Abstract

THE DIFFERENCES  DECREASING PLATELET IN DENGUE HEMORRHAGIC FEVER GRADE I AND II AT BHAYANGKARA TRIJATA HOSPITALDengue hemorrhagic fever was presented by thrombocytopenia with hemoconcentration. The infectious disease caused by dengue virus which is transmitted by the bite of Aedes Aegypti. The degree of severity of dengue hemorrhagic fever is divided into four, namely grade I, II, III, and IV. This study aims to see the mean count of platelets in patients with dengue hemorrhagic fever grade I and II in Bhayangkara Trijata Hospital. Design of the study used cross sectional with study hipotesis is the decreasing platelet count of dengue hemorrhagic fever grade II higher than grade I in Trijata Bhayangkara Hospital. The data was took from the medical records of patients whom were diagnosed with dengue fever by a doctor at the Bhayangkara Trijata Hospital since June 2012 until October 2013. The study result showed the mean of platelet count in dengue hemorrhagic fever grade I was 103.000/mm3 and mean of platelet count dengue hemorrhagic fever grade II was 94.000/mm3. The independent sample t - test obtained p value = 0.244 then is obtained no significant results.

Abstract

The diagnosis of primary or secondary Dengue virus infection is quite difficult because of the wide spectrum of symptoms and signs. Complete blood count testing may be the initial screening to determine diagnosis and therapy accurately. This study aimed to determine the general description of the results of a complete blood count includes the number of platelets, leukocytes, hematocrit, and hemoglobin in patients with primary and secondary Dengue virus infection. This was a descriptive cross-sectional study by taking data from medical record retrospectively with total sampling methode. Samples were patients with primary and secondary Dengue virus infection listed on Sanglah Denpasar Hospital during July 2013 - June 2014. The variables were analyzed by SPSS for Windows. The total sample were 202 peoples (46 primary-infected patients and 156 secondary-infected patients). The proportion of primary-infected patients and secondary-infected patients who has platelet count ?47.750 x 103/mL was 17.4% and 59.6%, the proportion of patients with leukocyte counts >3.965 x 103/mL was 30.4% and 55.8%, the proportion of patients with hematocrit >45.04% was 41.3% and 55.1%, and the proportion of patients with a hemoglobin level of >15.3 g/dL was 39.1% and 51.9%. It can be concluded that the majority of secondary-infected patients had lower platelet counts, but has the leukocyte count, hematocrit, and hemoglobin higher than the primary-infected patients. However, further research is needed to determine the relationship between the variables. Keywords: platelet, leukocyte, hematocrit, hemoglobin, primary and secondary Dengue virus infection.

Abstract

Aplastic Anemia describes a disorder of the clinical syndrome is marked by a deficiency of red blood cells, neutrophils, monocytes and platelets in the absence of other forms of bone marrow damage. Aplastic anemia is classified as a rare disease in developed countries the incidence of 3-6 cases / 1 million inhabitants / year. The exact cause of someone suffering from aplastic anemia also can not be established with certainty, but there are several sources of potential risk factors. Prognosis or course of the disease varies widely aplastic anemia, but without treatment generally gives a poor prognosis

Abstract

Hepatitis B is a liver disease caused by a virus known as Hepatitis B virus (HBV) and is a major global health probelm. The liver is susceptible to a wide variety of adverse effects caused by an excess of alcohol, drugs, infections such as viral health department. Vaccination for children, adolescents and high-risk adults could also prevent hepatitis B. The prognosis is variable hepatitis, cancer and other metabolic disorders. Hepatitis B is the causative agent and belongs to the hepadnaviridae family of viruses. Transmission of HBV virus is through exposure to infected blood and body fluids. One of the important diagnostic criteria of Hepatitis B is by performing a thorough laboratory investigation.

