Abstract

Introduction: The reimbursement mechanism is a fundamental component in an insurance-based financing system that affects service effectiveness, operational efficiency, and patient satisfaction. Objective: This study aims to conduct a systematic review of the literature that discusses reimbursement mechanisms with a focus on these three main dimensions. Method: Using the Systematic Literature Review approach following the PRISMA 2020 guidelines, a comprehensive search was conducted through four electronic databases to identify peer-reviewed articles for the 2021-2025 period. Of the 30 articles initially identified, six studies met the inclusion criteria and were analyzed in depth. Results and Discussion: Results show that the Diagnosis Related Groups system is effective in reducing costs but has the potential to negatively impact the quality of service. Differential reimbursement and cost-sharing mechanisms have been shown to be able to direct healthcare seeking behavior, although their effectiveness is highly contextual dependent on geographic and socio-economic factors. Patient satisfaction shows significant variation by service class with disparities in the dimensions of effectiveness, accessibility, patient-centeredness, and equity. The utilization rate of health services in Indonesia's National Health Insurance system is still relatively low, indicating the need for a comprehensive evaluation of the design of reimbursement mechanisms. Conclusion: This study emphasizes that optimizing the reimbursement system requires a holistic approach that balances cost efficiency, service quality, and patient satisfaction as interrelated outcomes in the health financing ecosystem

Abstract

Introduction: Service quality is a key determinant of patient satisfaction in hospital settings, particularly for National Health Insurance (BPJS) participants. Raden Mattaher Hospital Jambi, as a referral hospital, experiences increasing BPJS inpatient visits. However, complaints and fluctuations in satisfaction index indicate potential service quality gaps. This study aimed to analyze the influence of service quality dimensions on BPJS inpatient satisfaction. Objective: This study aimed to analyze the influence of service quality dimensions on BPJS inpatient satisfaction. Method: This quantitative study used a cross-sectional design conducted in the inpatient wards of Raden Mattaher Hospital in 2025. A total of 424 BPJS inpatients were selected using purposive sampling. Data were collected using a SERVQUAL-based Likert questionnaire covering tangibles, reliability, responsiveness, assurance, and empathy. Data were analyzed using univariate, bivariate (simple linear regression), and multivariate (multiple linear regression) analysis with SPSS at α=0.05. Result and Discussion: Most respondents rated service quality as good (58.3%). The mean satisfaction score was 79.10±12.71. Partially, reliability (p=0.048) and assurance (p=0.000) significantly influenced satisfaction, while tangibles (p=0.312), responsiveness (p=0.087), and empathy (p=0.421) were not significant. Simultaneously, all dimensions significantly affected satisfaction (p=0.000) with R²=0.106. Assurance was the dominant factor (β=1.238). Conclusions: Service quality significantly influences BPJS inpatient satisfaction, with assurance as the strongest determinant. Strengthening professional competence and patient trust is essential to improve satisfaction levels.

Abstract

Introduction: Mobile JKN is a digital innovation developed by BPJS Kesehatan to improve healthcare access for National Health Insurance (JKN) participants. However, its utilization among informal workers remains suboptimal. Objective: This study aimed to analyze the determinants of Mobile JKN utilization in supporting healthcare access among informal workers. Method: A cross-sectional study was conducted among 126 informal workers who were JKN participants in the working area of Kenali Besar Primary Health Center, Jambi City, Indonesia. Respondents were selected using accidental sampling. Data were analyzed using chi-square tests and multiple logistic regression. Results and Discussion: A total of 58.73% of respondents had utilized the Mobile JKN application. Significant associations were found between perceived ease of use (p=0.002), perceived usefulness (p=0.006), behavioral intention (p=0.022), attitude toward use (p<0.001), digital literacy (p=0.038), healthcare service needs (p=0.007), and Mobile JKN utilization. Multivariate analysis identified attitude toward use as the most dominant determinant (aOR=3.19; 95% CI=1.38–7.36; p=0.006). Conclusion: Perceptions and behavioral factors significantly influence Mobile JKN utilization. Improving users' attitudes toward the application may increase its utilization among informal workers

