Abstract

This study aims to analyze the implementation of open systems theory in the management of human resources (HR) within a hospital setting. As complex and dynamic healthcare service organizations, hospitals are required to adapt to external changes such as regulations, technology, and patient expectations. The open systems approach introduced by Katz and Kahn (1978) emphasizes the importance of input, process, output, feedback, and environmental adaptation for organizational sustainability. This research employs a qualitative approach through a case study at Hospital X. Data were collected through in-depth interviews, participatory observation, and document analysis, and were analyzed using NVivo software to identify dominant themes. The findings reveal that all elements of the open systems model have been gradually implemented, indicated by demand-based recruitment, cross-unit training, periodic performance evaluations, and the hospital's ability to respond to external changes such as the pandemic and digital transformation. Thematic analysis shows that the five elements interact to form an adaptive and responsive HR management ecosystem. These findings reinforce the argument that a managerial system based on open systems theory enhances productivity, inter-unit coordination, and service quality. This study contributes theoretically to strengthening organizational systems in the healthcare sector and offers practical recommendations for hospital management to optimize HR processes through data, technology, and continuous evaluation.

Abstract

This study aims to provide an in-depth analysis of the application of organizational structure theory within the context of public organizations, using a case study of the Provincial Mental Hospital of Bali. The research focuses on how organizational design, authority distribution systems, and both formal and informal power dynamics influence coordination effectiveness and the quality of mental health services. A qualitative descriptive approach with an intrinsic case study method was employed. Data were collected through document analysis, non-participant observation, and semi-structured interviews with five key informants from various managerial levels. The findings reveal that the organization’s classical bureaucratic structure, characterized by five hierarchical layers, tends to hinder decision-making processes and cause fragmentation in inter-unit collaboration. Moreover, informal power dynamics were found to often override formal command lines, thereby disrupting organizational cohesion and reducing operational effectiveness. Based on these findings, the study recommends a redesign of the organizational structure to be more adaptive, decentralized, and collaborative, in order to improve service performance—particularly in the mental health sector, which demands rapid response and a holistic approach. These results not only offer theoretical contributions to the development of public organization theory but also serve as a practical reference for institutional reform in mental health services at both local and national levels.

Abstract

Healthcare services are a vital component of social development, demanding effective, efficient, and adaptive organizational governance in response to changing environments. This study aims to analyze the contribution of organizational theories in enhancing the performance of healthcare service organizations, focusing on four key approaches: bureaucracy theory, open systems theory, contingency theory, and transformational leadership theory. This research employs a Systematic Literature Review (SLR) method with a qualitative approach, conducted in a systematic, transparent, and replicable manner. Ten scholarly articles published between 2019 and 2024 were analyzed thematically to explore the relationships among theories, patterns of implementation, and their impact on the effectiveness of hospitals and community health centers (puskesmas). The results indicate that bureaucratic theory remains foundational in the structural arrangement of public hospitals, but must be complemented by flexibility to avoid hindering innovation. Open systems theory emphasizes the importance of integration across organizational units and responsiveness to external influences. Meanwhile, contingency theory highlights the necessity of aligning organizational structures and strategies with local contexts and resource capacities. Transformational leadership plays a crucial role in driving positive change, increasing staff participation, and fostering service innovation. This study concludes that integrating all four organizational approaches is essential to shaping a healthcare management model that is adaptive, collaborative, and oriented toward quality and sustainability. The findings serve as both conceptual and practical references for healthcare facility managers, policymakers, and scholars in the field of health management.

Abstract

Digitalization in the healthcare sector, particularly through the implementation of Hospital Management Information System (SIMRS), has become a primary focus in enhancing the effectiveness of patient administration. The aim of this study is to analyze the impact of SIMRS digitalization on the effectiveness of patient administration. The method employed is a literature review, which examines various relevant journals and previous studies on this topic. The analysis results indicate that SIMRS digitalization improves patient administration, as evidenced by service speed, data accuracy, ease of access to information, and patient satisfaction. Although there are challenges in implementation, such as infrastructure constraints and human resource readiness, the benefits derived from this digital system far outweigh the drawbacks. The conclusion of this study demonstrates that digitalization through SIMRS has a significant positive impact on the effectiveness of patient administration.
