Abstract

Purpose: To analyze the relationship between fever duration before admission, hematocrit, platelet count, and leukocyte count with length of hospital stay in pediatric patients with dengue infection. Methodology: This observational study with cross-sectional design was conducted at PKU Muhammadiyah Sampangan Hospital, Surakarta, using secondary data from medical records. Forty pediatric patients were selected through consecutive non-probability sampling technique. Statistical analysis was performed with Spearman correlation test using statistical applications. Results: There was a significant association between fever duration before admission and length of hospitalization (r = -0.407, p = 0.009). However, there was no significant association between hematocrit (r = 0.136, p = 0.403), platelet count (r = 0.101, p = 0.536), or leukocyte count (r = 0.111, p = 0.496) with length of hospitalization. Applications/Originality/Value: This study highlights the importance of early detection and intervention in managing pediatric dengue cases. The significant correlation found between fever duration prior to admission and length of stay underscores the need for prompt treatment to minimize hospital stay due to associated inflammation in the body. In contrast, the lack of significant associations between hematocrit, platelet count, leukocyte level and duration of hospitalization highlights the complexity of factors influencing outcomes and the need to explore other prognostic markers. This study conducted in an endemic region of Indonesia contributes to understanding the management of dengue virus infection and underscores the importance of a comprehensive multifactorial approach to improve patient care strategies.

Abstract

Background: Helminthiasis is a significant public health issue, particularly in tropical and subtropical regions. The conventional staining method using 2% eosin, while effective, poses environmental and health concerns due to its carcinogenic properties and poor biodegradability. An alternative, natural, and environmentally friendly dye is necessary. Beetroot (Beta vulgaris L.), rich in betalain pigments, offers a potential solution. Aim: This study aims to evaluate the effectiveness of beet juice as a natural dye for microscopic examination of helminthiasis and assess its performance when combined with 9% citric acid. Methodology: A true experimental design with a posttest-only control group approach was employed. Beet juice concentrations of 70%, 80%, 90%, and 100% were prepared, both with and without 9% citric acid. Stool samples confirmed with helminthiasis were stained using these solutions and examined microscopically. The criteria assessed were effectiveness of coloration, visualization of parasite structure, and quality of staining results. Data were analyzed using the Kruskal-Wallis test. Results: Beet juice at all concentrations showed comparable effectiveness to 2% eosin across all parameters. Adding 9% citric acid enhanced the stability and longevity of the beet juice, maintaining color quality for up to 21 days, compared to 8 days without citric acid. Statistical analysis showed no significant differences between the staining results of beet juice and eosin (p > 0.05). Discussion: beetroot (Beta vulgaris L.) contains betasiani pigments and flavonoids which are antioxidants with an acidic pH that can color the protein layer of worm eggs which are alkaline in nature. Conclusion: Beet juice, particularly when combined with 9% citric acid, is a viable, natural alternative to 2% eosin for helminthiasis staining. It offers a safer and environmentally friendly option with comparable effectiveness and longer durability.

Abstract

Kasus demam berdarah dengue (DBD) atau Infeksi Virus Dengue (IVD) masih dilaporkan tinggi bahkan terus meningkat dan memberikan dampak beban perawatan pasien DBD. Gejala DBD bervariasi dari gejala ringan hingga berat berisiko menyebabkan kematian. Terapi suportif masih menjadi pilihan dan diterapkan untuk mengurangi gejala serta mencegah komplikasi. Upaya pencegahan menghindari gigitan nyamuk dan pengendalian vektor masih menjadi prioritas untuk menghindari risiko IVD. Kasus DBD sebesar 3% yang dilaporkan terjadi di wilayah RW 12 Desa Gonilan Kecamatan Kartasura merepresentasikan angka kejadian yang cukup tinggi di suatu lingkup kecil wilayah sehingga perlu upaya konkrit untuk mencegah angka kejadian lebih besar lagi. Salah satu upaya pencegahan tersebut adalah dengan memberikan edukasi IVD kepada masyarakat setempat. Peran seorang ibu dalam keluarga sangatlah vital dalam mengaja kesehatan keluarga karena keluarga merupakan ujung tombak menuju masyarakat yang sehat dan berpengetahuan. Kegiatan ini bertujuan memberikan edukasi kesehatan tentang pentingnya pengetahuan tentang DBD serta peran anggota keluarga dan masyarakat dalam melakukan upaya pencegahan penularannya. Metode pada kegiatan ini berupa edukasi kesehatan tentang serba-serbi DBD kepada kelompok PKK RT 03 RW 012 Desa Gonilan dalam bentuk penyuluhan dan diskusi interaktif serta pemberian soal pre-test dan post-test. Hasil menunjukkan rata-rata peningkatan pengetahuan peserta tentang DBD sebesar 14,3% dengan hasil yang berbeda bermakna antara sebelum dan setelah mendapatkan edukasi (p=0,015). Dapat disimpulkan bahwa kegiatan ini mampu meningkatkan pengetahuan peserta tentang demam berdarah dengue.

