Abstract

Nurses are frontliners during the COVID-19 pandemic and are vulnerable to ethical challenges when caring for patients with COVID-19. The ethical challenge of nurses is basically due to the gap between nursing practice and nursing goals. This study aims to identify empirical evidence related to the ethical challenges nurses face when caring for COVID-19 patients. This research method uses a literature study that analyzes articles from five international reputable databases such as CINAHL, Pubmed, Proquest, Science Direct, and search engines Google Scholar. The keywords used to facilitate the search for articles were 'COVID-19', 'critical care nursing', 'intensive care nursing', 'intensive care nurses', 'critical nurses', 'intensive care nurses', 'caring', 'ethical challenges', 'ethical issues', 'ethic dilemmas', ethical challenges', 'ethical issues', and 'ethical dilemmas'. with the help of boolean operators and then the articles are selected gradually using PRISMA. From the search results obtained 7 articles that deserve to be analyzed. There are three themes related to ethical challenges that occur, namely ethical challenges in the nursing care process, ethical challenges in nursing professionals, and ethical challenges in health interprofessional relationships. Ethical challenges cause nurse discomfort at work, and can even cause job stress. Regarding these results, nurses need to get support to overcome ethical dilemmas that occur through the provision of resources according to needs, preparation of SOPs regarding the roles and duties of nurses, as well as increasing knowledge regarding the care of COVID-19 patients.

Abstract

ABSTRACT Disaster preparedness among hospital nurses is crucial in disaster response, impacting the capacity of healthcare services in emergency situations. This study aims to identify the forms of disaster preparedness among nurses, assess the level of preparedness, and identify the factors influencing it. This study uses a literature review based on the framework by Arksey and O’Malley, with literature searches conducted through EBSCO host-CINAHL Plus and Scopus using relevant keywords. Articles were selected based on inclusion criteria and analyzed using a data mapping table. From 16 analyzed articles (13 quantitative, 3 qualitative, 2013-2023), it was found that disaster preparedness among nurses is generally moderate to low. Forms of preparedness include knowledge, attitude, skills, self-preparedness, crisis management, perception, and operational competence. Disaster preparedness is influenced by demographic factors, training experience, psychological conditions, and hospital infrastructure. This study highlights the importance of a multi-faceted approach to improving disaster preparedness among hospital nurses. Keywords: Disaster Preparedness, Hospital, Nurse.  ABSTRAK Kesiapsiagaan bencana pada perawat di Rumah sakit adalah kunci dalam respons bencana, yang juga mempengaruhi kemampuan layanan kesehatan dalam situasi darurat. Penelitian ini bertujuan untuk mengidentifikasi bentuk kesiapsiagaan bencana pada perawat, menilai tingkat kesiapsiagaan, dan mengidentifikasi faktor-faktor yang mempengaruhinya. Penelitian ini menggunakan scoping review berdasarkan framework Arskey dan O’Malley, dengan pencarian literatur melalui EBSCO host-CINAHL Plus dan Scopus menggunakan kata kunci terkait. Artikel dipilih berdasarkan kriteria inklusi dan dianalisis dengan tabel pemetaan data. Dari 16 artikel yang dianalisis (13 kuantitatif, 3 kualitatif, 2013-2023), ditemukan bahwa kesiapsiagaan bencana pada perawat umumnya sedang hingga rendah. Bentuk kesiapsiagaan mencakup pengetahuan, sikap, keterampilan, kesiapan diri, manajemen krisis, persepsi, dan kompetensi operasional. Kesiapsiagaan bencana dipengaruhi oleh faktor demografi, pengalaman pelatihan, kondisi psikologis, dan fasilitas infrastruktur. Abstract

