Abstract

Background: COVID-19 in Indonesia has caused 142,173 deaths, East Java is one of the provinces and Surabaya is the city with the highest death rate in Indonesia. Hypertension and cardiovascular disease are the most common comorbidities found in COVID-19 patients. Objectives: The purpose of this study was to analyze the relationship of comorbid hypertension with the degree of symptoms of COVID-19 patients. Methods: The type of this research is quantitative analytic observational with the type of case control study design with a sample of 104 patients. The sampling technique used is simple random sampling. The data used in this study were the medical records of patients who were confirmed positive for COVID-19 at RSUD Dr. Soetomo Surabaya for the period March 2020 to September 2021. Data analysis uses the estimated value of the Odd Ratio (OR) Results: The results showed that the majority of elderly patients as many as 60 patients or 57.6% were male as many as 53 patients or 51%, had comorbid hypertension as many as 63 patients or 60.5% and the most common symptoms experienced by patients were colds and stones. . The results of the analysis showed that 34 patients or 72.5% had comorbid hypertension and had severe symptoms, 14 patients or 27.5% had no comorbidities and had moderate symptoms. Obtained p-value = 0.015> 0.05 so that comorbid hypertension has a significant relationship with the patient's symptoms. The estimated OR value is 2.744 (95% CI = 1.211-6215) which means that patients with hypertension have a 2.744 times greater chance of having severe symptoms than patients without hypertension..Conclusions: The conclusion of this study is that patients with hypertension have a 2.744 times greater chance of being symptomatic than patients without hypertension. Comorbid hypertension is a risk factor for severe symptoms of COVID-19 patients.

Abstract

Background: The city of Tasikmalaya experienced a significant increase in cases of Dengue in 2020. This was evidenced by the incidence rate of Dengue, which was 229 per 100,000 people, and the Dengue mortality rate of 1.42%. The implementation of Dengue surveillance in the Tasikmalaya City Health Office faced delays in reporting suspects, cases, and Larva-free rates. Tasikmalaya City Health Office needs a surveillance system that can detect cases early to ensure timely reporting. Objectives The aim of this study is to design the development of a recording and reporting system for Dengue surveillance in the Tasikmalaya City Health Office. Methods: This study employed a qualitative research design with an action research approach for system development. Primary data were collected through in-depth interviews with officers and manager of Dengue surveillance. Secondary data were obtained through document studies on the surveillance forms used. The data analysis was conducted descriptively. The development design of the Dengue surveillance system was created using a Data Flow Diagram. Results: The types of information needed for the development of Dengue surveillance include the number of suspects, cases, and deaths from Dengue Fever on a weekly basis based on person, place, and time; trends in suspects, cases, and deaths from Dengue on a weekly basis; as well as the monthly Larva-free rates. The required data consist of suspect/case data and Dengue death data from weekly reports by primary health centers; a recapitulation of the number of suspects, cases, and Dengue deaths; Larva-free rates; and the population reported monthly by the primary health centers. Conclusions: The Tasikmalaya City Health Office aims to improve Dengue surveillance by implementing weekly data processing and analysis activities, weekly reports from primary health centers, and biweekly meetings. This will improve workflow procedures and monitor the increase in Dengue cases.

Abstract

Introduction: Immunization is an effective effort to prevent some dangerous diseases in children. By providing complete basic immunization, it can reduce child mortality. East Nusa Tenggara province reached a low number of complete basic immunization coverage in 2016 (69.29%), increased in 2017 (72.2%), then decreased again in 2018 (51.72%).Aims: of this study is to analyze risk factors for incomplete immunization status in children aged 12-23 months in NTT province. Method. The method used is analytical observational with a cross-sectional design. The data used are  secondary data of Basic Health Research 2018. The study population was household members who could be interviewed at the time of Basic Health Research 2018 in the NTT provincial census block area. Total sample was 674 households. The sampling technique used is two stage one phase stratified sampling. The independent  variables include mother's age, father's age, mother's education level, father's education level, number of children, residence, AEFI, and ANC. The dependent variable is the incomplete basic immunization status. Results: In this study, it is known that the candidate in the regression test variables are father's age (p value= 0.005), mother's educational level (p value= 0.000), number of children (p value= 0.007), residence (p value= 0.000), and ANC (p value= 0.000). Conclusion: According to this study,  most children under two years were not fully immunized (63.2%). And in this research, the candidate variables will then become a prediction model of incomplete immunization status in children 12-23 months in East Nusa Tenggara  province.