Abstract

Diabetes mellitus involves many mechanisms, the collagen metabolism pathway with very complex interactions and its regulation can contribute to the development of diabetes complications. The collagen 1 alpha 1  (COL1A1) gene plays an important role in the diabetes melitus pathway in adipose tissue by regulating adipocyte function, insulin resistance, and glucose metabolism. COL1A1 expression  is positively correlated with insulin resistance and negatively correlated with insulin sensitivity and is identified as a potential therapeutic target for type 2 diabetes. The cat's whisker plant (Ortosiphon aristatus) pharmacologically has hypoglycemic activity and increases insulin expression. This study aims to observe the potential of cat whisker leaf extract (Ortosiphon aristatus) in increasing the expression of the COL1A1 gene and is protective against increased blood sugar levels in  hyperglycemia-induced Rattus norvegicus. The experimental design with pretest and posttest controlled group design was carried out by dividing 18 test animals into six groups of non-diabetic negative control, diabetic negative control, metformin as positive control, doses of 100, 200, and 400 mg/kgBW. Induction of hyperglycemia with streptozotocin 65 mg/kgBW and nicotinamide 230 mg/kgBW peritoneal injection. The extract treatment is given for 14 days. Blood sugar level measurements were carried out at the treatment’s beginning and end. Adipose tissue was taken to examine COL1A1 gene expression by real time PCR technique by calculating the ratio of the cycle of tresh-hold COL1A1 to the internal ?-actin gene. The results of Kruskall Wallis test showed no significant difference in COL1A1 gene expression between groups and the Spearmen correlation test showed no relationship between blood sugar levels and COL1A1 gene expression (p>0.05). There was a decrease in blood sugar levels before and after treatment for all treatment groups.

Abstract

Background: Dengue fever, caused by the dengue virus which is transmitted by the Aedes aegypti mosquito, is a significant global health problem. Controlling the Aedes aegypti vector is key in efforts to reduce the spread of this disease. However, the use of chemical larvicides in vector control has raised concerns regarding environmental impacts and mosquito resistance. Therefore, research related to the development of natural larvicides based on pomegranate peel extract contain flavonoids, terpenoids, tannins, steroids, and saponins, in microemulsion form has an urgency to provide a safer and more sustainable alternative. Objective: To determine the larvicidal activity of microemulsion ethanol extract of pomegranate peel (Punica granatum) against Aedes Aegypti larvae. Methods: This research uses an experimental study design, post-test only controlled group design. The samples used were Aedes aegypti larvae instar III-IV. There were 12 treatment groups consisting of positive control, negative control, and treatment groups with microemulsion ethanol extract and ethyl acetate fraction of pomegranate peel extract, each with five different concentrations. Results: The probit analysis results indicate that the LC50 (Lethal Concentration for 50% mortality) for the ethanol extract microemulsion of pomegranate peel is 51,607 ppm, while the LC50 for the ethyl acetate fraction microemulsion of pomegranate peel is 52,401 ppm against Aedes aegypti larvae. Conclusion: the probit analysis shows that the ethanol extract microemulsion of pomegranate peel is slightly more effective in killing Aedes aegypti larvae compared to the ethyl acetate fraction microemulsion. These results emphasize the potential of pomegranate peel extracts as natural larvicides, contributing to sustainable and environmentally friendly mosquito control measures.