ABSTRACT The increase in HIV cases is in line with the development of antiretroviral therapy (ART), which has an impact on increasing the life expectancy of HIV/AIDS patients. New challenges that accompany the increase in life expectancy of HIV/AIDS patients, namely how to ensure good quality of life (QoL) in patients. The methodology used in this Scoping Review adheres to the methodological guidelines outlined by Levac and Colquhoun. Cultural and cultural differences play a very important role in determining the quality of life in people with HIV/AIDS. A good understanding of the cultural context in a population can develop more effective interventions to address the challenges faced by people with HIV/AIDS as an effort to improve the quality of life in patients with HIV/AIDS Keywords: Quality of Life, Human Immunodeficiency Virus (Hiv), Acquired Immune Deficiency Syndrome (AIDS).  ABSTRAK Peningkatan kasus HIV tersebut sejalan dengan perkembangan terapi antiretroviral (ART), yang berdampak pada peningkatan angka harapan hidup pasien HIV/AIDS. Tantangan baru yang menyertai peningkatan harapan hidup pasien HIV/AIDS, yakni bagaimana memastikan kualitas hidup (Quality of Life/QoL) yang baik pada pasien. Metodologi yang digunakan dalam Scoping Riview ini mematuhi pedoman metodologis yang digariskan oleh Levac dan Colquhoun. Perbedaan kultur dan budaya memiliki peran yang sangat penting dalam menentukan kualitas hidup pada penderita HIV/AIDS. Pemahaman konteks budaya, yang baik pada suatu populasi dapat mengembangkan intervensi yang lebih efektif untuk mengatasi tantangan yang dihadapi oleh penderita HIV/AIDS sebagai upaya meningkatkan kualitas hidup pada pasien dengan HIV/AIDS. Kata Kunci: Kualitas Hidup, Human Immunodeficiency Virus (Hiv), Acquired Immune Deficiency Syndrome (AIDS).

Abstract

Disasters, whether natural or human-induced, frequently lead to significant physical injuries and complex health conditions among affected populations. In such contexts, nurses serve as frontline responders who deliver critical care aimed at promoting physical recovery and preventing further complications. This scoping review explores and synthesizes existing literature on the impact of nursing interventions on the physical recovery of disaster victims. The review was conducted in accordance with the PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews) guidelines. A systematic search was carried out in three major electronic databases PubMed, Scopus, and Web of Science targeting articles published between 2015 and 2025. The following Boolean search string was used: ((disasters) OR ("disaster victims") OR ("disaster survivors")) AND ((nurses) OR ("nursing intervention") OR ("nursing care")) AND (("physical recovery") OR ("wound healing") OR ("pain management") OR ("mobility improvement") OR ("quality of life")). Following the application of predefined inclusion and exclusion criteria, three studies were selected for the final synthesis. The findings reveal that nursing interventions such as wound care, pain management, infection control, physical rehabilitation, and health education play a vital role in enhancing the physical recovery of individuals affected by disasters. The success of these interventions is often shaped by factors including timeliness, cultural appropriateness, and interdisciplinary collaboration. In conclusion, this review underscores the essential contribution of nurses in disaster recovery efforts and highlights the need for continued development of evidence-based, culturally sensitive, and standardized nursing practices to optimize health outcomes in post-disaster settings.

Abstract

Many emergency situations require immediate first aid. The community plays a crucial role in saving victims of cardiac arrest or traffic accidents, which are among the leading causes of premature death in Indonesia. Unfortunately, the level of public knowledge and preparedness in providing first aid remains low, highlighting the need for improvement through education and training. Community health cadres were chosen as the target group because they are closely connected to the community, easily accessible, and can act as first responders before professional medical personnel arrive. This study aimed to enhance the knowledge and readiness of community cadres in recognizing and providing effective first aid through CPR and first aid training in Pangandaran Village. A pre-experimental one-group pre-test and post-test design was used, involving 33 participants selected through accidental sampling. Data analysis was conducted using a paired sample t-test. The results showed a significant improvement (p<0.05) in the cadres’ knowledge and preparedness after the training. Based on these findings, it is recommended that similar training programs be conducted regularly by health institutions and community organizations to strengthen the capacity of cadres in providing first aid, thereby increasing the survival rate of victims of accidents and cardiac arrest within the community.Keywords: Accident, cardiac aArrest, knowledge, preparedness.

