Abstract

Self-nano-emulsifying drug delivery systems (SNEDDS) address the challenges of poor solubility and bioavailability in oral drug delivery. These challenges lead to poor therapeutic results and an increased frequency of dosing. This review assesses the challenges of oral drug delivery due to poor solubility and absorption and the potential of Self-Nanoemulsifying Drug Delivery Systems (SNEDDS) to deliver poorly water-soluble drugs. This also entails an assessment of the formulation components of SNEDDS, particularly oils, surfactants, and co-surfactants, and their influence on the physicochemical properties, release profile, stability, and anti-inflammatory activity of the formulation. A thorough assessment of 25 of the most recent primary studies revealed the significant influence of formulation design on the size and distribution of droplets and their subsequent emulsification, drug release, and bioavailability. Oils that are easier to solubilize, along with high HLB surfactants and co-surfactants, such as PEG 400 and Transcutol, are instrumental in producing smaller, stable emulsions that facilitate permeability. Nevertheless, human pharmacokinetic data are lacking, and challenges remain in the large-scale production of solid dosage forms from liquid SNEDDS. This problem is further complicated by the variability of the excipients. The review highlights the need for standardized predictive models and more refined plans for in vivo validation and excipient development. What is unique is the integration of specific information from formulation optimization with translational pharmacological outcomes. This integration has the potential to greatly improve the design and utilization of SNEDDS, thereby expanding its therapeutic benefits.

Abstract

Chronic constipation is a common gastrointestinal disorder that negatively affects quality of life and increases healthcare burden worldwide. Elobixibat, an ileal bile acid transporter inhibitor, has been introduced as a novel therapy to enhance intestinal fluid secretion and improve colonic motility in patients with chronic constipation. However, concerns remain regarding gastrointestinal adverse effects, particularly their potential relationship with dosage. Therefore, this study aimed to evaluate gastrointestinal adverse effects associated with elobixibat and to assess whether these events demonstrate a dose-dependent pattern. This systematic review and meta-analysis followed the PRISMA 2020 guidelines and was registered in PROSPERO (CRD420251155537). A literature search was conducted in PubMed and ScienceDirect to identify randomized controlled trials investigating elobixibat in chronic constipation patients published up to December 2025. Study eligibility was determined using the PICOS framework. Data on adverse effects, dosage, and treatment duration were extracted and analyzed using proportional meta-analysis with a random-effects model in R Studio, and risk of bias was assessed using the RoB2 tool. Five randomized controlled trials involving 922 participants are included in the analysis. The pooled proportion of diarrhea is 21%, abdominal distension is 26%, and abdominal pain is 16%, while unspecified gastrointestinal disorders represent the most frequently reported adverse events at 49%. Overall, gastrointestinal adverse events show a dose-dependent pattern, with higher doses associated with increased event rates. These findings indicate that elobixibat is associated with dose-dependent gastrointestinal adverse effects, highlighting the importance of individualized dosing and gradual titration to improve treatment tolerability.

Abstract

The use of nanotechnology in pharmaceuticals, especially for cancer therapy, has shown significant development through the innovation of liposome-based drug delivery systems. Liposomes as delivery nanosystems improve stability, bioavailability, and drug specificity to target cells, while lowering systemic toxicity. Various strategic approaches such as conjugation with antibodies (trastuzumab), enzyme (hyaluronidase), RNA, and combination with siRNA and gold nanoparticles demonstrate remarkable ability to penetrate biological barriers and overcome drug resistance. In addition, multifunctional liposomes allow simultaneous delivery of chemotherapy and immunotherapy synergistically. Based on the results of our literature/review study, it shows that nanotechnology-based liposomes significantly increase the effectiveness of cancer therapy by increasing stability, bioavailability, and drug specificity, and addressing resistance through various strategies such as conjugation and delivery of combinations of therapies. As shown in pancreatic, lung, cervical, and HER2-positive breast cancer therapy. These studies have confirmed that nanotechnology-based liposome formulations are not just a delivery device, but a precision, effective, and potentially changing the paradigm of cancer treatment.