Abstract

Dengue is the most important emerging tropical viral disease of humans in the world today. It is estimated that there are between 50 and 100 million cases of dengue fever (DF) and about 500.000 cases of dengue haemorrhagic fever (DHF) each year which require hospitalization. Persons of all ages can be infected and develop dengue fever. However, children younger than 15 years typically present with only a nonspecific self-limited febrile illness. Untreated dengue hemorrhagic fever mostly likely progresses to dengue shock syndrome which is defined as dengue fever with signs of circulatory failure. The prognosis depends on prevention or early recognition and treatment with case fatality rate is as high as 12% to 14% once shock has set in. Management of dengue fever requires rest, oral fluids to compensate for losses via diarrhoea or vomiting, antipyretics and analgesics. Patients who present with shock may require central venous pressure monitoring. An arterial line may be required in unstable patients for the assessment of blood gases, electrolytes, and coagulation profile to help identify patients needing ventilatory support.

Abstract

The symptoms of liver diseases are very diverging, from the mild one till the severe one. Sometimes we may find that severe heart disorders but the symptoms are too less. We need some tools to make a good diagnosis. We can not only use a good anamnesis, but also have to use good physical examination and the other support test. Transaminase also called aminotransferase. This aminotransferase catalyzes the transfer of the amino group (?NH2) of an amino acid to a carbonyl compound. The liver contains specific transaminases for the transfer of an amino group from glutamic acid to ?-keto acids that correspond to most of the other amino acids. Other transaminases catalyze reactions in which an amino group is transferred from glutamic acid to other compounds. Transamination is one of the principal mechanisms for the formation of necessary amino acids in the metabolism of proteins. Transaminase as a sign to cell damage may divided into Serum Glutamic Oxalocetic Transaminase (SGOT), Serum Glutamic Pyruvic Transaminase (SGPT), and Lactic Dehydrogenase (LDH). Gamma GT and alkali fosfatase correlate with cholestasis. Cholinestrase correlate with liver synthesis capacity.

Abstract

BACKGROUND: Hypoxia has been shown to be able to induce adiposity. However, the mechanism and factors involved in this effect still remains unclear. Hence, we sought to investigate the role of oxygensensitive factors regarding hypoxia-induced adiposity in nematode Caenorhabditis elegans.METHODS: The C. elegans were grown on nematode growth medium (NGM) agar plates seeded with Escherichia coli OP50 at 20°C. The ratio of width/body length was measured using the morphometry analysis. Fat accumulation was examined using Sudan Black methods. Protein levels of sterol binding protein (SBP)-1 were assessed by immunoblotting. Lifespan assay was performed at 20°C and was monitored every two days.RESULTS: The results showed that of all mutant used, only hif-1 mutant which did not experience an increase in the ratio of width/body length (p>0.05) and fat accumulation (p>0.05), indicating that hypoxia-inducible factors (HIF)-1 plays an important role in the pathogenesis of hypoxia-induced adiposity. Both siah-1 and skn-1 mutants experienced SBP-1 protein elevation (p<0.05), and increased fat-6 mRNA expression (p<0.05) which were not experienced by a hif-1 mutant (p>0.05) further supporting that HIF-1 acts as an upstream regulator fromSBP-1.CONCLUSION: In general, the results of this study provide evidences of the involvement of the transcription factor HIF-1 in inducing adiposity under the hypoxic conditions. However, we did not find the involvement of seven in absentia homolog-1 (Siah-1) and skinhead-1 (SKN-1).KEYWORDS: hypoxia, adiposity, fat, HIF-1, Siah-1, SKN-1, C. elegans

