Abstract

Background: Leprosy is strongly influenced by the host immunity response. One of the cytokines that play a role in cellular immunity is IL-2. IL-2 will stimulate macrophages to activate phagocytosis and form granulomas, and this occurs in the tuberculoid type of leprosy.Objective: Aim of this study was to verify the IL-2 serum levels are negatively correlated or not with bacterial index in leprosy.Methods: This study was a cross-sectional observational analytic study involving 50 leprosy subjects (41 MB and 9 PB types). Study was conducted at Dermatolovenerology Polyclinic of Sanglah General Hospital from December 2019 to March 2020. Sample was taken using a consecutive sampling method. IL-2 serum levels were measured by ELISA. Result: The mean of IL-2 serum in PB was 75.53±10.32 pg/mL and in MB was 37.59±17.13 pg/mL. Strong negative correlation (r= -0.67; p<0.001) between serum IL-2 levels with bacterial index (CI 95%= 25.96–49.93; p<0.001) was found. Every 1 pg/mL increase in IL-2 serum levels will be accompanied by a decrease in the bacterial index of 0.039 and vice versa. Determinant coefficient shows that 42.2% of bacterial index affected by the IL-2 serum levels. Low IL-2 serum levels were the risk factor of high bacterial index 2.8 times (PR= 2.8; CI 95%= 1.444–5.519; p<0.001).Conclusion: IL-2 serum levels were found to be higher in PB than MB. There was a negative correlation between IL-2 serum levels with bacterial index. Low IL-2 serum levels were the risk factor of a high bacterial index in leprosy patients.

Abstract

Background: Melanoma is the most serious lethal skin cancer, affects the melanin producer cells (melanocytes). Surgery is the most common treatment, whereas for the advance stage the development of a treatment is recommended. BRAF (Dabrafenib and Vemurafenib) inhibitor or MEK inhibitor (Trametinib) is used as the most frequently targeted therapy of melanoma due to more than 80% patient with positive BRAF mutation. In this review, those treatments will be investigated systematically to identify their clinical outcome.Method: This systematic literature review (SLR) was performed from Cochrane, Science Direct, Google Scholar, and Pubmed. Cochrane Risk-of-Bias Tool RoB2 is used to assess RCT studies and New-castle Ottawa Scale Assessment to assess cohort studies by 3 different assessors. Data analysis was carried out by using Review Manager (RevMan 5.4). Heterogenicity test was assessed by I2  and Chi2 statisticResult: There are 20 studies used in this article (13 RCT and 7 cohorts). The overall survival (OS) and progression-free survival (PFS) of study that using targeted therapy (vemurafenib, trametinib, or dabrafenib) compare other therapies (chemotherapy, immunotherapy,etc) showed risk ratio (RR) was 1.12 (95%CI 1.07,1.17;  I2=100%; p<0,00001). The OS and PFS with monotherapy compare of vemurafenib, trametinib, or dabrafenib with combination therapy showed RR was 1.09 (95%CI.06,1.13;I2=99%; p<0,00001). Conclusion: BRAF and MEK targeted therapy has a good prognosis for a patient with a positive BRAF gene mutation and could be combined with other therapy for a better clinical outcome rather than monotherapy.Keyword: melanoma, dabrafenib, vemurafenib, and trametinib

Abstract

Background: Varicella-zoster virus (VZV) is a virus of human neurotropic that remains within ganglionic neurons in a latent state after the primary infection throughout the entire neuroaxis.Case: A male patient, 32 years old Javanese, consulted from the pulmonary division with complaints of vesicular eruption in the facial area on September 27, 2020. The patient was diagnosed with ophthalmic herpes zoster and a mild COVID-19 infection since 2 days ago and was hospitalized. Management in patients was acyclovir, methylprednisolone, with analgesics and vitamins. COVID-19 as co-infection and other viruses has been known, but infrequently affect the respiratory tract.Conclusion: Herpes zoster patient should ruled out the COVID-19 and maximize the precaution for personal safety equipment until the SARS-CoV-2 is excluded.

