Abstract

BACKGROUND: Malaria remains a global health concern and an endemic disease in Indonesia. Sambiloto (Andrographis paniculata) and spirulina (Spirulina platensis) are two potential antimalarial agents which act as antioxidants and antiinflammatories that can suppress morbidities during chronic inflammation in malaria, such as proliferation in the colon. This study aims to investigate the effects of A. paniculata extract and S. platensis administration on Ki-67 expression in medial colon epithelial cells of Plasmodium berghei-infected mice measured by H-score.METHODS: Thirty P. berghei-infected male Swiss-Webster mice were divided into five groups: negative controls (carboxymethyl cellulose/CMC); positive controls (dihydroartemisinin-piperaquine/DHP); A. paniculata extract alone (AP); A. paniculata extract in combination with S. platensis extract (AP+ES); and with S. platensis powder (AP+PS). All mice were infected with P. berghei on day 0. The treatment for each group were given 3 days before infection (D-3) until the day of infection (D0) for 28 days after infection. Colon tissues were processed with immunohistochemistry to detect Ki-67.RESULTS: A difference in Ki-67 expression was observed among the groups (p<0.01). The mean H-score for the CMC control group is 135.503±6.723. The lowest level of Ki-67 expression was observed in the AP+PS group (H-score= 110.941±7.079). AP group did not show a significant difference from the CMC group (p=0.514) and neither did the AP+ES group (p=0.234).CONCLUSION: In conclusion, administration of A. paniculata extract and S. platensis powder lowers Ki-67 expression in medial colon epithelial cells of P. berghei-infected mice.KEYWORDS: malaria, spirulina, Ki-67, Andrographis paniculata, Spirulina platensis

Abstract

Since the coronavirus was first discovered in Wuhan, it has widely spread and was finally declared a global pandemic by the WHO. Image processing plays an essential role in examining the lungs of affected patients. Computed Tomography (CT) and X-ray images have been popularly used to examine the lungs of COVID-19 patients. This research aims to design a simple Convolution Neural Network (CNN) architecture called SCOV-CNN for the classification of the virus based on CT images and implementation on the web-based application. The data used in this work were CT images of 120 patients from hospitals in Brazil. SCOV-CNN was inspired by the LeNet architecture, but it has a deeper convolution and pooling layer structure. Combining seven and five kernel sizes for convolution and padding schemes can preserve the feature information from the images.  Furthermore, it has three fully connected (FC) layers with a dropout of 0.3 on each. In addition, the model was evaluated using the sensitivity, specificity, precision, F1 score, and ROC curve values. The results showed that the architecture we proposed was comparable to some prominent deep learning techniques in terms of accuracy (0.96), precision (0.98), and F1 score (0.95). The best model was integrated into a website-based system to help and facilitate the users' activities. We use Python Flask Pam tools as a web server on the server side and JavaScript for the User Interface (UI) Design

Abstract

Infiltration of T lymphocyte cells in tumor tissue is a good prognosis in cancer patients. The infiltrating lymphocyte(TIL) tumor is the phenotype of CD4+ T lymphocytes and CD8+ T lymphocytes that infiltrate tumors. TIL can have abeneficial effect as well as a detrimental effect on breast carcinoma. There are three subtypes of T lymphocytes thatinfiltrate tumors which are T-helper (Th), T-citotoxic (Tc) and T-regulators (Treg). The role of Th lymphocytes in tumorimmunity can be divided into three phases, that are the initial induction phase, the phase maintaining the Tc responseas an effector and the memory formation phase. Th lymphocytes will secrete cytokines that can stimulate tumor cellsto increase the expression of MHC class I molecules, so that it will further optimize the function of Tc lymphocyticcytotoxicity which has the most dominant role in cytolytic tumor cells. Treg lymphocytes effectively suppressantitumor responses from Tc lymphocytes and other immune cells so that they can facilitate tumor development. Inbreast carcinoma, Treg lymphocytes have a different influence on each of the cancer subtypes.