Abstract

Tumour necrosis factor alpha (TNF-α) is a cytokine produced by macrophages and other mononuclear cells, is a good antibacterial agent against Salmonella spp, especially Salmonella typhi. Centella asiatica is an alternative drug that is expected as an immunostimulant in patients with typhoid fever. Comparing the effectiveness of Centella asiatica extract the original Bali as an immunostimulant and without stimulants in mice infected Salmonella typhi in terms of TNF-α secretion. This study is an experimental study with a post test only with control group design. A total of 20 mice were divided into 4 groups. The first and second groups each given Centella asiatica extract 75 mg/20 g bw (0.5 cc) and without a given extract for 4 weeks. Both groups were inoculated orally Salmonella typhi 106 per mL of bacteria in the second week. The third and fourth groups were given thiamphenicol with Centella asiatica extract 75 mg/20 g bw (0.5 cc) and thiamphenicol without any extract for 4 weeks respectively. Both groups were inoculated orally Salmonella typhi 106 per mL of bacteria in the first day. All groups terminated on fourth week and examination levels of TNF-α by ELISA and gall culture. The mean levels of TNF-α in groups (1–4) is 86.10±2.67 pg/mL, 32.81±11.33 pg/mL, 35.87±3.90 pg/mL and 19.21±2.19 pg/mL respectively. Based on the examination of the gall cultures this study showed positive results in the first and second groups, while a negative result on the third and fourth groups. Based on the One way ANOVA analysis on levels of TNF-α, there are significant differences between the first group with the second group (p<0.05), and between the third and fourth groups also found significant differences (p<0.05) increased levels of TNF-α in mice with Salmonella typhi infection given Centella asiatica extract.

Abstract

Research regardless the effect of noni fruit for increasing bleeding time have already been carried out widely. The similar activity ofnoni fruit extract and acetosal can be concerned that the fruit extract has a potential activity for prolonged bleeding time. This study aimsto know the present of prolonged of bleeding time as a results of intake of combination of noni fruit extract with acetosal on mice. Thisresearch was carried out at Unit Binatang Percobaan, Departemen Farmakologi Universitas Udayana. This is an experimental study withpre and post-test control group design. Subject was compromised of 3 groups of mice and each group contain of 7 mice. The first groupwas treated with a dose of 40 mg/kg bw acetosal, the second group treated with a dose of 100 mg/kg bw ethanol noni fruit extract, andthe third group treated with combination of 40 mg/kg bw acetosal and 100 mg/kg bw ethanol extract of noni fruit. All groups werefed once per day for a week. Bleeding time was determined on the basis of tail bleeding method. This study results that the first groupexperience bleeding time increased from 61.42±9.43 second to 160.71±19.77 second. Increase bleeding time of the second group is from59.14±7.12 to 138.14±59.91 second. For the third group, the bleeding time increases from 65.00±7.91 to 213.00±20.92 second.One Way ANOVA analysis indicates that there is a significant different among these three groups after treatment p = 0.006 (p<0.05).Bleeding time of the third group which was treated with combination of noni fruit and acetosal results in the highest increase compareto the other two groups. In conclusions, combination of noni fruit and acetosal treatment results in increase of bleeding time on mice.

Abstract

Diabetic retinopathy is a complication of diabetes mellitus in the form of microvascular (aneurysma) which can cause blindness.Interleukin-6 is a pro - inflammatory cytokine produced by monocyte macrophage in the early inflammation. Chronic inflammationplays the role on artherosclerosis event, because it is directly correlated with all phases of it and cardiovascular disease as well. Theaim of this study is to know the difference in plasma IL-6 levels among patients suffering from diabetes mellitus together with diabeticretinopathy and without diabetic retinopathy. This study is an observational analytical research with cross sectional design. The sampleswere taken randomly as many as 16 patients, divided in each group suffering from or without retinopathy. The diabetic mellitus patientswere established through their related history and were examined randomly for blood glucose and their fundus or retinal abnormality.The level of IL-6 was examined by ELISA method with double antibody sandwich streptavidin - biotin. Based on this study it was foundthat the mean plasma levels of interleukin 6 (IL-6) of the diabetic mellitus patients with retinopathy was 1.165 ± 0.296 pg/mL andthe mean plasma levels of IL-6 with diabetes mellitus without retinopathy was 0.879 ± 0.230 pg/mL. Based on the normality test withShapiro-Wilk the data showed a normal distribution (p>0.05), while the Levene’s homogenity test obtained homogeneous with p= 0.369(p>0,05). The statistical analysis with two independent samples t-test showed that there were significant differences between the groupsof patients who suffered diabetes mellitus with retinopathy and without retinopathy (p=0.005). It can be concluded so far that the meanlevels of interleukin-6 (IL-6) plasma in diabetic mellitus patients with retinopathy is higher compared to patients with the same diseasebut without retinopathy.