Abstract

Many countries have high earthquake activities. As an integral part of the emergency response team, nurses play a critical role in disaster management. This review aimed to identify nurses' skills in dealing with earthquakes and their implications for the importance of nurse education in this area. This study used a narrative review. The literature was searched using three databases: CINAHL, PubMed, and Scopus, using the keywords "Nurses OR Nurse OR Personnel Nurses OR Registered Nurses AND Clinical Competence OR Competency, Clinical OR Clinical Skill AND Earthquakes." The analysis was carried out descriptively and qualitatively using a thematic analysis approach. The resultsshows that eight studies were analyzed in this review. Nursing competency in earthquake disasters includes skills and knowledge important for preparing, responding to, and recovering from pre- and post-disaster conditions. The results of this research classify nurses' skills into five categories: knowledge of disaster management, clinical skills and patient care, protective measures and safety leadership, communication and coordination, and psychological and social support. Conclusion: The review results highlight the importance of increasing nurses' skills through training and knowledge in disaster preparedness for nurses, emphasizing a deep understanding of emergency medical management and post-disaster recovery. The importance of adapting nursing education curricula and using effective teaching strategies to ensure that nurses have sufficient skills and knowledge to face complex disaster challenges. Keywords: Clinical competence, disaster management, nursing skills.

Abstract

ABSTRACT Hypovolemic shock is a life-threatening condition resulting from loss of large amounts of blood or fluid, which requires immediate detection and treatment to prevent organ damage and death. Nurses’ knowledge of hypovolemic shock is essential to improve patient outcomes, especially in early recognition and intervention. This study aimed to explore the factors that influence nurses' knowledge about hypovolemic shock. This narrative review was conducted by identifying articles from two databases (PubMed and Cinahl) and Google Scholar. The keywords used were “Nurses OR Nursing AND Knowledge AND Hypovolemic Shock AND Factor OR Predictor”. Eight articles from various countries were analyzed using cross-sectional methods. The results showed that the level of nurses’ knowledge was influenced by age, education, work experience, and training that had been attended. Conclusion: Nurses’ knowledge of hypovolemic shock tends to be still lacking. This study suggests the need for increased access to continuing education and structured clinical training to strengthen nurses’ competence in managing hypovolemic shock, with a focus on simulation and clinical practice-based learning. Keywords: Hipovolemic Shock, Knowledge, Nurses  ABSTRAK Syok hipovolemik adalah kondisi yang mengancam jiwa akibat kehilangan darah atau cairan dalam jumlah besar, yang membutuhkan deteksi dan penanganan segera untuk mencegah kerusakan organ dan kematian. Pengetahuan perawat tentang syok hipovolemik sangat penting untuk meningkatkan hasil perawatan pasien, terutama dalam pengenalan dini dan intervensi. Studi ini bertujuan untuk mengeksplorasi faktor-faktor yang mempengaruhi pengetahuan perawat tentang syok hipovolemik. Narrative review ini dilakukan dengan mengidentifikasi artikel dari dua database (PubMed dan Cinahl) dan Google Scholar. Kata kunci yang digunakan adalah “Nurses OR Nursing AND Knowledge AND Hypovolemic Shock AND Factor OR Predictor”. Delapan artikel dari berbagai negara dianalisis dengan menggunakan metode cross-sectional. Hasilnya menunjukkan bahwa tingkat pengetahuan perawat dipengaruhi oleh usia, pendidikan, pengalaman kerja, serta pelatihan yang pernah diikuti. Pengetahuan perawat tentang syok hypovolemia cenderung masih kurang. Penelitian ini menyarankan perlunya peningkatan akses terhadap pendidikan berkelanjutan dan pelatihan klinis yang terstruktur untuk memperkuat kompetensi perawat dalam menangani syok hipovolemik, dengan fokus pada simulasi dan pembelajaran berbasis praktik klinis. Kata Kunci: Syok Hipovolemia, Pengetahuan, Perawat

