Abstract

                Metode Hanlon adalah sebuah metode yang digunakan untuk membandingkan berbagai macam masalah kesehatan secara adil dan obyektif, serta sebagai dasar untuk merencanakan strategi perbaikan guna meningkatkan pelayanan. Tujuan penelitian ini untuk mengetahui gambaran pengelolaan obat tahap perencanaan dan pengadaan di RS Wijayakusuma Purwokerto dibandingkan dengan indikator standar dan dilakukan penetapan prioritas masalah dengan metode Hanlon kemudian memberikan rekomendasi perbaikan untuk penyelesaian masalah. Rancangan penelitian bersifat deskriptif, pengambilan data secara retrospektif dan concurent. Data yang dihasilkan berupa data kuantitatif dari penelusuran dokumen tahun 2022 dan penilaian metode Hanlon pada saat penelitian. Indikator standar yaitu kemenkes RI (2010), Satibi et al. (2019) dan Pudjaningsih (1996). Pemilihan responden penilaian metode Hanlon secara purposive sampling. Hasil penelitian menunjukkan, tiga indikator tidak memenuhi standar yaitu persentase kesesuaian jumlah permintaan (83,21%) dengan nilai OPR (22,65), persentase dana yang tersedia terhadap dana yang dibutuhkan (75,38%) dengan nilai OPR (26,15) dan frekuensi tertundanya pembayaran oleh RS terhadap waktu yang telah ditetapkan (32 kali) dengan nilai OPR (23,52). Kesimpulan dari penelitian ini bahwa prioritas utama diselesaikan yaitu indikator kecukupan dana, kedua indikator tertundanya pembayaran faktur dan ketiga indikator kesesuaian jumlah permintaan. Rekomendasi perbaikan: melakukan evaluasi dan penyesuaian anggaran, penyediaan anggaran yang memadai, menerapkan sistem pengadaan yang kompetitif dan meningkatkan fleksibilitas pengadaan obat.

Abstract

Background: Telepharmacy has emerged as the latest approach for remote pharmaceutical services. However, some factors influence the success of this technology, such as pharmacists knowledge, readiness to adopt, and perspectives. Objective: This study aims to evaluate the level of knowledge, perception, and readiness to use telepharmacy systems among pharmacists. Methods: Articles from PubMed, Google Scholar, and Scopus (November 2023) were reviewed following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines in 2022. Inclusion criteria encompassed cross-sectional studies examining knowledge, perception, and readiness to use telepharmacy among pharmacists within the last ten years. The quality of studies was assessed using the Joanna Briggs Institute Critical Appraisal Checklist. Results: Eight articles were included in the study, with the largest respondent pool from a study in Vietnam (414) and the smallest from Indonesia (78). All included articles demonstrated good quality with no significant bias. Five studies discussed knowledge levels, with four showing good knowledge (89.74%, 82.67%, 62.3%, 96.83%). Regarding telepharmacy perspectives, two studies had less favorable perspectives. Three studies indicated adequate readiness for telepharmacy implementation (87%, 72%, 63.23%), while one was unprepared (27.74%). Conclusion: Most studies show good knowledge, perception, and readiness to implement telepharmacy. There is a need for telepharmacy training programs in pharmacy schools or hospitals. Abstract

Drug planning is essential to ensure the fulfillment of the right type, amount, and time criteria. Forecasting can be utilized during the planning stage to predict future drug needs. Perfect forecasting is impossible due to uncertainties in various factors, necessitating selecting the best method. This study aimed to identify the optimal forecasting method for healthcare facilities based on the smallest Mean Absolute Deviation (MAD), Mean Square Error (MSE), and Mean Absolute Percent Error (MAPE) values obtained from forecasting results using time series methods like Single Moving Average (SMA), Weight Moving Average (WMA), (Single Exponential Smoothing) SES, Double Exponential Smoothing (DES), and Triple Exponential Smoothing (TES). This research involved a descriptive observational study with retrospective data and adhered to PRISMA guidelines. PubMed, Google Scholar, and Garuda served as the data sources. Nine articles meeting the eligibility criteria were employed. The findings revealed that the SES, DES, and TES methods produced forecasts with MAPE values below 10%, indicating highly accurate forecasting. The MAPE values for the SMA and WMA methods were less than 50%, which is still acceptable. Therefore, the ES methods, particularly SES, are highly recommended for accurate drug planning. Forecasting accuracy factors include data stability, pattern consistency, and smoothing constants. The SES method emerged as the best forecasting method, generating the smallest MAD, MSE, and MAPE values compared to other methods, falling below 10%, reflecting highly accurate forecasting.

