Abstract

Tuberkulosis (TB) merupakan penyakit menular yang masih menjadi masalah utama di Indonesia dan dunia. Tantangan utama dalam mendiagnosis TB secara konvensional yaitu rendahnya sensitivitas deteksi pada pemeriksaan mikroskopis dan lamanya waktu yang diperlukan untuk kultur. Penelitian ini bertujuan untuk mengevaluasi metode GeneXpert MTB/RIF untuk mendeteksiMycobacterium tuberculosis dengan sampel dahak langsung di RSUD Dr. Moewardi (RSDM), Surakarta. Analisis observasional dengan pendekatan kohort retrospektif menggunakan data sekunder hasil pemeriksaan GeneXpert MTB/RIF pada sampel dahak langsung di Laboratorium Mikrobiologi Klinik RSDM pada tahun 2012 2015. Sampel didapatkan dari pasien yang memenuhi kriteria suspek multidrug-resistant tuberculosis (MDR-TB) dan sampel tersebut telah dikultur di Balai Laboratorium Kesehatan (BLK), Jawa Tengah. Data dianalisis dengan OpenEpi versi 3, Epi Info 7, dan MedCalc. Pada penelitian ini didapatkan sensitivitas, spesifisitas, nilai duga positif dan negatif (NDP dan NDN) serta akurasi metode GeneXpert MTB/RIF sebesar 93,62%, 27,17%, 68,89%, 71,21%, dan 69,21%. Prevalensi TB paru pada sampel yang diperiksa sebesar 63,27%. Rendahnya spesifisitas GeneXpert MTB/RIF mengindikasikan perlunya kultur sebagai baku emas. Namun demikian, perlu standarisasi pemrosesan sampel dahak dalam segi teknik dan waktu pengambilan sampel disertai data klinis yang memadai untuk melihat riwayat terapi yang telah diberikan pada pasien.

Abstract

Background: Obesity is a risk factor for metabolic syndrome which can increase mortality. Obese sufferers often fail to lose body weight (BW) through non-pharmacological therapy. Obesity synthetic drugs can cause side effects, so natural ingredients are needed as alternative therapies.Objectives: This study aims to determine the effect of raja banana peel extract (RBPE) on BW, body mass index (BMI), body fat percentage (BFP), and visceral fat mass in obese male rats.Methods: The research subjects were 30 male Wistar rats weighing 125-200 g, aged eight weeks. Obesity was induced by being given high-fat high fructose (HFHFr) feed for 28 days. Rats were randomized and divided into five groups: the negative control group (C-) was given standard feed and distilled water, the positive control (C+) was given standard feed and orlistat, treatment 1 (T1), treatment 2 (T2), and treatment 3 (T3). Which were given standard feed and RBPE doses of 200 mg/kgBW/day, 400 mg/kgBW/day, and 800 mg/kgBW/day. Data before and after the induction of obesity were analyzed using paired t-tests. BW, BMI, and visceral fat mass were analyzed using the One-Way Analysis of Variance (ANOVA) and Repeated Measure ANOVA tests. BFP was analyzed using the Kruskal-Wallis test and the Friedman test.Results: RBPE can significantly reduce BW (p=0.026), BMI (p<0.001), and BFP (p<0.001). However, all groups had no significant difference in visceral fat mass (p=0.187). T3 was the group with the lowest average BW, BMI, BFP, and visceral fat mass although the highest weight loss during the intervention period occurred in C+.Conclusions: RBPE has the potential as an alternative therapy for obesity because it can reduce BW, BMI, and BFP. Future studies can investigate the effect of RBPE on other obesity parameters such as lipid profiles.