Abstract

Placenta accreta spectrum (PAS) is a cause of serious maternal morbidity and mortality. FIGO 2020 designated PAS into grade 1 (non-invasive), grade 2 (superficial invasion), grade 3A (deep invasion), grade 3D (deep invasion with disruption of the serosa), and grade 3E (deep invasion with adherence to extrauterine structures). Currently, the trophoblast cell in accreta spectrum is associated with similar characteristics to cancer. Both conditions can induce angiogenesis, marked by upregulation of vascular endothelial growth factor (VEGF). This resulted in the formation of extensive neovascularization. The secretion of VEGF is induced by hypoxia, which was related to risk factor of PAS occurrence; defect of the decidua due to endometrial tissue damage.

Abstract

Female breast cancer is the most common diagnosed cancer worldwide in 2020, with an estimated 2.3 million new cases or 11.7% from total 18,1 million cases, followed by lung, colorectal, prostate, and stomach cancers. In 2040, the breast cancer cases are predicted will be increased into 3 million cases per year or grow 40% from 2020 data. It is also estimated the number of deaths due to breast cancer will be one million death per year in 2040. In Indonesia, the number of new breast cancers reached 68,858 cases (16.6%) of a total of 396,914 new cases of cancer in 2020. ​ The incidence and mortality rates of breast cancer are 44 and 15.3 per 100,000 population, respectively in Indonesia. The most common breast cancer treatments used currently are surgery, chemotherapy, and radiation. These three treatments involve a rather invasive approach and often causes side effects. Serious side effects can reduce a patient's quality of life, lead to non-compliance to chemotherapy, and lead to drug resistance.​ Some cancer patients use alternative therapy such as complementary medicine including herbal medicine to treat the cancer. Patients try to discover and utilize natural-derived anti-cancer drugs because it believes has fewer side effects and more affordable. One of Indonesia plant which is therapeutically beneficial is Ficus deltoidea. Ficus deltoidea leaves had been used in traditional medicine to treat diabetes, inflammation, diarrhoea, cancer and infections. F. deltoidea leaves contained flavonoids, saponins, tannins, steroids and terpenes which known for its anticancer effect.

Abstract

Breast cancer remains a significant cause of cancer-related mortality among women globally, highlighting the importance for preventive strategies targeting early molecular events. BRCA1 plays a critical role in maintaining genomic stability through DNA repair mechanisms. However, the potential of soybean phytochemicals to modulate BRCA1 activity at the molecular level, particularly through computational approaches, has not been extensively explored. This study aimed to evaluate the chemopreventive potential of soybean phytochemicals targeting the BRCA1 protein using an in silico approach. A total of 32 compounds were prepared and docked into the BRCA1 binding site using Autodock Tools 1.5.7, followed by interaction analysis and visualization, prediction of pharmacokinetic and toxicity profiles using SwissADME, pkCSM, and ProTox. The results showed that the top compounds exhibited binding energy ranging from -6.04 to -8.07 kcal/mol, which were lower than the reference compound. Interaction analysis revealed stable binding with key amino acid residues, including Met1775, Leu1839, and Lys1702 through hydrogen and hydrophobic interactions. Among the evaluated compounds, daidzin showed the most balanced profile in terms of binding affinity, interaction relevance, and favorable ADMET properties. This study provides a systematic in silico evaluation of soybean phytochemicals targeting BRCA1 and highlights their potential as candidates for breast cancer chemoprevention.

