Abstract

Background: Medical students who get community-involved medical education will be better equipped to address the health needs of their communities by integrating public health services into their practice. The importance of public health disciplines for physicians needs to be reinforced in the medical curriculum. Many studies, findings, and discussions report on the importance of this integration. This study aims to identify, decipher, and map the available scientific evidence regarding the importance of public health integration in medical education. Subjects and Method: This was a scoping review study using PUBMED, ProQuest, SCOPUS, Google Scholar, and other related sources. The keywords used in article searches are as follows: "public health" AND "medical education" OR "support" OR "against" OR "proportion" OR "role". From the database used, 1,775 articles were obtained and those who met the criteria were 45 articles. Results: The results were presented in three sub-discussions, namely role, perception, and form of integration. 1) Role: doctors are able to facilitate individual health and address public health problems through health promotion, health policy, evidence-based medicine practices, improving the quality of health data and health services; 2) Perception: Both positive and negative percep­tions were reported in these integration-related findings; and 3) Form of integration: public health collaboration in medical education is carried out at the academic and professional stages of doctors. Conclusion: The importance of integrating public health principles into medical education is to encourage the formation of doctors who act as doctors in the curative department as well as public health experts who understand community problems and can address them holistically and comprehensively. Abstract

Background: Military personnel has a high risk of being exposed to HIV. The transmission of HIV from male military personnel is like an iceberg phenomenon. Based on studies in the US, UK, and France, the military personnel from these countries are much more likely to be contaminated with HIV than the equal age/sex group in the civilian population. This study aimed to analyze the risk factors of HIV among male military personnel by using meta-analysis.Subjects and Method: This study was a systematic and meta-analysis study. The articles used in this study were obtained from several electronic databases including PubMed, Scopus, Springer Link, ScienceDirect, and Clinical key. These articles were collected for 2 weeks. The keywords used in searching for the articles were as follows: "HIV", "soldier", "military", "armed force", "army", "navy", "air force", "armed force", "sex partner", "man having sex with man (MSM)", "homosexual", number of marriage","prostitute","sexually transmitted infection (STI)", and "conscription". The articles included in this study were full-text articles with cross-sectional design. The articles were collected using PRISMA flow diagram. These articles were analyzed using meta-analysis with the Review Manager 5.3 application.Results: MSM (aOR= 9.37; 95%CI= 5.31 to 16.55; p <0.001), sexually transmitted infections (aOR = 3.75; 95%CI= 3.00 to 4.68; p<0.001), sexual partners> 10 (aOR= 3.05; 95 CI= 2.12 to 4.38; p<0.001) increased the incidence of HIV among male military personnel, and it was statistically significant. Sex with CSWs (aOR= 3.93; 95% CI= 0.93 to 16.62; p= 0.060) increased the incidence of HIV among male military personnel, but it was not statistically significant.Conclusion: MSM, STI, and sexual partners >10 increase the incidence of HIV among male military personnels.Keywords: HIV, MSM, commercial sex workers, sexually transmitted infection, CSW, militaryCorrespondence: Ayu Anulus. Abstract

Background: The case fatality rate (CFR) of Co­vid­-19 death­s in Indonesia on June 16, 2020 was 5.52% and 4.19% in West Nusa Tenggara. One of the causes of this high percentage of deaths is the lack of preventive behavior against Covid-19. This study aims to analyze factors as­so­ciated with Co­vid-19 preventive behavior among people in West Nusa Tenggara, Indonesia.Subjects and Method: A cross-sectional study was conducted in 10 cities/districts in West Nusa Tenggara from May-June 2020. A total of 385 people were enrolled in this study. Data were collected using a questionnaire distributed in the form of Google form and analyzed using multiple linear regressions with Stata 13.Results: High education level (≥high school) (aOR= 0.43; 95%CI=0.03 to 0.82; p=0.033), perceived benefits (aOR=0.16; 95%CI= 0.07 to 0.25; p=0.001), and good knowledge (aOR=0.19; 95%CI= 0.01 to 0.37; p=0.034) were increased the preventive behavior. While, male (aOR= 0.54; 95% CI= -0.89 to -0.19; p=0.002), perceived barriers (aOR= -0.16; 95%CI= -0.25 to -0.07; p= 0.001) were decreased the preventive behavior toward Covid-19, and they were statis­ti­cally sig­nificant.Conclusion: Education higher than high school, high perceived benefit, and good knowledge are increase the local people behavior to prevent Co­vid-19 in West Nusa Tenggara.Keywords: Covid-19, preventive behavior, West Nusa TenggaraCorrespondence: Artha Budi Susila Duarsa. Abstract

