Abstract

Latar belakang: TNF-α berperan sentral dalam patogenesis tuberkulosis (TB) paru, namun data deskriptif lokal mengenai kadar serum pada pasien terkonfirmasi molekular masih terbatas. Tujuan: Mendeskripsikan profil kadar serum TNF-α pada pasien TB paru terkonfirmasi Xpert MTB/RIF di RS Paru Sumatera Barat. Metode: Studi potong lintang deskriptif pada pasien poliklinik. Darah vena diproses menjadi serum dan kadar TNF-α diukur dengan ELISA sesuai protokol pabrikan. Analisis terbatas pada statistik deskriptif—rerata±SD. Hasil: Sebanyak 40 pasien dianalisis; usia 45,5 ± 15,7 tahun (rentang 18–75). Kadar serum TNF-α 150,84 ± 96,61 pg/mL, median 121,72 pg/mL, rentang 79,63–517,36 pg/mL; sebaran cenderung menceng ke kanan dengan heterogenitas antarindividu yang nyata. Kesimpulan: Pada pasien TB paru terkonfirmasi Xpert, kadar serum TNF-α menunjukkan nilai rerata yang tinggi dengan variasi luas, mencerminkan heterogenitas respons inflamasi sistemik. Temuan ini menyediakan data dasar bagi populasi setempat dan dapat menjadi acuan untuk studi longitudinal dan panel biomarker multimarker ke depan.

Abstract

Latar Belakang: Ibadah haji menuntut aktivitas fisik tinggi, paparan panas, dan perubahan pola makan/obat yang dapat memicu disglikemia. Pemetaan profil glikemik pra-keberangkatan diperlukan untuk perencanaan pembinaan klinis calon jemaah haji (CJH). Tujuan : Menilai profil glikemik CJH 2025 di RSU Aisyiyah Padang menggunakan glukosa puasa (FPG), glukosa 2 jam post-prandial (2JPP), dan HbA1c; serta menggambarkan karakteristik dasar dan keterkaitan antarmarker. Metode : Studi potong lintang pada CJH yang menjalani pemeriksaan rutin tahun 2025. Dikumpulkan data demografi, FPG, 2JPP, dan HbA1c. dievaluasi dengan Spearman.Hasil: Sebanyak 30 peserta (usia rata-rata 57,9±13,6 tahun; perempuan 70%). Rerata (±SD) FPG 102,9±34,2 mg/dL; 2JPP 142,3±50,7 mg/dL; HbA1c 6,1±1,4%. Kategori FPG: normal 56,7%, prediabetes 23,3%, diabetes 20,0%. Kategori 2JPP: normal 63,3%, prediabetes 30,0%, diabetes 6,7%. Kategori HbA1c: normal 43,3%, prediabetes 36,7%, diabetes 20,0%; HbA1c ≥7% pada 13,3%. Kesimpulan: Beban gangguan glikemik pra-keberangkatan pada CJH cukup bermakna, dengan proporsi prediabetes tinggi dan sepertiga respon “diabetes” menurut sebagian marker. Penilaian kesiapan medis sebaiknya mengintegrasikan multi-parameter (FPG, 2JPP/OGTT, HbA1c), disertai edukasi dan optimasi terapi. Ketidaksesuaian antarmarker perlu konfirmasi diagnostik, khususnya karena 2JPP klinik tidak ekuivalen dengan OGTT terstandar.

Abstract

Latar Belakang : Kanker serviks merupakan salah satu kanker terbanyak pada perempuan di Indonesia dan sering terdiagnosis pada stadium lanjut. Deteksi dini dan edukasi faktor risiko sangat penting dalam menurunkan angka kejadian dan mortalitas. Tujuan : Penelitian ini bertujuan untuk mengidentifikasi faktor risiko kanker serviks pada wanita usia subur berbasis data klinis pasien serta mengintegrasikannya dalam kegiatan edukasi masyarakat. Metode: Kegiatan dilaksanakan di RSUP Dr. M. Djamil Padang pada September 2020–April 2021 dengan desain deskriptif analitik cross sectional. Populasi penelitian adalah 70 pasien kanker serviks yang dipilih dengan teknik simple random sampling. Data yang dikumpulkan meliputi usia menikah, jumlah paritas, serta lama penggunaan kontrasepsi hormonal. Kegiatan pengabdian meliputi penyuluhan, diskusi kelompok, distribusi leaflet, dan informasi mengenai skrining deteksi dini (Pap smear/IVA). Hasil: Sebanyak 42,9% pasien menikah pada usia <20 tahun, dan 80% pasien adalah multipara. Lama penggunaan kontrasepsi hormonal terbanyak <5 tahun (55,7%) dengan jenis terbanyak kontrasepsi suntik (62,9%). Sebagian besar pasien terdiagnosis pada stadium lanjut: stadium III (47,1%) dan stadium II (30%). Edukasi melalui penyuluhan dan distribusi media informasi berhasil meningkatkan pemahaman peserta mengenai faktor risiko dan pentingnya deteksi dini. Kesimpulan: Faktor risiko yang dominan pada pasien kanker serviks adalah usia menikah muda, multiparitas, serta penggunaan kontrasepsi hormonal jangka panjang. Edukasi kesehatan berbasis data klinis pasien efektif meningkatkan kesadaran wanita usia subur terhadap pencegahan dan deteksi dini kanker serviks.