Abstract

Hemorrhoids are a common anorectal disorder characterized by vascular dilation, tissue edema, and inflammation mediated by cytokines such as interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α), and cyclooxygenase-2 (COX-2). Current therapies mainly relieve symptoms without adequately targeting the underlying inflammatory process. Metformin, widely used as an antidiabetic drug, has demonstrated anti-inflammatory properties through activation of the adenosine monophosphate-activated protein kinase (AMPK) pathway and inhibition of nuclear factor kappa B (NF-κB). Topical administration may enhance local therapeutic effects while minimizing systemic exposure. This literature review aimed to evaluate the potential of topical metformin as an innovative treatment for hemorrhoids and to examine its anti-inflammatory effects in rat hemorrhoid models. Relevant studies were identified from PubMed, Scopus, ScienceDirect, and Google Scholar using predefined keywords related to topical metformin, inflammation, hemorrhoids, AMPK, and COX-2. Eligible full-text articles were narratively synthesized to evaluate mechanisms of action, therapeutic efficacy, and formulation strategies. The reviewed evidence indicates that metformin suppresses inflammatory responses by activating AMPK, inhibiting NF-κB signaling, and reducing TNF-α, IL-6, and COX-2 expression. Animal studies consistently reported reduced inflammatory cell infiltration, decreased edema, improved tissue architecture, and enhanced local drug retention with topical formulations. Overall, topical metformin represents a promising therapeutic strategy for hemorrhoids by targeting inflammation and promoting tissue repair. Nevertheless, further experimental studies and well-designed clinical trials are necessary to establish its safety, efficacy, and optimal formulation for clinical use.

Abstract

Degenerative diseases such as hypertension, diabetes mellitus, and gout have developed and threaten the quality of life of the community, especially in rural areas. This community service aims to enhance awareness among the residents of Sungai Petai Village about the benefits of instant herbal medicine, infused water, and health education for early detection and maintaining a healthy lifestyle. The method used is service learning with health counseling, demonstration of instant herbal medicine and infused water, and health checks (blood pressure, blood sugar, and uric acid), followed by a pretest and posttest to assess the improvement of health education, with a pretest and posttest to determine the improvement of knowledge. The results showed that the education significantly improved participants' understanding of diseases and health products, with a considerable and sizable overall increase in knowledge at the posttest (80.83). The health check also showed that some participants had health conditions that required further attention.

Abstract

Carrot extract (Daucus carota L.) contains various bioactive compounds, including vitamins A, B, and C, alkaloids, flavonoids, tannins, anthraquinones, saponins, diterpenes, steroids, beta-carotene, phenols, terpenoids, and minerals, all of which possess antioxidant properties. These compounds are known to help slow down the aging process. The aim of this study was to determine the optimal concentrations of gelling and alkalizing agents, assess their interactions, and evaluate the anti-aging effects of the most effective serum formulation. The formulation was optimized using the Box–Behnken design with two key factors: Carbopol 940 concentration (0.5–1%) and triethanolamine (0.5–1%). The effects on pH, adhesion, and spreadability were evaluated. The best formulation was achieved with 0.837% Carbopol 940 and 0.855% triethanolamine,  showing a pH error of 2.45%, adhesion error of 0.76%, and spreadability error of 1.01%, all within acceptable limits (errors < 10%). After four weeks of stability testing, the formulation remained stable, well-mixed, with a pH of 5.16, an adhesion time of 1.60 seconds, and spreadability of 6.73 cm, with no discomfort. The combination of Carbopol 940 and triethanolamine improved the physical properties, enhancing anti-aging effects compared to the base formula. The optimized serum increased skin moisture by 84.62%, reduced pore size by 64.71%, lightened spots by 62.79%, and reduced wrinkles by 67.50%. This indicates the optimized carrot extract serum is stable, safe, and effective as an anti-aging agent, making it a promising natural skincare product.

