Abstract

Background: Herpes zoster ophthalmicus occurs in 10-20% of cases of herpes zoster. Herpes zoster ophthalmicus occurs when the human herpes virus type 3 reactivates the first division of the trigeminal nerve, which is also called the ophthalmic division. In some patients, it may show ocular involvement.Case: A 60 years old male patient came with swelling and wounds on the left forehead and around the left eye. Initially appear as watery spots and accompanied by pain. The pain felt like being stabbed, hot like burning sensation. The patient had chickenpox as a child. In the dermatological status of the superior palpebra, frontalis and left parietalis, multiple erythema patches were seen with multiple erosions. It appears that the lesion is unilaterally distributed according to the dermatome of the trigeminal cranial nerve of the left ophthalmic branch. Gram examination on erosive lesions showed the presence of 1-5 leukocytes / large field of view with Gram positive cocci and no Gram negative rods. The patient was diagnosed with herpes zoster ophthalmicus left with secondary infection. Consultation to ophthalmology division revealed left ocular herpes zoster ophthalmic (blepharoconjunctivitis), corneal erosions, iridoplegia and mature senile cataracts. The patient discharged on the fifth day. On the 11th day, complaints of watery eyes, swelling and eye discharge were gone.Conclusion: A case of herpes zoster ophthalmicus in old age with ocular complications and secondary infection was reported. For erosional lesions, open compresses was applied with NaCl and sodium fusidate cream. Patients respond well to therapy. Latar Belakang: Herpes zoster oftalmikus terjadi pada 10-20% dari kasus herpes zoster. Herpes zoster oftalmikus terjadi ketika human herpes virus tipe 3 mengalami reaktivasi pada divisi pertama dari saraf trigeminalis, yang disebut juga divisi oftalmik. Pada beberapa pasien, dapat menunjukkan keterlibatan okular.Laporan Kasus: Pasien laki-laki, usia 60 tahun datang dengan keluhan bengkak dan luka pada dahi kiri serta area sekitar mata kiri. Awalnya muncul berupa bintik-bintik berair dan disertai nyeri. Nyeri dirasakan seperti ditusuk-tusuk, panas seperti terbakar, dan mata terasa seperti tertarik. Pasien pernah menderita penyakit cacar air pada saat kanak-kanak. Pada status dermatologis palpebra superior, frontalis dan parietalis sinistra tampak patch eritema multipel dan pada beberapa tempat disertai erosi multipel. Tampak lesi terdistribusi secara unilateral sesuai dermatom nervus kranial trigeminus cabang oftalmikus sinistra. Pemeriksaan Gram pada lesi erosi menunjukkan adanya leukosit 1-5/lapang pandang besar, ditemukan adanya kokus Gram positif dan tidak didapatkan batang Gram negatif. Pasien didiagnosis dengan herpes zoster oftalmikus sinistra disertai infeksi sekunder. Konsultasi pada bagian mata didapatkan okuli sinistra herpes zoster optalmika (blefarokonjungtivitis), erosi kornea, iridoplegia dan katarak sinilis matur. Pasien dipulangkan pada hari kelima perawatan. Pada hari ke 11, Keluhan mata berair, bengkak dan kotoran mata sudah tidak ada.Simpulan: Telah dilaporkan sebuah kasus herpes zoster oftalmikus pada usia tua dengan komplikasi okular dan didapatkan juga infeksi sekunder. Pada lesi erosi diberikan kompres terbuka dengan NaCl dan krim natrium fusidat. Pasien memberikan respon yang baik dengan terapi.

