Abstract

Stroke iskemik terjadi ketika arteri otak terhambat sehingga aliran darah berkurang menuju otak. Stroke iskemik membutuhkan perawatan yang lama dan pemakaian regimen obat yang komplek sehingga berpotensi terjadi Drug Related Problems (DRPs), kejadian yang tidak diharapkan berkaitan dengan terapi obat yang dapat menggangu keberhasilan dalam proses penyembuhan. Penelitian bertujuan mengidentifikasi penyebab terjadinya DRPs pada pasien geriatri yang didiagnosa penyakit stroke iskemik. Studi ini merupakan observasional dengan menggunakan desain penelitian cross sectional. Pengumpulan data dilakukan secara retrospektif melalui rekam medis tahun 2020 di Rumah sakit Bhayangkara Anton Soedjarwo, Kota Pontianak. Sejumlah 94 pasien memenuhi kriteria inklusi meliputi pasien dengan diagnosa stroke iskemik berumur 60 tahun atau lebih dan rekam medis lengkap. DRPs diklasifikasikan berdasarkan penyebabnya menggunakan PCNE (Pharmaceutical Care Network Europe) ver 9.0. Analisis deskriptif dilakukan untuk mendiskripsikan jumlah dan jenis DRPs. DRPs ditemukan pada 28 pasien (29,78%) dengan total kejadian DRPs sebanyak 44 kejadian. DRPs diklasifikasikan dalam penyebab yang berkaitan dengan pemilihan obat yang tidak tepat sebanyak 24 pasien (25,53%) dan dosis tidak tepat sebanyak 20 pasien (21,26%). Jenis obat yang banyak menimbulkan DRPs adalah Amlodipin, dan ditemukan sebanyak 14 (23,33%). Adanya DRPs pasien geriatri stroke iskemik menjadi tantangan bagi farmasis untuk dapat menyelesaikan masalah terkait obat sehingga meningkatkan optimasi terapi pasien stroke iskemik.

Abstract

Flebitis merupakan infeksi yang sering terjadi selama perawatan dan dapat menyebabkan komplikasi lebih lanjut seperti septikemia, sehingga dapat memperpanjang waktu perawatan di rumah sakit. Kondisi ini dapat dipengaruhi oleh pengetahuan perawat mengenai administrasi obat injeksi intravena. Tujuan penelitian ini untuk mengetahui pengetahuan perawat mengenai faktor risiko sediaan intravena yang berkaitan dengan kejadian flebitis. Penelitian ini menggunakan rancangan cross-sectional. Subyek penelitian perawat pada suatu rumah sakit pemerintah di Kabupaten Karanganyar, Jawa Tengah. Sejumlah 159 perawat terlibat dalam penelitian yang dilakukan bulan September hingga Oktober 2020. Pengetahuan perawat mengenai faktor risiko sediaan intravena yang berkaitan dengan kejadian flebitis dinilai menggunakan instrumen kuesioner. Kuesioner terdiri dari karakteristik responden, pengetahuan perawat mengenai faktor risiko sediaan intravena yang berkaitan dengan kejadian flebitis, dan pernyataan perawat mengenai perawatan dan pemeliharaan kanula intravena perifer. Kuesioner telah diuji validitas dengan hasil valid. Analisis data dilakukan secara deskriptif. Hasil penelitian menunjukkan 83,08% responden memiliki pengetahuan yang baik mengenai faktor risiko sediaan intravena, dan 86,30% responden menjawab dengan benar terkait pernyataan perawat mengenai perawatan dan pemeliharaan kanula intravena perifer. Pelatihan obat suntik dan teknik aseptik masih sangat diperlukan mengingat masih terdapat 10,69% responden yang belum mendapatkan pelatihan serta diperlukan juga workshop penyegaran agar dapat memaksimalkan pelayanan di rumah sakit