Abstract

Background: Melasma is a chronic cutaneous hypermelanosis in areas exposed to sunlight. Hormonal factors are known to influence melanogenesis processes such as prolactin as a neuroendocrine modulator in skin epithelial growth and immune system. This study aims to prove the inverse relationship between plasma prolactin levels and the severity of melasma.Methods: A cross-sectional study was carried out on 59 patients in the skin and genital olyclinic at Sanglah Hospital in Denpasar involving 46 subjects of melasma and 13 subjects who were not melasma who met the inclusion and exclusion criteria. Plasma prolactin levels were assessed by the Chemiluminescent microparticle immunoassay (CMIA) method. Data analysis was performed using SPSS ver. 23 software with a P-value <0.05 was considered statistically significant.Results: Plasma prolactin levels in patients with melasma were significantly lower than non-melasma patients (p <0.05). Patients with moderate and severe degrees of melasma had significantly lower plasma prolactin levels compared with mild degree of melasma (p <0.05). Low plasma prolactin levels can increase the likelihood of melasma by 4.79 times (PR 4.79; 95% CI = 0.94-24.27; p <0.05). Low plasma prolactin levels were significantly strong negative correlated with melasma severity (r = -0.820; p <0.05).Conclusion: There is a strong negative correlation of low plasma prolactin levels with the severity of melasma in patients with skin disorders at Sanglah General Hospital

Abstract

Background: Atopic dermatitis (AD) is a chronic and residual skin inflammatory disease in which the symptoms include intense itching, chronic exacerbations and remissions, with multifactorial etiologies. The role of leptin in DA is closely related to polarization of T cells towards Th1 cells resulting in the release of pro-inflammatory cytokines.Methods: A cross-sectional study was conducted among 30 atopic dermatitis patients at Sanglah General Hospital in the period of July-August 2018. The leptin examination was carried out using the ELISA method. Data analysis were performed using SPSS version 25.0 software with the Spearman correlation test to determine the relationship between levels of serum leptin and the severity of atopic dermatitis based on SCORAD.Results: Most of the respondents were females (66.7%) with an average leptin level was 25.75 + 18.21 pg/dL. The results showed a significant negative correlation between the level of serum leptin and the severity of DA (SCORAD) (r: -0.127; p: 0.043). However, there was a significant positive correlation between pH of the skin and SCORAD (r: 0.810 and p: <0.001).Conclusions: There is a negative correlation between level of serum leptin and SCORAD but it has a strong positive correlation between the pH of the skin and SCORAD.

Abstract

ABSTRACTIntroduction: Ectodermal dysplasia (ED) is a rare genetic disorder characterized by defect in two or more ectodermal structures such as teeth, hair, nails and sweat glands. Ectodermal dysplasia divided into 2 types, which is hidrotic and hypohidrotic. The hypohidrotic type was found in 80% of cases. There is no specific treatment for this disorder, which is more of a symptomatic measures depends on the structure involved.Case: A 1 year old male, Balinese, consulted from Department of Pediatrics to Department of Dermatovenereology with the chief complaint of dry skin, absence of sweat, teeth, scalp hair and eyebrow since birth. Recurrent fever present especially in hot climates. No growth abnormalities. No history of incest between parents. His grandfather also complained about the same symptoms, which are sparse hair and decreased sweating. General examination revealed frontal bossing, saddle nose, prominent chin and anodontia. Dermatologic examination revealed multiple hypopigmented macules, well defined margin, oval-round shaped, size ø 0,5-1 cm, distributed over entire skin and anhidrosis. Department of Dermatovenereology gives moisturizer every 12 hours topically. Department of Pediatrics gives zinc tablets, 20 mg every 24 hours orally.Conclusion:The diagnosis of ED was mainly clinical. Treatment is symptomatic and effective education is needed to increase patient's quality of life. Early fetal detection can be performed if there is a risk in the family. Currently, there is a team joined under SIDIC program in Sanglah Hospital whose main purpose is to detect and prevent congenital disorders early.