Abstract

Digestive surgery is a treatment for diseases of the parts of the body involved in digestion, which has a fairly high risk of infection. The study aimed to obtain a description of the types and specific characteristics of patients and the rationality of antibiotic use among digestive surgery patients. This study was conducted using observational analysis. Data on antibiotic use was collected prospectively from medical records of digestive surgery inpatients at the Academic Hospital Universitas Gadjah Mada (UGM) from January to March 2023. The Gyssens method was used to evaluate qualitatively the use of antibiotics. A total of 76 patients met the inclusion and exclusion criteria. No significant difference between the number of male and female patients was identified. The results showed that 24 (31.58%) acute appendicitis patients are the primary diagnosis most often encountered in patients undergoing digestive surgery. The most frequently used prophylactic antibiotics for digestive surgery patients were ceftriaxone (53.85%) and cefazolin (41.03%). Meanwhile, the most frequently used therapeutic antibiotics were ceftriaxone (41.94%) and levofloxacin (29.03%). The Gyssens analysis shows that large irrational use of antibiotics is still observed.

Abstract

Knowledge of cardiovascular disease is essential for providing information about healthy behavior and to be proactive in reducing modifiable risk factors, control prevention, and improving early recognition. The existing study about knowledge of CVD in Indonesia is still limited. Hence, this study aimed to describe knowledge of CVD among general population in Indonesia and its association. A cross-sectional survey was conducted in 34 provinces in Indonesia from November-December 2021. Data were retrieved using a self-administered questionnaire. The Chi-Square analysis was performed to determine the association that contributes to overall knowledge of CVD. Among the 694 participants, 32.1% originated from Java, with the median age was 24 years and dominated by women (72.5%). The mean score of overall CVD knowledge was 59.6%, and only 39.1% of the participants had good knowledge of CVD. Cerebrovascular disease (94.7%) was the most identified various type of CVD. Likewise, unhealthy diet (77.2%), obesity (74.6%), and lack of exercise (72.5%) were the most identified risk factors. More than 50% of the participants recognized heart attack symptoms such as chest pain (77.1%) and shortness of breath (66.1%). Meanwhile, numbness or weakness (84.7%), trouble speaking (76.9%), and loss of balance (72.3%) were recognized as stroke symptoms. Gender, age, education level, working status, healthy food, smoking status, family history of CVD, ever checked blood pressure, fasting blood glucose, and total cholesterol were associated with overall knowledge of CVD. The disparity between level of CVD knowledge is not prominent among general population in Indonesia and further study need to establish.

