Abstract

Pasien diabetes melitus yang mendapatkan terapi insulin rata-rata mempunyai kadar HbA1c relatif tinggi. Kurangnya pengetahuan dan kepatuhan pasien terkait terapi insulin dapat menjadi menyebab terjadinya hipoglikemia yang dapat memperburuk kondisi klinik pasien. Medication Therapy Management merupakan sebuah model intervensi apoteker berkolaborasi dengan tenaga kesehatan lainnya untuk mengatasi permasalahan terkait obat dan meningkatkan outcome pasien secara individu. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh penerapan medication therapy management terhadap kadar HbA1c pasien diabetes melitus yang menggunakan terapi insulin. Penelitian ini menggunakan rancangan kuasi eksperimental dengan one group pretest-posttest design dilakukan pada 25 pasien diabetes melitus yang mendapatkan terapi insulin yang rutin berkunjung ke poliklinik RSUD Toto Kabila Gorontalo dan memenuhi kriteria inklusi. Pasien mendapatkan intervensi pelayanan berbasis medication therapy management dan dilakukan pengukuran kadar HbA1c pre dan post intervensi. Analisis perubahan kadar HbA1c pre dan post intervensi menggunakan uji paired t-test. Setelah pelayanan berbasis MTM terjadi penurunan kadar HbA1c dengan rata-rata 10,93 ± 2,22 menjadi 9,42 ± 2,54 dengan p = 0,006. Hal ini menunjukkan bahwa penerapan medication therapy management dapat menurunkan kadar HbA1c pasien diabetes melitus yang mendapatkan terapi insulin.

Abstract

Warfarin telah lama digunakan sebagai antikoagulan utama untuk fibrilasi atrium. Dengan indeks terapi sempit, penggunaannya membutuhkan pemantauan ketat, terutama pada lansia yang mengalami perubahan fisiologis yang memengaruhi farmakokinetik dan farmakodinamiknya. Time in therapeutic range (TTR) menjadi parameter utama untuk menilai kualitas kontrol antikoagulasi Warfarin. Penelitian ini bertujuan untuk mengeksplorasi faktor-faktor determinan TTR pada pasien lanjut usia dengan fibrilasi atrium kronik. Penelitian ini adalah penelitian observasional dengan desain cross sectional. Data diperoleh dari rekam medis pasien fibrilasi atrium yang menjalani rawat jalan di Poli Jantung RSUP Dr. Sardjito Yogyakarta. Pengambilan sampel dilakukan dengan metode consecutive sampling. Sebanyak 40 pasien yang memenuhi kriteria inklusi dan eksklusi diikutsertakan dalam penelitian ini. Hasil penelitian menunjukkan bahwa indeks massa tubuh dan risiko perdarahan (HAS-BLED) berpengaruh terhadap kualitas kontrol antikoagulasi Warfarin. Pasien non-obesitas berpotensi kecil untuk mencapai kontrol antikoagulasi yang baik (TTR ≥65%) dibandingkan pasien obesitas (OR = 0,14, CI 95% = 0,02 – 0,91, p = 0,039), pasien dengan risiko perdarahan rendah-sedang (skor HAS-BLELD 1-2) 9,1 kali lebih berpotensi mencapai TTR ≥65% dibandingkan pasien berisiko tinggi mengalami perdarahan (skor HAS-BLED ≥3) (OR = 9,1 , 95% CI = 1,29 – 64,11, p = 0,027).

Abstract

Background: Health applications are a crucial technological advancement in healthcare, enhancing service quality and access to essential information. Despite this, challenges in implementation and user adaptation remain. Evaluating these applications is vital, and the Technology Acceptance Model (TAM) provides a dependable framework for assessing user acceptance and meeting its intended goals.Objectives: This review aims to explore the use of the TAM method for evaluating health applications adoption in Asia, focusing on ease of use, usefulness, user attitudes, and factors affecting technology acceptance.Methods: The method used is Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Articles were gathered using PubMed and ScienceDirect employing Boolean search terms. The search focuses on technology acceptance and health applications, combined with the names of Asian countries. Eligible studies must describe the technology acceptance model of a health application in Asia. Quality assessment is performed using JBI and data are systematically extracted and analyzed.Results: A total of 11 original articles meet the inclusion criteria from 2428 articles. TAM plays a pivotal role in understanding the adoption of healthcare technology in Asia. TAM original constructs are widely used, with some integrating additional factors. Continued integration and modification of TAM will enhance its utility in addressing evolving healthcare challenges, ultimately improving healthcare outcomes across diverse user settings. TAM's adaptability and integration with theories like PMT and SUS improve its relevance across various healthcare settings and user contexts.Conclusion: Future research should mitigate biases, employ longitudinal and comparative analyses, and integrate TAM with complementary theories to advance healthcare technology adoption.

Abstract

Background: Individuals with visual impairments tend to have low health outcomes. Exploring health literacy among this group can provide a basis for designing effective strategies for healthcare services that assist this special population.Objectives: This study aimed to determine the health literacy level of individuals with visual impairments.Methods: A cross-sectional study was employed in this study. We administered a survey using the Health Literacy Scale European Union Q16 (HLS-EU-Q16) Indonesian version questionnaire to individuals with visual impairments in Yogyakarta. Members of two Non-Government Organisations (NGOs), namely Ikatan Tunanetra Muslim Indonesia/Indonesian Muslim Blind association (ITMI) and Persatuan Tuna Netra Indonesia/Indonesian Blind Association (Pertuni) were invited as respondents using a purposive sampling method. Data was collected during the NGOs regular meetings in August and September 2019, which took place in some members’ houses or public places throughout Yogyakarta province.Results: A total of 95 eligible respondents who consented to join the study had a health literacy index of 31.29 ± 8.91, classified as problematic, with an index range between 3.13 and 50.00 (inadequate to excellent).Conclusion: Our findings revealed that individuals with visual impairments have problematic health literacy and face multiple determinants to improve their health literacy. This condition requires pharmacists and other health professionals to be aware of it and to develop an appropriate strategy to deal with it comprehensively.