Abstract

ABSTRACT Nurses in intensive care units (ICU) often face ethical dilemmas when caring for palliative patients. Decisions made at this late stage of a patient's life have a major impact on the quality of life of the patient and their family, and can trigger conflicts between professional values, patient expectations, and family needs. This study aimed toexplore the experiences of ICU nurses in facing ethical dilemmas in palliative patients and to map the main ethicalissues faced. Methods: This study used a scoping review approach with literature search through several databases, such as Pubmed, Ebsco, and Science Direct. The article searches and analysis was from November to December 2024. Inclusion criteria included studies published from 2014-2024 related to ethical dilemmas in palliative care in the ICU, with experimental, case study, and qualitative descriptive research designs. Relevant articles were screened following PRISMA guidelines and analyzed qualitatively descriptively. Of the 19,765 articles identified, 8 articles metthe inclusion criteria and were analyzed further. Nurses faced ethical dilemmas that included conflicts in carrying outprofessional roles, feeling sandwiched between patient wishes and family expectations, and organizational policylimitations. The two main categories found were conflict within the professional role and conflict between personal autonomy and external demands, including pressure from family and coworkers. The importance of improvingeffective communication, emotional support, multidisciplinary teamwork, clinical ethics training and education, developing adaptive coping strategies, and support policies in an effort to minimize the stress experienced by nurses infacing ethical dilemmas in caring for palliative patients. Keywords: Ethics, Nurse, Palliative Care ABSTRAK Perawat di unit perawatan intensif (ICU) sering menghadapi dilema etik ketika merawat pasien paliatif. Keputusan yang diambil pada tahap akhir kehidupan pasien ini memiliki dampak besar terhadap kualitas hidup pasien dan keluarganya, serta dapat memicu konflik antara nilai profesional, harapan pasien, dan kebutuhan keluarga. Penelitian ini bertujuan untuk menggali pengalaman perawat ICU dalam menghadapi dilema etis pada pasien paliatif dan dalammemetakan isu-isu etika utama yang dihadapi. Studi ini menggunakan metode pendekatan scoping review dengan pencarian literature melalui beberapa database, seperti Pubmed, Ebsco, dan Science Direct. Pencarian serta analisis artikel dilakukan bulan November hingga Desember 2024. Dengan Kriteria inklusi meliputi studi yang dipublikasikansejak 2014-2024 yang berkaitan dengan dilema etika dalam perawatan paliatif di ICU, dengan desain penelitian eksperimental, studi kasus, dan deskriptif kualitatif. Artikel yang relevan diseleksi mengikuti pedoman PRISMA dan dianalisis secara kualitatif deskriptif. Dari 19.765 artikel yang teridentifikasi, 8 artikel memenuhi kriteria inklusi dan dianalisis lebih lanjut. Perawat menghadapi dilema etis yang mencakup konflik dalam melaksanakan peran profesional, perasaan terjepit antara keinginan pasien dan harapan keluarga, serta keterbatasan kebijakan organisasi.Dua kategori utama yang ditemukan adalah konflik dalam peran profesional dan konflik antara otonomi pribadi dengan tuntutan eksternal, termasuk tekanan dari keluarga dan rekan kerja. Pentingnya peningkatan komunikasi efektif, dukungan emosional, kerja sama tim multidisplin, pelatihan dan edukasi etik klinik, pengembangan strategi koping adaptif, dan dukungan organisasi dalam upaya meminimalisir stress yang dialami oleh perawat dalam menghadapi dilema etik merawat pasien paliatif. Kata Kunci: Dilema Etik, Perawat, Perawatan Paliatif

Abstract

Digital National Early Warning Score (NEWS) is a system designed to detect early clinical deterioration in patients by monitoring vital parameters using digital technology. The implementation of Digital NEWS in hospitals is increasingly gaining attention because of its ability to improve patient safety, efficiency in clinical decision-making, and reduce the workload of health workers. However, although many benefits have been identified, implementing Digital NEWS is open to various challenges and obstacles. This study aimed to explore the benefits, factors, and obstacles faced in implementing digital NEWS in Hospitals. This scoping review used the Arksey and O'Malley framework following the PRISMA for Scoping Review. Literature searches used three main databases: EbscoHost: Medline Ultimate, Pubmed, and Scopus, and also one search engine, Google Scholar. Ten relevant articles were analyzed in this study. Implementation of digital NEWS can improve early detection of deteriorating patient conditions, reduce medical response time, and improve communication between health teams. However, the success of its implementation is influenced by several factors, such as the availability of technological infrastructure, adequate training, and technological literacy among health staff. The main challenges include lack of resources, alarm fatigue, and cultural resistance to technological change. This review concludes that implementing digital NEWS has many benefits, but health workers still face many challenges and barriers. A comprehensive approach is needed to optimize the benefits of digital NEWS implementation, including investment in infrastructure, training, and managerial support.