Abstract

Background: Indonesia ranks third highest of new cases of leprosy after India and Brazil. Transmission of leprosy through inhalation and prolonged close contact. Families who live in the same house have the highest risk for infection. Currently, several studies have found a correlation between IgM anti-PGL-1 levels with micronutrient deficiencies, especially zinc, which is associated with cellular immunity. This study aimed to investigate the effect of 40 mg elemental zinc supplementation per day for four weeks on IgM anti-PGL-1 levels. Methods: An experimental one-group pre-test and post-test design conducted from February to May 2021. The samples were all close contact persons with multibacillary leprosy patients aged 18-65 years who visited the Dermatology and Venereology Outpatient Clinic, Sanglah General Hospital, Denpasar during the study period by consecutive sampling. The history taking, physical examination, and blood sampling were carried out at the to evaluate IgM anti-PGL-1 and zinc levels. Results: Mean serum zinc level before and after supplementation were 79.00±9.67 g/dl and 95.16±31.84 g/dl, mean difference 25.21±32.12 g/dl (p<0.001). IgM anti-PGL-1 mean level before and after supplementation were 843.21±878.51 u/ml and 465.68±735.21 u/ml, mean difference 377.52±309.62 u/ml (p<0.001). Correlation test of different mean levels of zinc with different mean levels of IgM anti-PGL-1 resulted with r=0.630 and p=0.004. Conclusion: Forty mg elemental zinc per day for four weeks may increase serum zinc levels and reduce IgM anti-PGL-1 in close contact with multibacillary leprosy patients (p<0.05). Higher mean increase in serum zinc levels follower by higher mean decrease of IgM anti-PGL-1 levels.   Latar Belakang: Indonesia menduduki peringkat ketiga jumlah kasus baru kusta terbanyak setelah India dan Brazil. Keluarga yang tinggal serumah memiliki resiko yang paling tinggi untuk terkena infeksi. Saat ini ditemukan beberapa penelitian yang menghubungkan kadar PGL-1 dengan defisiensi mikronutrien, salah satunya yaitu seng. Penelitian ini bertujuan untuk meneliti efek pemberian suplementasi seng elemental 40 mg per hari selama 4 minggu terhadap kadar IgM anti PGL-1. Metode: Penelitian eksperimental one group pre-test post-test design yang dilakukan pada Februari hingga Mei 2021. Sampel adalah seluruh individu narakontak dengan pasien kusta tipe multibasiler berusia 18-65 tahun yang berkunjung ke Poliklinik Kulit dan Kelamin RSUP Sanglah Denpasar selama periode penelitian berlangsung secara consecutive sampling. Anamnesis, pemeriksaan fisik, dan pengambilan sampel darah dilakukan untuk pemeriksaan IgM anti PGL-1 dan kadar serum seng. Hasil: Rerata kadar serum seng sebelum dan sesudah diberikan suplementasi yaitu 79,00±9,67 µg/dl dan 95,16±31,84 µg/dl dengan beda rerata 25,21±32,12 µg/dl (p<0,001). Rerata kadar IgM anti PGL-1 sebelum dan sesudah diberikan suplementasi yaitu 843,21±878,51 u/ml dan 465,68 ± 735,21 u/ml dengan beda rerata 377,52±309,62 u/ml (p<0,001). Uji korelasi beda rerata kadar seng dengan beda rerata kadar IgM anti PGL-1 yaitu r=0,630 dan p=0,004. Simpulan: Pemberian suplementasi seng elemental 40 mg perhari selama 4 minggu dapat meningkatkan kadar serum seng dan menurunkan kadar serum IgM anti PGL-1 pada narakontak pasien kusta tipe multibasiler (p<0,05). Semakin tinggi selisih rerata peningkatan kadar seng serum, semakin tinggi selisih rerata penurunan kadar IgM anti PGL-1.