Abstract

Background: Acute urticaria is a skin disorder that is often encountered that can cause redness to respiratory obstruction so that emergency emergencies and urticaria lesions that last more than 72 hours are indicative of hospitalized patients. This study aims to describe the characteristics of acute urticaria at Dermato-Venerology Polyclinic, Sanglah General Hospital during October 2017-2018 period.Methods: A cross-sectional study was conducted using secondary data from medical records of patients with urticaria in Sanglah General Hospital in the October 2017-2018 period. Data retrieval was taken by the Total Sampling method. Distribution of research variables namely, gender, age, genetic, causative factors, were collected. Data analysis is described in the form of tables and narratives using SPSS version 20 for Windows.Results: The results showed that there were 23 acute urticaria patients in this study, the age of the most research samples was in the Adult age category with a large percentage of 60.87%, the most gender-based study sample was female gender with a percentage of 78.3%, research genetic samples most of them are in the absence of genetics, with a percentage of 91.3%, the work of most research samples in housewives with a percentage of 34.8%, the most common cause of acute urticaria is in food allergies with a percentage of 56.5%.Conclusion: The incidence of acute urticaria is most common in adulthood, female sex, housewives, and food allergies.

Abstract

Introduction: Leprosy is still a burden on endemic countries such as Indonesia. As well as leprosy reaction is a problem associated with the cessation of treatment from leprosy patients. The purpose of this study was to do a description of the characteristics of leprosy patients with ENL reactions in the Skin and Sex Polyclinic of Sanglah Hospital Denpasar.Method: This study used a descriptive design with a retrospective approach conducted at the Skin and Sex Polyclinic of Sanglah General Hospital. A search related to the characteristics of leprosy patients who experienced an ENL reaction, then the data was presented in descriptive form.Results: Female gender tend to have more severe leprosy reactions (50%) compared to men. High income (50%), non-endemic area origin (44.8%), elementary school education level (44.4%), and active working (31.0%) tended to experience more severe leprosy reactions. And in this study more severe leprosy reactions were found in the multi-bacillary leprosy group (65.7%), whereas mild leprosy reactions were higher in the pauci-bacillary leprosy group (34.2%).Conclusion: The high rate of severe leprosy reaction requires special attention from both the government and health practitioners, so that there is an integrated management of leprosy, so that the population in Indonesia becomes healthier.

Abstract

Background: The development of the garment industry in Bali is increasing as a tourism supporting industry. As a consequence the number of garment and textile companies is growing in quantity and quality. According to data in the Denpasar City, there were around 125 large garment companies employing at least 100 workers. A small garment company as a home industry is almost five hundred. The impact of the development of the garment industry requires a lot of human resources by recruiting, opening jobs both for trained and untrained workers, most of whom are casual workers, without getting attention in terms of health. This research was conducted to collect data about the pattern of skin lesions (work-related contact dermatitis) in garment workers in Denpasar City because they do not have complete and accurate data.Methods: The method used in this study was a survey on three large garment companies in Denpasar with interviews and examinations of their skin disease patterns.Results: Total 288 workers included in this study consisted of 105 (36.5%) men and 183 (63.5%) women. From 288 respondents, 74 workers (25.7%) suffer from skin disorders related to their work. The occupational category that suffered the most was colouring workers of 30 people.Conclusion: The pattern of skin disease in garment workers by 25.7% suffering from DKAK. Disease categories are often workers who come into contact with colour materials. Latar Belakang: Perkembangan industri garment di Bali semakin meningkat sebagai salah satu industri penunjang pariwisata. Sebagai konsekuensi jumlah perusahan garmen dan tekstil semakin banyak secara kuantitas dan kualitas. Menurut data di kodya Denpasar, tercatat sekitar 125 perusahan garmen yang besar dengan memperkerjakan sedikitnya 100 orang pekerja. Perusahan garmen yang kecil sebagai industri rumah tangga hampir sebanyak lima rastusan. Dampak dari perkembangan industri garment membutuhkan banyak tenaga kerja dengan merekrut, membuka lapangan pekerjaan baik itu untuk pekerja yang terlatih maupun yang tidak terlatih, kebanyakan merupakan pekerja lepas, tanpa mendapat perhatian dari segi kesehatan. Penelitian ini dilakukan untuk mengumpulkan data tentang pola kelainaan kulit pada(DKAK) pekerja garment  di Kodya Denpasar, karena belum memiliki data yang lengkap dan akurat.Metode: Metode yang digunakan pda penelitian ini adalah survelanse, pada 3 perusahan garment yang besar di Denpasar dengan wawancara dan pemeriksaan terhadap pola penyakit kulitnya.Hasil: Dari 288 pekerja yang di ikut sertakan dalam penelitian ini terdiri dari 105 (36,5%) laki-laki dan 183 (63,5%) perempuan. Dari 288 responden, sebanyak 74 pekerja (25,7%) yang menderita kelainan kulit yang berhubungan dengan pekerjaannya. Katagori pekerjaan yang paling banyak menderita adalah pada pekerja pencelupan (coloring) sebesar 30 orang.Simpulan: Pola penyakit kulit pada pekerja garment sebesar 25,7% menderita DKAK. Katagori penyakit sering adalah pekerja yang berhubungan kontak dengan bahan warna.