Abstract

The diabetic foot ulcer prevalence was 6.3% globally and 7.3% in Indonesia. This study analyzes the relationships between the type of germs and antimicrobial susceptibilities to the Wagner’s degree of diabetic-foot-ulcers on patients with type-2 diabetes mellitus admitted to Dr. Moewardi regional hospital. The method used in this observational analysis is a cross-sectional approach. The variables used were the type-of-germs, antimicrobial susceptibilities, and the degree of diabetic-foot-ulcers with the data extracted from 149 inpatients between the period of 2021-2022 and tested with Fisher’s-exact and multinomial-logistic-regressions. Data for the 4-most-frequent germs on the diabetic-foot-ulcers are Pseudomonas aeruginosa (17.4%; 26/149), Escherichia coli (12.8%; 19/149), Klebsiella pneumoniae ss. Pneumoniae (11.4%, 17/149) and Proteus mirabilis (10.7%; 16/149). Data for the 4-most-frequent antimicrobial susceptibilities are ampicillin (20,8%; 31/149), sulfamethoxazole (20,8%; 31/149), gentamicin (18,8%; 28/149) and amikacin (16,1%; 24/149). Data for the 3-most-frequent degree of diabetic-foot-ulcers are the 4th (39.6%; 59/149), 3rd (31.5%; 47/149) and 2nd (19.5%; 29/149) degrees. There is no relationship between the type-of-germs and the degree of diabetic-foot-ulcers (p=0.111). There is a relationship between antimicrobial susceptibilities and the degree of diabetic-foot-ulcers (p=0.009). There is a relationship between the type-of-germs and antimicrobial susceptibilities (p=0,0001). There is a simultaneous relationship between the type-of-germs (p=0.029) and antimicrobial susceptibilities (p=0.032) to the degree of diabetic-foot-ulcers (p=0.005). Therefore, it is necessary to conduct microbial culture and antimicrobial susceptibility tests on all patients with diabetic foot ulcers.

Abstract

: Covid-19 pandemic has caused dramatic changes in daily activities. In particular, human interactions were limited, and most daily duties were directed into an online environment. This study aimed to describe the characteristics of research protocols submitted to the Research Ethics Committee (REC), Faculty of Medicine, Universitas Sebelas Maret (UNS), and evaluate the effectiveness of ethical review procedures implemented during the pandemic era. The following data were retrieved: protocol identity, researcher's institution, researcher's educational background, type of study, the date of protocol submission, and the date when the ethical clearance (EC) certificate was issued. Two hundred seventeen research protocols were submitted to the REC from 1 January 2021 until 30 June 2022. Most researchers were university students, lecturers, physicians, and service providers. Observational studies were more prevalent than interventional studies. The ethical reviews were usually completed within 2-3 weeks unless a few protocols required major revisions. Most applicants and reviewers were satisfied with the process of online review. The REC secretariat played a crucial role in keeping everyone working on schedule. In conclusion, restricting direct human interactions during the COVID-19 pandemic does not affect the ethical review process. With familiar tools, online review becomes more effective than face-to-face meetings.

Abstract

Pendahuluan: Jumlah kasus penyakit kardiovaskular (PKV) di dunia menunjukkan kecenderungan meningkat termasuk di Indonesia. Kabupaten Sukoharjo di Jawa Tengah melaporkan adanya peningkatan penyakit hipertensi dan diabetes mellitus (DM) yang menjadi faktor risiko penting PKV. Penyakit hipertensi dan DM merupakan penyakit penyerta utama pada kasus coronavirus disease 2019 (COVID-19) sehingga dapat memperburuk gejala dan menyebabkan kematian. Tujuan dari kegiatan pengabdian ini adalah memberikan edukasi tentang pencegahan dan deteksi dini PKV di wilayah Kecamatan Kartasura, Sukoharjo, Jawa Tengah di masa pandemi COVID-19.Metode: Kegiatan pengabdian dilaksanakan di RT 03 RW 02 desa Gowongan, Pucangan, Kartasura, Sukoharjo, Jawa Tengah. Metode pengabdian yang dilakukan berupa pelatihan pengukuran denyut nadi latihan, talk show  mengenai pencegahan dan deteksi dini PKV serta pemeriksaan kesehatan berupa pemeriksaan tekanan darah dan kadar gula darah.Hasil dan pembahasan: Hasil kegiatan pengabdian ini peserta mendapatkan informasi yang valid mengenai pencegahan dan deteksi dini PKV. Selain itu, kegiatan ini menstimulasi keperdulian peserta untuk melakukan upaya pencegahan dan deteksi dini melalui pemeriksaan tekanan darah dan kadar gula darah. Hasil lain yang didapatkan adalah peserta dapat melakukan pengukuran denyut nadi latihan secara mandiri untuk mengoptimalkan olahraga yang dilakukan.Kesimpulan: Edukasi pencegahan dan deteksi dini PKV kepada masyarakat talkshow, pelatihan dan layanan kesehatan telah terlaksana dengan baik. Kegiatan ini dapat memberikan informasi valid kepada masyarakat dan memacu keperdulian masyarakat untuk melakukan upaya pencegahan dan deteksi dini PKV.

