Abstract

Asupan tinggi fruktosa dapat menyebabkan gangguan metabolisme terhadap timbulnya sindrom metabolik dan diabetes melitus tipe 2. Dalam penelitian ini akan disampaikan beberapa dampak yang di dari konsumsi fruktosa. Penelitian ini bertujuan untuk membuat pemodelan hewan uji dengan pemberian fruktosa 60% terhadap resistensi insulin. Metode penelitian menggunakan 8 ekor tikus putih jantan galur Wistar yang dikelompokkan menjadi 2 kelompok yaitu kelompok normal dan kelompok fruktosa 60%. Parameter yang diamati terhadap bobot badan, kadar glukosa darah, Konstanta Tes Toleransi Insulin (KTTI), kadar adiponektin dan kadar TNF-α serta histologi jaringan adiposa. Hasil penelitian menunjukkan bahwa pemberian fruktosa 60% selama 60 hari menunjukan adanya perbedaan untuk kadar glukosa darah yang dumulai pada hari 30-60, Nilai KTTI kelompok fruktosa lebih kecil dibandingkan kelompok normal, kadar adiponektin kelompok fruktosa lebih kecil dibandingkan kelompok normal , sedangkan  kadar TNF-α kelompok fruktosa lebih besar dibandingkan kelompok normal  dan terjadi pembesaran diameter jaringan adiposa putih dan pengecilan adiposa cokelat  pada kelompok fruktosa, namun belum terjadi perbedaan pada bobot badan. Dapat disimpulkan bahwa pemberian fruktosa 60% selama 60 hari dapat digunakan sebagai pemodelan hewan uji resistensi insulin.

Abstract

This study aimed to summarize the pharmacological activities of the sungkai plant (Peronema canescens Jack.) as a potential Indonesian herbal medicine. A literature review was conducted using Google Scholar and Scopus, with keywords such as "pharmacology," "activity," "sungkai," and "Peronema canescens." Inclusion criteria were articles with these keywords, published between 2014 and 2024, in English or Indonesian. Exclusion criteria were review articles, articles not related to sungkai plants, and in vitro and in vivo studies not available in full text. The study identified seven pharmacological activities of the sungkai plant: antioxidant, antibacterial, analgesic, antidiabetic, antihyperuricemic, anti-inflammatory, and immunomodulatory. The antioxidant activity was demonstrated using Thin Layer Chromatography (TLC) and IC50 values. The antibacterial activity was shown through bacterial growth inhibition using the Kirby-Bauer disk diffusion method. The analgesic activity was tested using the writhing test and Hot Plate methods. The antidiabetic activity was studied with alloxan induction method. Overall, the sungkai plant exhibits various pharmacological activities, suggesting its potential as an herbal medicine for treating various diseases.

Abstract

Hyperglycemia in individuals with type 2 diabetes mellitus is primarily driven by the rapid hydrolysis of starch by the enzyme α-amylase in the pancreas and the breakdown of oligosaccharides by α-glucosidase in the intestine. Peronema canescens Jack. (PC) has shown promise as a potential antidiabetic agent. This study aimed to evaluate the total flavonoid, phenolic, and α-amylase inhibitory activity of extracts and fractions derived from PC leaves using both in vitro and in silico approaches. The ethanol extract of PC leaves was fractionated through liquid-liquid extraction using n-hexane, ethyl acetate, and water as solvents. Preliminary phytochemical screening of the extracts and fractions identified the presence of alkaloids, flavonoids, saponins, tannins, and steroids/triterpenoids. The n-hexane fraction exhibited the highest total flavonoid content, averaging 203.37±4.38 mg QE/gram, while the ethyl acetate fraction demonstrated the highest total phenolic content, averaging 147.04±0.79 mg GAE/gram. Furthermore, the ethyl acetate fraction showed the strongest α-amylase inhibitory activity, with an average inhibition rate of 70.38±1.26%. In silico analysis, combined with GC-MS identification, suggested that three compounds, bis(2-ethylhexyl) phthalate, myristyl oleate, and 14 beta H-pregna may contribute to the observed α-amylase inhibitory activity. These findings highlight the potential of PC as a source of natural antidiabetic agents.

