Abstract

Abstract

This study aims to analyze the implementation of the marketing mix strategy (7P) in the Executive Outpatient Services at Nirmala Suri Hospital, Sukoharjo. The background of this research is based on the significant decline in patient visits over three consecutive years, from 5,493 visits in 2022 to 5,048 in 2023, and only reaching 5,436 in 2024, well below the target of 10,000 visits per year. This study uses a descriptive qualitative approach with in-depth interviews, observations, and documentation with 111 informants. The results indicate that the aspects of process (28%), physical evidence (22%), and human resources (19%) are the most appreciated marketing mix elements by patients. Weaknesses were found in the promotional strategy, which has not been optimal, the inaccuracy of doctors' schedules, and the lack of supporting facilities such as lactation rooms and children's play areas. Strategic recommendations include enhancing digital promotion, improving the registration and queuing system, strengthening human resources training, and adding physical facilities to reinforce the premium service image. These findings are expected to serve as a foundation for managerial decision-making in improving competitiveness and patient loyalty.

Abstract

This study aims to analyze the implementation of the 4C marketing strategy—Co-creation, Currency, Communal Activation, and Conversation—in increasing patient visits, particularly among pregnant women covered by the BPJS national health insurance program, to the Obgyn Clinic at Level IV Hospital Samarinda through 4D ultrasound services. These services are positioned as a flagship offering, providing vivid fetal imaging experiences at no additional cost, thereby enhancing the hospital's appeal amidst competition in health services. The research employed a descriptive qualitative approach, with data collected through structured interviews involving hospital management, medical personnel, and patients. It was further supported by quantitative data obtained from questionnaires distributed to 40 pregnant patients. The findings indicate that the Co-creation dimension fosters active patient participation through direct communication and engagement in interpreting ultrasound results. The Currency dimension was perceived very positively, as patients felt they received high-value services without extra charges. Communal Activation strategies through health cadres, social media, and maternal support groups were effective in raising awareness and interest in the 4D ultrasound services. Meanwhile, the Conversation dimension, encompassing both face-to-face and digital interactions, played a vital role in building trust and long-term patient loyalty. Based on these findings, the 4C marketing strategy has proven to be relevant and effective when implemented in National Health Insurance-based hospital services, with an increasing trend in average patient visits from 180 patients per month to 220 patients per month (a 22% increase). This study recommends the use of mixed methods in future research to obtain a more complete understanding of how each dimension influences patient visitation behavior.

Abstract

This study aims to analyze the implementation of strategic management in a community health center (puskesmas) within the framework of the Indonesian National Health Insurance (BPJS Kesehatan). Using a qualitative approach, data were collected through in-depth interviews with the head of the health center, medical staff, and patients, supported by observation and documentation. The findings reveal that the Ogotua Health Center applies strategic management by strengthening primary health care services to reduce unnecessary referrals, improving administrative systems for BPJS claims, and conducting continuous community education about participants’ rights and obligations. Challenges identified include limited human resources, particularly the shortage of physicians, and recurrent technical disruptions in the BPJS digital system, which slow down service delivery. Despite these barriers, the health center mitigates the issues through intensive coordination with BPJS regional offices and monthly evaluation meetings involving all staff. From the patient’s perspective, the service is generally perceived as fast, friendly, and accessible, although sporadic drug stock-outs remain a concern. This study concludes that strategic management contributes significantly to improving the quality and efficiency of BPJS-based services at the primary care level. Future studies should explore digital innovation, supply chain governance, and patient education strategies to further strengthen the role of puskesmas as the frontline of Indonesia’s health system.