Abstract

Fenomena underreporting insiden keselamatan pasien masih menjadi tantangan utama dalam sistem keselamatan pasien di berbagai fasilitas pelayanan kesehatan. Rendahnya pelaporan insiden tidak hanya menunjukkan hambatan teknis, tetapi mencerminkan lemahnya budaya organisasi yang mendukung keterbukaan dan pembelajaran dari kesalahan. Menganalisis peran budaya keselamatan pasien, khususnya just culture, dukungan manajemen, dan respons nonpunitif dalam mengatasi fenomena underreporting insiden keselamatan pasien di rumah sakit melalui tinjauan literatur sistematis. Penelitian ini menggunakan desain Systematic Literature Review (SLR) dengan panduan PRISMA 2020. Pencarian literatur dilakukan pada lima basis data (Scopus, PubMed, ScienceDirect, Scite, Google Scholar) menggunakan kombinasi kata kunci terstruktur. Dari 500 artikel teridentifikasi, sebanyak 27 artikel memenuhi seluruh kriteria inklusi dan dianalisis menggunakan pendekatan sintesis naratif. Hasil sintesis menunjukkan bahwa just culture, dukungan manajemen, dan respons nonpunitif merupakan determinan organisasi yang secara sinergis berkontribusi dalam meningkatkan pelaporan insiden dan mengurangi fenomena underreporting. Penerapan just culture membangun persepsi keadilan dan keamanan psikologis; dukungan manajemen memperkuat komitmen organisasi; dan respons nonpunitif menghapus ketakutan tenaga kesehatan terhadap sanksi sebagai hambatan utama pelaporan. Upaya mengatasi underreporting memerlukan pendekatan sistemik yang mengintegrasikan transformasi budaya organisasi. Rumah sakit perlu mengembangkan kebijakan yang mendukung just culture, memperkuat komitmen pimpinan, dan menjamin bahwa pelaporan insiden dipandang sebagai sarana pembelajaran, bukan sebagai dasar pemberian sanksi.

Abstract

Stunting remains a significant chronic nutritional challenge in Indonesia. Early Initiation of Breastfeeding (EIB) and Exclusive Breastfeeding (EBF) are critical interventions that support child growth and serve as foundational strategies for stunting prevention. This study aims to investigate the relationship between a history of EIB and EBF and the severity of stunting (classified as stunted and severely stunted) among affected children under five within the catchment area of Public Health Center "X" in Indragiri Hilir Regency.An observational analytic design with a cross-sectional approach was employed, utilizing secondary data from the 2025 Stunting Audit. The study population consisted of 45 stunted children under five, selected through a total sampling technique. Data were analyzed using the Chi-Square test with a 95% confidence level (α=0,05) and the calculation of Odds Ratios (OR).The results indicated that the majority of respondents were categorized as severely stunted (66.7%), had a history of EIB (55.6%), and received EBF (55.6%). No significant correlation was found between a history of EIB and stunting severity (p=0,670; OR=1,313; 95%CI=0,373–4,616). Conversely, a significant association was observed between EBF and stunting severity (p=0,034; OR=0,250; 95%CI=0,067–0,931). These findings suggest that exclusive breastfeeding serves as a protective factor against increased stunting severity, whereas EIB did not demonstrate a significant relationship in this specific population. Consequently, enhancing EBF coverage must be prioritized as a pivotal component of stunting prevention and control programs.

Abstract

Entrepreneurial readiness is increasingly shaped by psychological factors that enable individuals to adapt and persist in uncertain conditions. Previous, studies on this topic have largely examined psychological factors such as mental health, resilience, and adversity quotient in isolation, resulting in a fragmented understanding of their combined influence on entrepreneurial outcomes.Therefore this study aims to address the stated gap by integrating the outlined constructs within a unified framework. In order to achieve this objective, a sequential mixed-method design was adopted, combining bibliometric analysis of 356 Scopus-indexed documents (2010–2025) with Structural Equation Modeling (PLS-SEM) based on data from 412 health students. The bibliometric findings identified resilience, adversity quotient, and mental well-being as growing themes in entrepreneurship exploration. SEM results showed that adversity quotient had the strongest effect on entrepreneurial readiness (β=.734, p<.001), followed by resilience (β=.466, p<.001) and mental health (β=.260, p=.046). Resilience also significantly mediated these relationships. TCollectively, these findings suggest that resilience, adversity quotient, and mental health are foundational components of entrepreneurial readiness and should be integrated into entrepreneurship education and student development programs.

