Abstract

Introduction: MTBS is a comprehensive approaches for child illnes and one of the way to reduce an infant and under fi ve years morbidity and mortality rateas well as appropriately to apply in public health center. In fact, MTBS has not been implemented as procedure guidance. The objective of the study was to examie the implementation of MTBS viewed from Van Meter and Van Horn model as basic measurement and the goal of policy, the sources of policy, a communication among organisation and the implementation of MTBS.Method: The study was qualitative design. Data were collected by in depth interview. Six Nurse and midwife were recruited as a sample by purposive sampling. Informant triangulation were head of PHC and head of deputy health services. Analysis was by means of content analysis.Result showed that the written and indicator of basic measurement and goal policy was not available yet. Sources of policy, namely health.Result: For the health workers was sufficient and they have a good competency to carry out MTBS and have workload focus on three types. Fasilities was no room, form was difficult to provide, instruments and medication was sufficient available, but there was no specific funding. Communication was carried out only on the training program by MTBS fascilitator. Characteristic of beaurocracy structure (SOP) consists of preparation, implementation, eporting and recording.There was no specific preparation, it only an introduction dessimination was done. The implementation was not as modul and the form of MTBS was not used anymore. The implementation of MTBS lack support from Public Health center and Health Care District of Surabaya. Healh worker unmotivated to implement MTBS.Conclusio: the implementation of MTBS lack support from health worker, Public Health Center and Health Care District of Surabaya.It is recomended that Health Care District & Public Health Center cooperatively should have commitment to assess the implementation of MTBS in Surabaya.

Abstract

Background: Hospitals utilize technology with the application of software used to process data, namely the Hospital Management Information System (SIMRS). The implementation of SIMRS by some hospitals in Indonesia is aimed at Service and access, Performance improvement, Efficiency, effectiveness, and professionalism. Objective: To evaluate the implementation of SIMRS in the medical records section of Panti Rini Hospital with PIECES analysis. Methods: This research is a descriptive analytic study. The data collection technique used a questionnaire instrument and interviews with respondents using the PIECES (Performance, Information, Economic, Control, Efficiency, and Service) evaluation method. The research sample consisted of 10 people including 9 medical recorders including unit coordinator and head of medical records and 1 head of IT. Result: The Performance aspect is said to be poor, in the Information aspect, system Control, economy, and Service are said to be good, which is 100% for each assessment item.Conclusion: Overall from the system assessment, it can be categorized as good, the resulting Information can be considered by the management for decision making. With the system as well, work becomes more efficient and effective in the use of resources and in Service to patients and management.

Abstract

The hospital is one of the businesses engaged in the health sector by emphasizing the products and health facilities that are able to be provided by the hospital. Considering the high level of competition in hospitals in Indonesia, both state-owned and private hospitals, many hospitals have developed marketing strategies to meet sales targets and realize marketing goals. One of them is the Diponegoro National Hospital which runs a Global Innovative Marketing strategy by providing exclusive Private Wing services for special customers or VIPs (Very Important Person). In this case, the researcher reviews the marketing mix of Private Wings Services through seven variables, namely product, price, place, physical evidence, people, promotion, process. The process of collecting data was carried out by in-depth interview techniques and literature studies on several related literatures that were used as research references. The research was conducted qualitatively and the data that had been collected were analyzed descriptively. The results of this study indicate that the promotion and place variables need to be a concern for the Diponegoro National Hospital in developing Private Wing services.

Abstract

Pending klaim artinya tidak terklaim atau tertunda. Klaim yang dipending menyebabkan aliran kas rumah sakit terganggu akibat permasalahan dalam pembayaran klaim. Klaim yang di pending juga dapat menyebabkan kerugian bagi rumah sakit pemerintah yang banyak menerima pasien jaminan kesehatan sehingga mengalami kerugian akibat ketidaksesuaian pembiayaan pelayanan dengan jumlah klaim yang dibayarkan. Tujuan dari penelitian ini yaitu untuk meninjau evaluasi administrasi klaim BPJS kesehatan dalam menurunkan klaim pending. Jenis penelitian ini adalah kualitatif dengan pendekatan fenomenologi. Fokus penelitian ini pada evaluasi administrasi klaim BPJS kesehatan dalam menurunkan klaim pending. Teknik pengumpulan data yang digunakan yaitu observasi, wawancara mendalam kepada petugas BPJS, dan dokumen melalui studi kepustakaan dari buku dan jurnal ilmiah yang relevan dengan penelitian ini. Setelah memperoleh data, peneliti menganalisis nya menggunakan adopsi dari Miles dan Huberman dengan 3 tahapan yaitu reduksi; penyajian data; dan penarikan kesimpulan dan verifikasi. Hasil penelitian ini pengaruh terbesar terjadinya pending klaim BPJS Kesehatan di rumah sakit ketetapan kode diagnosa dan ketidaklengkapan resume medis. Beberapa faktor yang menyebabkan pending klaim BPJS Kesehatan di rumah sakit dapat berupa minimya pengetahuan koder, penulisan diagnosa yang sulit dipahami/ kurang lengkap, dan kurangnya kelengkapan rekapitulasi pelayanan yang diberikan Rumah Sakit karena banyaknya pasien BPJS yang harus ditangani. Upaya untuk mengurangi terjadinya pending klaim BPJS Kesehatan sebaiknya antara tenaga kesehatan dan coder harus berjalan baik, dan bagi perawat dan dokter agar lebih teliti dalam penulisan data pasien di dalam berkas rekapitulasi pelayanan, dan tenaga kesehatan lebih banyak berhati-hatilah saat mengisi resume medis.

