Abstract

Background: Sepsis is one of the leading causes of mortality in hospitals, with a significant death rate. The management of sepsis bundles was developed by the Surviving Sepsis Campaign (SSC).  Sepsis bundles include early identification, lactate measurement, antibiotic administration, and fluid resuscitation, and have been effective in reducing mortality rates. Nurses play a strategic role in implementing these bundles, as they are often the first to recognize the early signs of sepsis. However, implementation in clinical settings often faces challenges, including high workloads, limited resources, and insufficient understanding of protocols. Methods: This study used a scoping review method to explore the implementation of sepsis bundles by nurses in hospitals. Literature searches were conducted in PubMed, CINAHL/MEDLINE, Scopus, and ScienceDirect, using the following inclusion criteria: English-language articles with free full-text access and published from January 2014 to June 2024. Results: Of the 211 identified articles, 9 met the inclusion criteria and were analyzed. The findings reveal that effective implementation of sepsis bundles can improve nurses' adherence to protocols and reduce mortality rates. Factors such as continuous education, protocol revisions, leadership support, and optimal staffing policies are key to successful implementation. Conclusion: This study concludes that nurse-led sepsis bundles require a multidimensional approach to enhance clinical outcomes and operational efficiency for hospital sepsis patients.

Abstract

Breast cancer is one of the most common malignancies among women worldwide and is associated with multidimensional burdens throughout the disease trajectory, including diagnosis, treatment, and survivorship. Beyond physical symptoms, women with breast cancer frequently experience psychosocial problems such as anxiety, depression, emotional distress, and impaired quality of life, which may persist even after active treatment and influence treatment adherence and long-term outcomes. Conventional psychosocial support is typically delivered through face-to-face interactions; however, access to such services is often limited by geographical, temporal, and resource-related constraints. Telehealth has emerged as a promising approach to support psychosocial care by utilizing information and communication technologies to deliver health services remotely. In the context of breast cancer, telehealth encompasses various modalities, including mobile health applications, web-based platforms, teleconsultations, text messaging, and remote monitoring systems. Recent developments also indicate a growing integration of artificial intelligence, such as chatbots and conversational agents, to enhance information delivery and emotional support. This narrative review aims to critically discuss the role of telehealth in providing psychosocial support for women with breast cancer. Relevant literature was identified through searches of major scientific databases and analyzed descriptively to explore psychosocial challenges, telehealth concepts, and potential benefits for mental health and patient engagement. The findings suggest that telehealth may contribute to improved access to psychosocial support, enhanced coping, and increased patient involvement in care, although evidence remains heterogeneous across intervention types and outcomes. Telehealth should be considered a complementary approach within holistic breast cancer care, with careful attention to individual needs, digital literacy, and implementation context.

Abstract

ABSTRACT Natural disasters and health crises have experienced a significant increase in the last decade. Nurses have a role in disaster mitigation and response, including providing holistic care to victims in every phase of response. Previous studies reported that nurses are often faced with various situations and challenges in every phase of emergency response such as limited resources and low preparedness. Previous studies have focused more on evaluating disaster preparedness training programs or the effectiveness of standard protocol-based nursing interventions, but have not sufficiently explored how nurses' direct experiences shape their perceptions of preparedness and the challenges they face. This study aims to explore nurses' perceptions of preparedness and challenges in caring for disaster survivors. This study method uses a scoping review approach with the Arksey O'Malley methodology framework. Literature searches were conducted on the Scopus, PubMed, EBSCOhost, and ScienceDirect databases for the journal period 2015 - 2025 resulting in 791 articles. In the process of screening and selecting articles, a systematic approach followed the PRISMA protocol. The process of selecting studies and reporting results followed the PRISMA-ScR guidelines. Of the 791 articles found, 10 final articles met all inclusion criteria. This study identified three nurses' perceptions of preparedness and challenges in caring for disaster survivors, namely perceptions of lack of preparedness, perceptions of limited resources and coordination, and perceptions of emotional distress during the disaster response process. Conclusions: Nurses face psychosocial and technical barriers when caring for disaster survivors. Resource support plays a role in supporting nurses' preparedness in the field. These results emphasize the need for increased disaster training programs and policy development to strengthen nurses' preparedness in dealing with crisis situations. Keywords: Nurse Preparedness, Disaster Survivors, Nurse Perception, Disaster Response, Nursing Challenges.

