Abstract

Introduction: Hyperemesis gravidarum (HEG) or continuous nausea and vomiting can result in loss or even death in the mother and fetus. For most women who recover from HEG well, there is clear evidence of an increased risk of pregnancy outcomes.Purpose: The purpose of this scoping review is to map the evidence and conclude the literature about the effects of Hypermesis Gravidarum on pregnancy outcomes.Methods: In this scoping review, we evaluate the emerging evidence that HEG impacts pregnancy outcomes, by using two databases (Pubmed and ScienceDirect).Results: some of the effects of HCG, namely: LBW and Premature, Repetition of hyperemesis gravidarum across generations, Reduction of insulin sensitivity in children in childhood, Risk of placental abruption and increased risk of birth for small for gestational age (SGA), Provides a protective effect on the risk of cleft lip in new babies birth, Delayed nerve development in children, increased blood pressure and abnormal fetal brain development.Conclusion: HEG has an adverse risk to the outcome of pregnancy and can have an impact on the fetus, childhood and adulthood.

Abstract

Abstract: Teenagers with premarital pregnancy can experience more stress, feelings of helplessness, hopelessness, depression, suicidal willingness, feelings of failure, and loss of self-esteem. This study aimed to analyze the anxiety in teenagers who were pregnant before marriage in Manado. This was a mixed method study with quantitative sampling technique using total sampling (67 samples). The instrument used for quantitative research was the PASS questionnaire. Qualitative samples obtained from the results of quantitative research with purposive sampling technique, the type of research used phenomenology. Quantitative data were analyzed univariately and bivariately. The results showed that the highest percentages were found in age 15-18 years (71.6%), senior high school education (59.7%), gestational age of second trimester of pregnancy (46.3%) and third trimester of pregnancy (44.8%). The qualitative analysis obtained four major themes, namely: the anxiety of adolescents with premarital pregnancy, the impact of pregnancy, the experience of adolescents, the factors of premarital pregnancy. The chi-square test for the relationship between anxiety level and premarital pregnancy had a p-value of 0.040. In conclusion, there is a relationship between anxiety level and premarital pregnancy in adolescents in Manado. The contributing factors are inappropriate parents and couple support, and readiness for being pregnancy. Keywords: anxiety; adolescents; pre-marital pregnancy    Abstrak: Remaja hamil di luar nikah lebih mengalami stres, perasaan tidak berdaya, putus asa, depresi, keinginan bunuh diri, merasa diri gagal, serta kehilangan harga diri. Penelitian ini bertujuan untuk menganalisis kecemasan pada remaja hamil di luar nikah di Kota Manado. Penelitian ini menggunakan mixed method dengan pendekatan explanatory sequential design. Populasi dan sampel yakni remaja usia 15-24 tahun yang hamil di luar nikah di Kota Manado. Desain kuantitatif yaitu deskriptif analitik. Pengambilan sampel kuantitatif menggunakan total sampling berjumlah 67 sampel. Instrumen penelitian yaitu kuesioner PASS. Sampel kualitatif berasal dari hasil penelitian kuantitatif dengan purposive sampling, dan jenis penelitian fenomenologi. Analisis data kuantitatif secara univariat dan bivariat. Hasil penelitian mendapatkan persentase tertinggi pada kategori usia remaja 15-18 tahun (71,6%), pendidikan SMA (59,7%), serta usia kehamilan trimester II (46,3%) dan trimester III (44,8%). Hasil analisis kualitatif mendapatkan  empat tema besar, yaitu: kecemasan remaja yang hamil di luar nikah, dampak hamil, pengalaman remaja, dan faktor terjadinya kehamilan di luar nikah. Hasil uji chi-square terhadap hubungan antara tingkat kecemasan dengan hamil di luar nikah mendapatkan nilai p=0,040. Simpulan penelitian ini terdapat hubungan bermakna antara tingkat kecemasan dengan remaja hamil di luar nikah di Kota Manado. Faktor yang memengaruhi kecemasan pada remaja hamil yakni kurangnya dukungan keluarga dan pasangan, dan kesiapan menjalani kehamilan. Kata kunci: kecemasan; remaja; hamil di luar nikah