Abstract

ABSTRACT Chronic Kidney Disease is a global health problem that occurs every year. Patients can survive by undergoing hemodialysis therapy, but it still leaves a number of important issues as a result of hemodialysis therapy. The purpose of this study was to identify the life experiences of chronic kidney disease patients undergoing hemodialysis. This study used a literature review method with a scoping review approach to explore the experiences of chronic kidney failure patients undergoing hemodialysis. Searches of three databases includes EBSCOhost, PubMed, and Scopus. The keywords used in English were “chronic kidney disease” OR “chronic renal failure” OR “CKD” OR “ESRD” AND “life experiences” OR “lived experiences” AND “during hemodialysis” OR “undergoing hemodialysis”. There were several interrelated themes in exploring the experiences of chronic kidney failure patients undergoing hemodialysis, including emotional and social support, physical and life limitations, psychological impact, economic difficulties, changes in health and knowledge about treatment, and dependence. These findings suggest that the experiences felt by patients with chronic renal failure undergoing hemodialysis are interconnected and have a significant influence on patients' overall quality of life. The implications of these findings suggest that a holistic approach in the care of hemodialysis patients is necessary so that medical interventions cover the physical, emotional, social, and economic aspects of patients. This study provides important insights for healthcare professionals to design more comprehensive and patient-centered interventions. Keywords: Chronic Kidney Disease, Hemodialysis, Life experience  ABSTRAK Chronic Kidney Disease adalah masalah kesehatan global yang sering terjadi di setiap tahunnya. Pasien bisa bertahan hidup dengan menjalani terapi hemodialisis, namun masih menyisakan sejumlah persoalan penting sebagai dampak dari terapi hemodialisis. Tujuan penelitian ini adalah untuk mengidentifikasi pengalaman hidup pasien chronic kidney disease yang menjalani hemodialisis. Penelitian ini menggunakan metode literature review dengan pendekatan scoping review untuk mengeksplorasi pengalaman pasien gagal ginjal kronis yang menjalani hemodialisis. Pencarian menggunakan tiga database terdiri dari EBSCOhost, PubMed, dan Scopus. Kata kunci yang digunakan dalam bahasa Inggris yaitu “chronic kidney disease” OR “chronic renal failure” OR “CKD” OR “ESRD” AND “life experiences” OR “lived experience” AND “during hemodialysis” OR “undergoing hemodialysis”. Terdapat beberapa tema yang saling berkaitan satu sama lain dalam mengeksplorasi pengalaman pasien gagal ginjal kronis yang menjalani hemodialisis, diantaranya dukungan emosional dan sosial, keterbatasan fisik dan hidup, dampak psikologis, kesulitan ekonomi, perubahan kesehatan dan pengetahuan tentang perawatan, serta ketergantungan. Temuan ini menunjukkan bahwa pengalaman yang dirasakan oleh pasien dengan gagal ginjal kronis yang menjalani hemodialisis saling berhubungan dan memiliki pengaruh signifikan terhadap kualitas hidup pasien secara keseluruhan. Implikasi dari temuan ini menunjukkan bahwa pendekatan holistik dalam perawatan pasien hemodialisis sangat diperlukan agar intervensi medis mencakup aspek fisik, emosional, sosial, dan ekonomi pasien. Penelitian ini memberikan wawasan penting bagi tenaga kesehatan untuk merancang intervensi yang lebih komprehenif dan berpusat pada pasien.  Kata Kunci: Chronic Kidney Disease, Hemodialisis, Pengalaman Hidup
