Abstract

Diabetes melitus merupakan salah satu penyakit kronis yang prevalensinya terus meningkat secara global, termasuk di Indonesia. Untuk menanggulangi hal ini, pemerintah melalui BPJS Kesehatan menginisiasi Program Pengelolaan Penyakit Kronis (Prolanis) yang bertujuan untuk meningkatkan kualitas hidup penderita penyakit kronis melalui pelayanan kesehatan proaktif dan berkelanjutan. Penelitian ini bertujuan untuk mengevaluasi pelaksanaan Program Prolanis dalam pengendalian diabetes melitus di wilayah kerja Puskesmas Tirto I. Metode penelitian yang digunakan adalah deskriptif kualitatif dengan teknik pengumpulan data melalui wawancara mendalam kepada pengelola program, tenaga kesehatan, dan peserta Prolanis. Hasil penelitian menunjukkan bahwa pelaksanaan Prolanis di Puskesmas Tirto I masih menghadapi berbagai tantangan, seperti keterbatasan sumber daya manusia, partisipasi peserta yang fluktuatif, keterbatasan akses transportasi, serta kurangnya inovasi dalam edukasi kesehatan. Meskipun demikian, program ini memberikan manfaat nyata bagi peserta dalam hal pemantauan kesehatan rutin dan peningkatan pengetahuan tentang pengelolaan diabetes. Diperlukan perbaikan dalam koordinasi, evaluasi program, dan pendekatan edukatif untuk meningkatkan efektivitas pelaksanaan Prolanis.

Abstract

Adolescent participation in the Integrated Primary Care (ILP) Program remains low in many rural areas in Indonesia, despite national efforts to strengthen primary health care as part of the Healthy Indonesia 2045 agenda. Limited studies have examined the determinants of adolescent engagement, as previous research has predominantly focused on ILP implementation among mothers, toddlers, and health cadres. This study aims to analyze the predisposing, enabling, and reinforcing factors influencing adolescents participation in ILP activities using Lawrence Green’s PRECEDE-PROCEED Model. A descriptive qualitative design was employed, involving 10 adolescents aged 10-24 years registered in the ILP Posyandu of Karanganyar Village, Tirto District, Pekalongan Regency. Additional triangulation informants included one village midwife, two ILP cadres, and two policy makers, all selected through purposive sampling. Data were collected through in-depth semi-structured interviews and analyzed thematically. The findings indicate that predisposing factors, particularly adolescents limited understanding of ILP objectives and benefits, contribute to low engagement, with some expressing fear of examination results. Enabling factors show that facilities, equipment, and financing are adequate, yet participation is hindered by scheduling conflicts with school hours, despite the close proximity of ILP locations to adolescents homes. Reinforcing factors emerged as the strongest influence, with active communication from health workers, moral encouragement from families, and strong peer support playing crucial roles in motivating adolescents to attend ILP activities. Overall, the study reveals that low participation is shaped by interacting barriers across cognitive, structural, and social domains. Strengthening adolescent health literacy, adjusting activity schedules, and enhancing family and peer-based reinforcement are essential to improving sustainable adolescent engagement in ILP services. These findings highlight the importance of community-centered strategies and cross-sector collaboration to support the on going transformation of primary health care at the village level.

Abstract

Indonesia is in a state of emergency due to narcotics and illegal drugs (narcotics). By 2024, the prevalence of drug abuse in Indonesia had reached 3.3 million people, dominated by the younger generation, especially teenagers aged 15 to 24. Drug abuse was the highest or most common case in Pekalongan Regency throughout 2024, with 29 cases. This is based on data from the Pekalongan Police Department regarding all cases uncovered over the past year. Drug cases in Pekalongan Regency were spread across several sub-districts, one of which was Sragi Sub-district. The purpose of this Community Service (PKM) activity was to provide education on the dangers of drug abuse using lectures, jigsaw puzzles, and role-play methods. Results: This activity took place on Sunday, March 5, 2023, at Purworejo Public Elementary School. The community service participants consisted of four female cadres and 16 adolescent members of the youth integrated health post (Posyandu) in Purwoharjo Village, Sragi Sub-district.

Abstract

Primary care integration (ILP) is a pillar of health transformation to strengthen primary health care by bringing health services closer to the village level, targeting the entire life cycle, and strengthening local area monitoring (PWS). The purpose of the study was to analyze the implementation of Primary Care Integration (ILP) at Puskesmas in East Pekalongan District. The research method used qualitative analysis with a case study approach. Data collection was carried out at the Puskesmas in the East Pekalongan sub-district of Pekalongan City. Data collection with in-depth interviews involving 20 informants and data processing using thematic analysis with CIPP variables. This study found that in the context aspect, the goal has not been achieved optimally to bring health services closer and expand due to the lack of cadres, influential factors are family support and officer involvement and infrastructure, targeting the entire life cycle.  In the input aspect, human resources are not sufficient, especially medical personnel and cadres for many targets, funding is insufficient for activities, infrastructure for tools is sufficient but the place of implementation is still not large enough and health test kits are still lacking. In the process aspect, planning has gone well, organizing is well structured, it's just that the number of cadres and health workers is still lacking to serve all life cycles, for Pekalsanaan it is still running as much as possible and for evaluation it still refers to the old mechanism and for evaluations carried out regularly every month. In the product aspect, the results of activities can increase the level of visits and the community becomes more enthusiastic in participating in the ILP posyandu.
