Abstract

Premature rupture of membranes is a rupture of the amniotic membrane before delivery. It occurs in 3% of pregnancies and carries a risk of preterm delivery. Each year there are 15 million babies born and more than 1 in 10 babies born prematurely (gestation less than 37 weeks). Obtained patient Mrs. IR, 36 years old, came with complaints of preterm pregnancy accompanied by water-discharge. This complaint was felt since two days before entered the hospital, clear color, odor less and changed sanitary napkins three times a day. The patient appeared to be moderately ill, consciousness compos mentis, blood pressure 118/87 mmHg, pulse 83x/minute, breathing 23x/minute and, temperature 36 ° C. The generalist status of the patient was within normal limits. On obstetric examination, there was a TFU of 3 fingers above the umbilicus (24 cm). The lower part of the mother's fetus was round, bouncy, hard with the impression of the head, not yet in PAP. His (-), fetal heart rate 148x/minute. Inspeculo examination found closed OUE, fluorine -, fluxus: +, amniotic (+) inactive, erosion / laceration / poly (-), red litmus (+) to blue. The patient was diagnosed as 28 weeks pregnant G1P0A0 with promiscuity of single live fetal head presentation and oligohydramnios. To prevent preterm labor in this case, conservative therapy is given in the form of antibiotics (prevent in increase dinfection), corticosteroids (fetal lung maturation), and tocolytics (suppress in uterine contractions). To assess the success of tocolytic therapy, the tocolytic index was used.

Abstract

One of the activities routinely carried out in antenatal care is the measurement of Fundus Uterine Height (FUH) to determine Estimated Fetal Weight (EFW). Fetal weight growth can be indirectly affected by the basic nutritional knowledge of  pregnant  women  about the  importance of  fulfillinf nutrition during pregnancy to  support the  health  and development  of the  fetus. The aim of the study was to determine the relation between the level of nutritional knowledge of pregnant women and the estimated fetal weight at Kedaton Primary Health Center Bandar Lampung City. The research design used was analytic observational with cross sectional approach using primary data by measuring the knowledge of pregnant  women  using  a  questionnaire  consisting of 28 question  items  and  secondary  data  in  the   form  of uterine fundal height in the KIA book. Sampling was conducted at the Kedaton Primary Health Center in 2020. The sample was taken using purposive sampling with a total sample of 46 samples that met the inclusion and exclusion criteria. Data analysis used Chi Square with α = 0.05 and CI = 95%. The results  showed  that  there  were  8.7%  mothers with good nutritional knowledge, 71.7% mothers with moderate nutritional knowledge and 19.6% mothers with poor nutrition knowledge. Then pregnant women with small EWF during pregnancy and large EFW during pregnancy consisted of 10.9% and 78.3% respectively of mothers with EFW according to gestational age. Bivariate analysis showed that there was a relationship between the level of nutritional knowledge in pregnant women and the estimated fetal weight at Kedaton Health Center with a value of p = 0.003. There is a correlation between the level of nutritional knowledge of pregnant women with the estimated fetal weight.

Abstract

Mahasiswa kedokteran cenderung memiliki tingkat aktivitas fisik yang ringan. Penurunan tingkat aktivitas fisik dapat memengaruhi fleksibilitas. Gaya hidup yang cenderung kurang aktif akan menimbulkan terjadinya kekakuan pada otot atau jaringan ikat pada sendinya. Hal ini disebabkan oleh adanya pemendekan atau atrofi otot karena otot tersebut kurang aktif digunakan. Penelitian ini bertujuan untuk mengetahui hubungan antara tingkat aktivitas fisik terhadap fleksibilitas lumbal. Penelitian cross sectional dilakukan terhadap 42 orang mahasiswa Fakultas Kedokteran Universitas Lampung angkatan 2017 berjenis kelamin laki-laki. Penelitian dilakukan dengan melakukan pengukuran terhadap tingkat aktivitas fisik menggunakan Global Physical Activity Questionnaire (GPAQ) dan fleksibilitas lumbal menggunakan metode toe touch test dengan alat standing trunk flexion meter. Penelitian ini menunjukkan hasil berupa 19 responden (45,2%) memiliki tingkat aktivitas fisik rendah, 17 responden (40,5%) memiliki tingkat aktivitas fisik sedang dan 6 responden (14,3%) memiliki tingkat aktivitas fisik tinggi. Nilai rerata fleksibilitas lumbal responden sebesar 3,676 cm dengan standar deviasi 9,0734. Tingkat aktivitas fisik tidak memengaruhi fleksibilitas lumbal dengan nilai p = 0,628. Tidak terdapat hubungan antara tingkat aktivitas fisik terhadap fleksibilitas lumbal pada mahasiswa Fakultas Kedokteran Universitas Lampung.

Abstract

Preeclampsia is a serious pregnancy complication characterized by high blood pressure and organ damage. This condition poses a health risk for both the mother and the fetus. This article reviews the epidemiology and diagnosis of preeclampsia. Its epidemiology is complex with varying global prevalence. Risk factors include maternal age, history of preeclampsia and family, multiple pregnancies, obesity, diabetes, and kidney disease. Common diagnostic methods include blood pressure measurement, urine analysis, laboratory tests, and Doppler ultrasonography to assess the placenta and fetus. Prevention and early detection are the major priorities to reduce maternal and infant mortality rates. Further research is needed to develop better diagnostic and prevention strategies. Increasing understanding of the epidemiology and diagnostic methods of preeclampsia, more mothers and babies can be protected from the risks of this condition in the future.