Abstract

Good Pharmacy Practice (GPP) merupakan standar praktik farmasi yang ditetapkan oleh International Pharmaceutical Federation (IPF) dan World Health Organization (WHO) untuk menjamin kualitas pelayanan kefarmasian. Implementasi GPP di berbagai negara menunjukkan tingkat kepatuhan yang bervariasi. Melakukan tinjauan sistematis terhadap implementasi GPP di apotek komunitas di berbagai negara, mengidentifikasi faktor-faktor yang mempengaruhi kepatuhan, serta peluang peningkatan kualitas pelayanan. Pencarian literatur dilakukan terhadap artikel yang dipublikasikan tahun 2015-2025. Sepuluh artikel yang memenuhi kriteria inklusi dianalisis menggunakan metode literature review. Tingkat kepatuhan terhadap standar GPP bervariasi dari 11,7% hingga 65,09%. Faktor-faktor yang mempengaruhi kepatuhan meliputi kualifikasi tenaga farmasi, lokasi geografis apotek, dukungan regulasi, infrastruktur, dan sumber daya. Kategori dengan kepatuhan tertinggi adalah pengelolaan sediaan farmasi (66,1%-86,6%), sedangkan terendah adalah pelayanan farmasi klinik (11,02%-41,4%). Implementasi GPP di apotek komunitas masih suboptimal, terutama di negara berkembang. Diperlukan penguatan regulasi, peningkatan kompetensi tenaga farmasi, dan dukungan infrastruktur untuk meningkatkan kepatuhan terhadap standar GPP.

Abstract

Diabetic wounds remain one of the most challenging complications of diabetes mellitus, largely due to persistent inflammation, oxidative stress and impaired tissue regeneration. These conditions contribute to delayed healing and increase the risk of infection and amputation. In recent years, topical metformin has gained attention as a potential therapeutic option to improve wound healing outcomes. This review aims to summarize current preclinical evidence on the effectiveness of topical metformin, with a particular focus on its underlying molecular mechanisms. Evidence from experimental studies suggests that topical metformin can accelerate wound healing through several interconnected pathways, including activation of AMP-activated protein kinase (AMPK), suppression of NF-κB–mediated inflammation, reduction of reactive oxygen species (ROS) and regulation of extracellular matrix remodeling. Moreover, the advancement of sophisticated delivery technologies, such as hydrogels and nanocomposites has significantly augmented the localized therapeutic efficacy of metformin. The evidence suggests that topical metformin may be a new way to treat diabetic wounds. However, additional rigorously planned studies and clinical trials are necessary to validate its safety, enhance its formulation and facilitate its clinical use. Abstract

This study aims to assess the rationality of drug prescribing practices in the Outpatient Department of XYZ Hospital Pekanbaru using the World Health Organization (WHO) prescribing indicators. The research is an observational study with a descriptive nature, employing systematic random sampling to analyze data extracted from prescription records in the pharmacy department of XYZ Hospital Pekanbaru. A total of 660 prescription samples were evaluated, revealing an average of 2.91 drugs prescribed per prescription sheet, indicating a lack of adherence to diagnostic considerations. The findings show that the percentage of generic drug prescriptions was 64.04%, while antibiotic prescriptions accounted for 32.57%. Additionally, prescriptions for injectable forms constituted 1.36%, and adherence to the national formulary was at 72.27%. Out of the five prescribing indicators assessed, only the prescribing of injectable forms met the WHO’s prescribing indicators standard. These results highlight the necessity for improved adherence to rational prescribing practices to enhance treatment efficacy, minimize adverse drug reactions, and ensure cost-effectiveness in patient care. Regular monitoring and education of healthcare providers regarding rational drug use principles are essential for improving healthcare service quality.
