Abstract

Keputusan memulai hemodialisis sangat penting untuk mengurangi morbiditas dan mortalitas pasien, namun dipengaruhi oleh berbagai faktor. Penelitian ini bertujuan untuk mengetahui factor-faktor yang berhubungan dengan pengambilan keputusan pasien gagal ginjal kronik tahap akhir dalam memulai terapi hemodialisis. Penelitian ini menggunakan desain kuantitatif dengan pendekatan studi retrospektif yang dilaksanakan di RSUD Arifin Achmad pada 75 pasien yang menjalani terapi hemodialisis, dengan teknik purposive sampling. Hasil penelitian menunjukkan mayoritas responden berusia >46 tahun (58,7%), berjenis kelamin laki-laki (52,0%), berpendidikan tinggi (60,0%), tidak bekerja (52,0%), dan memiliki asuransi kesehatan (98,7%). Sebagian besar responden memiliki dukungan sosial yang baik (77,3%), self-efficacy tinggi (70,7%), pengetahuan baik (53,3%), serta tidak menunda dalam memulai terapi hemodialisis (69,3%). Berdasarkan hasil uji chi-square, terdapat hubungan antara usia (p value = 0,011), dukungan sosial (p value = 0,002), self-efficacy (p value = 0,000), dan pengetahuan (p value = 0,001) dengan pengambilan keputusan dalam memulai terapi hemodialisis. Sementara itu, tidak terdapat hubungan antara jenis kelamin (p value = 0,464), tingkat pendidikan (p value = 0,240), pekerjaan (p value = 1,000), dan kepemilikan asuransi (p value = 1,000) dengan pengambilan keputusan dalam memulai terapi hemodialisis. Kesimpulan penelitian menunjukkan bahwa dukungan sosial, self-efficacy, dan pengetahuan yang tinggi meningkatkan kemungkinan pasien mengambil keputusan untuk memulai terapi hemodialisis secara tepat waktu. Implikasi penelitian ini menekankan pentingnya peran tenaga kesehatan dalam meningkatkan edukasi, memperkuat dukungan sosial, serta membangun self-efficacy pasien guna mencegah keterlambatan memulai terapi hemodialisis.

Abstract

Tuberculosis (TB) remains a major health problem in Indonesia, with Pekanbaru City—particularly Marpoyan Damai Subdistrict—recording the highest number of cases (1,558). This study aimed to formulate evidence-based strategies to sustainably reduce TB incidence and transmission. A descriptive quantitative approach was applied using primary and secondary data collected through questionnaires, field observations, and documentation. A total of 91 respondents were selected by simple random sampling from 1,558 TB households. Data were analyzed descriptively and through SWOT analysis. Findings indicated an increasing TB trend, from 1,200 cases in 2020 to 1,650 in 2024, including a rise in pediatric TB. Risk factors included poor ventilation and lighting (40.7%), smoking habits (50.5%), low mask use, and high household density. SWOT analysis revealed IFAS = 0.67 and EFAS = 1.10, placing the strategy in Quadrant I (aggressive). Recommended strategies include community education, optimizing the role of health cadres, maximizing healthcare facilities, improving housing environments, and strengthening cross-sectoral collaboration to accelerate TB control