Abstract

Introduction: Compliance with National Health Insurance premium payments is essential for ensuring the sustainability of Indonesia's health insurance system. However, many independent participants fail to pay premiums on time. Objective: This study aimed to identify factors associated with premium payment compliance among Non-Wage Recipient Participants in Jambi City. Method: A quantitative cross-sectional study was conducted among 106 participants selected through random sampling. Data were collected using a structured questionnaire and analyzed using descriptive statistics and the Chi-square test with a significance level of 0.05. Results and Discussion: Employment, knowledge, income, payment method, history of catastrophic disease, perceptions of healthcare services, and family support were significantly associated with premium payment compliance. Participants with stable employment, better knowledge, higher income, convenient payment methods, positive healthcare service perceptions, a history of catastrophic disease, and stronger family support were more likely to comply with premium payments. These findings indicate that compliance is influenced by multiple socioeconomic, behavioral, and healthcare-related factors. Conclusion: Strengthening health education, improving healthcare service quality, expanding accessible payment channels, and increasing family support may enhance premium payment compliance and contribute to the sustainability of the National Health Insurance program

Abstract

Purpose: Despite the National Health Insurance (JKN) coverage, outpatient service utilization remains suboptimal. In 2023, only 48.20% of the Indonesian population utilized health insurance for primary care. This preliminary study aims to provide an initial exploration of the factors related to health service utilization among subsidized beneficiaries (PBI) at Public Health Centers (Puskesmas) in West Tanjung Jabung District. Methods: This quantitative study employed an analytical cross-sectional survey. The research was conducted at Merlung and Suban Puskesmas. A sample of 106 PBI participants was selected from a population of 26,043 using proportional random sampling. Data were collected using a validated and reliable questionnaire, then analyzed using univariate and bivariate (chi-square) tests. Results: The study found that 62.3% of respondents did not utilize health services in the last month (utilization rate 37.7%). Bivariate analysis revealed that significant factors associated with service utilization were JKN knowledge (p=0.001), employment status (p=0.003), perception of illness (p=0.035), history of chronic disease (p=0.000), and accessibility of information (p=0.041). Conversely, age (p=0.051), education level (p=0.18), and geographical distance (p=0.534) showed no significant correlation. Conclusion: The study identifies several significant factors related to utilization in a bivariate context. As a preliminary finding, these results provide a baseline for future multivariate research to determine the independent predictors of healthcare access among the PBI population.

Abstract

Stunting in children aged 24 to 59 months is a health issue related to parenting patterns, social conditions, and family habits in meeting children's needs. This research aims to describe the factors related to the occurrence of stunting based on a review of six field research articles published between 2020 and 2025. Systematic review was conducted by searching articles in PubMed, Google Scholar, and Science Direct databases using keywords "stunting", "factors", "children 24 to 59 months", “health”, and "economy" with inclusion criteria: journal articles from the last 5 years, full text, English/Indonesian language. The results found 6 articles that meet the criteria. The results show that the occurrence of stunting in children aged 24 to 59 months is related to the dynamics of family life, starting from parenting patterns, feeding habits, social conditions, to the household economic situation. Abstract

Low back pain (LBP) adalah nyeri yang dirasakan pada daerah punggung bawah di daerah diskus intervertebralis lumbal dan sakralis (L5-S1). Faktor resiko low back pain, yaitu Usia, Sikap, Durasi kerja, masa kerja. Penelitian ini bertujuan untuk menganalisis hubungan antara Usia, sikap kerja, durasi kerja, dan masa kerja pada pengendara ojek online di Kota Jambi. Penelitian kuantitatif dengan desain observasional merupakan metodologi yang digunakan. Tujuan dari pendekatan cross sectional adalah untuk mengetahui hubungan antara usia, sikap kerja, durasi kerja, dan masa kerja dengan resiko terjadinya Low Back Pain (LBP) pada pengendara ojek online di Kota Jambi. Teknik pengambilan sampel dalam penelitian ini adalah Accidental sampling mengambil responden yang kebetulan ditemui di suatu tempat yang sesuai dengan konteks penelitian. Jumlah sampel yaitu 100 sampel yang merupakan pengendara Ojek Online di Kota Jambi. Hasil penelitian terhadap 72 responden yang mengalami keluhan (LBP) dan 28 orang yang tidak mengalami keluhan low back pain (LBP). Ditemukan hubungan yang signifikan antara variabel durasi kerja dan masa kerja dengan keluhan (LBP). Tidak ditemukan hubungan yang signifikan antara variabel usia dan sikap kerja hal ini disebabkan oleh homogenitas postur berkendara responden. Dari hasil tersebut dapat disimpulkan bahwa lebih banyak pengendara ojek online di Kota Jambi yang mengalami keluhan LBP dari pada yang tidak mengalami keluhan LBP.