Abstract

Background: Incontinentia pigmenti (IP) is a rare X-linked dominant inherited genodermatosis that occurs almost in females and is usually accompanied by other ectodermal tissue diseases such as the central nervous system, eyes, hair, nails, teeth and skeletal system. This case report aims to enhance understanding of incontinentia pigmenti and provide appropriate treatment to patients and proper education to families. Case report: A 3-days-od baby girl was consulted by the Pediatric department with complaints of blisters and erythematous rashes on almost her entire body with the Blaschko line distribution. The nails on the right and left big toes appeared inward. No history of fever and seizure. No family had similar complaints. There were no eye and nerve abnormalities involved. She treated with hydrocortisone 1% cream every 12 hours on erythematous papules and vesicles, open compresses with 0.9% NaCl every 8 hours for 10-15 minutes on lesions with yellowish crusts. Thus, the patient diagnosed with incontinentia pigmenti. Conclusion: The diagnosis of incontinentia pigmenti is based on history and physical examination. This case is very rare, so the family needs to understand the course of this disease. Appropriate management and education can prevent secondary infection.

Abstract

Introduction: A healthy person who contacts people diagnosed with leprosy can be at a high risk of being infected with leprosy with a predominance of the subclinical stage. The MB type infection has a 4-10 times greater risk of developing into a clinical form. Early detection of subclinical leprosy is important so that it can play a role in the process of eliminating leprosy. If it is not screened, most of the subclinical stage leprosy can develop into the clinical stage and can cause physical deformities. Phenolic glycolipid-1 (PGL-1) is a specific antigen for M.leprae and Interleukin-10 (IL-10) is an anti-inflammatory cytokine that shows the body's immune response. Research on the relationship between levels of IL-10 and IgM anti-PGL-1, especially in Bali, has not been carried out, so this study aims to find a relationship between levels of IL-10 and levels of IgM anti-PGL-1 in household contacts of multibacillary leprosy patients. Methods: This research is an analytical observational study with a cross-sectional and was carried out in February-May 2021 at the Dermatology and Venereology Polyclinic and Clinical Pathology Laboratory, Sanglah Hospital Denpasar, and the Leprosy reference laboratory, Tropical Diseases Center, Universitas Airlangga. Samples aged 18-65 years were selected through Consecutive Sampling according to inclusion and exclusion criteria. The sample was taken from venous blood and then checked for levels of PGL-1 and IL-10. Data analysis using SPSS version 23. The results were considered significant if p<0.05. Results: The mean level of IgM anti-PGL-1 in the contact group vs. the non-contact group was 863.07 ± 789.04 u/ml vs 247.75 ± 161.60 u/ml. The mean levels of IL-10 in the contact and non-contact groups were 121.09 ± 144.11 pg/ml and 46.04 ± 11.27 pg/ml. There was a significant difference in levels of IL-10 and IgM anti-PGL-1 in the contact group and non-contact groups (p<0.05) and a weak positive relationship (r=0.296, p=0.02) between levels of IL-10 and anti-PGL-IgM. 1. Conclusion: The levels of IgM anti-PGL-1 and IL-10 in the contacts were higher than in the non-contacts with a significant difference.   Pendahuluan: Narakontak merupakan salah satu kelompok dengan risiko tinggi terinfeksi kusta dengan dominansi stadium subklinis. Pada infeksi tipe MB memiliki resiko 4-10 kali lebih besar berkembang menjadi bentuk klinis. Deteksi dini pada penyakit kusta subklinis ini menjadi penting sehingga dapat berperan dalam proses eliminasi kusta. Jika dibiarkan sebagian besar kusta stadium subklinis dapat berkembang menjadi kusta klinis dan dapat menimbulkan deformitas pada fisik. Phenolic glycolipid-1 (PGL-1) merupakan antigen spesifik untuk M. leprae dan Interleukin-10 (IL-10) merupakan sitokin anti-inflamasi yang menunjukkan respon imun tubuh. Penelitian mengenai hubungan antara kadar IL-10 dan IgM anti PGL-1 khusunya di Bali belum dilakukan, sehingga penelitian ini bertujuan mencari hubungan antara kadar IL-10 dengan kadar IgM anti PGL-1 pada narakontak serumah pasien kusta tipe multibasiler. Metode: Penelitian merupakan studi observasional analitik dengan pendekatan potong lintang dan dilaksanakan bulan Februari-Mei 2021 di Poliklinik Kulit dan Kelamin dan Laboratorium Patologi Klinik, RSUP Sanglah Denpasar dan laboratorium rujukan Leprosy, Tropical Diseases Centre, Universitas Airlangga. Sampel berusia 18-65 tahun dipilih melalui Consecutive Sampling sesuai kriteria inklusi dan eksklusi. Sampel dilakukan pengambilan darah vena kemudian diperiksa kadar PGL-1 dan IL-10. Analisis data menggunakan SPSS versi 23. Hasil dianggap signifikan jika p<0,05. Hasil: Rerata kadar IgM anti PGL-1 kelompok narakontak vs bukan narakontak sebesar 863,07 ± 789,04 u/ml vs 247,75 ± 161,60 u/ml. Rerata kadar IL-10 kelompok narakontak dan bukan narakontak sebesar 121,09 ± 144,11 pg/ml dan 46,04 ± 11,27 pg/ml. Terdapat perbedaan signifikan pada kadar IL-10 dan IgM anti PGL-1 pada kelompok narakontak dan bukan narakontak (p<0.05) serta hubungan positif lemah (r=0,296, p=0,02) antara kadar IL-10 dengan IgM anti PGL-1. Simpulan: Kadar IgM anti PGL-1 dan IL-10 pada narakontak lebih tinggi dari non narakontak dengan perbedaan yang signifikan.