Abstract

ABSTRACT It is very necessary to limit fluid intake in chronic kidney failure patients undergoing hemodialysis, this aims to prevent the risk of excess fluid between hemodialysis sessions (Interdialytic Weight Gain/IDWG).  There are several factors associated with long IDWG, namely undergoing hemodialysis and family support. This study aims to identify the relationship between duration of hemodialysis and family support on Interdialytic Weight Gain (IDWG) of Chronic Kidney Failure patients in the Hemodialysis Unit. This type of research is quantitative with a cross sectional approach. The population is kidney failure patients. The sample consisted of 96 respondents using an accidental sampling technique. The measuring instrument used is a questionnaire. The analysis used is bivariate analysis using the chi square test. The research results were obtained from 96 respondents, most of whom had long HD 1 year, 59.4%, 58.3% of respondents who had high family support and 41.7% of respondents who had mild IDWG. The statistical test results for the hemodialysis duration variable obtained p value = 0.009 α 0.05, while the family support variable obtained p value = 0.002 α 0.05, so it can be concluded that there is a relationship between hemodialysis duration and family support for IDWG in failed patients. chronic kidney disease in the Hemodialysis Unit. It is estimated that health workers can provide information to respondents and families, namely by providing health education and socializing the prevention program for increasing IDWG. Keywords: Duration of Hemodialysis, Family Support and IDWG  ABSTRAK Pendahuluan: Pembatasan asupan cairan pada pasien gagal ginjal kronik yang menjalani hemodialisis sangat perlu dilakukan hal ini bertujuan untuk mencegah terjadinya resiko kelebihan cairan antar sesi hemodialisis (Interdialytic Weight Gain/IDWG).  Ada beberapa faktor yang berhubungan dengan IDWG yaitu lama menjalani hemodialisis dan dukungan keluarga. Penelitian ini bertujuan untuk mengidentifikasi hubungan lama hemodialisis dan dukungan keluarga terhadap Interdialytic Weight Gain (IDWG) pasien Gagal Ginjal Kronik di Unit Hemodialisis. Metode: Jenis penelitian ini kuantitatif dengan pendekatan cross sectional. Penelitian ini dilakukan Unit Hemodialisis RSUD Arifin Achmad. Populasi adalah pasien gagal ginjal. Sampel berjumlah 96 responden dengan teknik pengambilan accidental sampling. Alat ukur yang digunakan adalah kuesioner Analisis yang digunakan adalah analisis bivariat menggunakan uji chi square. Hasil: hasil penelitian didapat dari 96 responden mayoritas memiliki lama HD 1 tahun sebanyak 59,4%, responden yang memiliki dukungan keluarga tinggi sebanyak 58,3% dan responden yang memiliki IDWG ringan sebanyak 41,7%. Hasil uji statistik untuk variabel lama hemodialisis didapat p value = 0,009  0,05, sedangkan variabel dukungan keluarga diperoleh nilai p value = 0,002  0,05, maka dapat disimpulkan bahwa ada hubungan lama hemodialisis  dan dukungan keluarga terhadap IDWG pada pasien gagal ginjal kronik di Unit Hemodialisis. Diharapkan bagi petugas kesehatan dapat memberikan informasi kepada responden dan keluarga yaitu dengan cara melakukan pendidikan kesehatan dan mensosialisasikan program pencegahan peningkatan IDWG. Kata Kunci: Lama Hemodialisis, Dukungan Keluarga dan IDWG

Abstract

ABSTRACT Benign Prostatic Hyperplasia (BPH) is a non-life-threatening condition that significantly impacts the quality of life of affected individuals. Patients with BPH commonly experience symptoms such as nocturia, increased frequency and urgency of urination, decreased urine flow rate, incomplete bladder emptying, and hesitancy to urinate (Lower Urinary Tract Symptoms/LUTS). LUTS directly impairs the quality of life of BPH patients and contributes to morbidity. The severity of LUTS in BPH patients can be subjectively assessed using the International Prostate Symptom Score (IPSS). This study aims to explore the relationship between Lower Urinary Tract Symptoms (LUTS) and quality of life in patients with Benign Prostatic Hyperplasia (BPH). This study employed a quantitative approach with a descriptive correlational design using a Cross-Sectional approach. The population consisted of all patients diagnosed with Benign Prostatic Hyperplasia (BPH) experiencing Lower Urinary Tract Symptoms (LUTS) at RSUD Arifin Achmad Provincial Hospital in Riau from October to December 2023, totaling 153 individuals. A purposive sampling technique was used to select 111 participants as the study sample. Bivariate analysis using the chi-square test was employed for data analysis. Among the respondents, 43 individuals (38.74%) were categorized with moderate LUTS. The quality of life of BPH patients was rated as moderate in 62 respondents (55.86%). The Pearson Chi-Square test yielded a p-value of 0.003 (p-value α (0.05)). There is a significant relationship between Lower Urinary Tract Symptoms (LUTS) and the quality of life of patients with Benign Prostatic Hyperplasia (BPH) at RSUD Arifin Achmad Provincial Hospital in Riau. Keywords: Lower Urinary Tract Symptoms, Quality of Life, Benigna Prostatic Hyperplasia  ABSTRAK Benign Prostatic Hyperplasia (BPH) merupakan kondisi yang tidak mengancam jiwa namun memberikan dampak signifikan terhadap kualitas hidup penderitanya. Penderita BPH umumnya mengalami gejala seperti nokturia, peningkatan frekuensi dan urgensi buang air kecil, penurunan laju aliran urin, kesulitan mengosongkan kandung kemih secara sempurna, dan keraguan saat ingin buang air kecil (Lower Urinary Tract Symptoms/LUTS). LUTS secara langsung mempengaruhi kualitas hidup penderita BPH dan berkontribusi terhadap morbiditas. Derajat keparahan LUTS pada penderita BPH dapat dinilai secara subjektif menggunakan International Prostate Symptom Score (IPSS). Penelitian ini bertujuan untuk mengeksplorasi hubungan antara Lower Urinary Tract Symptoms (LUTS) dan kualitas hidup pada pasien Benign Prostatic Hyperplasia (BPH). Penelitian ini menggunakan pendekatan kuantitatif dengan desain deskriptif korelasional melalui pendekatan Cross Sectional. Populasi penelitian terdiri dari seluruh pasien Benign Prostatic Hyperplasia (BPH) yang mengalami Lower Urinary Tract Symptoms (LUTS) di Rumah Sakit Umum Daerah (RSUD) Arifin Achmad Provinsi Riau pada bulan Oktober hingga Desember 2023, dengan jumlah 153 orang. Sampel penelitian sebanyak 111 orang dipilih dengan menggunakan teknik purposive sampling. Analisis data dilakukan dengan menggunakan analisis bivariat menggunakan uji chi-square. Abstract