Abstract

ABSTRACT Nurses are pivotal in disaster management. Suboptimal preparedness impairs response, communication, and patient safety. Virtual Reality-based training can enhance clinical skills, readiness, and decision-making, yet logistical and cost challenges persist. This scoping review aims to map evidence on the impact of Virtual Reality Simulation (VRS) in enhancing nurses’ competency in disaster management. The methodology framework utilized in this scoping review followed Arksey and O'Malley's approach. Literature was systematically searched across PubMed, Scopus, and EBSCO using keywords: nurses, Virtual Reality Simulation Training, nurses’ competency, and disaster management. Inclusion criteria required English-language publications. All extracted data were systematically tabulated to facilitate comparative analysis and thematic synthesis. A total of 164 articles were identified, nine studies met the inclusion criteria and were selected for this scoping review. VR training consistently improved nurses’ disaster preparedness, response capabilities, knowledge, and satisfaction compared to traditional methods. Immersive VR enhances practical skills, decision-making, and engagement, despite challenges related to resources, infrastructure, and standardization. VR-based training enhances nurses’ disaster preparedness more effectively than conventional methods. Despite its benefits, the widespread adoption of this approach is hindered by infrastructure, resource demands, and technical limitations in developing realistic simulation environments. Keywords: Disaster Management, Disaster Preparedness, Nurse’s Competency, Simulation Training, Virtual Reality.

Abstract

High-risk pregnancy is a major global health issue, marked by high maternal and infant mortality rates, and poses both physical and psychological challenges for women. This study aimed to explore the experiences of women with high-risk pregnancies through a scoping and literature review using Medline, PubMed, and Scopus. The search utilized keywords related to maternal experiences in high-risk pregnancies and included qualitative and mixed-method studies published in English from 2016 to 2024. A total of 15 articles were reviewed, revealing six key experiences: continuous care, communication, psychological support, non-pharmacological therapies, understanding of risks, and technological advancements. Findings indicate that social support, ongoing healthcare access, and health technologies can reduce anxiety and improve well-being. Effective communication, continuous care models, and telemonitoring during the pandemic have shown positive impacts on managing high-risk pregnancies.

Abstract

The Emergency Department (ED) is an important department or unit and the core clinical unit of a hospital, which functions to receive, stabilize, and manage patients who need immediate emergency treatment, both in daily conditions and in disasters. Overcrowding in ED is a problem in many countries and is one cause of increased mortality and decreased quality of optimal health and nursing services in the world today, including in Indonesia. There has been little research on overcrowding factors in the ED. Many factors are thought to cause overcrowding. This literature review identifies factors that can influence overcrowding in the ED. The literature search was carried out on the electronic databases of Medline, PubMed, CINAHL, PsycInfo and the Cochrane Library with the keywords factors overcrowding, AND, ED crowding. The inclusion criteria consisted of overcrowding factors in the ED, Fulltext, and publishing from 2013 to 2019 in English. The assessment of the quality of the articles was carried out using instruments from CONSORT. The first search results found 5,026 articles, and after screening and evaluation, 3 articles met the inclusion criteria. The results of the literature review show that the many factors that influence the occurrence of overcrowding in the ED are categorized into three factors, namely input factors (increased number of patient visits), throughput factors (patient disposition and delay in diagnostic examination results), and output factors (access block, inability to transfer patients out of the emergency room to inpatient, lack of inpatient beds, entry rooms, and other resources).