Abstract

Introduction: Melasma is an acquired pigmentation disorder characterized by symmetrical hyperpigmented patches, most commonly seen on the face. Melasma and iron deficiency are most common in women of reproductive age. Iron deficiency causes a decrease in ferritin levels, which are iron stores in the body. The mechanism of iron deficiency causing melasma is thought to be due to cell hypoxia in the tissue involving mediators that increase melanogenesis. This study aims to prove that the mean ferritin level is lower in melasma than without melasma and that low ferritin levels are a risk factor for melasma in women. Methods: A case-control research included 28 participants with melasma and 28 without melasma who visited the dermatovenereology department of Prof. Dr. I.G.N.G Ngoerah Hospital, medical cosmetic division, in May-July 2022. The two groups will compare ferritin mean and ferritin risk factor analysis on the incidence of melasma. Data were tabulated and analyzed using SPSS 26. Results: The mean age in the melasma group was 40.64±3.39 years old, and the group without melasma was 37.64±3.50 years old (p=0.75). Of the 28 melasma subjects, there were more subjects with low ferritin levels (60.7%) than the group without melasma (32.1%). From the results of the Mann-Whitney test, the mean ferritin level in the melasma group (36.32±31.05) was significantly lower than in the non-melasma group (58.26±5.39); p=0.006. From the results of the Chi-Square test, it was found that low ferritin levels had a risk of 3.2 times the occurrence of melasma [OR (95% CI) = 3.2 (1.098-9.776), p=0.032]. Conclusion: The mean ferritin level in women with melasma is lower than in women without melasma, and low ferritin levels are a risk factor for melasma.   Latar Belakang: Melasma merupakan kelainan pigmentasi didapat yang ditandai dengan adanya bercak hiperpigmentasi simetris, paling sering terlihat pada wajah. Melasma dan defisiensi besi paling sering dijumpai pada wanita usia reproduktif. Defisiensi besi menyebabkan turunnya kadar feritin yang merupakan cadangan besi dalam tubuh. Mekanisme defisiensi zat besi menyebabkan melasma diduga akibat hipoksia sel di dalam jaringan melibatkan mediator yang meningkatkan melanogenesis. Studi ini bertujuan untuk membuktikan rerata kadar feritin lebih rendah pada melasma dibandingkan tanpa melasma serta membuktikan kadar feritin yang rendah merupakan faktor risiko melasma pada wanita. Metode: Studi case-control yang melibatkan 28 subjek dengan melasma dan 28 subjek tanpa melasma yang berobat ke poliklinik kulit dan kelamin RSUP Prof. Dr. I.G.N.G Ngoerah divisi kosmetik medik periode Mei-Juli 2022. Kedua kelompok akan dibandingkan rerata feritin dan analisis faktor risiko feritin terhadap kejadian melasma. Data ditabulasi dan dianalisis dengan menggunakan SPSS 26. Hasil: Rerata usia pada kelompok melasma 40.64±3,39 tahun dan kelompok tanpa melasma 37,64±3,50 tahun (p=0,75). Dari 28 subjek melasma, didapatkan lebih banyak subjek dengan kadar feritin rendah (60,7%) dibandingkan kelompok tanpa melasma (32,1%). Dari hasil uji Mann-Whitney didapatkan rerata kadar feritin pada kelompok melasma (36,32±31,05) lebih rendah secara signifikan dibandingkan pada kelompok bukan melasma (58,26±5,39); p=0,006. Dari hasil uji Chi-Square didapatkan kadar feritin yang rendah memiliki risiko sebesar 3,2 kali terjadinya melasma [OR (IK 95%) =3,2 (1,098-9,776), p=0,032]. Simpulan: rerata kadar feritin pada wanita dengan melasma lebih rendah dibandingkan wanita tanpa melasma dan kadar feritin yang rendah merupakan faktor risiko melasma.