Abstract

Background: Indonesia has successfully eradicated wild poliovirus since 2005 using robust immunization and AFP surveillance programs. However, circulating vaccine-derived poliovirus type 2 has challenged these commitments, followed by the 2024 outbreak in East Java. Purpose: The study aims to analyze the polio eradication initiative by assessing polio immunization coverage and AFP surveillance in East Java. Methods: A descriptive, quantitative study design and retrospective analysis of polio immunization coverage and AFP surveillance indicators from the Provincial Health Office, East Java, from 2018–2022. The data was collected and analyzed using Excel, EpiInfo7, and Health Mapper software. Results: OPV4 coverage ranged from 89.95% to 100.76%, while IPV coverage ranged from 73.98 % to 94.18% from 2018 to 2020; the proportion of districts and cities for IPV coverage increased from 0% in 2018 to 44.74% in 2022, below the 95% WHO target. Of the 826 cases of AFP reported, 820 (99.27%) were actual AFP cases <15 years of age, with most (49.02%) being <5 years of age. Non-polio AFP rates ranged from 0.80 in 2020 to 2.59 in 2022 per 100,1000 <15 years of age, with the proportion of districts and cities slightly declining from 55.26% in 2018 to 50% in 2022. The annual mean percentage of AFP-adequate specimens is 61.15%, with the proportion of districts and cities significantly declining from 31.58% in 2018 to 10.53% in 2022, below the 80% target. Conclusion: IPV immunization coverage and AFP surveillance indicators are below the WHO-recommended targets across districts and cities in East Java.

Abstract

Background: Indonesia is facing a mental health crisis, with 1 in 10 people experiencing mental disorders. Among the 282,654 households with individuals suffering from severe mental disorders, only 0.6% are receiving treatment, and just 0.5% are regularly taking medication. In Surabaya, 0.19% of the population is identified as People with Mental Disorders (PMD). Objective: This study aims to evaluate the achievement of the Minimum Service Standards (MSS) Policy for Mental Health on PMD in Surabaya in 2023. Methods: This research utilized a cross-sectional design and descriptive analysis to analyze data. Secondary data was gathered from a cohort of PMD services within the Surabaya Health Service Working Area in 2023, involving a total sample of 5,766 PMD individuals. Results: In 2023, the MSS for People with Mental Disorders achieved 100.3%, surpassing previous years and meeting the target of 100%. However, that achievement was not consistent across all health centers in Surabaya. About 48% of community health centres did not meet the MSS target. This shortfall is attributed to the presence of specialized social institutions for mental health cases and the absence of mental health post activities in certain areas. Conclusion: The implementation of the Mental Health MSS for PMD in Surabaya in 2023 met the Ministry of Health's target, achieving 103.33%. The trend from 2020 to 2023 shows a continuous increase in MSS implementation, though the distribution of achievements is uneven across community health centers. Out of 63 health centers, 30 (48%) have not yet reached the 2023 MSS.

Abstract

The achievement of immunization has become a global focus, not only in Indonesia. Maternal perception is one of the factors in increasing complete basic immunization (IDL) status in an area. The aimed to analyze several perceptions and fear of the adverse effects of mothers with IDL status in non-universal child immunization (UCI) urban village areas in Surabaya City. A cross-sectional study was conducted from September 2019 to May 2020. The sample was the community taken from the baseline data survey on IDL problems in non-UCI villages in Surabaya City in 2017. The five variables are the mother's characteristics, the mother's fear of the adverse effects of immunization, the belief/culture, the mother's busyness, and the family support that can be changed in maternal perception. The Chi-square test and a logistic regression test were both used in the multivariate analysis. And 1,449 mothers were included. The mother's perception that has the most influence is the mother's fear of the adverse effects of immunization with an OR of 10.139 at 95% CI (4.063–25.302). The findings show that they need to get an education or socialization from health professionals, collaborate with religious community leaders, as well as getting health campaigns.

Abstract

Landslides are a natural disaster with a serious threat in Indonesia, especially in areas with vulnerable geographic conditions. Gresik Regency, East Java, is one such area that is prone to landslides. This research aims to identify mitigation measures that are appropriate to the level of landslide risk in Gresik Regency. The research method used is a descriptive approach by collecting secondary data from various reports and available data. The research results show that the proposed solution is to install an internet-based landslide Early Warning System (EWS) in locations that are prone to ground movement and landslides. This EWS is based on standard disaster risk assessment parameters, such as slope slope, slope direction, rock type, etc., which are adapted to regional conditions in Gresik Regency. Installation of this internet-based EWS has several innovative values, including providing added value for the organization and stakeholders, innovation in technology, can be replicated, can be applied sustainably, is in line with organizational values, and provides benefits to the organization. Short, medium and long-term activity stages are outlined to achieve successful implementation, including installing EWS, developing the latest formats and applications, as well as monitoring and outreach. The installation of an internet-based landslide EWS in Gresik Regency is an innovative step that has great potential to increase preparedness and reduce the impact of landslides. This EWS is in line with the values ​​of BPBD Gresik Regency and can be replicated by other agencies. Successful implementation depends on short, medium and long term planning steps, as well as ongoing efforts in system development and outreach to the community. It is hoped that this research can contribute to reducing the impact of landslides in Gresik Regency and similar areas in the future.