Abstract

Data Riset Kesehatan Dasar (Riskesdas) di Indonesia secara nasional memperlihatkan bahwa penyakit tidak menular (PTM) terutama penyakit kardiovaskular (PKV) cenderung mengalami peningkatan. Hal tersebut terjadi juga di wilayah kota Surakarta. Tim Pengabdi berinisiatif melaksanakan kegiatan edukasi dan pemeriksaan kesehatan yang bertujuan untuk meningkatkan pengetahuan anggota Klub Jantung Sehat (KJS) terkait PKV sekaligus untuk skrining faktor risiko PKV yang meliputi hipertensi, hiperkolesterol, diabetes mellitus (DM), dan obesitas. Metode yang digunakan berupa penyuluhan dan pemeriksaan tekanan darah, gula darah sewaktu, kolesterol dan antropometri. Kegiatan pengabdian masyarakat ini diikuti oleh 85 anggota KJS dan diselenggarakan pada hari Minggu, 18 Juni 2023 jam 07.00 - 09.00 di Fakultas Keolahragaan Universitas Sebelas Maret (FKOR UNS) Jl. Menteri Supeno, Manahan, Banjarsari Kota Surakarta Jawa Tengah. Dari kegiatan pengabdian masyarakat ini pemahaman para anggota KJS terkait PKV melalui diskusi dan tanya jawab selama penyuluhan dan konseling selama pemeriksaan kesehatan mengalami peningkatan. Kegiatan sejenis perlu dilakukan secara rutin dan berkesinambungan sebagai upaya preventif dan promotif terhadap penyakit tidak menular di masyarakat.

Abstract

Indonesia currently is experiencing a shift in disease patterns marked by a significant increase in the number of non-communicable diseases (NCDs), especially cardiovascular diseases, specifically diabetes mellitus (DM). A similar phenomenon also occurs in Surakarta city. Community service team conducts educational activities and health checks that aim to increase the knowledge of members of the Klub Jantung Sehat (KJS) related to cardiovascular disease as well as for screening risk factors for cardiovascular diseases including hypertension, hyperuricemia, diabetes mellitus (DM), and obesity. The methods used are counseling and checking blood pressure, uric acid, blood sugar at any time, and anthropometry. This community service activity was attended by 33 KJS members and was held on Sunday, 23 June 2024 at 07.00 - 09.00 at the Faculty of Sports, Sebelas Maret University (FKOR UNS) Jl. Menteri Supeno, Manahan, Banjarsari, Surakarta City, Central Java, however only 19 participants took part in the complete screening. The results achieved from this community service are an increase in the knowledge of KJS members related to cardiovascular diseases through discussions during counseling as well as during health checkups. The results of the health examination, it was found that 6 people (31.58%) had hypertension, 2 people (10.52%) had DM, and 11 people (57.89%) were found to meet the criteria of overweight and obesity. The members of KJS who had uric acid levels above normal were 10 people (52.63%). Similar activities should be carried out regularly and continuously as a preventive and promotive effort against non-communicable diseases in the community.