Abstract

Pasien lanjut usia atau geriatri rentan terhadap berbagai masalah klinis yang timbul akibat proses degeneratif dan berpengaruh dalam menjalankan kegiatan pribadi sehingga membutuhkan bantuan dari orang lain yang disebut caregiver. Seorang caregiver, selain membantu kegiatan pribadi pasien geriatri, juga berperan penuh dalam proses penyembuhan pasien. Hal ini dapat mengakibatkan caregiver akan memiliki beban mental yang harus diperhatikan agar proses caregiving pada pasien lanjut usia dapat berjalan dengan lancar. Penelitian ini dilakukan untuk mengidentifikasi aspek penguasaan emosional individu caregiver serta menelaah perannya dalam proses pengobatan pasien geriatri, terutama pada kepatuhan penggunaan obat. Penelitian ini menggunakan rancangan cross sectional yang melibatkan 131 responden. Pengumpulan data dilakukan melalui wawancara terhadap caregiver pasien usia lanjut yang mendapatkan layanan kesehatan di Puskesmas daerah Sulawesi Selatan. Instrumen kuesioner Pearlin Mastery Score digunakan untuk mengukur penguasaan emosional individu caregiver, dan pill count method untuk mengetahui tingkat kepatuhan pasien. Analisis data menggunakan statistik deskriptif terkait data demografi pasien dan caregiver serta Spearman-rho untuk mengetahui hubungan antar variabel. Hasil analisis menunjukkan angka koefisien korelasi sebesar 0,417** yaitu tingkat hubungan antara variabel penguasaan emosional caregiver terhadap tingkat kepatuhan pasien geriatri adalah sebesar 0,417 atau korelasi sedang dengan hubungan antar variabel searah dan bernilai signifikan (p<0,05). Semakin tinggi tingkat penguasaan emosional caregiver maka semakin tinggi tingkat kepatuhan pasien geriatri. Kata kunci: Geriatri, Caregiver, Preventive Health Behaviour, Kepatuhan Penggunaan Obat

Abstract

Schizophrenia psychiatric disorder is characterized by the emergence of delusions, hallucinations, and the inability to distinguish between reality and fantasy. In the acute phase, people with schizophrenia can experience agitation that can endanger themselves and those around them. Rapid-acting antipsychotics with minimal side effects are needed in treating acute agitation in schizophrenia patients. Haloperidol is a first-generation antipsychotic that acts on dopamine D2 receptors, which helps reduce positive and negative symptoms. Haloperidol has been widely used for the treatment of acute agitation because of its rapid effects, but the prevalence of extrapyramidal symptoms is relatively high. Diphenhydramine is an antihistamine with anticholinergic activity used to treat extrapyramidal symptoms. Several guidelines recommend the use of diphenhydramine as a pharmacological therapy for extrapyramidal symptoms, but in practice, psychiatry provides diphenhydramine together with haloperidol as prophylaxis. This combination helps treat acute agitation, where haloperidol can reduce symptoms of schizophrenia, and diphenhydramine can provide sedation and treatment of extrapyramidal symptoms. The challenge in managing acute agitation in schizophrenia patients continues due to drug side effects. A better understanding of the pathophysiology of schizophrenia is expected to produce effective pharmacological therapy alternatives in treating acute agitation

Abstract

Tuberkulosis Multi Drug Resistant (TB MDR) merupakan kondisi kuman Mycobacterium tuberculosis yang telah resisten terhadap isoniazid dan rifampisin dengan atau tanpa OAT lainnya. Banyaknya jumlah obat pada terapi TB MDR serta rendahnya angka keberhasilan pengobatan mendorong untuk mengevaluasi permasalahan terkait pengobatan (DRPs) yang terjadi pada pasien TB MDR tersebut. Penelitian ini dilakukan dengan pendekatan deskriptif observasional metode potong lintang. Pengamatan sampel dilakukan secara prospektif pada pasien TB MDR rawat jalan di RSUD Dr. Moewardi Surakarta dan RSP Ario Wirawan Salatiga. Klasifikasi DRPs mengacu pada klasifikasi pharmaceutical care network Europe versi 8.02. Kejadian DRPs pada pasien rawat jalan TB MDR terjadi pada seluruh pasien (117 pasien) dengan total 168 kasus. DRPs kategori efektifitas terapi 29,51% (51 kasus), DRPs kategori keamanan terapi 70,49% (117 kasus).