Abstract

Indonesia targeting that at least 70% of the population vaccinated and formed the herd immunity. However, there are several challenges in reaching that target, such as the anti-vaccine movements, disinformation or hoaxes, fear of the Adverse Event Following Immunization (AEFI), and fear of needles. Based on the preliminary study, the area of Bandarharjo Public Health Centre was in the lowest 3rd place related to the COVID-19 vaccination coverage in Semarang. This was due to lack of education to the community regarding the vaccination, the high resistance to vaccination due to concerns about AEFI, and so on. Health cadres are one of the main actors in the supporting the health programs in PHC, therefore the role of health cadres need to be optimized in education and management of AEFI related to COVID-19 vaccination. This community service program was carried out offline in 30 health cadres in the working area of Bandarharjo PHC. The activity began with a pre-test, continued with the presentation of the topic, discussion session, and ends with post-test. The results of the pre-test and post-test showed that there was an increase in the knowledge of cadres between before the presentation and after the presentation session. Health cadres also received booklets containing general information about COVID-18 vaccination and also related to AEFI. Booklets given to cadres not only to increase their knowledge but also can be used to educate the community. Hopefully, with the increase in knowledge and understanding of cadres, the community will be easily get the right information. The health cadres commitment to educate the community regarding COVID-19 vaccination also hoped to increase the community knowledge related to the vaccination and also the willingness of community to receive the COVID-19 vaccination, especially in the working area of Bandarharjo PHC, Semarang City, Central Java Province.

Abstract

In order to improve the quality of hospital services in Indonesia, one of the government's efforts is to engage a system for the Public Service Board (PSB). RSUD Semarang that has changed its status to become Regional Public Service Board (RPSB) in 2007 have an obligation to implement the Minimum Service Standards (MSS) in their service but until now RSUD Semarang have never assessing SPM on the outpatient Installation so it is necessary to investigate the SPM monitoring and compliance efforts at the outpatient installation in RSUD Semarang. The method used is qualitative method with cross sectional approach, analysis of data using content analysis. From the survey results revealed that the monitoring in fulfillment of MSS at outpatient installation in RSUD Semarang has not gone well and also the SPM evaluation and assessment has not been carried out. For the indicator at a specialist clinic providers already met supported by the specialist in accordance with the specialty. Indicator of the availability of the service has been fulfilled which medical personnel, administrative personnel and other health facilities are already available. Indicator of the opening hours of service have not been met due to delays in the arrival of the doctor because of their visit schedule is at the same time with the opening hours of outpatient, so doctors come late to outpatient polyclinic. Indicators of service waiting time is unmet because the doctor came in late for reason above, the shortage of specialist doctor where the number of specialist doctor in some clinic only one person so that when that doctor performed emergency measures then the outpatient patient cannot be serviced. The delay of medical record file delivery due to the narrowness of the patient's medical record file storage area and the lack of human resources in outpatient registers lead the search process of old patient files become slowly. Indicator of customer satisfaction is unmet because of the long waiting time and inhospitable nurses, but it is also due to the lounge facilities are less comfortable and less clean toilets. Indicator of TB diagnosis enforcement through microscopic examination and recording, reporting of TB cases are met with the facilities and capabilities of the laboratory personnel, in addition, also because of the availability of a complete TB form and laptops provided by the Department of Health to assist the process of TB recording and reporting. Suggestions that need to be done to evaluate MSS periodically, adding space and patients file storage rack, improve hospitality personnel, increasing the number of specialists, make regulations in doing visit hours and practice hours of polyclinic doctor.