Abstract

Background: Technological advancements in healthcare have driven BPJS Kesehatan Indonesia to develop the Mobile JKN application to enhance accessibility, efficiency, and quality of services. Despite its potential benefits, the implementation of this application faces various challenges, such as application errors, differing levels of digital literacy, and data security issues, necessitating an evaluation of its adoption and acceptance. Aims: To analyse the public acceptance of Mobile JKN based on the Technology Acceptance Model (TAM) variable while also exploring user experiences towards the application. Methods: A descriptive-analytical approach with a cross-sectional design. Data were collected through a questionnaire developed based on the TAM model, involving 406 respondents across Indonesia. Data analysis was performed using a regression linear test with SPSS after all basic assumptions were met. Results:  Perceived Ease of Use, Perceived Usefulness, and Attitude Toward Use individually and simultaneously impact Actual System Use (Sig. 0.000; R-Square 0.827). Respondents generally answered “Agree” to all TAM indicators, with mean scores of 4.20 for Perceived Usefulness, 4.16 for Perceived Ease of Use, 4.25 for Attitude Toward Use, and 4.33 for Actual System Use. Most users accessed the app through self-access (48%), frequently used features were membership information (28%), and encountered issues such as application errors (25%). Conclusion: The app was deemed relevant, accurate, easy to use, and beneficial. To enhance public acceptance, BPJS Kesehatan should focus on developing a user-friendly Mobile JKN app by incorporating key TAM factors, optimizing popular features, improving technical stability, and regularly addressing user feedback to ensure effective health administration and service delivery. Abstract

Flebitis merupakan infeksi yang sering terjadi selama perawatan dan dapat menyebabkan komplikasi lebih lanjut seperti septikemia, sehingga dapat memperpanjang waktu perawatan di rumah sakit. Kondisi ini dapat dipengaruhi oleh pengetahuan perawat mengenai administrasi obat injeksi intravena. Tujuan penelitian ini untuk mengetahui pengetahuan perawat mengenai faktor risiko sediaan intravena yang berkaitan dengan kejadian flebitis. Penelitian ini menggunakan rancangan cross-sectional. Subyek penelitian perawat pada suatu rumah sakit pemerintah di Kabupaten Karanganyar, Jawa Tengah. Sejumlah 159 perawat terlibat dalam penelitian yang dilakukan bulan September hingga Oktober 2020. Pengetahuan perawat mengenai faktor risiko sediaan intravena yang berkaitan dengan kejadian flebitis dinilai menggunakan instrumen kuesioner. Kuesioner terdiri dari karakteristik responden, pengetahuan perawat mengenai faktor risiko sediaan intravena yang berkaitan dengan kejadian flebitis, dan pernyataan perawat mengenai perawatan dan pemeliharaan kanula intravena perifer. Kuesioner telah diuji validitas dengan hasil valid. Analisis data dilakukan secara deskriptif. Hasil penelitian menunjukkan 83,08% responden memiliki pengetahuan yang baik mengenai faktor risiko sediaan intravena, dan 86,30% responden menjawab dengan benar terkait pernyataan perawat mengenai perawatan dan pemeliharaan kanula intravena perifer. Pelatihan obat suntik dan teknik aseptik masih sangat diperlukan mengingat masih terdapat 10,69% responden yang belum mendapatkan pelatihan serta diperlukan juga workshop penyegaran agar dapat memaksimalkan pelayanan di rumah sakit

Abstract

According to the 2018 Indonesian Basic Health Research data, the prevalence of hypertension in the Yogyakarta Special Region (11.0%) is higher than the national average of 8.8%. Hypertension with chronic kidney failure is currently a prevalent and increasing issue. This study aimed to investigate the pattern and rationality of antihypertensive use in hypertension patients with chronic kidney failure (CKF) participating in the National Health Insurance (Jaminan Kesehatan Nasional/JKN) program at Universitas Gadjah Mada (UGM) Academic Hospital period of 2020 to 2022. The study was conducted using quantitative descriptive non-experimental methods. Data collection was carried out retrospectively through medical records of patients diagnosed with hypertension with CKF (ICD I12.0). Samples were selected using purposive techniques and analyzed by comparing the suitability of drug selection with standard hypertension therapy with CKF based on JNC 8 and NKF-K/DOQI, including the rationality of their use. A total of 68 patients over the age of 18 were used as samples. The The majority of drug groupsresults showed that a combination of 2 drugs is more dominant (52.9%) compared to monotherapy (29.4%) and a combination of 3 drugs (17.6%). The majority of drug groups used were angiotensin receptor blockers (ARBs), especially candesartan (45%). ARB and calcium channel blockers (CCB), especially candesartan+amlodipin (50%), and ARB +CCB+diuretic, especially andesartan +amlodipine + furosemide (33%).. The rationality of drug use showed that 66.2% in the right indications, 89.7% the right drugs, 100% in the right patients and 85% the right doses. Therefore, health workers still need to make efforts to encourage rational drug use.

