Abstract

Boilers experience frequent problems, such as scale formation, unstable steam pressure, incomplete combustion, and difficulty starting. Therefore, this study uses a quantitative method with the Failure Mode and Effect Analysis (FMEA) approach, which aims to determine maintenance and prevention to minimize these problems. This method determines the failure priority obtained from the Risk Priority Number (RPN) calculation based on assessment factors such as severity, frequency of occurrence, and the likelihood of failure being detected in each component. The results of these calculations indicate that the steam drum component falls into the highest category, with an RPN of 32. The other 12 components fall into the medium category: the feed water pump, electrode, photocell, nozzle, blower fan, solenoid valve, fuel pump, blowdown valve, main steam valve, scum valve, non-return valve, and gauge glass, with RPN values ranging from 12 to 24. Furthermore, the components in lowest category include the fuel filter, safety valve, and manometer, with RPN values ranging from 8 to 10. Therefore, the recommended improvements based on the FMEA results can be used as evaluation material to ensure the effectiveness of the improvements. Furthermore, regular boiler maintenance and monitoring are required, especially for components such as the steam drum, which requires additional routine maintenance schedules, repeated evaluations related to water treatment, and determining appropriate repairs for any steam drum failures.

Abstract

The air compressor is an essential auxiliary machinery onboard MT Seroja III, playing a critical role in supporting vessel maneuverability, operational safety, and main engine starting readiness. During operations, a critical reduction in compressed air production was encountered, leading to prolonged air reservoir filling times and potential risks during ship maneuvering. This study aims to identify the root causes of decreased compressed air production in the main air compressor onboard MT Seroja III and to formulate corrective and preventive maintenance actions based on onboard observations and Fault Tree Analysis (FTA). This research employs a qualitative descriptive method, utilizing continuous monitoring parameter logs, field inspections, and semi-structured interviews with engine officers over a 12-month sea service period. The diagnostic findings using FTA indicate that the decrease in volumetric efficiency was primarily caused by high-resistance suction filter blockages, severe carbon fouling on suction and delivery valves, and piston ring degradation. Corrective maintenance involving filter purging, valve refacing, and piston ring replacement successfully restored the compressor discharge pressure to 30 bar and reduced the air receiver filling time from 45 minutes to 15 minutes. The study concludes that strict adherence to the vessel's Planned Maintenance System (PMS) is mandatory to guarantee operational safety and prevent compression loss.

Abstract

The ship engine cooling system is critical for maintaining the operational stability of main and auxiliary machinery. However, biofouling within sea chests and seawater pipelines remains a prevalent issue, leading to flow obstruction, reduced heat transfer efficiency, and potential engine malfunction. While the Marine Growth Prevention System (MGPS) is designed to mitigate this via electrolysis, marine growth persisted on MV Ever Clear, indicating suboptimal system performance. This study aims to analyze the effectiveness of the MGPS in preventing biofouling and to identify factors contributing to its performance degradation. Using a qualitative descriptive method with a case study approach, data were collected through direct onboard observation, interviews with the engineering crew, and document analysis. Results indicate that the decreased effectiveness was primarily caused by insufficient electrode maintenance, unstable electrical parameters (current and voltage), and inadequate operational monitoring. These deficiencies compromised the electrolysis process, preventing sufficient ion release. It is concluded that MGPS efficacy is contingent upon stable electrical parameters, rigorous preventive maintenance, and consistent operational supervision in accordance with established procedures.

Abstract

The port is one of the distribution centers for goods in which there are loading and unloading activities. Loading and unloading activities sometimes involve loading and unloading personnel and heavy equipment which raises the potential for hazards that can cause work accidents and damage to goods. The purpose of this research is to conduct a risk assessment using the HIRARC (Hazard Identification, Risk Assessment and Risk Control) method on bagged fertilizer unloading work in the TUKS PUSRI Banyuwangi Branch Port area. The stages of bagged fertilizer unloading work activities include the unloading activity process, which consists of five work stages: opening the ship's hatch, workers (stevedores) entering the hatch, stevedoring, cargodoring, and delivery. The results stated that from a total of 5 work stages, 26 sources of hazards were identified with 6 sources of low risk level hazards at 23%, 7 sources of moderate risk level hazards at 27%, 6 sources of high risk level hazards at 23%, and 7 sources of very high risk level hazards (extreme) at 27%. Control measures that can be implemented are conducting briefings on OHS (Occupational Health and Safety) before starting work activities, supervision of compliance with the use of complete personal protective equipment and also the need for sanctions for workers who do not comply with the rules for using personal protective equipment.

Abstract

Siltation of shipping channels presents significant challenges that can heighten maritime safety and security risks while impeding economic activities. Sedimentation, defined as the deposition process of solid particles including sand and rock materials, necessitates regular maintenance interventions. This phenomenon has been particularly pronounced in the Perak River shipping channel in recent years, with no dredging operations conducted to date. The occurrence of sedimentation significantly impacts the operational activities of shipyard companies located along the Perak River shipping channel. This study aims to investigate the impacts of sedimentation in the Perak River on shipyard operations. The research employs a quantitative methodology utilizing SPSS software for data analysis. The study sample comprises employees from the shipyard companies PT. Dumas Tanjung Perak Shipyard and Pelni Surya Shipyard. Simple linear regression analysis was employed, yielding the equation Y = 5.943 + 1.112X. The findings demonstrate that the impacts of sedimentation on shipping channels exert a positive and significant effect on shipyard operations. These impacts include reductions in the number and size of vessels capable of transiting the channel, as well as increased operational costs for shipyard companies and ship owners due to process delays while awaiting optimal water conditions during highest tide periods.
