Abstract

Background: Perineal rupture is a tear that occurs when a baby is born, either spontaneously or by using tools/actions. Preventive efforts can be made to reduce the risk of perineal rupture by massaging the perineum and using good pressure techniques during labor. Objective: To determine the effect of perineal massage techniques and pressure on perineal rupture in primigravida mothers who give birth. Method: This research uses experimental research methods; this type of research is quantitative with a pretest-posttest research design with a control group and an analytical study design with a retrospective approach. The total research sample required was 30 primigravidas, with details, namely 15 people from the perineal massage group and 15 from the gentle technique. Sampling was taken using an accidental sampling technique, where subjects were selected as research samples based on cases/respondents who happened to be available in a place according to the context. Results: research results of primigravid mothers who experienced perineal rupture in the perineal massage group were two people (20%), and in the menstrual technique group, there were eight people (80%), based on the results of data analysis using the Wilcoxon test, the p-value was obtained. In the perineal massage group, it was 0.000; in the gentle technique group, it was 0.005. Conclusion: It can be concluded that perineal massage is more effective than gentle techniques in preventing perineal rupture in primigravida mothers. To ensure the effectiveness of the chosen method, it is advisable to consult with medical personnel experienced in this field and follow the recommendations and protocols approved by the relevant health authorities. In addition, careful monitoring and care during labor is also essential to minimize the risk of perineal rupture.

Abstract

Labor pain is one of the factors that can hinder the progress of the labor process. The mother's inability to manage pain during labor has the potential to cause uterine contractions to become uncoordinated, which has an impact on prolonging the duration of the first stage and increasing the risk of disruption to the fetal condition. Various nonpharmacological methods have been applied to accelerate labor and reduce pain intensity, including the use of gym ball and rebozo (pelvic rocking) techniques. This study aims to evaluate the effectiveness of using gym ball and rebozo techniques on pain intensity and duration of active phase I in laboring women at the Ota Independent Midwife Practice Site (TPMB), South Lampung, in 2025. The research used a case study approach to two participants who were in the active phase of labor. Primary data were obtained through direct observation of the interventions provided, namely the use of gym ball and rebozo techniques. The results showed that laboring women who received the gym ball intervention experienced a decrease in pain intensity from the moderate category to mild, with a duration of labor of 5 hours. Meanwhile, mothers who received the rebozo technique showed no change in pain intensity, with a duration of labor of 6 hours. It can be concluded that the use of gym ball is more effective in reducing pain intensity and shortening labor time in active phase I compared to the rebozo technique. Therefore, the use of gym ball can be recommended as an alternative nonpharmacological method to support a more comfortable and efficient labor process.

Abstract

Fluor albus (leukorrhea) or commonly referred to as vaginal discharge is excessive fluid discharge from the feminine area. Fluor albus is the most common gynecological symptom that occurs in pre-pubertal girls and teenagers. Vaginal discharge can be normal or abnormal vaginal discharge. Physiological vaginal discharge (normal ) is the discharge of excessive fluid/mucus from the vagina which is white or clear, odorless, painless/itchy, and smooth. Meanwhile, pathological (abnormal) vaginal discharge is the discharge of fluid/mucus from the vagina with an abnormal color change (yellow-green or white -grey), cloudy, and accompanied by pain, odor or itching (Kurniadi, 2017). The aim of this service is to increase knowledge among young women in recognizing flour albus. This counseling was carried out to 45 participants and they received the information well as proven by the correct answers after being given the counseling, namely 97%.

Abstract

Abstract

Labor pain during the active phase of the first stage of labor is a major challenge that can affect both the comfort and progress of the birthing process. Non-pharmacological interventions such as the use of a birth ball and hypnobirthing techniques are widely employed as safe and effective alternatives for pain relief. This study aimed to analyze the effect of birth ball and hypnobirthing interventions on reducing labor pain during the active phase of the first stage of labor among mothers delivering at PMB Mira Dahlia, Toboali District, South Bangka Regency, in 2025. A quantitative study was conducted using a quasi-experimental design with a pre-test–post-test non-equivalent control group approach. A total of 46 respondents were selected through purposive sampling. Pain levels were measured using a combination of the Visual Analogue Scale and Wong-Baker Scale before and after the interventions. Data were analyzed using univariate and bivariate analysis with the Wilcoxon Signed Rank Test and Mann-Whitney U Test. The results showed a significant reduction in labor pain after the birth ball intervention, with all respondents experiencing decreased pain levels (Wilcoxon test p = 0.000; p < 0.05). The hypnobirthing intervention also resulted in a significant reduction in pain (Z = -4.260; p = 0.000). However, the Mann-Whitney test showed no significant difference in effectiveness between the birth ball and hypnobirthing interventions (p = 0.061 > 0.05), although the average pain reduction was greater in the birth ball group. It can be concluded that both birth ball and hypnobirthing interventions are effective in reducing labor pain during the active phase of the first stage of labor. While no significant difference was found between the two methods, birth ball showed a slightly higher average reduction in pain compared to hypnobirthing.