Background: Garbage is a major issue in many countries throughout the world, including Indonesia. Every year, the amount of waste produced rises in tandem with population growth. As a result, it is vital to manage liquid fertilizer made from household waste, which will aid in the reduction of community problems. The general public still has little knowledge of how liquid fertilizer is made from household garbage. This study aimed to increase community understanding and practice through training and help in the processing of household trash into liquid organic fertilizer in the tourist village of Bonjeruk, Central Lombok, West Nusa Tenggara.Subjects and Method: This was a cross-sectional study conducted in the tourist village of Bonjeruk, Central Lombok, West Nusa Tenggara, from March to December 2021. A total of 27 subjects were selected in this study. The variables in this study were knowledge and behavior. The training in management household waste into organic liquid fertilizer was carried out for 4 months. The stages of intervention were as follows: 1) Education and simulation of sorting organic and non-organic waste; 2) Separating vegetable and fruit waste from rice, meat, and bone residue. Vegetable and fruit waste was transferred into composer barrels; 3) Spraying diluted bio-activator every time they enter organic waste; 4) Filling cans continuously with garbage; 5) Every 14th day, the liquid fertilizer can be harvested; 6) Liquid fertilizer can be used after being left for 14 days outside the composer barrel by dilution. The data were collected by questionnaire and analyzed descriptively.Results: After 4 months of training, the community was able to process liquid fertilizer from the use of compost barrel and natural bio-activators from household waste. Community knowledge related to liquid fertilizer after training (Mean= 91.48; SD= 11.12) was higher than before training (Mean= 85.56; SD= 12.57). Household waste management practice after training (Mean= 94.07; SD= 11.94) was better than before training (Mean= 45.93; SD= 22.32).Conclusion: Community knowledge and practice increase after going through training in processing household waste into liquid organic fertilizer in the tourist village of Bonjeruk, Central Lombok, West Nusa Tenggara.Keywords: organic liquid fertilizer, tourist village, household waste. Correspondence:Rozikin. Abstract

The prevalence of DM disease in West Nusa Tenggara Province is not much different from that in Indonesia. DM cases in NTB are included in the ten most non-communicable illnesses suffered by the community and the incidence continues to increase from year to year. An increase in the levels of pro-inflammatory cytokines in the body is one of the causes of insulin resistance in cells which can further develop into type 2 diabetes. This study involved diabetic patients at the Mataram Community Health Center, who were assigned into 2 groups, namely the controlled diabetes group and the uncontrolled diabetes group and involved a standard group which was a group consisted of healthy people. Each group was examined for Fasting Blood Glucose (FBG) and HbA1c levels. The results of the examination in the standard group, controlled diabetes group and uncontrolled diabetes group obtained the FBG levels of 89.22 mg/dl, 110.0 mg/dl, and 245.80 mg/dl, respectively. Furthermore, the results of the HbA1c test in the standard group, controlled diabetes group and uncontrolled diabetes group were 5.44%, 6.03%, and 10.49%, respectively. The results of the examination of IL-6 levels in the standard group, controlled diabetes group and uncontrolled diabetes were 329.36 pg/ml, 331.52 pg/ml, and 320.33 pg/ml, respectively. The results of the IL-10 test in the standard group, controlled diabetes group and uncontrolled diabetes were 71.80 pg/ml, 116.60 pg/ml, and 128.10 pg/ml, respectively. Based on the results of the study, there was no significant difference in the levels of interleukin 6 and interleukin 10 between respondents with diabetes mellitus and healthy respondents (p>0.05). It can be concluded that there were no differences in interleukin 6 and 10 levels between healthy people with patients with controlled and uncontrolled diabetes.

