Abstract

Congenital Megacalyces is a rare anomaly of the renal pelvis-calyceal system characterized by non-obstructive calyx dilatation due to renal medulla hypoplasia. This condition is often misinterpreted as hydronephrosis, leading to unnecessary surgical interventions. Advances in genomic technology and precision imaging have opened up opportunities to understand the molecular basis and anatomical structure of this anomaly more deeply. A literature review was conducted through PubMed, ScienceDirect, and Google Scholar, covering publications that discuss the relationship between genomics, imaging, and clinical management of congenital kidney abnormalities. The integration of Next Generation Sequencing (NGS), 3D MRI reconstruction imaging, and AI-based radiomics analysis has proven to enhance diagnostic accuracy, differentiate between obstructive and non-obstructive abnormalities, and assist in determining appropriate conservative therapies. Case studies demonstrate the association between SETBP1 mutations and the development of bilateral megacalyces, as well as the effectiveness of long-term monitoring based on multimodal data. The integration of genetic, imaging, and clinical data is a strategic step toward precision medicine in the management of Congenital Megacalyces. This approach improves diagnostic accuracy, reduces unnecessary invasive interventions, and supports individualized therapy based on genetic and anatomical risk factors.

Abstract

Pendahuluan: Bronkiektasis adalah pelebaran permanen saluran bronkial akibat peradangan kronis dan infeksi saluran napas yang berulang. Salah satu penyebab utama bronkiektasis adalah tuberkulosis paru, yang dapat menyebabkan pembentukan sekuel, dan infeksi. Mikosis paru juga berperan dalam patogenesis dan eksaserbasi bronkiektasis. Jamur seperti Candida albicans dan Aspergillus spp., yang memiliki potensi patogenik tinggi, sering ditemukan pada pasien bronkiektasis, terutama karena gangguan pembersihan mukosilier dan kemampuan jamur untuk menghindari pertahanan imun inang. Laporan kasus ini mengkaji patofisiologi, diagnosis, dan penatalaksanaan bronkiektasis terinfeksi pasca-TB dengan infeksi mikosis paru. Ilustrasi Kasus: Seorang laki-laki berusia 51 tahun datang dengan keluhan sesak napas dan batuk hilang timbul yang memberat dalam tiga hari terakhir, disertai penurunan berat badan dan nafsu makan. Riwayat menunjukkan TB Paru terkonfirmasi tahun 2024 dengan pengobatan OAT selama enam bulan. Pemeriksaan di RS Abdoel Moeloek menunjukkan bronkiektasis pasca-TB dengan infeksi jamur paru. Diskusi: Kerusakan paru pasca-TB memicu gangguan mukosilier yang mendukung kolonisasi jamur oportunistik. Hasil kultur menunjukkan infeksi Candida glabrata dan Acinetobacter baumannii. Terapi kombinasi antibiotik, antijamur, dan antiinflamasi memberikan respons klinis baik. Kasus ini menegaskan pentingnya diagnosis dini infeksi jamur pada bronkiektasis pasca TB untuk mencegah perburukan. Simpulan: Bronkiektasis pasca-TB meningkatkan risiko infeksi mikosis paru. Diagnosis cepat dan terapi tepat dapat memperbaiki kondisi klinis serta mencegah komplikasi lanjut

Abstract

Background: Antidepressant medications are widely used as a first-line treatment for depressive disorders and various other mental health conditions in both primary and hospital-based healthcare settings. Despite their proven efficacy in alleviating depressive symptoms, antidepressants are frequently associated with a range of adverse effects that may negatively impact patient adherence, treatment satisfaction, and long-term therapeutic outcomes. Understanding the safety profile of antidepressants is therefore essential for optimizing clinical management and improving treatment success. Purpose: To review and synthesize current scientific evidence regarding the commonly encountered adverse effect profiles of antidepressant medications and the strategies used for their management in clinical practice. Results: A literature review of studies published between 2020 and 2025 identified several frequently reported adverse effects associated with antidepressant use, including gastrointestinal disturbances, sleep disorders, sexual dysfunction, weight changes, and neurological symptoms such as headache and tremor. The adverse effect profiles varied across antidepressant classes, including selective serotonin reuptake inhibitors (SSRIs), serotonin–norepinephrine reuptake inhibitors (SNRIs), tricyclic antidepressants, and atypical antidepressants. Management strategies commonly reported in the literature included dose adjustment, switching antidepressant agents or drug classes, the use of adjunctive therapies to alleviate specific adverse effects, and continuous patient education and monitoring to enhance treatment tolerability and adherence. Conclusion: Antidepressant medications exhibit diverse adverse effect profiles that may compromise treatment effectiveness if not appropriately addressed. This literature review highlights the importance of recognizing commonly encountered adverse effects and implementing individualized, evidence-based management strategies to improve drug tolerability and patient adherence. An integrated and patient-centered approach is essential to optimize antidepressant therapy in clinical practice. Abstract