Abstract

Background: Leprosy is a chronic granulomatous infectious disease caused by Mycobacterium leprae (M. leprae) that primarily infects Schwann cells in the peripheral nerves, leading to nerve damage and the development of disabilities. In 2018, Indonesia was the third country with the most leprosy cases in the world. Erythema nodosum leprosum (ENL), also known as type II leprosy reaction, is a severe immune-mediated complication of multibacillary leprosy. Purpose: To report a case of borderline lepromatous leprosy with severe ENL. Case: A 49-year-old Balinese man presented with multiple tender erythematous skin nodules all over his body, fever, arthralgia, bilateral cervical lymphadenopathy, and sensory loss for the past week. The acid-fast bacilli bacteriological examination showed a positive result. The patient was diagnosed with borderline lepromatous (BL) leprosy with severe ENL and was treated with multibacillary multidrug therapy (MB MDT), methylprednisolone, and other symptomatic medications. After 1 month of treatment, there was an improvement in skin lesions. The MB-MDT treatment was continued and methylprednisolone was planned to be tapered down gradually. Discussion: Approximately 20-50% of all leprosy patients show leprosy reactions in the course of the disease. The goals of treatment for severe ENL are to control inflammation, reduce pain, treat neuritis to prevent nerve dysfunction and contractures, and prevent recurring ENL. The prognosis of leprosy with ENL reactions depends on the severity of the occurring leprosy reaction; early diagnosis and prompt treatment; and patient compliance with treatment. Conclusion: Early diagnosis and treatment are essential to avoid deformities in leprosy patients.