Abstract

Background: The prevalence of hypertension is higher among the elderly population. Elderly patients are considered a high-risk population for Drug-Drug Interactions (DDIs) due to multi-morbidity-related polypharmacy, age-related physiological changes, as well as pharmacokinetic and pharmacodynamic changes.Objectives: This study conducted a drug use survey in the inpatient department of the Academic Hospital of Universitas Gadjah Mada Yogyakarta to assess Drug-drug interactions (DDIs) in individual prescriptions for elderly hypertensive patients.Methods: This research employs an observational study design with a retrospective cohort approach. This study was conducted at the Academic Hospital of Universitas Gadjah Mada Yogyakarta in January-December, 2021. The inclusion criteria for this study included patients diagnosed with hypertension, aged 60 years or older and those hospitalized. Ethical approval for this research was granted by the Faculty of Medicine, Public Health and Nursing Research Ethics Commission at Universitas Gadjah Mada, Yogyakarta under approval number KE-FK-0500-EC-2022.Results: 120 prescriptions from 120 patients were collected from the medical records maintained by the Academic Hospital of Universitas Gadjah Mada Yogyakarta. This study identified 66 cases of Drug-Drug Interactions (DDIs), mainly due to drug interactions comprising 8 interactions unrelated to hypertension, and 58 related to hypertension. Three actual drug interaction events resulted in increased blood pressure, while the others were potential drug interactions.Conclusion: From this study, it can be concluded that elderly patients are considered a natural high-risk population for DDIs. Each patient should be assessed individually based on their prescriptions, although in many cases, DDIs are unavoidable.Keywords: Drug-Drug Interactions; Elderly; Hypertension; Inpatients

Abstract

Penelitian ini dilatarbelakangi oleh kesulitan yang dialami oleh PAK Pusat di DIY dalam mendapatkan sertifikat CDAKB dari Kemenkes RI. Tujuan penelitian ini adalah untuk mengetahui tingkat kesesuaian pelaksanaan kegiatan distribusi PAK Pusat di DIY terhadap standar CDAKB dan memahami faktor yang memengaruhi tercapainya kesesuaian kegiatan distribusi PAK Pusat dengan standar CDAKB. Desain penelitian adalah deskriptif evaluatif pada 20 PAK Pusat secara purposive sampling di DIY selama 3 bulan penelitian. Pendekatan kuantitatif menggunakan instrumen kuesioner dan pendekatan kualitatif menggunakan pedoman wawancara kepada PJT Alkes. Analisa data kuantitatif dengan perhitungan persentase tingkat kesesuaian pelaksanaan 13 aspek standar CDAKB pada PAK Pusat, kemudian dikategorikan dalam katagori sesuai dan belum sesuai, sedangkan data kualitatif disajikan dengan menarasikan faktor yang memengaruhi tercapainya kesesuaian dengan standar CDAKB. Hasil penelitian menunjukkan tingkat kesesuaian kegiatan distribusi PAK Pusat di DIY terhadap standar CDAKB adalah 75%, sedangkan faktor yang memengaruhi kesesuaian kegiatan distribusi PAK Pusat dengan standar CDAKB adalah karakteristik PAK Pusat, faktor manusia, faktor sistem dan faktor lingkungan. Kesimpulan penelitian, masih terdapat ketidaksesuaian kegiatan distribusi PAK Pusat di DIY dengan standar CDAKB dan faktor yang memengaruhi kesesuaian perlu ditindaklanjuti dengan usaha melaksanakan kegiatan distribusi sesuai standar CDAKB baik dari sisi ketersediaan sarana-prasarana serta dokumen pendukung.
