private radiology clinic. Data were collected through interviews using modified questionnaires on responsiveness from WHO. Data were analyzed by univariate, bivariate (chi-square test) and multivariate logistic regression method. Results: Univariate analysis indicated that patient satisfaction was 69% but still below the minimum service standards (>80,0%). Multivariate analysis showed that factors associated with satisfaction were the domain of attention (AOR=3.77; 95%CI: 1.62-8.76), the quality of infrastructure (AOR=4.57; 95%CI: 1.61-12.93) and communication (AOR=6.30; 95%CI: 1.75-22.64). Conclusion: Patient satisfaction was generally still below the standard. Domains of responsiveness associated with patient satisfaction were the quality of infrastructure, level of attention and communication skills of service providers.