Abstract

This study aimed to develop comic "Pade Tolak Stunting" on increasing the knowledge of female adolescents related to infectious diseases (worms, diarrhea, malaria, and ARI) as risk factors for stunting. This study was a pre-experimental study with a pre-post-test with control group design. The study subject was female adolescents (10-19 years old.) in Dusun Sade, Central Lombok. A total of 23 female adolescents were involved in this study. The comic media "Pade Tolak Stunting" was developed in the ADDIE model which consists of five stages, namely analysis, design, development, implementation, and evaluation. Data were analyzed using the Wilcoxon Rank Test. After tested, media is applicable for stunting education media. The results showed that there was an increase in the knowledge of prospective brides and grooms about infectious diseases as risk factors for stunting in Dusun Sade, Central Lombok (p <0.001). The conclusion is that the comic "Pade Tolak Stunting" is feasible and also can increase the knowledge of female adolescents about infectious diseases as risk factors for stunting.

Abstract

Abstract Background: Infection prevention and control training and access to running water are essential components of infection prevention. However, gaps in their implementation have been linked to suboptimal practices among healthcare workers, highlighting the need for improved training programs and infrastructure to mitigate infection risks globally. Aim: The objective of this systematic review was to assess the role of running water availability and infection prevention and control training in enhancing healthcare workers' practices. Methods: A systematic review analyzed studies (2014–2024) on running water availability and infection prevention and control training's impact on healthcare workers' practices. A random-effects model calculated adjusted odds ratios, with heterogeneity and publication bias assessed using forest and funnel plots, respectively, highlighting the importance of these factors in improving infection prevention practices. Results: We analyzed 171 abstracts, selecting 10 studies (7 on infection prevention and control training, 5 on running water) with a total of 3,818 healthcare workers. Infection prevention and control training improved good practices (AOR = 2.26; 95%CI = 2.01–2.53). Access to running water also enhanced practices (AOR = 1.96; 95%CI = 1.53–2.50). Publication bias was not significant. Discussion: Infection prevention and control training and running water access significantly improve healthcare workers’ practices. Healthcare facilities should implement mandatory infection prevention and control training and ensure access to running water to enhance infection prevention practices, particularly in resource-limited settings. Further research is needed to address publication bias and assess long-term outcomes.

Abstract

Health problems occur in the elderly due to a decrease in body function, one of which is a decrease in musculoskeletal function, namely Arthritis Gout. Ergonomic gymnastics is a physical activity that can help lower uric acid levels. This study aims to determine the effect of ergonomic gymnastics on uric acid levels in the elderly. The research uses a quantitative design with a pretest-postest group with control group design. The sample consisted of 30 elderly people who were selected by random sampling. The research instrument is the assessment of uric acid levels carried out before and after the intervention is given.  The research analysis consisted of univariate analysis and bivariate analysis using the Mann Whitney test with STATA software version 17. The results of the analysis showed that the mean uric acid level was 6.34 ± 0.97, before intervention and the mean decreased to 5.30 ± 0.7, after intervention. There was an effect of ergonomic gymnastics on the reduction of uric acid levels in the elderly with a Mann-Whitney p-value test of 0.0001. Ergonomic gymnastics can be a non-pharmacological treatment option to lower uric acid levels in the elderly. Ergonomic gymnastics can lower uric acid levels with muscle relaxation which can improve the process of reabsorption of uric acid in the urine. The application of ergonomic gymnastics 3 times a week showed a significant difference in uric acid levels in the elderly.