Abstract

With high mortality rates worldwide, cervical cancer is a major threat to women's health. The World Health Organization (WHO) has set a global strategy for elimination through HPV vaccination. Indonesia has implemented the HPV Immunization Introduction Program, including its implementation in the School Children Immunization Month, aimed at grade 5 and 6 elementary schools. The prevalence of cervical lesions has been reduced by HPV vaccination, but screening and adapting to changing epidemiology remain challenging. The influence of socioeconomic factors on risk highlights the need for prevention strategies that address social disparities. Successful vaccination not only protects individuals but also contributes to overall public health. Therefore, it requires adaptation and collaboration across sectors to address cervical cancer globally.

Abstract

Myoma or uterine fibroids are benign tumors that often occur in the uterine wall. Myomas can be asymptomatic or cause symptoms such as abnormal bleeding, pelvic pain, and disorders of the urinary tract. Risk factors include age, race, age of menarche, parity, family history, hormonal contraceptive use, and obesity. Diagnostic examination involves pelvic examination, ultrasound, hysteroscopy, and MRI. Medical therapies used such as NSAIDs, oral contraceptives, Levonorgestrel-releasing intrauterine system (Mirena), tranexamic acid (cyclocapron), GnRH agonists and SPRMs and surgeries such as myomectomy, hysterectomy, uterine artery embolization, and MRgFUS. Management of Myoma must consider various factors, namely tumor size, patient age, symptom and the desire to maintain fertility in order to provide optimal treatment according to the patient's needs and preferences.

Abstract

Menstruation is a natural process in which the inner uterine lining of the uterus is regularly released every month in women. The most vulnerable period for a person to experience menstrual disorders is during the first year, about 75% of adolescent girls experience menstrual disorders. According to the World Health Organization (WHO), 75% of adolescent girls experience menstrual cycle disorders. Risk factors that can cause disruptions in the menstrual cycle include hormonal imbalances due to environmental stressors and excessive physical activity and low energy intake. In addition, exposure to pollutants such as cigarette smoke, stress, depression, short sleep, Body Mass Index (BMI), and certain medical conditions can also affect the menstrual cycle. This study is an analytical study with cross-sectional design. The study was conducted at SMAN 5 Bandar Lampung in October - December 2023. The research instruments used in this study were questionnaires to assess sleep quality, namely PSQI, questionnaires from who, namely GPAQ to measure physical activity, and menstrual cycle questionnaires to assess menstrual disorders in respondents. Chi-square test results with a confidence level of 95% showed a p-value of 0.265 (p-value > 0.05) on sleep quality variables and p-value of 0.006 (p-value < 0.05) on physical activity variables. There is no relationship between sleep quality and physical activity with the menstrual cycle in adolescent girls at SMAN 5 Bandar Lampung in 2023. Abstract

The female reproductive system undergoes regular cycles in preparation for pregnancy and fertilization, known as the menstrual cycle. This cycle involves hormonal changes and complex physiological processes, with the follicular phase dominated by estrogen and the luteal phase dominated by progesterone. Menstrual cycle disorders, such as amenorrhea, polymenorrhea, and oligomenorrhea, can be caused by various factors including hormonal imbalances, unhealthy lifestyles, and certain medical conditions. Management of menstrual disorders varies depending on their causes, which may require a holistic and individualized approach. A good understanding of the menstrual cycle and its disorders is crucial for accurate diagnosis and appropriate treatment. It is also important to promote overall women's reproductive health.

Abstract

Breast cancer is the uncontrolled growth of cells in the lobules, ductus and supporting tissue of the breast that surrounds them. An examination for early detection of breast cancer is breast self-examination (BSE) which can reduce the mortality rate by 25-30%. Pesawaran District in 2018 had a coverage of the PTM indicator for BSE examination of 16,20%, with the highest achievement at the Gedong Tataan Community Health Center of 27,83% and the lowest in Tegineneng Community Health Center of 4,17%. This research was conducted to determine the factors associated with BSE behavior in women of childbearing age (WUS) in Tegineneng Community Health Center, Pesawaran Regency. The research used an analytical observational method with a cross sectional approach. The sampling technique was cluster sampling. Data was collected using a questionnaire and analyzed with chi-square test. The results showed that there was a significant correlation between knowledge (p-value 0.003) and media information (p-value 0.000) on BSE behavior. There was a significant relationship between knowledge and information media on BSE behavior. Meanwhile, there was no significant relationship between age, motivation, and the role of health workers on BSE behavior.

Abstract

Preeclampsia and HELLP syndrome are cases of obstetric emergencies that are still serious problems and have a high level of complexity. Preeclampsia is characterized by hypertension and proteinuria after 20 weeks of gestation, while HELLP syndrome is characterized by hemolysis, elevated liver enzymes, and thrombocytopenia. A 32 year old woman, term pregnant, G2P1A0, presented with a history of hypertension since pregnancy. The results of the physical examination, blood pressure was 165/120 mmHg, laboratory results showed that platelets were 150,000/µL, LDH 731 U/L, SGOT/SGPT 84/46 U/L and urine excretion showed proteinuria 500. The importance of knowing the risk factors for preeclampsia include the mother's age at the time. pregnant for more than 35 years, obesity, history of preeclampsia in a previous pregnancy, multiple pregnancies, and a history of certain diseases such as chronic hypertension, diabetes, kidney disease or degenerative diseases such as rheumatoid arthritis or lupus. Regular antenatal examinations and recognizing signs of preeclampsia as early as possible can prevent severe preeclampsia and its complications. Management of these patients includes stabilization, fluid management, preventing eclampsia, administering antihypertensive drugs and corticosteroids. Appropriate management in this case can prevent further complications and reduce maternal mortality. Diagnosis is based on anamnesis, physical examination and supporting support in patients given medical therapy in accordance with the theory and termination of pregnancy by caesarean section.
