Refusal of COVID-19 booster vaccination among people living in remote tourism area, Samosir, Indonesia Sitompul, Jos Iswadi; Anak Agung Sagung Sawitri; Dyah Pradnyaparamita Duarsa

Abstract

Background and purpose: Samosir Island is a tourist area located in the middle of Toba Lake, Sumatra. It was reported that the coverage of COVID-19 booster vaccination had not reached the national target. This study aims to determine factors associated with the refusal of the COVID-19 booster vaccination. Methods: A community-based survey was conducted in the Sitiotio Sub-district of Samosir, from January to March 2023. Four out of eight villages were selected to represent the farthest and closer area. A multistage systematic random sampling was applied to select 156 out of 2073 households (HH). We interviewed 376 respondents to explore sociodemographic, experience of side effects of previous COVID-19 vaccination, comorbidities, family support, information, knowledge and perceptions. Booster vaccination status was determined through p-Care and immunization card. Descriptive, simple and multiple logistic regression were conducted. Results: About 32.4% of respondents refused the COVID-19 booster for fear of vaccine side effects (95.5%), had been vaccinated against COVID-19 (91.8%), and believed that the COVID-19 virus was gone (66.4%). The refusal of boosters was higher for those with low education, experienced side effects, have comorbidities, no family support and insufficient information, had poor knowledge and perception about COVID-19 vaccinations. Lack of trust of COVID-19 vaccine (AOR=2.31, 95%CI: 1.05-5.07), high perceived barriers (AOR=4.75, 95%CI: 1.96-11.49) and low self-efficacy (AOR=4.94, 95%CI: 1.44-16.98), less family support (AOR=6.34, 95%CI: 1.89-21.26) and poor knowledge (AOR=7.74, 95%CI: 1.74-34.02) significantly associated with COVID-19 booster vaccination refusal. Conclusion: Poor knowledge and perception of the COVID-19 vaccine and lack of family support increase refusal of the COVID-19 booster. Continuous evidence-based health promotion and family involvement can be carried out to increase public trust.

Abstract

Pandemi COVID-19 yang melanda dunia sejak Maret 2020 menyebabkan peningkatan beban kerja akibat angka kasus COVID-19. Peserta Program Pendidikan Kedokteran Spesialis (PPDS) merupakan salah satu kelompok tenaga kesehatan yang diikutsertakan dalam perang melawan virus SARS-CoV-2. Salah satu faktor yang mempengaruhi keselamatan peserta PPDS dalam menangani pasien COVID-19 adalah ketersediaan Alat Pelindung Diri (APD) di rumah sakit. Penelitian ini bertujuan untuk mengetahui gambaran persepsi peserta PPDS Fakultas Kedokteran Universitas Udayana di Program Studi (Prodi) Ilmu Penyakit Paru, Ilmu Kesehatan THT-KL, Ilmu Penyakit Jantung dan Pembuluh Darah, Ilmu Penyakit Dalam, Prodi Anestesiologi dan Reanimasi, Prodi Ilmu Kebidanan dan Penyakit Kandungan, serta Prodi Ilmu Kesehatan Anak selama pandemi COVID-19. Penelitian ini merupakan penelitian kuantitatif dengan rancangan deskriptif cross sectional. Data dikumpulkan melalui pengisian kuisioner mandiri secara daring dan luring pada satu waktu yang bersamaan. Responden dipilih secara convenient hingga didapatkan 224 sampel. Data yang terkumpul dianalisis dengan analisis univariat. Penelitian ini menunjukkan bahwa di ketujuh prodi rerata peserta PPDS mengaku merasa bahwa intitusi sudah memberikan APD yang cukup untuk kebutuhan di rumah sakit adalah sebesar 88,8%. Hal yang sama diakui 94,9% peserta PPDS Prodi Anestesiologi dan Reanimasi, 94,4% peserta PPDS Prodi Ilmu Penyakit Dalam sebesar, 93,3% peserta PPDS Prodi Ilmu Kesehatan THT-KL, 92,3% peserta PPDS Prodi Ilmu Kebidanan dan penyakit Kandungan, 90,0% peserta PPDS Prodi Ilmu Kesehatan Paru, 78,7% peserta PPDS Prodi Ilmu Kesehatan Anak, dan 66,7% peserta PPDS Prodi Ilmu Penyakit Jantung dan Pembuluh Darah sebesar 66,7%. Abstract

Background: Diabetes management has not been optimally implemented by patients, considering the high incidence of diabetes complications. Therefore, family empowerment is important for diabetes management. Objective: This study aims to determine factors related to diabetes management, inhibiting, and supporting factors for diabetes management to develop a family-based educational model in diabetes management through expert testing.Methods: This research used Mixed Methods design with an Explanatory Sequential Design approach. A cross-sectional approach to 207 people with DM selection using a multistage sampling technique. Data was collected by questionnaire, in-depth interview, and Delphi study. Data analysis using chi-square and logistic regression. The development of family empowerment model is based on the theory Ladder of Empowerment, Social Cohesion, and Family Support. Results: Research results showed that good knowledge of patients, good family empowerment, and absence of DM risk factors are related to diabetes management. Model development and expert testing (Delphi Study) of the model find that a family-based diabetes management module is needed. Conclusion: A good family-based diabetes management module includes aspects of Self-Instructional, Self-Contained, Stand Alone, Adaptive, and User-Friendly aspects. This module is expected to improve diabetes management for patients and their families so that diabetes complications can be prevented and the patient's quality of life increases.

