Abstract

Background: Diabetic foot ulcer (DFU) is a chronic complication of diabetes mellitus (DM) and remains a significant challenge in morbidity, mortality, and disability among diabetic patients. This study aims to identify the sociodemographic and clinical characteristics of hospitalized DFU patients at Raden Mattaher General Hospital, Jambi. Method: This retrospective cross-sectional study was conducted at Raden Mattaher General Hospital, Jambi, Indonesia. A total of 31 patients data were collected from the medical records of DFU patients hospitalized at Raden Mattaher General Hospital in 2022. The variables observed were age, sex, residence, comorbidities, type of DM, random blood glucose levels (RBG), amputation rate, mortality, and length of stay (LOS). Results: The mean age of DFU patients was 55.6 ± 12.6 years, with females accounting for the majority (67.7%). Most patients lived outside Jambi City (74.2%), with more than one comorbidity (48.4%). All patients had type 2 diabetes mellitus (100.0%) with RBG level of 303.8 ± 205.8 mg/dL (mean±SD), amputation rate (19.4%), mortality rate (12.9%), and the mean LOS was 10.3 ± 6.9 days. Conclusion: This study indicates that most DFU patients have multiple comorbidities and poorly controlled blood glucose levels, which contribute to a higher risk of amputation, mortality, and prolonged hospitalization.

Abstract

Dadap serep stems contain secondary metabolite compounds of flavonoids, saponins, isoflavonoids, alkaloids and lectins which can act as reducing agents in the biosynthesis of nanosilver. This research aims to determine the effect of the concentration of liquid extract of dadap serep stem on the green synthesis silver nanoparticle (AgNPs) process. The aqueous extract of dadap serep stem reacted with one mM AgNO3 in a ratio of volume 1: 1 (FI), 1: 2 (FII), and 2: 1 (FIII). Characterization of AgNPs includes Surface Plasmon resonance (SPR) using UV/Vis spectrophotometer at wavelengths of 300-700 nm, particle size, and zeta potential using a particle size analyzer (PSA). The result showed that the SPR of AgNP values is 429-436 nm. AgNPs FI, II, and III particle sizes were 66.13 nm, 75.76 nm, and 75.96 nm, respectively. The PDI values below 0.5 confirmed that the distribution of nanoparticles was uniform. The most stable nanoparticle is Formula I.

Abstract

Facial masks are cosmetic products for topical use on facial skin. Using organic components from nature can prevent irritation and skin pigment damage due to the side effects of harmful chemicals in cosmetic products in the global market. This study aims to develop a combination organic mask formulation from Moringa leaves and turmeric and test its effectiveness. This experimental study, which had a one-shot case study design, was conducted at the Pharmaceutical Technology Laboratory and Phytochemical Laboratory, Pharmacy Department, Health Polytechnic, Ministry of Health, Jambi. The formulation consisted of Moringa leaf powder and turmeric with formulas F1 (40%, 30%), F2 (50%, 25%) and F3 (60%, 20%). Further data analysis used the paired t-test. The evaluation consists of phytochemical screening, characteristic tests of the organic mask (organoleptic, pH, drying time, and homogeneity), stability tests (organoleptic, fungal growth), effectiveness tests of mask preparations (skin moisture), and antioxidant activity tests. Based on the results, formula 3 showed the best results as an organic mask preparation with moisture test results of 58.87 ± 6.24 (mean ± SD), p <0.001. The antioxidant activity results showed IC50 values of each formula were F1 (73.43 µg/ml), F2 (56.33 µg/ml), and F3 (182.11 µg/ml). This study recommends that Moringa leaves and turmeric have potential and effectiveness as antioxidant compounds that can be developed as organic mask preparations.

Abstract

During the COVID-19 pandemic, the mandatory lockdown and government policy in prioritizing COVID-19 patients affected patients with diabetic foot ulcers (DFU). Most DFU patients experienced difficulties visiting hospitals and covering the cost of treatment, which led to changes in patient characteristics, poor clinical outcomes, and higher total costs. This study aimed to determine the sociodemographic and clinical outcomes profile of DFU patients under National Health Insurance (NHI-DFU) during the COVID-19 pandemic and their effect on the total direct medical cost. This cross-sectional study enrolled all JKN-DFU hospitalized patients at the National Referral Hospital, Cipto Mangunkusumo Hospital (RSCM), Jakarta, Indonesia, during COVID-19 pandemic (March 2nd, 2020, to December 2022). We used electronic health records, electronic billing, and foot registry systems. Participants were over 18 years old in a single admission period. Statistical analysis used Mann-Whitney and Kruskal-Wallis; Post hoc used the Mann-Whitney test. There were 158 JKN-DFU patients with predominated criteria: female patients (55.1%), aged over 60 years old (55.1%) with a mean age of 59.3 ± 11.0 years, unemployed (63.9%), intermediate educated (41.8%), Jakarta residents (68.4%), and mostly severe infection (63.3%). The total cost was much higher than INA-CBG’s coverage. The occupation (p=0.048), length of stay (LOS) (p=0.001), and amputation status (p<0.001) significantly affected total cost. Civil servants, longer LOS, and major amputation status leading higher total costs.