Abstract

Indonesia masih memiliki prevalensi stunting di atas batas yang ditetapkan WHO yaitu 20%. Prevalensistunting pada Kabupaten Pamekasan belum mencapai target nasional sebesar 14% pada tahun 2024.Salah satu wilayah yang masih mengalami permasalahan ini adalah Puskesmas Sopaah di KecamatanPademawu. Penelitian ini bertujuan untuk menganalisis pengaruh riwayat pemberian ASI eksklusif danriwayat penyakit infeksi (ISPA dan diare) terhadap kejadian stunting pada balita usia 24-59 bulan.Desain penelitian yang digunakan adalah case control dengan total sampel sebanyak 54 balita, terdiridari 27 balita stunting (kasus) dan 27 balita tidak stunting (kontrol). Data diperoleh dari wawancaralangsung dengan responden dan dari Buku Kesehatan Ibu dan Anak (KIA). Analisis data dilakukanmenggunakan uji chi-square dan perhitungan Odds Ratio (OR). Hasil penelitian menunjukkan bahwariwayat ASI eksklusif (p = 0,013; OR = 4,156), riwayat ISPA (p = 0,014; OR = 4,038), dan diare (p =0,006; OR = 4,857) memiliki hubungan signifikan dengan kejadian stunting. Balita yang tidak mendapatASI eksklusif dan yang memiliki riwayat ISPA atau diare lebih berisiko mengalami stunting. Olehkarena itu, diperlukan sosialisasi rutin tentang pentingnya ASI eksklusif dan pelatihan manajemen ASIperah bagi ibu bekerja. Pemanfaatan Buku KIA secara optimal juga perlu ditingkatkan sebagai saranakomunikasi, informasi, dan edukasi serta pemantauan tumbuh kembang balita.

Abstract

Bacillus Calmette–Guérin (BCG) immunization plays a crucial role in preventing tuberculosis, especially in developing countries like Indonesia. Its coverage is influenced by various factors that differ between urban and rural areas. This study aimed to identify factors associated with BCG immunization among infants aged 0–3 months in urban and rural Indonesia. This study used data from the 2017 Indonesia Demographic and Health Survey (IDHS), including 562 rural and 476 urban respondents. Variables analyzed included maternal age, education, household wealth, contraceptive use, and ANC visits. Chi-square tests and multiple logistic regression were conducted separately for urban and rural areas. The findings showed that 64.2% of urban and 48.7% of rural children received BCG immunization. In rural areas, children from the richest households were more likely to be immunized compared to those from the poorest households (OR = 2.29; 95%CI = 1.1–4.6). Children of mothers with higher education in urban areas were more likely to receive BCG compared to those with lower education (OR = 3.43; 95%CI = 1.3–9.0), as were those with secondary education (OR = 2.28; 95%CI = 1.1–4.4). Mothers who used contraception were also more likely to immunize their children compared to those who did not (OR = 2.71; 95%CI = 1.5–4.8). BCG coverage remains lower in rural communities. Improving coverage requires expanding access to health services in rural areas and strengthening education and contraceptive programs in urban settings.

Abstract

Ringkasan: Latar belakang: Indonesia masih menghadapi tantangan besar dalam pencegahan stunting dengan prevalensi 21,5% tahun 2023, sementara BPJS Kesehatan telah mencakup 83% penduduk namun akses layanan kesehatan belum merata terutama di daerah terpencil. Tujuan: Menganalisis peran BPJS Kesehatan dalam pelayanan promotif, preventif, dan kuratif untuk pencegahan stunting guna mendukung pencapaian Generasi Emas 2045. Metode: Penelitian kualitatif deskriptif-analitis dengan pendekatan literature review menggunakan artikel ilmiah dan sumber resmi terkait pencegahan stunting, analisis data model Miles dan Huberman. Hasil: BPJS Kesehatan berperan dalam edukasi gizi dan pemeriksaan rutin (promotif-preventif), pemberian TTD dan suplemen (preventif), serta rujukan anak stunting untuk intervensi gizi spesifik (kuratif). Simpulan: BPJS Kesehatan memberikan kontribusi signifikan melalui layanan komprehensif namun masih menghadapi keterbatasan alat medis dan tenaga terlatih. Saran: Diperlukan peningkatan ketersediaan fasilitas medis, perluasan cakupan layanan, evaluasi rutin program pencegahan, dan penguatan kolaborasi lintas sektor.
