Abstract

Sepsis remains a major global healthcare problem, indicate as most frequently cause of morbidity and mortality worldwide. The last consensus of sepsis in 2016 defined sepsis as life threatening organ dysfunction caused by a dysregulated host response to infection. Dysfunction of organs can be represented by Sequential [Sepsis-Related] Organ Failure Assessment (SOFA) score. Score2 points or more consequent to the infection. Nowadays, there is ideal biomarkers of sepsis such as procalcitonin (PCT). However, the use of that markers in developing countries are hardly accessible. Eosinopenia is an prepossess biomarker because eosinophil count is always measured in daily practice and considered as a forgotten marker. The study purpose is to determine the validity of absolute eosinopenia in bacterial sepsis patients. This study is a descriptive observational study, collecting 118 patient’s medical record data from the past, diagnosed as sepsis using consensus criteria of Sepsis-3 between January 1st 2018–December 31st 2019. Eosinopenia validity test in sepsis patients showed 92.7% specificity and 71.4% sensitivity. This study also showed significant differences of absolute eosinophil count between positive sepsis patients and negative group with p value <0.001. Eosinopenia had high specificity so it could be used as a marker of diagnostic in septic patients.

Abstract

Indonesia is the third highest number of leprosy in the world with pockets of high endemicity spread across the archipelago. Leprosy diagnosis in control program include finding of acid fast bacilli (AFB) from slit skin smear (SSS) that assessed for the bacillary index (BI). The solid AFB form assessed for morphological index (MI). This study aimed to analyze the proportion of BI and MI of untreated and treated suspected leprosy SSS. This was a retrospective observational study on BI and MI from 117 suspected leprosy SSS secondary data. Of this, being males was the most prevalent in the age group 19-40 years with 91 untreated (84.6% acid fast bacilli, AFB) and 26 treated (80.8% AFB). Treated 2+ BI still high 23.8% from untreated BI 2+ 23.4%. Untreated MI >50 was found in 25.4% and treated MI >50 is still found in 19.1%. Bacillary index and MI support the diagnosis and detect the transmission of leprosy infection.

Abstract

Madrasah has a very important role in nation building. There are two distinct periods in the history of madrasas. The first was the establishment of the Ministerial Decree 3 in 1975 which marked the beginning of the recognition of madrassas as Islamic educational institutions equivalent to general education. The presence of the Three Ministerial Decree is a step in the right direction towards improving school standards. The purpose of this study is to describe the existence of madrasah in formal education in the digital and global era. The method used in this study is descriptive qualitative. The techniques carried out in taking data are direct interviews with school principals, curriculum departments, teachers, administrative and financial taps at MAN 1 Serang City. The results of this research show that MAN 1 Serang City is a madrasah that follows the digital age era, it can be seen from all classes starting from grades ten to twelve learning media have used digital media such as laptops and infocus. In addition, all children are encouraged when KBM to practice directly using computers. However, the obstacle is that not all field teachers can use digital media, due to their limited time and age. In addition, in managing financial tap data, it is currently using digital media in the form of an Institution-Level Financial Application System (SAKTI) which contains planning, implementation and reporting.

Abstract

The condition of the COVID-19 Pandemic which is full of uncertainty makes the elderly easily plagued by feelings of excessive anxiety which has an impact on physical health conditions. The purpose of the community service was to provide mental and psychosocial support (MHPSS) during Covid-19 pandemic through booklets. The method used health education by distributing booklets. The activity was conducted in Blitar district area through the online network on February 2021 with 21 elderly people as the respondents. The results showed that the elderly were able to follow all the activities given and able to re-practice physical activities that could be done to deal with anxiety as a psychological impact of COVID-19 properly. Batik activity is one form of occupational therapy for People with mental disorders to continue to be carried out so that independence and entrepreneurial spirit can increase, so they will be more productive. The Mental and Psychosocial Support (MHPSS) for all elderly groups in the community needs to be continuously carried out by involving elderly cadres as an effort to control anxiety as a result of the COVID-19 Pandemic

Abstract

Abstract. Tuberculosis (TB) remains a critical global health crisis, demanding non-sputum biomarkers to overcome the limitations of traditional diagnostics. This study investigated Interleukin-10 (IL-10), a cytokine linked to Mycobacterium tuberculosis survival, as a novel, non-invasive diagnostic marker for active TB. A cross-sectional study (Jan–Mar 2024) at DR. Hasan Sadikin General Hospital in Bandung measured serum IL-10 levels via ELISA in 84 individuals. Levels were compared between patients with bacteriologically confirmed TB and those with unconfirmed or suspected TB. Confirmed TB patients showed significantly higher median IL-10 levels (1.58 pg/mL) than unconfirmed cases (1.10 pg/mL; p=0.0001). Diagnostic utility was strong, with the Receiver Operating Characteristic (ROC) curve analysis yielding an Area Under the Curve (AUC) of 0.787 (P < 0.001), achieving 80.0% sensitivity and 67.3% specificity. The significant difference in IL-10 levels confirms its potential as a non-sputum-based clinical biomarker to enhance the diagnosis and global management of active Tuberculosis. Abstrak. Tuberkulosis (TB) masih menjadi permasalahan kesehatan global, perlunya ditemukan biomarker non-sputum untuk mengatasi keterbatasan diagnostik tradisional. Studi ini meneliti Interleukin-10 (IL-10), sitokin yang terkait dengan kelangsungan hidup Mycobacterium tuberculosis, sebagai penanda diagnostik non-invasif yang baru untuk TB aktif. Studi cross-sectional (Januari–Maret 2024) dilakukan di RSUP Dr. Hasan Sadikin, Bandung. Sebanyak 84 subjek diukur kadar IL-10 serumnya menggunakan metode ELISA. Kadar IL-10 dibandingkan antara pasien TB yang terkonfirmasi secara bakteriologis dan yang tidak terkonfirmasi atau suspek TB. Pasien TB terkonfirmasi menunjukkan kadar IL-10 median yang jauh lebih tinggi (1,58 pg/mL) dibandingkan kasus tidak terkonfirmasi (1,10 pg/mL; p=0,0001). Nilai diagnostik yang kuat didukung analisis kurva ROC dengan Area Under the Curve (AUC) sebesar 0,787 (P < 0,001), mencapai sensitivitas 80,0% dan spesifisitas 67,3%. Perbedaan signifikan pada kadar IL-10 menunjukan potensinya sebagai biomarker klinis non-sputum untuk meningkatkan diagnosis dan penatalaksanaan Tuberkulosis aktif