Abstract

The Scube1 gene is expressed in atherosclerosis (plaque buildup in blood vessels), while Scube2 is expressed in DIT (Diffuse Intimal Thickening) which is used to describe diffuse thickening of the intima of blood vessels. This study aims to determine the differences in the expression of the scube1 gene in blood samples of type II DM patients with dyslipidemia and type II DM with non-dyslipidemia and differences in the expression of the scube2 gene in blood samples of type II DM patients with dyslipidemia and type II DM with non-dyslipidemia. With a comparative cross-sectional research method, the sample size of this study was determined based on the Lameshow hypothesis test formula which obtained 18 blood samples of type II DM with dyslipidemia, and 18 blood samples of type II DM with non-dyslipidemia. Blood samples of type II DM patients with dyslipidemia and blood samples of type II DM patients without dyslipidemia showed significantly different expression of the scube1 gene, with a p-value of 0.000. Similarly, blood samples from type II DM patients with dyslipidemia and those from type II DM patients without dyslipidemia showed a significant difference in scube2 gene expression (p = 0.001). Thus, it can be said that the expression of the Scube1 and Scube2 genes is significantly influenced by the complications of diabetes mellitus with dyslipidemia. This happens because it is believed that hyperglycemia in type II DM contributes to endothelial cell dysfunction, which occurs prior to blood vessel damage and results in progressively more severe blood vessel problems. Along with hyperglycemia, type II diabetes also results in dyslipidemia, an alteration in the normal lipid profile. Both dyslipidemia and hyperglycemia contribute to macrovascular and microvascular problems in type II diabetes.

Abstract

The high incidence of Type 2 diabetes mellitus (DM) in the elderly in Padang is due to several factors, one of which is an unhealthy lifestyle, namely consumption of foods high in fat, so preventive and promotive efforts are needed in the form of early detection of DM risk factors as well as providing education and health education. to the public about matters relating to DM. The increase in the incidence of DM is related to the increase in the elderly population, economic development, decreased consumption of healthy foods, physical activity and obesity, especially central obesity. Elderly is an advanced stage of the life process that shows signs of decreased physical ability to adapt to environmental pressures which will be measured using the Global Physical Activity Questionnaire (GPAQ).

Abstract

Pulmonary tuberculosis remains a significant global public health challenge. In efforts to overcome this disease, a deeper understanding of the role of individual genetics, such as IL-10 genetic variation, in the response to M. tuberculosis infection is critical. Research that has been conducted shows that IL-10, which has an important role in regulating the immune response, can also influence the development of TB. Genetic variations in the IL-10 gene play a role in determining the extent of the immune response to TB infection and an individual's risk of this disease. The interaction between Treg cells, IL-10, and TB is also an important aspect in the pathogenesis and management of TB. Although Treg cells and IL-10 have a role in controlling excessive inflammation, too much of either can dampen the immune response needed to overcome infections. The implication of this research is that the development of more targeted and personalized therapy is an important step in overcoming TB. The use of individual genetic knowledge, such as IL-10 genetic variations, can help design more effective therapies and improve patient prognosis. However, challenges such as drug resistance and the complexity of genetic-immunological interactions remain challenges that need to be overcome in TB management. Overall, this study shows the importance of involving the fields of genetics and immunology in global efforts to address pulmonary tuberculosis. With a deeper understanding of the factors that influence the immune response to TB infection, we can hope to develop more effective strategies in the prevention, diagnosis and treatment of this disease and reduce the burden of TB worldwide.