Abstract

A total of 30,199 retail pharmaceutical services were recorded in Indonesia in 2022, distributed across the country. The significant number of retail pharmacies intensifies competition within the sector. To compete effectively, retail pharmacies must gain and retain customer loyalty. It is widely recognized that service quality and price are key determinants of customer loyalty in the pharmacy sector. Notably, delivering high-quality services requires substantial financial investment. On the other hand, reducing prices to gain a competitive advantage may risk the company’s financial stability. This study aims to examine the comparative impact of service quality and price on customer loyalty in the pharmacy context. A narrative review methodology was employed, using the ScienceDirect, Summon, ProQuest, and Google Scholar databases to gather relevant literature. A total of 10 articles were analysed, examining the influence of service quality versus price on loyalty in pharmacies. Of these, eight articles concluded that service quality had a greater impact on customer loyalty, while two suggested that service quality and price had an equivalent influence. The findings of this study indicate that service quality has a more significant impact on customer loyalty in pharmacies.

Abstract

Neonates with low birth weight (LBW) have a higher risk of growth failure. To optimize their growth to target of 15-20 g/kg/d, neonates must receive adequate nutrition. Parenteral nutrition implementation is one of the approaches to provide adequate energy and nutrition to LBW neonates who can not tolerate enteral feeding. This study aims to investigate growth velocity in LBW neonates receiving parenteral nutrition in hospital settings and observe if the target growth velocity was achieved, also describes current parenteral nutrition practices in tertiary hospital settings. A retrospective observational study was conducted in the Neonatal Intensive Care Unit (NICU), Kandou General Hospital, Manado. Sample in this study were neonates with birth weight <2500 g and received only parenteral nutrition for minimum 7 d. Daily weight measurement data was collected through medical records and parenteral nutrition regimens was collected through electronic prescriptions in the Pharmacy Department. Growth velocity was assessed using two parameters, gain weight velocity and change in weight for age Z-scores (WAZ). A total of 73 neonates were used as samples. The results demonstrated that only the extremely low birth weight (ELBW) group achieved the growth velocity recommendation during parenteral nutrition, 16.17 g/kg/d. All the neonates had a negative change of weight Z-scores indicated a declining growth rate during parenteral nutrition administration. We also found lipid emulsion initiation time was later and relatively low energy intake in parenteral nutrition compared to the recommendation. Vitamin as micronutrient was not yet added in parenteral nutrition regimens. Therefore, we suggest starting lipid emulsion from the first day of parenteral nutrition and add vitamin to parenteral nutrition regimen to optimize nutrient intake in order to improve growth velocity in LBW neonates during NICU stay.

Abstract

Background: The paradigm shift from a drug-oriented to a patient-oriented approach requires health facilities to improve the quality of pharmaceutical services. Meeting customer needs and expectations can increase satisfaction and support the achievement of therapeutic goals.Objective: Conduct an in-depth analysis of the available literature to identify customer satisfaction factors towards pharmaceutical services in community pharmacies.Method: A systematic review was conducted by searching articles across three databases: ScienceDirect, Scopus, and PubMed. The search flow is described in the PRISMA Diagram. Inclusion criteria included customer satisfaction with pharmaceutical service articles and research settings in community pharmacies. Exclusion criteria included online or hospital pharmacy articles and review articles, books, theses, dissertations, and reports. Articles are assessed using the Joanna Briggs Institute's critical appraisal instrument.Result:  The findings from the nine articles revealed various factors that influence customer satisfaction, which can be categorized into intangible and tangible factors. Intangible factors include the attitude and communication of pharmacy staff, as well as consultations regarding medications and health conditions. Tangible factors include the facilities and convenience, the availability and variety of medications, prices, and customer demographics.Conclusion: The identified customer satisfaction factors can be useful to increase pharmacy customer loyalty. Thus, it can encourage customers to make repeat orders.

Abstract

Background: Tobacco farm workers are exposed to numerous occupational health hazards, involving chemical, biological, and physical concerns. These hazards can cause acute and chronic health problems, affecting workers’ well-being and productivity. Objective: This systematic review aims to integrate available information on occupational health hazards among tobacco farm workers, identify important risks, and provide feasible mitigation techniques. Methods: A systematic search was conducted in Scopus, ScienceDirect, and PubMed for studies published between 2015-2023. Inclusion criteria comprised reviewed articles focusing on occupational health hazards among tobacco farm workers. Studies were screened, and data were extracted according to PRISMA guidelines. The Joanna Briggs Institute (JBI) and Mixed Methods Appraisal Tool (MMAT) were used to assess quality for the Critical Appraisal Tool. Results: This review found five studies that highlighted major health hazards, such as green tobacco sickness (GTS) caused by nicotine exposure, pesticide poisoning, respiratory problems, musculoskeletal disease, and cancer. Risk factors include extended exposure, a lack of protective equipment, and poor working conditions were frequently noted. Preventive strategies, such as the use of personal protective equipment (PPE), education programs, and government policies, must be prepared. There are research gaps in long-term health impacts and gender-specific concerns among tobacco farm workers. Conclusion: Tobacco workers face several occupational health hazards, including green tobacco sickness (GTS), pesticide exposure, respiratory illnesses, and ergonomic issues that require several interventions for prevention.