Abstract

Background: A positive childbirth experience, stating that it must meet or exceed a woman's personal and socio-cultural beliefs and expectations. This includes giving birth to a healthy baby in a clinically and psychologically safe environment, with ongoing practical and emotional support from birth attendants and competent clinical staff. Purpose: This scoping review aimed to identify the best evidence about positive experiences during childbirth, evaluating what factors control and contribute to positive experiences during childbirth. Methods: Databases were searched from 2019 to 2023 from various databases, including PubMed, ScienceDirect, and Cochrane Library/Wiley. As well as using search engines such as Google Scholar and Research Rabbit and selecting them using Rayyan assistance. Critical appraisal and data charting are adopted from the JBI checklist, including cross-sectional, and Qualitative method. Of the 1600 articles identified, only Six met the criteria. Results: The results of this review identified two themes and four subthemes. The two themes are including factors controlling and contribute, and four subthemes such us support and communication factor, participation and empowerment, medical factor and psychology factors. Conclusion: Recommendation for health professionals, especially midwives, is to continue to develop a good and holistic service strategy, by providing support, empowering mothers and also providing good support in order to improve the positive experience during childbirth and minimize negative feedback during childbirth.

Abstract

Air Susu Ibu (ASI) Eksklusif adalah ASI yang diberikan kepada bayi sejak dilahirkan selama enam bulan, tanpa menambahkan atau mengganti makanan atau minuman lain. Berdasarkan data UNICEF, sebanyak 136,7 juta bayi lahir diseluruh dunia hanya 32,6% dari me reka yang disusui secara ekslusif dalam 6 bulan pertama. sementara di negara berkembang hanya 39% ibu-ibu yang memberikan ASI Ekslusif. Hal ini dipengaruhi prespektif ibu tentang pemberian ASI Eklusif. Unit kesehatan yang paling berperan dalam meningkatkan pengetahuan pemberian ASI Eksklusif adalah puskesmas melalui kegiatan posyandu yang dibantu oleh kader Kesehatan. Namun demikian kader di Indonesia belum melaksanakan perannya secara maksimal karena kurangnya kemampuan dan pengetahuan kader tentang ASI Eksklusif. Penelitian bertujuan untuk mengetahui pengaruh Pendidikan kesehatan terhadap tingkat pengetahuan kader mengenai asi eklusif. Desain penelitian yang digunakan dalam penelitian ini adalah desain Pre Eksperimen dengan rancangan one grup pretest postest.teknik pengambilan sampel dengan menggunakan Simple Random sampling dengan jumlah sampel 33 responden. Instrumen penelitian menggunakan kuesioner. Hasil penelitian rata-rata tingkat pengetahuan kader sebelum mendapatkan Pendidikan kesehatan mengenai ASI Ekslusif adalah 6,9697 dan setelah mendapatkan Pendidikan kesehatan mengenai ASI eklusif mengalami peningkatan yaitu 7.773. setelah dilakukan analisis menggunakan uji Wilcoxon didapatkan nilai P value 0,000 < 0,05 maka dapat disimpulkan bahwa terdapat perbedaan yang signifikan anatara pengetahuan kader sebelum dan sesudah mendapatkan Pendidikan kesehatan mengenai ASI eklusif.

Abstract

HIGHLIGHTS Analysis of maternal and fetal risk factors associated with postpartum hemorrhage, comparing data from both developed and developing countries. There are differences in the risk factors for postpartum hemorrhage between developed and developing countries.   ABSTRACT Objective: This study aimed to assess maternal and fetal risk factors for post-partum hemorrhage in both developed and developing countries. Materials and Methods: A systematic review methodology was employed, incorporating a descriptive design. The study followed Joanna Briggs Institute (JBI) techniques, the PCC framework, and the PRISMA-ScR checklist. Article searches were conducted across four databases: Research Rabbit, Science Direct, Wiley, and PubMed. Articles published in English between 2019 and 2024 that met the inclusion criteria were selected. The search terms included combinations of “Postpartum women” OR “Puerperium” AND “Risk factors” OR "Risky conditions” AND “Postpartum hemorrhag*” “Postpartum bleeding” and “Developed countr*” AND “Developing countr*”. Results: Out of 906 articles, 20 met the inclusion criteria, representing both developed and developing countries. The findings were categorized into two themes: risk factors in developed versus developing countries. In developed countries, PPH risk factors are often linked to medical interventions and prolonged labor management, while in developing countries, the focus shifts to conditions like anemia, uterine overdistension, and trauma from delivery practices. Sociocultural disparities further exacerbate risks in both settings. Conclusion: The study reveals significant differences in PPH risk factors between developed and developing countries. By reviewing existing research, it identifies factors that can influence PPH occurrence. The findings emphasize the need for developing policies to screen pregnant, maternity, and postpartum women to mitigate PPH risk.