Abstract

On providing health services Hospital chance to be prone of deviation in the presence of health services especially finance and assets management it can cause the hospital difficult achieving vision and goals. Therefore, to ensure implementation of health services, hospitals must conduct an internal performance audit by the Internal Control Unit (SPI) according Under Law No 44 of 2009. This study aims to analyze the implementation of internal supervision in RSUD Tugurejo, Central Java Province. This research used Qualitative Approach in the form of Descriptive Analysis with data collection using in-depth interview method and based on purposive sampling technique. The subjects of the study were all the SPI Tugurejo Regional Hospital of Central Java Province, while the triangulation informants were Deputy Director of Services, service staff, finance staff and general head of the Tugurejo Regional Hospital of Central Java Province. The variables studied by this research appear of human resource, funds, infrastructure/ facility and Regulation (SOP). The results of this study are mechanisms for appointment that have not been well related to the specific competence of internal supervision, have not been facilitated by training for all SPI team personnel related to technical internal supervision, there is no clear and written translation of assignments for each SPI team personnel, internal supervision SPI team personnel, the implementation of internal supervision is more focused on the financial section because the active SPI team is only 2 personnel and both have economic background and lack of employee understanding of the role, function and position of SPI.. This study recommends to the mechanism of appointment through analysis of workload according to the magnitude and complexity of hospital activities, increasing competence by facilitating internal and external training for all SPI team personnel, periodically disseminating information to all employees regarding the role, function and position of SPI for implementation the main tasks and functions of internal supervision can be carried out optimally at the Tugurejo Regional Hospital in Central Java Province.

Abstract

The waiting time at laboratory services is still often a problem in some hospitals and has an impact on the quality of the hospital. Patients at Jakarta City X Private Owned Women and Children Hospital complained about the length of the waiting time in laboratory service, especially during peak hours. Achievements of the laboratory waiting time indicator still do not meet the standards set by the Jakarta City X Private Owned Women and Children Hospital. This purpose of this study is to describe and identify the factors that lead to long periods of waiting time in laboratory service. This research is a quantitative research and qualitative research. The data of this research were collected with an observation sheet of waiting time and with an in-depth interviews with 6 informants.The result of this research indicating that average waiting time of laboratory service is 64.11 minutes and the biggest delay is in the process of taking laboratory results. The waiting time for laboratory service that do not reach the waiting time standard is 56.7% on hospital quality indicators standard. The factors that lead this longer waiting period are the lacking numbers of employee, the layout and the space of the room that has not been optimal, and there are still standard operating procedure (SPO) that are not used as guidelines for work. The few suggestion for this case are increasing the number of employees, make effective supervision of the implementation of the SPO, and improve the layout of the laboratory room.

Abstract

Angka kematian bayi dapat diturunkan dengan meningkatkan program ASI Eksklusif. Cakupan ASI Eksklusif Kabupaten Kebumen adalah 51,37%, 56,98%, dan 62,79% pada tahun 2011, 2012, dan 2013. Cakupan ASI Eksklusif tersebut masih rendah jika dibandingkan dengan Standar Nasional sebesar 80%. Hasil survei pendahuluan dengan 8 bidan desa menunjukkan bahwa semua bidan desa tidak pernah melakukan kunjungan ke rumah Ibu hamil dan tidak membentuk kelompok pendukung ASI (KP-ASI). Sebanyak 3 bidan desa tidak mensosialisasikan pengertian, manfaat dan kerugian susu formula. Penelitian bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan kinerja bidan desa dalam pelaksanaan program program ASI Eksklusif di Kabupaten Kebumen. Penelitian ini menggunakan metode kuantitatif dengan pendekatan cross sectional. Sampel adalah 82 bidan desa dari total 460 bidan desa. Analisis bivariat dilakukan dengan menggunakan uji chi square. Hasil analisis bivariat menunjukan bahwa ada hubungan antara persepsi kompensasi (p =0,028) dan motivasi (p=0,008) dengan kinerja bidan desa dalam pelaksanaan program ASI Eksklusif. Sedangkan faktor yang tidak berhubungan adalah pengetahuan (p =0,162), persepsi kepemimpinan (p= 0,829), sarana dan prasarana (p =0,279), dan  persepsi supervisi (p = 0,079). Disarankan kepada Dinas Kesehatan untuk membuat SOP pelaksanaan program ASI Eksklusif untuk bidan desa dan bidan koordinator meningkatkan supervisi baik secara kuantitas maupun kualitas.

Abstract

Indonesian government has made efforts to fulfill the community’s needs about health care. One of the innovation program was The Semarang City Alert Ambulance Program. This program aims to improve health degrees by easier the community having healthcare access. This study was conducted to determine the implementation of The Semarang City Alert Ambulance Program. The research was located in the postal territories of the Ngesrep Public Health Center. This was qualitative research with a descriptive approach using an in-depth interview method with purposive sampling. The analyzed variables were human resources, funds, facilities, SOP or policies, planning, organizational structure, implementation, evaluation, and environment. In input variables, the competency of human resources was good, but the number of human resources still not enough, the funding for the program execution was enough but the facilities still not enough yet, and the SOP has not been formed. Then, in the environment variable, there was no regulation to govern, it caused the implementation of this program interrupted. The Semarang City Government need making specific mayor rules related to the program. The Semarang Health Service needs making regulation and SOP, making ideal planning.