Abstract

Latar belakang Akibat gambaran morfologik dan imunohistokimia yang tumpang tindih, sulit dibedakan antara karsinoma medular dan karsinoma invasif no special type (NST) dengan gambaran medular derajat 3. Padahal kedua entitas tersebut memiliki perbedaan tata laksana dan prognosis. Tujuan penelitian ini untuk menganalisis penggunaan indeks apoptosis (IA) untuk mempertajam diagnosis karsinoma payudara medular secara obyektif. Metode Penelitian retrospektif analitik secara potong lintang telah dilakukan terhadap 20 kasus karsinoma medular dan 20 kasus karsinoma invasif NST derajat 3. Penilaian indeks apoptosis dilakukan dengan metode terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate in situ nick endlabeling (TUNEL). Kedua kasus dilakukan simulasi core biopsy. Indeks apoptosis antara kedua kasus dibandingkan dan dihitung titik potongnya. Hasil Indeks apoptosis pada karsinoma medular lebih tinggi secara bermakna dibandingkan karsinoma invasif NST derajat 3 (p=0,001). Berdasarkan kurva ROC, didapatkan titik potong yang optimal pada IA 1,25. Uji kappa terhadap keselarasan sediaan simulasi core biopsy dan eksisi/mastektomi mendapatkan hasil 0,3. Kesimpulan Indeks apoptosis digunakan untuk mempertajam diagnosis karsinoma medular payudara pada sediaan eksisi/mastektomi. Indeks Apoptosis sediaan simulasi core biopsy tidak selaras dengan eksisi/mastektominya sehingga penggunaannya pada core biopsy dapat membantu jika IA>1,25 dengan morfologi mencurigakan karsinoma medular. Kata kunci : core biopsy, indeks apoptosis, karsinoma invasif NST, TUNEL.

Abstract

Latar belakangKarsinoma kolorektal merupakan tumor ganas ketiga di dunia. Sembilan puluh lima persen karsinoma kolorektal merupakan adenokarsinoma yang berasal dari lesi prekursor adenoma. Dilaporkan 15%-20% karsinoma terkait dengan infeksi virus. Virus yang diduga berhubungan dengan karsinoma kolorektal adalah human papilloma virus (HPV) dan tipe tersering adalah 16 dan 18. Hubungan antara HPV dan karsinoma kolorektal masih menjadi perdebatan. Penelitian ini bertujuan untuk mengetahui perbedaan prevalensi infeksi HPV pada adenoma dan adenokarsinoma kolorektal di Departemen Patologi Anatomik FKUI/RSCM Jakarta dengan menggunakan teknik polymerase chain reaction (PCR).MetodePemeriksaaan DNA HPV pada 33 kasus adenoma dan 33 kasus adenokarsinoma kolorektal dengan teknik nested PCR MY/GP dan elektroforesis. Pada kasus dengan hasil HPV positif, dilanjutkan PCR menggunakan primer spesifik HPV 16 dan HPV 18. Subjek penelitian berasal dari Departemen Patologi Anatomik FKUI/RSCM.HasilSatu dari 33 kasus (3,0%) adenoma dan 3 dari 33 kasus (9,1%) adenokarsinoma positif infeksi HPV. Satu kasus adenoma positif HPV bukan merupakan tipe 16 dan 18. Satu kasus adenokarsinoma dengan positif HPV merupakan tipe 16, 2 kasus merupakan gabungan tipe 16 dan 18.KesimpulanPrevalensi infeksi HPV pada adenokarsinoma lebih tinggi dibandingkan adenoma kolorektal. Tipe HPV pada kasus adenokarsinoma kolorektal merupakan tipe 16 dan 18.