Abstract

Episiotomi merupakan tindakan insisi perineum yang umum dilakukan dalam proses persalinan, namun praktik ini masih menuai kontroversi. Sejak tahun 1996, sebagian besar literatur menyimpulkan bahwa episiotomi rutin tidak diperlukan dan bahkan berisiko meningkatkan kejadian laserasi perineum derajat tiga atau empat. Meskipun Organisasi Kesehatan Dunia (WHO) telah merekomendasikan pembatasan praktik episiotomi rutin sejak 1990-an, kenyataannya tindakan ini masih sering dilakukan, terutama di negara berkembang, termasuk Indonesia. Scoping review ini bertujuan untuk menganalisis faktor yang mendasari tindakan episiotomi pada ibu bersalin. Scoping review menggunakan PRISMA-ScR Ceklist. Pencarian database dari Pubmed, Scopus, Wiley Online Library, dan Science Direct. dengan kata kunci pencarian “Factor” AND “indication” OR “episiotomy” OR “laceration” OR “rupture perineum”.  Kriteria inklusi yang digunakan meliputi artikel orisinal yang membahas topik relevan serta artikel yang diterbitkan dalam lima tahun terakhir, yaitu antara tahun 2018 hingga 2023. Sebanyak 18 artikel terpilih dianalisis menggunakan checklist dari Joanna Briggs Institute (JBI) melalui proses Critical Appraisal. Tinjauan ini menghasilkan sub tema tehadap faktor yang berhubungan dengan tindakan episiotomy yaitu jarak kelahiran, BMI ibu, gawat janin, berat lahir, paritas, kehamilan ganda, Perineum pendek, perineum kaku, kala II lama, TFU >32 cm, penggunaan oksitosin, dan persalinan instrumen. Dapat disimpulkan bahwa paritas dan berat janin yang paling banyak ditemukan dalam kasus episiotomy. Temuan ini memiliki implikasi penting bagi praktik klinis dan kebijakan pelayanan kebidanan, khususnya dalam mengembangkan panduan pengambilan keputusan berbasis bukti yang lebih selektif dan rasional terkait tindakan episiotomi, guna meminimalkan risiko komplikasi serta meningkatkan keselamatan ibu dan bayi.

Abstract

Background: Delaying cord clamping can increase the baby’s hemoglobin levels. Most of these studies compared delayed cord clamping with a baby’s hemoglobin levels at a few months of birth and not with hemoglobin levels in newborns. After the baby is born, the baby’s circulatory system is no longer related to the mother’s circulatory system. In the fetus, nutrients are obtained directly from the mother’s blood circulation through diffusion, so all the substances from the diffusion may enter the fetus’s body. Thus, if the baby’s hemoglobin level is not measured immediately after birth, the results will likely differ. This study checked the baby’s hemoglobin levels within 48 hours after birth.Objectives: This study aims to determine the relationship between the timing of cord clamping and newborn hemoglobin levels. Methods: This study is correlational research. The study population consists of medical records of maternity mothers and newborns from the Midwife’s Independent Practice in the Bantul area. There were 118 records total, with 84 meeting the inclusion criteria. Data analysis employs Spearman’s Rank correlation. Results: Of the 84 newborns, one had a hemoglobin level of 12.3 g/dL, indicating anemia. In this baby, cord clamping was performed 11 minutes after birth or after the umbilical cord stopped pulsating. The rest have levels above 20 g/dl, ranging from 20.3 to 25.9 g/dl, which includes 65 babies (77.4%). When referencing normal newborn hemoglobin levels, the largest range appears in the polycythemia category. The Spearman Rank test showed a p-value of 0.271. Conclusions: The timing of cord clamping was unrelated to newborn hemoglobin levels.