Abstract

Background: Adolescents are a group of people with a fairly high vulnerability to reproductive health problems in Indonesia. To reduce vulnerability to reproductive health problems, the Youth Counseling Information Center Program (PIK-R) program was introduced, one of which was at Bali Mandara senior high school, but its utilization was still low. The purpose of this study was to determine the relationship between completeness of facilities and teacher support on the knowledge, attitudes, and behavior of using youth counseling information center.Subjects and Method: The cross-sectional survey research was conducted on all students in Bali Mandara senior high school from October 2017 to March 2018. A total of 118 students were selected by total sampling. The dependent variables were know­ledge, attitude, and behavior. The independent variables were youth counseling infor­mation center facilities and teacher support. Data were collected using a questionnaire and analyzed by a multiple logistic regression.Results: Factors related to student knowledge were completeness of facilities (aOR= 3.14; 95%CI= 1.25 to 7.85; p= 0.014) and teacher support (aOR= 3.22; 95% CI= 1.27 to 8.15; p= 0.013). Completeness of facilities (aOR= 9.26; 95% CI= 3.30 to 25.97; p= 0.001) and teacher support (aOR= 6.27; 95% CI= 2.23 to 17.59; p= 0.001) were associated with student attitude. While the factors related to the behavior of using youth counseling information center were the completeness of the facilities (aOR= 11.56; 95% CI= 3.96 to 33.70; p= 0.001) and teacher support (aOR= 4.59; 95% CI= 1.60 to 13.17; p= 0.005).Conclusion: The completeness of facilities and teacher support for the youth counseling information center program is related to the knowledge, attitudes, and behavior of using youth counseling information center.Keywords: youth counseling information center program, knowledge, attitude, behavior.Correspondence:Ketut Eka Larasati Wardana. Abstract

In Indonesia, the number of Covid-19 cases in pregnant women is 3.9% and the prevalence of morbidity and mortality of pregnant women with Covid-19 is still unknown. However, several studies have shown that pregnant women may be very susceptible to Covid-19 infection so that it does little to cause fear and anxiety in pregnant women. Perceptions and behaviors of pregnant women need to be explored to see how health behavior is in making decisions for ANC visits. This study uses a qualitative method with an exploratory approach. Primary data were obtained through in-depth interviews with 12 pregnant women. The informants were pregnant women in Tabanan Regency who experienced an increase in the number of ANC visits at the Puskesmas. Data collection was carried out during January-February 2021. The data were analyzed thematically and presented using a narrative approach. Knowledge of pregnant women about the benefits of ANC and how to spread Covid-19 plays a very important role in the attitude of pregnant women in carrying out ANC. Adaptations in the application of health protocols continue to be carried out, this is because they feel vulnerable to being exposed to Covid-19. The feeling of fear and anxiety they felt did not stop them from doing ANC during the pandemic. The perception of severity is felt because of the experience of previous pregnancies, but the support and motivation makes pregnant women feel more able to avoid feelings of anxiety. ANC behavior of pregnant women can be seen in the selection of ANC locations that are felt to be safe and not crowded, such as Puskesmas and midwives. This study reveals that the anxiety of pregnant women during the Covid-10 pandemic can lead to levels of depression and anxiety if left untreated. There is a need for psychosocial support and information through online media for pregnant women during the Covid-19 pandemic. Keywords: perception, pregnant women, ANC, Covid-19

Abstract

The research method used an exploratory study design. Data were collected using in-depth interviews and direct observation. The research data were analyzed using thematic methods. The performance of midwives, seen from the input aspect, is lacking, where there is an additional workload for midwives, most of the midwives have not attended training, and there is a lack of examination room infrastructure. From the process aspect, there is lacking of supervision that more focus on early detection of high-risk pregnancies. The output aspect, the midwife's performance achievement was lacking in the two sample Puskesmas as seen from the coverage of each program indicator that had not yet reached the SPM target and the Kupang City Strategic Plan. It is necessary to increase the competence of midwives through integrated antenatal training and early detection of high-risk pregnancies that will support their performance achievement in ANC services

Abstract

Oral health in children plays a very important role as it lays down the foundation for healthy permanent teeth. Parents’ knowledge have impact on maintenance of young children’s oral health. Purpose: To explore dental health knowledge of preschool children’s parent, so can be designed the right education media for them. Explorative study was conducted to 129 mother of 4-6 year old from four kindergarten-schools was chosen by quote sample at the Saraswati Denpasar Foundation. Data were collected using questionnaires and structured interview. The parents have good knowledge of some aspect of oral health, but lacked knowledge about the oral hygiene maintain and transmission of dental infections. Their good knowledge is not supported by positive support behaviors of oral health for their children.All of mother using whats app (WA) social media. Preschoolers parents have good dental health knowledge, but it is necessary to increase awareness and behavior of oral health in their children considering their health of primary teeth is very important. WA can be used as an educational medium.