Abstract

Background: Fungal infections are one of the most common dermatologic conditions affecting elderly population. Elderly population is susceptible to all of the superficial mycoses such as tinea pedis, candidiasis, and onychomycosis in geriatric populations compared to other age groups.Objective:  To determine the profile of onychomycosis among elderly patients over a period of 1 years (January 2018–August 2019) at Dermatology and Venereology outpatient clinic, Sanglah General HospitalMethods:  Retrospective study was performed at Department of Dermatology and Venereology, Sanglah General Hospital, Denpasar, Bali. The data collected from patient’s register data of outpatient clinic.Result:  A total of 11 patients were diagnosed with onychomycosis, consist of 5 women and 6 men. From these, 4 (36,3%) was reported with chronic systemic diseases and 7 (63,6%) reported with no concomitant diseases. The most common clinical sign were onychodystrophy, hyperkeratotic subungual and dyschromia (6 patients, 54,54%). Potassium hydroxide examination reveal positive results in all patient. From 5 patients, on culture examination, Candida (2 patients, 40%), Trichophyton rubrum (2 patients, 40%), and Trychophyton mentagrophytes (1 patient, 20%) were found. For therapy, there were 7 patients got combination therapy (fluconazole 150 mg and ciclopirox lacquer 8%) and 4 patients with single therapy (ciclopirox lacquer 8%).Conclusion: Onychomycosis is more common in the elderly or geriatric patients, this occurs because old age can change the body's immune function, including decreased immune response to fight infections against viruses, bacteria, and fungi.  Pendahuluan: Infeksi jamur adalah salah satu kondisi dermatologis paling umum yang mempengaruhi populasi lansia. Populasi lansia rentan terhadap semua mikosis superfisial seperti tinea pedis, kandidiasis, dan onikomikosis pada populasi geriatri dibandingkan dengan kelompok usia lainnya.Tujuan:  Untuk menentukan profil onikomikosis pada pasien usia lanjut selama 1 tahun (Januari 2018 - Agustus 2019) di poliklinik rawat jalan Dermatologi dan Venereologi, Rumah Sakit Umum Pusat Sanglah.Metode:  Studi retrospektif dilakukan di Departemen Dermatologi dan Venereologi, Rumah Sakit Umum Pusat Sanglah, Denpasar, Bali. Data dikumpulkan dari data pendaftaran pasien di klinik rawat jalan.Hasil:  Sebanyak 11 pasien didiagnosis menderita onikomikosis, terdiri dari 5 wanita dan 6 pria. Dari hasil ini, sebanyak 4 orang (36,3%) dilaporkan dengan penyakit sistemik kronis dan 7 orang (63,6%) dilaporkan tanpa penyakit bersamaan. Tanda klinis yang paling umum adalah onikodistrofi, hiperkeratotik subungual dan dischromia (6 pasien, 54,54%). Pemeriksaan kalium hidroksida mengungkapkan hasil positif pada semua pasien. Dari 5 pasien pada hasil pemeriksaan kultur ditemukan adanya infeksi Candida (2 pasien, 40%), Trichophyton rubrum (2 pasien, 40%), dan Trychophyton mentagrophytes (1 pasien, 20%). Untuk penatalaksanaan, terdapat sejumlah 7 orang pasien mendapatkan terapi kombinasi (fluconazol 150 mg dan ciclopirox lacquer 8%) dan 4 orang pasien dengan terapi tunggal (ciclopirox lacquer 8%).Simpulan: Onikomikosis lebih sering terjadi pada pasien usia lanjut atau usia lanjut dimana hal ini terjadi karena usia tua dapat mengubah fungsi kekebalan tubuh, termasuk penurunan respons imun untuk melawan infeksi terhadap virus, bakteri, dan jamur.