Pendahuluan: Penyakit Paru Obstruktif Kronik (PPOK) merupakan masalah kesehatan global yang ditandai dengan hambatan aliran udara, air trapping, serta penurunan oksigenasi yang progresif. Intervensi nonfarmakologis seperti Pursed Lips Breathing (PLB) direkomendasikan untuk meningkatkan ventilasi dan saturasi oksigen. Tujuan: menganalisis dan memahami penerapan filosofi Teori Patricia Benner tentang From Novice to Expert dalam proses kredensial keperawatan. Metode penelitian: Desain penelitian menggunakan pendekatan deskriptif dengan durasi implementasi enam hari berturut-turut, waktu 23-28 oktober 2025 dilaksanakan di RSUD Arifin Achmad Propinsi Riau Ruang Jasmin melibatkan tiga pasien PPOK yang memenuhi kriteria inklusi. Intervensi PLB diberikan selama 15–20 menit setiap sesi, dilakukan tiga kali sehari. Pengukuran SpO₂ dilakukan sebelum dan sesudah intervensi menggunakan pulse oximeter terkalibrasi, kemudian data dicatat, dibandingkan, dan dianalisis secara deskriptif untuk melihat perubahan harian dan keseluruhan, Hasil/temuan: adanya peningkatan konsisten pada nilai SpO₂ setelah intervensi pada seluruh pasien, dengan peningkatan harian berkisar antara 1% hingga 3%. Rata-rata SpO₂ sebelum intervensi adalah 94,39%, meningkat menjadi 96,17% setelah PLB, dengan rata-rata peningkatan sebesar 1,78%. Tidak ditemukan efek samping, dan seluruh pasien dapat mengikuti teknik PLB dengan baik. Temuan ini menegaskan bahwa PLB efektif meningkatkan oksigenasi melalui perpanjangan fase ekshalasi, pengurangan air trapping, dan peningkatan efisiensi pertukaran gas. Simpulan: PLB merupakan terapi tambahan yang sederhana, aman, dan bermanfaat bagi pasien PPOK, serta dapat diintegrasikan ke dalam asuhan keperawatan rutin untuk meningkatkan fungsi pernapasan dan kenyamanan pasien

Abstract

Introduction: Each patient has a different experience undergoing hemodialysis, depending on individual perceptions, social support, and self-care abilities. This study aims to explore the experiences of chronic kidney disease patients undergoing hemodialysis. Methods: This is a qualitative study with a phenomenological approach. The researcher used a purposive sampling technique to select eight informants for the participants, and analyzed using Colaizzi thematic analysis. Data validity was tested to increase trustworthiness through triangulation, member checking, dependability, confirmability, and transferability. Results: This study yielded three themes: 1) initial response to hemodialysis, 2) the process of accepting hemodialysis, and 3) motivation for regular hemodialysis. Conclusion: Nurses need to actively introduce coping strategies, involve families, and integrate medical and psychological aspects to support patient compliance and resilience