Abstract

Severe dengue virus infection (DVI) is characterized by thrombocytopenia and plasma leakage (PL) due to the release of vasoactive cytokines by immune cells such as monocytes, lymphocytes, neutrophils, and platelets in response to dengue virus (DENV). This immune response triggers systemic inflammation, altering monocyte, lymphocyte, and neutrophil kinetics, which in turn affects leukocyte differentials. PL is indicated by an increase in hematocrit (Hct). This study aims to evaluate the potential use of the Aggregate Index of Systemic Inflammation (AISI) as an indicator of plasma leakage. A correlative analytical study with a cross-sectional approach was conducted on 70 pediatric DVI patients diagnosed according to the 2009 WHO criteria and confirmed by anti-DENV IgM/IgG tests. Patients with a history of malignancy, autoimmune thrombocytopenic purpura, or secondary DVI were excluded. Absolute neutrophil, monocyte, platelet, and lymphocyte counts, AISI, and Hct levels were obtained from complete blood count results using a hematology analyzer. The results showed that AISI and lymphocytes correlated with Hct, with correlation values of (r=0.410, p<0.001) and (r=0.446, p<0.001), respectively. In contrast, monocytes, neutrophils, and platelets did not correlate with Hct, with correlation values of (r=-0.009, p=0.942), (r=-0.059, p=0.628), and (r=-0.175, p=0.147), respectively. In conclusion, AISI levels in DVI were low and negatively correlated with Hct, a known indicator of PL. These findings suggest that AISI has potential as a marker for PL and disease severity in DVI.

Abstract

Metabolic syndrome is a collection of metabolic disorders that are closely related to an increased risk of developing diabetes mellitus and cardiovascular diseases in obese adolescents. Metabolic syndrome in adolescents is still reversible with varying timeframes. In adolescents with metabolic syndrome, the transition from homeostasis with normal blood sugar levels to type 2 diabetes occurs more rapidly compared to adults. Early diagnosis of metabolic syndrome is essential for prevention through lifestyle changes. Living in urban areas has a higher risk of obesity compared to living in rural areas after controlling for other variables. This activity aims to initially screen metabolic syndrome in middle school adolescents in urban and rural areas. The measurement of metabolic syndrome parameters involved 107 students, consisting of 47 middle school students in urban areas and 60 in rural areas. The measurement and data analysis results showed significant differences in body mass index (p<0,001), waist size (p=0.03), waist circumference (p=0.007), and upper arm circumference (p=0.002) between students in urban and rural areas, while blood sugar levels, blood pressure, and neck circumference did not show significant differences.

Abstract

  ABSTRAK Stunting merupakan gangguan pertumbuhan linier yang ditandai dengan panjang atau tinggi badan menurut umur di bawah -2 SD. Kondisi ini mencerminkan masalah gizi kronik dan masih menjadi tantangan besar di Indonesia. Kegiatan pengabdian masyarakat ini bertujuan untuk meningkatkan pemahaman dan keterampilan kader serta orang tua balita dalam pencegahan stunting melalui edukasi, pelatihan antropometri, promosi pemberian ASI eksklusif, MPASI bergizi seimbang, dan pemahaman tentang pangan olahan untuk keperluan medis khusus (PKMK). Metode pelaksanaan mencakup survei awal, koordinasi dengan Puskesmas Colomadu II, pelatihan kader, edukasi kepada orang tua, serta evaluasi menggunakan pretest dan posttest. Hasil analisis dengan uji Wilcoxon Signed-Rank menunjukkan tidak terdapat perbedaan signifikan secara statistik, namun terdapat peningkatan nilai deskriptif yang menunjukkan adanya dampak positif terhadap peningkatan pengetahuan. Hasil ini mengindikasikan bahwa pendekatan edukatif berbasis komunitas berpotensi efektif untuk pencegahan stunting bila dilaksanakan secara berkelanjutan dan adaptif.   ABSTRACT Stunting is a linear growth disorder characterized by height or length below -2 SD for age. This condition reflects chronic malnutrition and remains a major challenge in Indonesia. This community service activity aims to improve the understanding and skills of cadres and parents of toddlers in preventing stunting through education, anthropometry training, promotion of exclusive breastfeeding, nutritionally balanced complementary foods, and understanding of processed foods for special medical purposes (PKMK). The implementation methods included an initial survey, coordination with the Colomadu II Community Health Center, cadre training, education for parents, and evaluation using pre-tests and post-tests. The results of the analysis using the Wilcoxon Signed-Rank test showed no statistically significant differences, but there was an increase in descriptive values, indicating a positive impact on knowledge improvement. These results indicate that a community-based educational approach has the potential to be effective in preventing stunting when implemented in a sustainable and adaptive manner.

