The Combination Effect of Simvastatin and Virgin Coconut Oil on Total Cholesterol Levels in Dislipidemic Male Albino Rats (Rattus norvegicus) Lutfi Dewanda Nugroho; Soebagijo Adi Soelistijo; Jusak Nugraha

Abstract

Introduction: Simvastatin is a primary pharmacological therapy for dyslipidemia (hypercholesterolemia). Virgin coconut oil (VCO) is a companion intake of statin drugs considered capable of lowering blood cholesterol levels. This study aimed to determine the combination effect of statin drug groups with VCO on total cholesterol.Methods: The stages of this study started with the adaptation process of animals, giving a high-fat diet (HFD), and treatment with simvastatin and VCO. The adaptation process was conducted where the experimental animals were fed pellets and drank clean water in a container. A HFD was given by mixing pork brains with distilled water with a ratio of 1:3 as much as 2 ml per day for 21 days. In simvastatin and VCO treatment for 7 days, the experimental animals were divided into 4 groups; group I was given aquades, group II was given VCO, group III was given simvastatin, and group IV was given simvastatin and VCO. The total cholesterol levels in the blood of albino rats were examined by employing a BS-300 Chemistry Analyzer through a spectrophotometric method. The total cholesterol levels data obtained were then examined for normality (Shapiro-Wilk test) and analyzed parametrically by utilizing One-way ANOVA.Results: The average total cholesterol level in the negative control group (being given a standard diet) was 45.57 mg/dL. The averages of blood total cholesterol levels were 47.28 mg/dL, 45.00 mg/dL, 46.85 mg/dL, 41.42 mg/dL, and 44.28 mg/dL in the positive control group (given an HFD), in the treatment group I, in the treatment group II, in the treatment group III, and in the treatment group IV, respectively. There were no significant distinctions in the statistical test on decreasing blood total cholesterol among the negative control group, positive controls I, II, III, and IV.Conclusion: Combination of simvastatin and VCO tends to reduce the total cholesterol level in albino rats. This is better than just giving VCO simvastatin alone.

Abstract

Introduction: Rheumatoid arthritis (RA) is an autoimmune disease which mainly attacks synovial membrane and causes systemic manifestation. During treatment, controlling disease activity is needed to prevent further complication. On the other hand, medications used in the treatment of RA may bring various side effects. It is important to evaluate side effects from the given therapy.Methods: This study aimed to evaluate response and side effects of therapy in RA patients. The samples were collected from 59 RA patients at Rheumatology Division of Outpatient Clinic in Department of Internal Medicine Dr. Soetomo General Hospital Surabaya in 2017. This study method was descriptive observational with cross sectional design using medical records.Results: Pain was reduced in 83.1% patients, Erythrocyte Sedimentation Rate (ESR) increased in 61.4% patients, and C-Reactive Protein (CRP) decreased in 50% patients. Based on the statistic analysis, ESR decreased significantly (p = 0.012) while CRP decreased not significantly (p = 0.415). The side effects were observed from clinical and laboratory data. Based on clinical symptoms, there were alopecia in 1.7% patient, dyspepsia in 78% patients, infection in 27.1% patients, and other symptoms including itchy skin, neuropathy, hyperuricemia, hyperkalemia and Acute Kidney Injury (AKI). Meanwhile, abnormalities in laboratory data include increased aspartate transaminase (AST) in 3.8% patients, increased alanine transaminase (ALT) in 26.1% patients, increased Blood Urea Nitrogen (BUN) in 7.9% patients, increased creatinine serum in 7.9% patients, decreased hemoglobin in 15.5% patients, and decreased leukocytes in 3.4% patients.Conclusion: Most patients had a good therapeutic response based on decreased pain, while ESR had a significant decrease and CRP did not have significant decrease. Abstract

Highlights: The majority of HIV/AIDS patients who succumbed to the effects of co-infection at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, were of productive age, with the predominant complications being anemia and bacterial infections. Tuberculosis was listed as the most common opportunistic infection that exacerbated the condition of HIV/AIDS patients and caused death during the ongoing pandemic of COVID-19.   Abstract Introduction: Human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) is an immune system disease caused by the HIV infection, making individuals susceptible to various diseases, including coronavirus disease (COVID-19). Co-infection in HIV/AIDS patients can worsen the severity of the diseases, especially in those with comorbidities, complications, and opportunistic infections, potentially leading to death. This study aimed to determine the mortality profile of COVID-19 co-infection in HIV/AIDS patients at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, from January 2020 to December 2022. Methods: This study employed a descriptive cross-sectional method, utilizing secondary data from 48 medical records of COVID-19 co-infection in HIV/AIDS patients at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, from January 2020 to December 2022, collected through a total sampling technique. Results: Of the 105 total HIV/AIDS patients with COVID-19 co-infection, 48 were selected in this study based on predefined inclusion and exclusion criteria. The mortality profile showed a predominance of males (75%), with an average age of 36.5 years. The most common comorbidity was bacterial infection (38.6%), with the highest complication being anemia (55.6%), and the most prevalent opportunistic infection being tuberculosis/TB (50%). Conclusion: The mortality profile of COVID-19 co-infection in HIV/AIDS patients indicated a higher occurrence among males, with an average age of 36.5 years old. The most common comorbidity was bacterial infection, whilst the most prevalent complications include anemia, with TB being the most frequent opportunistic infection.