Abstract

Penyakit Ginjal Kronis (PGK) merupakan kondisi progresif yang memerlukan terapi hemodialisis jangka panjang sehingga penerapan Continuity of Care penting untuk menjamin kesinambungan pelayanan, meningkatkan manajemen diri, dan mencegah komplikasi, yang tercermin dari persepsi pasien terhadap pelayanan yang terkoordinasi dan berkelanjutan. Penelitian ini bertujuan untuk menggambarkan persepsi pasien dalam penerapan prosedur Continuity of Care pada penyakit ginjal kronis yang menjalani hemodialisis. Metode penelitian ini menggunakan desain deskriptif kuantitatif dengan pendekatan cross-sectional. Sampel berjumlah 53 responden yang dipilih menggunakan teknik purposive sampling. Instrumen yang digunakan adalah kuesioner Patient Continuity of Care Questionnaire (PCCQ) digunakan untuk mengukur variabel Continuity of Care. Data dianalisis menggunakan analisis univariat dan disajikan dalam bentuk distribusi frekuensi dan persentase. Hasil penelitian menunjukkan bahwa mayoritas responden memiliki persepsi baik terhadap penerapan prosedur Continuity of Care yaitu sebanyak 36 responden (67,9%), sedangkan 17 responden (32,1%) memiliki persepsi yang buruk. Kesimpulannya, gambaran persepsi pasien dalam penerapan prosedur Continuity of Care pada pasien penyakit ginjal kronis di RSUD Arifin Achmad tergolong baik.

Abstract

Hypertension is a chronic disease, called the silent killer. WHO data shows that the number of people with hypertension continues to increase. In Riau Province, the prevalence of hypertension increased from 20.9% (2013) to 29.1% (2018). Hypertension will cause various complications such as encephalopathy, heart disease, hemorrhagic stroke, kidney failure, heart failure, and others. Action needs to be taken to reduce the increasing number of hypertension cases. Objective: This study examines the effectiveness of a physical exercise program in reducing blood pressure in hypertension patients at the Pangkalan Kuras II Health Center. Method: It used a pre-test and post-test experimental design involving 101 hypertensive patients, primarily aged 46-64 years (85.1%) and mostly female (86.14%). The program included walking three times a week and exercising twice a week for three weeks. Blood pressure was measured using a manual device. The majority of participants had grade II hypertension. Result: The Wilcoxon Signed Rank Test showed significant reductions in both systolic (4.72 mmHg) and diastolic (3.13 mmHg) blood pressure, with a p-value of 0.000. Conclusions: This numbers indicates that regular physical exercise, such as walking and exercise, effectively lowers blood pressure in hypertensive patients.

Abstract

Hypertension is a chronic disease with a high prevalence and is a major cause of global morbidity and mortality. Low medication adherence and inadequate patient knowledge are the main factors contributing to uncontrolled blood pressure. Peer group–based education has become one of the effective approaches to improving understanding, motivation, and health behavior changes among patients with hypertension. This review aims to comprehensively examine the effect of peer group education on hypertension control, including blood pressure, medication adherence, and lifestyle changes in patients with hypertension. This literature review used the PRISMA 2020 approach. Article searches were conducted through online databases including PubMed, ScienceDirect, and Google Scholar within the 2020–2025 publication period. The inclusion criteria comprised randomized controlled trials (RCTs), quasi-experimental, and experimental studies investigating peer group education interventions in adult patients with hypertension. A total of 7 articles published between 2022–2025 that met the inclusion criteria were critically appraised and analyzed descriptively and narratively to identify the effectiveness of peer group education interventions among patients with hypertension. The findings showed that peer group education significantly contributed to reducing blood pressure, improving medication adherence, and enhancing health behaviors such as dietary patterns, physical activity, and self-management. This intervention also improved patients' knowledge, motivation, and self-efficacy in managing hypertension. Peer group education is an effective intervention for hypertension control because it can improve adherence, promote healthier lifestyles, and reduce blood pressure.

Abstract

Environmental sanitation management in port areas has become increasingly important amid the growing intensity of global logistics activities. Perawang Container Terminal (PCT), one of the major ports in Riau Province, continues to face several unresolved sanitation issues, including the absence of a waste segregation system, temporary waste storage facilities that do not meet technical standards, irregular monitoring of clean water quality, management of hazardous and toxic waste (Bahan Berbahaya dan Beracun [B3]) without the required permits, and the absence of a disease vector control program. These conditions indicate a gap between current practices and healthy port standards as stipulated in Indonesian Ministry of Health Regulation No. 44 of 2014. This study aimed to assess the existing condition of environmental sanitation management at PCT, identify supporting and inhibiting factors, and formulate optimization strategies to achieve a sustainable healthy port. A qualitative case study approach was employed, involving 10 key informants selected through purposive sampling from PCT management, the Class I Health Quarantine Office of Pekanbaru, and the surrounding port community. Primary data were collected through observations using the Healthy Port Self-Assessment Form, semi-structured interviews, and document review, while secondary data were obtained from relevant institutional records. Existing conditions were analyzed descriptively, followed by SWOT analysis and the Analytic Hierarchy Process (AHP) to determine priority optimization strategies. These strategies were integrated with the Triple Bottom Line framework, encompassing environmental, social, and economic dimensions, to support the development of a sustainable healthy port.