Latar belakangLimfoma sel NK/T ekstranodal tipe nasal (NKTCL; extranodal NK/T-cell lymphoma, nasal type) dapat dikategorikan menjadi beberapa subtipe berdasarkan ekspresi CD56 dan CD16 sesuai dengan pola perkembangan fenotip sel natural killer (NK) normal. Ekspresi CD56 banyak ditemukan pada tahap diferensiasi sel NK yang lebih awal, sedangkan ekspresi CD16 terdapat pada sel NK dengan diferensiasi yang lebih mature. Selain fenotip normal, adanya infeksi virus Epstein-Barr (EBV) sebagai etiologi NKTCL berkaitan dengan fenotip teraktivasi yang ditandai dengan ekspresi CD30, kemudian mengaktifkan jalur sinyal nuclear factor kappa beta (Nf-KB) sehingga dapat menyebabkan pertumbuhan dan proliferasi sel tumor. Pada penelitian ini diteliti hubungan antara subtipe NKTCL berdasarkan ekspresi CD56 dan CD16 dengan ekspresi CD30 serta indeks proliferasi Ki-67.MetodaPenelitian ini menggunakan metoda potong lintang. Sampel terdiri atas 32 kasus NKTCL di Departemen Patologi Anatomik Fakultas Kedokteran Universitas Indonesia/Rumah Sakit Cipto Mangunkusumo (FKUI/RSCM) periode Januari 2010 sampai April 2016. Dilakukan pulasan imunohistokimia CD16, CD30 dan Ki-67, sedangkan pewarnaan CD56 diambil dari arsip. Selanjutnya dihitung persentasis positivitas pada setiap pulasan tersebut.HasilPositivitas CD56 ditemukan pada semua kasus (100%). Positivitas CD16 sebanyak 16 (50%) kasus, positivitas CD30 sebanyak 17 (53,13%) kasus, dan indeks proliferasi Ki-67 tinggi sebanyak 16 (50%) kasus. Tidak terdapat perbedaan bermakna antara subtipe NKTCL (CD56+CD16- dan CD56+CD16+) dengan ekspresi CD30 (p=0,723). Tidak terdapat perbedaan bermakna antara subtipe NKTCL (CD56+CD16- dan CD56+CD16+) dengan indeks proliferasi Ki-67 (p=0,480). Tidak terdapat perbedaan bermakna antara ekspresi CD30 dengan indeks proliferasi Ki-67 (p=0,723).KesimpulanSubtipe imunofenotipik NKTCL (CD56+CD16- dan CD56+CD16+) tidak berhubungan dengan fenotip teraktivasi (CD30) maupun proliferasi (Ki-67).

Abstract

Latar belakangLimfoma non-Hodgkin sel B jenis sel besar difus (DLBCL; Diffuse Large B-Cell Lymphoma) adalah jenis limfoma non-Hodgkin yang paling banyak ditemukan dan merupakan suatu entitas yang sangat heterogen secara klinik maupun morfologik. Para ahli mengelompokkan DLBCL ke dalam dua subtipe yaitu subtipe Germinal Center B Cell-Like (GCB) dan non-Germinal Center B-Cell Like (non-GCB), di mana GCB memiliki prognosis yang lebih baik. Penjelasan tentang perbedaan dari kedua subtipe belum sepenuhnya diketahui. Penelitian ini dilakukan dengan tujuan untuk mengetahui perbedaan ekspresi Bcl-2 pada kedua subtipe DLBCL yang mungkin dapat menjelaskan perbedaan prognosisnya.MetodeStudi potong lintang dilakukan pada 41 kasus DLBCL, terdiri atas 18 kasus subtipe GCB dan 23 kasus subtipe non-GCB. Dilakukan review diagnosis histopatologik dari sediaan H&E serta review imunofenotip dari sediaan imunohistokimia. Dilakukan pemeriksaan ekspresi Bcl-2 secara imunohistokimia dan penilaian positivitas dengan nilai cut off 31%. Selanjutnya dilakukan analisis statistik menggunakan uji Mann-Whitney.HasilSeluruh kasus DLBCL, sejumlah 18 kasus subtipe GCB, 15 kasus ekspresi Bcl-2 dikategorikan tinggi. Sejumlah 23 kasus subtipe non-GCB ditemukan 22 kasus ekspresi Bcl-2 termasuk kategori tinggi. Ekspresi Bcl-2 pada DLBCL subtipe non-GCB (nilai rerata 78,70%) lebih tinggi secara bermakna dibandingkan ekspresi Bcl-2 subtipe GCB (nilai rerata 66,61%) (p<0,05).KesimpulanEkspresi Bcl-2 pada DLBCL subtipe non-GCB lebih tinggi dibandingkan ekspresi Bcl-2 subtipe GCB. Hal ini mungkin dapat menjadi salah satu penjelasan perbedaan prognosis diantara keduanya.