The preclinical phase is crucial stage in preparing medical students for clinical practice, as students are required not only to master theoretical concepts but also to apply them accurately and safely. One important educational strategy at this stage is Clinical Skill Lab (CSL), which serves as a bridge between theory and clinical practice within safe and controlled learning environment. Through CSL activities, students are expected to develop psychomotor skills, clinical reasoning, and professional confidence before interacting directly with patients. This study aimed to evaluate the effectiveness of various CSL learning models, including Self-Directed Learning (SDL), video-based learning media, and Peer-Assisted Learning (PAL), in improving medical students’ clinical competence. A narrative literature review approach was employed. Data were obtained from the PubMed, Google Scholar, and ScienceDirect databases, covering publications from 2016 to 2025, and six relevant articles were systematically analyzed. The findings indicate differences in effectiveness among the learning methods. SDL was found to be less effective when applied independently due to limited supervision and minimal feedback. Video-based learning was shown to enhance visual understanding and memory retention but tended to be passive. In contrast, PAL emerged as the most effective approach, as it promotes active interaction and provides immediate feedback.

Abstract

Allergic rhinitis is a chronic inflammatory disease that occurs widely in the global population. This disease is related to the body's hypersensitivity to allergens from the external environment. Available therapy for allergic rhinitis is only symptomatic, while primary therapy has several problems, such as access, time, and cost. Research related to renewable rhinitis therapy is needed, one of which is the use of probiotics as a therapeutic agent. The research method used was a literature review through the PubMed and GoogleScholar databases using the keywords "Rhinitis," "Probiotics," "NSS," and "TNSS." The literature search process was carried out systematically by applying several inclusion criteria. In total, four final articles were obtained from a total of 27 available articles. The results of nasal symptom score (NSS) and total nasal symptom score (TNSS) measurements in several analyzed studies indicated the potential of probiotics such as NVP1703, the combination of B. animalis Subsp. Lactis BB12 with Enterococcus faecium L3, and Lacticaseibacillus paracasei GM-080 in treating allergic rhinitis symptoms. Probiotics as a renewable therapeutic agent for allergic rhinitis have been shown to reduce symptoms, as measured by a decrease in mean NSS and TNSS values. The antiallergic activity of probiotics occurs through regulatory mechanisms of Th1 cells, Th2 cells, inflammatory mediators, and the production of pro-inflammatory antagonist compounds.

Abstract

Indonesia memiliki keanekaragaman hayati yang kaya, dan banyak spesies tumbuhan yang dapat dimanfaatkan sebagai agen antibakteri. Pemilihan metode ekstraksi diduga memengaruhi efektivitas antibakteri. Artikel ini bertujuan untuk membandingkan efektivitas metode ekstraksi konvensional dan modern dalam menghasilkan hasil optimal dan aktivitas antibakteri. Studi ini dilakukan dengan menggunakan pendekatan tinjauan sistematis. Penelaahan literatur menghasilkan 27 studi yang memenuhi kriteria inklusi. Metode konvensional yang paling umum diterapkan meliputi maserasi dan sokletasi, sedangkan teknik modern yang diidentifikasi meliputi ekstraksi berbantuan gelombang mikro (MAE), ekstraksi berbantuan ultrasonik (UAE), dan ekstraksi fluida superkritis (SFE). Bukti yang ditinjau menunjukkan bahwa metode ekstraksi modern mampu menghasilkan hasil yang lebih tinggi dalam durasi yang lebih singkat dibandingkan dengan teknik konvensional. Lebih lanjut, aktivitas antibakteri tertinggi juga dilaporkan pada ekstrak yang dihasilkan melalui metode modern. Meskipun demikian, beberapa studi menunjukkan bahwa metode konvensional masih dapat menghasilkan aktivitas antibakteri yang kompetitif, tergantung pada pilihan pelarut dan karakteristik fitokimia bahan tanaman. Secara keseluruhan, metode ekstraksi modern menawarkan peningkatan efisiensi dan potensi untuk meningkatkan aktivitas antibakteri, namun pemilihan metode yang ideal harus mempertimbangkan sifat bahan baku, senyawa target, dan aplikasi yang dimaksud.

Abstract

Incision wounds are injuries that result in the disruption of skin tissue continuity and are at risk of infection as well as delayed healing if not managed optimally. Standard treatment often uses chemical antiseptics which risk causing irritation and resistance side effects. Indonesia has a wealth of herbal plants that have the potential to be effective wound healing alternatives. To evaluate the effectiveness of herbal-based topical preparations on incision wound healing in in vivo studies of white rats (Rattus norvegicus). This study is a narrative review conducted by searching for experimental scientific articles (True Experimental) published between 2020–2025. Inclusion criteria included in vivo studies on white rats with incision wound models, using herbal topical preparations (gels, ointments, creams), and comparing them with positive controls (standard drugs) and negative controls. Nine selected articles were analyzed descriptively. The review results showed that plants such as Papaya Stem, Jatropha curcas Leaves, Jatropha multifida Leaves, Lantana camara, Stenochlaena palustris, Shallots, Allium chinense, Lime Peel, and Taro showed significant wound healing activity. Topical preparations with optimal extract concentrations were able to match the healing speed of standard drugs and improve histopathological parameters (collagen and fibroblasts). Based on the literature review, herbal-based topical preparations are proven effective in accelerating incision wound healing in rat models and have the potential to be developed as phytopharmaceuticals.
