Abstract

Along with the increase in tuberculosis (TB) cases, there has also been a rise in the number of Drug-Resistant Tuberculosis (DR-TB) cases, which poses a serious threat to global TB control efforts. Globally, there are 400,000 DR-TB cases according to the WHO report in 2024, with Indonesia contributing 7.4% of the total cases. In North Sumatra, the incidence of DR-TB cases reached 54%. This study aims to analyze and identify the factors influencing the incidence of DR-TB among DR-TB patients at the Special Lung Hospital of the Provincial Government of North Sumatra. The study uses a quantitative approach with a case-control design utilizing secondary data from the SITB application at the Special Lung Hospital of the Provincial Government of North Sumatra from 2021-2024. The sample consists of 204 individuals with a 1:1 ratio, comprising 102 individuals with DR-TB and 102 controls with non-DR-TB. The sampling technique used is purposive sampling. This study sets DR-TB incidence as the dependent variable, while independent variables include sex, employment status, age, treatment outcome, access to health service facilities, residence, contact examination, TB treatment history, diagnosis type, diabetes mellitus status, and HIV status. Data were analyzed using three tests: univariate analysis, bivariate analysis, and multivariate analysis with the use of statistical software version 22. The results show that factors significantly influencing DR-TB incidence include age (CI 95%: 4.7 [2.3-9.5]), employment (CI 95%: 3.4 [1.7-6.6]), and a history of previous TB treatment (CI 95%: 14.7 [5.5-39]).

Abstract

Neonatal tetanus remains a significant cause of infant mortality, particularly in developing countries, despite being preventable through Tetanus Toxoid (TT) immunization during pregnancy. This study aimed to analyze the factors influencing the utilization of TT immunization among pregnant women at Terjun Public Health Center, Marelan District. Using a cross-sectional quantitative approach, data were collected from 46 third-trimester pregnant women through questionnaires. Bivariate and multivariate analyses were conducted using chi-square tests and logistic regression. The results showed that knowledge (p=0.002) and attitude (p=0.001) were significantly associated with TT immunization uptake, while age (p=0.536) and spousal support (p=0.603) were not. Multivariate analysis revealed that attitude was the dominant factor influencing immunization utilization (OR=4.7; p=0.045). Although knowledge had a higher odds ratio (OR=5.59), it did not reach statistical significance (p=0.056). The model explained 38.9% of the variation in immunization status (Nagelkerke R² = 0.389). These findings underscore the importance of promoting positive maternal attitudes and improving knowledge through targeted health education to enhance immunization coverage and prevent neonatal tetanus

Abstract

This study aims to describe the implementation of Hospital Health Promotion (PKRS) at the Special Pulmonary Hospital Technical Implementation Unit of North Sumatra Province in 2025. The background of the study is based on the importance of PKRS in increasing the awareness, attitude, and healthy behavior of patients and their families, as well as supporting the achievement of public health status. The study uses a qualitative approach with observation and in-depth interviews with PKRS officers, as well as a review of hospital documents. The results of the study indicate that the implementation of PKRS has been going quite well, marked by the existence of a special PKRS unit and support from hospital management through the preparation of SOPs and regular monitoring and evaluation. Health promotion activities are carried out through individual or group counseling, as well as utilizing print, electronic, and social media. Thus, the study found several obstacles, including budget limitations, infrastructure, and patients' limited time to participate in education. The program's accessibility is considered quite good because health information can be accessed through various hospital services and social media. Overall, the PKRS at UPTD RS Khusus Paru PROVSU has been implemented according to standards, although it still requires reinforcement in terms of resources and budget support to reach a wider audience and be sustainable.