Abstract

Abstrak Lanjut usia (lansia) diketahui sering mengalami masalah kesehatan terutama risiko jatuh. Masalah ini dapat memperparah kondisi pasien sehingga perlu diketahui apa saja faktor yang berpotensi dalam risiko jatuh pada lansia. Penelitian literatur review ini bertujuan untuk menganalisis faktor intrinsik dan ekstrinsik yang mempengaruhi risiko jatuh pada lansia melalui sintesis dari artikel penelitian sebelumnya. Penelitian ini menggunakan 3 database (Google scholar, ProQuest, dan PubMed. Sejumlah 18.366 artikel didapatkan dengan penggunaan kata kunci “elderly” AND “fall” AND “intrinsic factors” AND “extrinsic factors”. Hasil analisis dari sintesis 6 artikel menunjukkan bahwa risiko jatuh pada lansia dipengaruhi oleh kombinasi faktor intrinsik dan ekstrinsik. Faktor intrinsik mencakup kondisi fisik, psikologis, dan kognitif, seperti usia lanjut, gangguan keseimbangan, gangguan kognitif, serta penyakit kronis, gangguan penglihatan, kelemahan otot, riwayat jatuh sebelumnya, dan inkontinensia urin. Faktor ekstrinsik yang dominan meliputi lingkungan fisik yang tidak aman, penggunaan alat bantu jalan yang tidak tepat, polifarmasi, serta kurangnya fasilitas keamanan di rumah atau fasilitas kesehatan. Temuan ini menegaskan bahwa pencegahan jatuh pada lansia memerlukan perhatian terhadap kondisi tubuh, lingkungan, serta dukungan sosial yang memadai untuk menciptakan lingkungan yang lebih aman bagi lansia. This problem can worsen the patient's condition so it is necessary to know what factors have the potential to cause the risk of falling in the elderly. This literature review study aims to analyze the intrinsic and extrinsic factors that influence the risk of falling in the elderly through a synthesis of previous research articles. This study used 3 databases (Google scholar, ProQuest, and PubMed. A total of 18,366 articles were obtained using the keywords "elderly" AND "fall" AND "intrinsic factors" AND "extrinsic factors". The results of the analysis of the synthesis of 6 articles showed that the risk of falling in the elderly is influenced by a combination of intrinsic and extrinsic factors. Intrinsic factors include physical, psychological, and cognitive conditions, such as advanced age, balance disorders, cognitive disorders, and chronic diseases, visual impairment, muscle weakness, history of previous falls, and urinary incontinence. The dominant extrinsic factors include an unsafe physical environment, inappropriate use of walking aids, polypharmacy, and lack of security facilities at home or in health facilities. Abstract

Background: Mental health literacy is important as the prevalence of mental health disorders increases, while access to information through digital media on the internet is becoming more widespread. With the different characteristics and patterns of internet consumption in each gene­ration, the level of understanding of internet-based mental health information is expected to vary. This study aims to examine the differences in internet-based mental health literacy among the Baby Boomer, X, Y, and Z generations in Indonesia. Subjects and Method: This study uses a cross-sectional study method with an online survey involving 400 respondents from various generation groups in Indonesia. The independent vari­ables were Baby Boomer, X, Y, and Z generations. The dependent variable was internet-based mental health literacy. Generational difference data was measured using a questionnaire and internet-based mental health literacy understanding was measured using a modified internet-based health literacy scale (eHEALS) questionnaire. The data were analyzed using the Kruskal-Wallis Test and the Mann-Whitney as the post hoc test. Results: The results of the study show that generation Z has a higher level of internet-based mental health literacy. The Kruskal-Wallis test shows that there is a statistically significant difference in internet-based mental health literacy between generations (p=0.025). The results of Mann-Whitney show that internet-based mental health literacy of baby boomers vs. generation X (p= 0.006) and baby boomers vs. generation Z (p=0.005) is significantly different. Conclusion: There are significant differences in internet-based mental health literacy between generations. These findings emphasize the importance of an intergenerational approach to online mental health education programs, so that information can be accessed and understood properly by all age groups.