Abstract

Electronic transactions or e-commerce there is an online mall or marketplace, which is a platform that can directly become an intermediary for all transactions between sellers and buyers. One of the payment methods known to the public is the COD (Cash On Delivery) system, where payment is made when the courier delivers the ordered goods to the customer's home. This causes problems that are often experienced by couriers delivering ordered goods. Often there are several customers who commit irresponsible acts by not being willing to pay for the COD package that has been ordered. Actions such as those mentioned certainly violate the agreement between buyers and sellers who use the Cash On Delivery method, where payment must be made when the courier has delivered the order to the buyer's address. This study aims to determine the legal protection of couriers and liability for COD (Cash On Delivery) packages. This research is made with a normative juridical method through a legal approach supported by primary, secondary and tertiary legal materials. The result of this research is the legal protection of courier rights for consumers who are not responsible for the COD package they bought.

Abstract

Monoclonal antibodies (mAbs) are powerful therapeutic agents known for their high specificity and effectiveness in treating complex diseases. Yet, one of their major challenges is their tendency to aggregate, which can reduce treatment efficacy and even trigger unwanted immune responses. This review focuses on how pharmaceutical excipients can help prevent antibody aggregation, particularly during the drug administration process, a phase often overlooked. We conducted a systematic literature search using Scopus, PubMed, and ScienceDirect, targeting studies on excipients and aggregation in therapeutic proteins. After applying the selection criteria, six original research articles were analyzed. The findings reveal that several excipients—such as L-arginine, polysorbates, trehalose derivatives, proline analogs, and cyclodextrins—can effectively stabilize mAbs. They work by reducing interfacial stress, minimizing protein–protein interactions, and preserving antibody structure during stress conditions like infusion or inhalation. These insights highlight the importance of choosing the right excipient based on the administration route to ensure antibody stability and therapeutic impact. By shifting the focus from formulation to administration, this review provides a practical perspective that can support the development of safer and more effective mAb therapies.

Abstract

Cardiovascular diseases (CVDs) are common diseases in the elderly as aging can cause changes in the heart and blood vessels. Patients with CVD are prone to multimorbid conditions which lead to the use of multiple medications (polypharmacy). Polypharmacy is susceptible to drug-drug interaction (DDI) and potentially inappropriate medication (PIM). This study aimed to identify polypharmacy, DDI, and PIM in hospitalized elderly patients with CVD. The design of this study is retrospective cross-sectional. A number of 100 medical records were employed and analyzed descriptively. Polypharmacy is defined as administering multiple medications at the same time or excessive medication administration of ≥4 drugs or ≥5 drugs. Potential DDI was detected by Lexicomp Application. PIM was identified based on Beers criteria 2019. The findings showed that most patients were male (55%) and aged 60-70 (72%). Most of the patients (48%) were categorized as moderate polypharmacy (8-10 medications/day). Potential DDI was revealed in 97% of patients, and Beers Criteria 2019 identified 280 cases of PIM. The study showed that polypharmacy, DDI, and PIM frequently occurred in hospitalized elderly patients with CVD. Pharmacists should pay more attention to reviewing the prescription. Therefore, the process of pharmacotherapy can be optimized and increase safety for elderly patients with CVDs.

Abstract

Geriatric patients are elderly patients (>60 years) who have various chronic diseases with polypharmacy that varies by age group. This study aims to determine the differences in the age of elderly patients according to age group in the internal medicine ward of Panti Wilasa RSUD Dr. Cipto. This study is a descriptive study with retrospective data collection on discharge prescriptions of geriatric patients after undergoing inpatient care in the internal medicine ward obtained from medical record data. Sampling used a purposive sampling technique. In this study were classified into three groups: age 60 to 74 years; age 75 to 90 years; and age > 90 years. During the study period 86% were aged 60-74 years and 14% were aged 75-90 years, while in this study there were no ages over 90 years. In this study the most common gender was male, namely in the 60-74 age group 50.5% while in the 75-90 age group 73.3%. The length of hospitalization in the 60-75 age group was 49.5% and the 75-90 age group 60%, namely 4-5 days. And the top 5 types of drugs that are widely used in this age group are omeprazole, aspirin, metoclopramide, spironolactone and warfarin. The number of diagnoses received by the age group 60-74 years with 2 diagnoses 34.7%, with 3 diagnoses 31.6%. While the number of diagnoses received by the age group 75-90 years with 2 diagnoses 40%, with 3 diagnoses 33.3%. This shows that elderly patients have the same therapeutic picture in terms of age group.   Keywords: Geriatric Patients, Internal Medicine Ward, Polypharmacy, Chronic Disease.