Abstract

ABSTRAK Kesehatan remaja, baik secara fisik maupun mental, telah menjadi isu kesehatan masyarakat yang semakin signifikan dan menjadi perhatian serius secara global, termasuk di Indonesia. Faktor-faktor yang berkontribusi terhadap masalah kesehatan ini meliputi kebiasaan gaya hidup dan kurangnya pengetahuan kesehatan. Keluarga memegang peran yang sangat krusial dan mendasar dalam membentuk perilaku kesehatan remaja. Siswa SMP Muhammadiyah perlu mengetahui pentingnya dukungan keluarga untuk gaya hidup sehat mereka. Tujuan kegiatan ini adalah untuk mengetahui adanya peningkatan pengetahuan tentang dukungan keluarga dalam penarapan gaya hidup sehat remaja pada siswa SMP Muhammadiyah PK Surakarta. Metode pelaksanaannya adalah pretes, penyampaian materi oleh dokter menggunakan presentasi slide, kemudian postes. Hasil yang didapatkan adalah rata-rata nilai pretes adalah 63,36, rata-rata nilai postes adalah 97,59, rata-rata kenaikan skor sebesar 34,54. Dari analisis t-test, didapatkan p<0,001 pada siswa 9A dan siswa 9B yang menunjukkan bahwa terdapat perbedaan yang signifikan pada pengetahuan siswa sebelum dan sesudah edukasi. Simpulan kegiatan ini adalah terdapat peningkatan pengetahuan siswa-siswi SMP tentang peran dukungan keluarga pada gaya hidup sehat mereka.   ABSTRACT Adolescent health, both physically and mentally, has become a significant public health issue and a serious concern globally, including in Indonesia. Factors contributing to this health problem include lifestyle habits and lack of health knowledge. Families play a crucial and fundamental role in shaping adolescent health behaviors. Muhammadiyah junior high school students need to know the importance of family support for their healthy lifestyles. The objective of this study was to determine whether there was an increase in knowledge about family support for healthy lifestyles among junior high school students at Muhammadiyah PK Surakarta. The study was conducted using a pretest, presentation of material by a doctor using slides, and a posttest. The results obtained were an average pretest score of 63.36, an average posttest score of 97.59, and an average score increase of 34.54. From the t-test analysis, p<0.001 was obtained for students in class 9A and class 9B, indicating a significant difference in students' knowledge before and after the education program. The conclusion of this activity is that there was an increase in the knowledge of junior high school students about the role of family support in their healthy lifestyle.

Abstract

Dengue haemorrhagic fever (DHF) cases are still high in America and Asia region, including Indonesia. Eradication of DHF can be done by larvacide. Temephos 1%, chemical larvacide used in Indonesia for decades, can cause environment degradation and allergic reaction in human. Papaya stem extract as biolarvacide contains saponins, tannins, alkaloids, flavonoids, papain and essential oils which have larvicidal activity. It can be applicable in all over the world. This study was conducted to determine the better suspending agent between CMC or PEG of papaya stem extract to kill Aedes aegypti larvae. Objective: To determine the better suspending agent to kill Aedes aegypti larvae. Method: This study was true experimental study in laboratory, used post-test only controlled group design. This study used samples consist of Aedes aegypti instar III-IV larvae. There were seven experimental groups, consist of positive control group, negative control group for CMC, negative control group for PEG, extract concentration of 1.5% with CMC suspending agent, extract concentration of 1.5% with PEG suspending agent, extract concentration of 2% with CMC suspending agent, extract concentration of 2% with PEG suspending agent. Each group was repeated 4 times and counted every 6 hours (6th, 12th, 18th, 24th). Data analysis using the SPSS application ver 23. Probit test used to determine the effectiveness of the larvacide, LC50 and LT50 to determine the concentration and time to kill 50% of Aedes aegypti larvae. It used Kruskal Wallis Test to compare four dependent groups. It used Mann Whitney Test to compare the differences of each group. Result: The study showed that LC50 PEG was 0.033%, LC50 CMC was 0.321%, LT50 (PEG+1.5%) was 20.8 hours, LT50 (PEG+2%) was 30.7 hours, LT50 (CMC+1.5%) was 23.5 hours, LT50 (CMC+2%) was 40.7 hours. Based on the probit test, the LC50 and LT50 results showed that papaya stem methanol extract with PEG suspending agent was more effective than CMC suspending agent, it needed lower concentration and lower time to kill 50% of larvae. Kruskall Wallis Test result p<0.05, so there was significantly difference between groups. Post hoc Mann-Whitney Test result p<0.05 between C(-) PEG group with P1, P2, P3, P4 and between C(-) CMC group with P1, P2, P3, P4, so there was significantly difference between C(-) PEG with each group and C(-) CMC with each group. Conclusion: It can be concluded that papaya stem methanol extract with PEG suspending agent was more effective than CMC suspending agent to kill Aedes aegypti larvae.