Abstract

BACKGROUND: Peronema canescens Jack is traditionally employed in the treatment of inflammation, malaria, and immune-related disorders. Despite its traditional use, scientific evidence on its immunomodulatory effects, interleukin (IL)-6 modulation, hematological impact, as well as its related acute toxicity data remains limited. Therefore, this study was conducted to investigate the immunostimulatory potential of P. canescens leaf extract through in vitro and in vivo assessments and evaluate its acute toxicity profile.METHODS: The in vitro immunomodulatory activity of P. canescens ethanolic extract, n-hexane fraction (NHF), ethyl acetate fraction (EAF), and water fraction (WF) were assessed by measuring IL-6 inhibition in lipopolysaccharide-stimulated RAW 264.7 macrophages. For in vivo analysis, Balb/C mice were divided into six groups: a normal control (Na-CMC), a positive control (50 mg/kg BW/day Stimuno Phyllanthus niruri extract), a negative control (80 mg/kg BW/day cyclophosphamide as immunosuppressant), and 3 treatment groups receiving P. canescens extract at doses of 100, 200, and 400 mg/kg BW/day. Hematological parameters, including white blood cell (WBC) counts, lymphocyte percentages, and neutrophil percentages, were analyzed. Acute toxicity studies were performed by administering P. canescens extract at doses of 300, 2000, and 5000 mg/kg BW over observation period.RESULTS: EAF exhibited the most pronounced IL-6 inhibition in vitro. In vivo, the administration of P. canescens extract at 200 and 400 mg/kg BW significantly elevated WBC and lymphocyte levels while concurrently reducing neutrophil counts. No mortality or neurotoxic manifestations were observed, confirming the P. canescens extract’s safety profile up to 5000 mg/kg BW.CONCLUSION: P. canescens leaf extract, particularly EAF, demonstrates robust immunomodulatory activity with a favorable safety margin. Abstract

Monascus purpureus red yeast rice (MpRYR), popularly known as angkak, is a Chinese fermented rice health product. The red pigments and monacolin K contained in MpRYR are sensitive to various factors, including acidity, temperature, oxidation, and light; thus, MpRYR is unstable and susceptible to damage during storage and processing. It was reported that monacolin K in red yeast rice solution was easily degraded under thermal treatment at 85–121°C, and the absorbance of MpRYR extract increased at an alkaline pH. This work aimed to study the effects of pH on the stability of pigment extract  MpRYR. MPRYR was purchased from Raja Godhong herbal supplier and extracted with 70% ethanol for 3 × 24 h. The resulting MpRYR extracts were subjected to various pH conditions, and the pH and absorbance were measured, and the degradation rate of MpRYR extracts was calculated. The initial pH of MpRYR extract is 5.75, which increased to 5.97 (+3.82%) at 3 h. The MpRYR extract was more stable in an acidic medium with a degradation rate of 3.80% at 3 h. Research results suggest that acidic conditions are preferable for storing and formulating MpRYR-based products to preserve their functional efficacy.

Abstract

Angiotensin receptor neprilysin inhibitor (ARNi) saat ini digunakan untuk pengobatan gagal jantung dengan penurunan fraksi ejeksi (HFrEF). Selain gagal jantung, obat golongan ARNi telah banyak diteliti potensinya untuk menangani berbagai kondisi penyakit. Sacubitril/valsartan merupakan obat golongan ARNi pertama yang disetujui oleh FDA. Penelitian ini adalah studi literatur yang dilakukan dengan mengumpulkan jurnal ilmiah menggunakan kata kunci “angiotensin receptor-neprilysin inhibitor (ARNi)”, “sacubitril/valsartan”, “heart failure”, “hypertension” menggunakan mesin pencarian Google Scholar dan PubMed dalam rentang waktu 10 tahun terakhir. Hasil yang diperoleh yaitu kombinasi mekanisme kerja dari obat ini telah menunjukkan manfaat klinis yang signifikan dalam pengobatan berbagai penyakit kardiovaskular. Studi klinis menunjukkan efikasi ARNi dalam mengurangi morbiditas dan mortalitas pada pasien dengan HFrEF, serta berpotensi untuk menangani berbagai kondisi kardiovaskular lainnya seperti hipertensi, infark miokardium, dan aritmia. Namun, beberapa efek samping seperti hipotensi, hiperkalemia, dan angioedema mungkin dapat terjadi sehingga dalam pemberian obat ARNi diperlukan pertimbangan berdasarkan karakteristik pasien dan faktor risiko yang terkait, serta pemantauan selama pengobatan.

Abstract

Penggunaan kortikosteroid jangka panjang dalam penatalaksanaan berbagai kondisi medis seringkali diperlukan, namun berhubungan dengan efek samping serius. Oleh karena itu, strategi tapering off menjadi sangat penting untuk diterapkan. Review artikel ini bertujuan untuk mengevaluasi berbagai strategi tapering off kortikosteroid yang telah diteliti serta implikasi klinisnya. Penelusuran literatur dilakukan pada basis data Google Scholar, PubMed, dan Sciencedirect selama 10 tahun terkahir. Studi yang diulas mengungkap bahwa keberhasilan tapering off sangat bergantung pada pendekatan yang dipersonalisasi, dengan mempertimbangkan kondisi klinis, tingkat keparahan penyakit, respons individu terhadap terapi, dan risiko kekambuhan. Temuan menunjukkan bahwa penghentian mendadak pada dosis kortikosteroid yang tinggi dapat memicu relapse dan perburukan kondisi, sementara penggunaan agen steroid-sparing sebagai bagian dari strategi tapering off dapat mengurangi kebutuhan kortikosteroid dan efek sampingnya. Implikasi klinis dari review ini menekankan pentingnya pemantauan ketat dan penyesuaian dosis secara bertahap untuk mengoptimalkan outcome terapi dan meningkatkan kualitas hidup pasien.