Abstract

Background: Leprosy is a bacterial infection that is still a neglected disease. This disease leads to peripheral neuropathy despite routine treatment. Moreover, early detection of peripheral neuropathy in leprosy patients is very important. ENMG is known to have fairly good sensitivity and specificity in detecting it. Therefore, this study aims to determine the relationship between type 2 leprosy reactions and the risk of peripheral neuropathy through ENMG examination. Methods: The study design was a prospective, cross-sectional observational study. The research was conducted in July-September 2022 at Prof. dr. I G.N.G. Ngoerah General Hospital, Denpasar. The sample will be selected based on inclusion and exclusion criteria and then divided into two groups as a group with type 2 leprosy reaction and a group without type 2 leprosy reaction. The data were analyzed using SPSS version 25. The analysis was carried out in the form of descriptive and analytical analyses. The analytical analysis used is fisher's exact. Results: This study obtained 20 samples with an average age of 36.2±13.3 years. There were 40% of patients with type 2 leprosy reactions. All patients had polyneuropathy with 30% axonal type polyneuropathy and 70% mixed type. Bivariate analysis showed that there was no significant relationship between type 2 leprosy reaction to the type of peripheral neuropathy and the number of nerves involved based on ENMG examination (p=0.64). Conclusion: From this study, it can be concluded that there was no statistically significant relationship between the type 2 leprosy reaction to the type of peripheral neuropathy and the number of nerves involved based on the ENMG examination.   Latar belakang: Kusta merupakan infeksi bakteri yang masih menjadi  penyakit terbengkalai. Penyakit ini dapat menimbulkan terjadinya neuropati perifer meskipun telah melakukan pengobatan yang rutin. Untuk itu deteksi dini neuropati perifer pada pasien kusta sangat penting. ENMG diketehui memiliki sensitivitas dan spesifisitas yang cukup baik dalam mendeteksi hal tersebut. Maka dari itu, penelitian ini bertujuan untuk mengetahui hubungan antara reaksi kusta tipe 2 dengan resiko neuropati perifer melalui pemeriksaan ENMG. Metode: Disain penelitian ini merupakan observasional prospektif, potong lintang. Penelitian dilakukan pada bulan Juli-September 2022 di RSUP Prof. dr. I G.N.G. Ngoerah, Denpasar. Sampel akan diseleksi berdasarkan kriteria inklusi dan eksklusi kemudian di bagi menjadi dua kelompok yaitu, kelompok dengan reaksi kusta tipe 2 dan kelompok tanpa rekasi kusta tipe 2. Data dianalisis menggunakan SPSS versi 25. Analisis yang dilakukan berupa analisis deskriptif, dan analitik. Analisis analitik yang digunakan berupa fisher’s exact. Hasil: Pada penelitian ini mendapatkan 20 sampel dengan rata-rata berusia 36,2±13,3 tahun. Terdapat 40% pasien dengan reaksi kusta tipe 2. Seluruh pasien mengalami polineuropati dengan 30% merupakan polineuropati tipe aksonal dan 70% tipe campuran. Analissi bivariat menunjukkan tidak ditemukannya hubungan yang signifikan antara rekasi kusta tipe 2 terhadap jenis nauropati perifer dan jumlah saraf yang terlibat berdasarkan pemeriksaan ENMG (p=0,64). Simpulan: Dari penelitian ini dapat disimpulkan bahwa tidak ditemukannya hubungan yang signifikan secara statistik antara rekasi kusta tipe 2 terhadap jenis nauropati perifer dan jumlah saraf yang terlibat berdasarkan pemeriksaan ENMG.

Abstract

Introduction: Leprosy is an emerging transmitted disease that still prevalent enough in Indonesia. Although its treatment modalities have proved to be effective, an effective preventive measure is still needed especially to prevent the development of the severe form of Leprosy. Several studies found that Vitamin D had a significant influence on the immune system and thus could potentially alter the susceptibility to Leprosy or its manifestation. Thus, this study aimed to evaluate whether the plasma vitamin D level had a significant effect on the manifestation of Leprosy. Methods: A case-control study was conducted from July to August 2016 at Dermatology and Venerology Department of Sanglah General Hospital. The leprosy was categorized into Paucibacillary (PB) and Multibacillary (MB) while plasma vitamin D level was classified into normal/high and low. Subjects with MB were included in the case group while those with PB act as acontrol.Results:50 subjects, 25 per group, were enrolled in this study in which no significant differences were found in baseline characteristics of both groups. Bivariate analysis showed that plasma vitamin D level was significantly higher in PB than in MB (24.4272 ng/ml vs. 20.1488ng/ml). In accordance to mean difference, it revealed that low plasma vitamin D significantly increase the risk of MB type leprosy (OR: 12.667; 95% CI: 3.308-48.504). Conclusion: Low plasma vitamin D level significantly increased the risk of MB type leprosy as much as 12.667 times higher than normal.Keywords: Paucibacillary Leprosy, Multibacillary Leprosy, Plasma vitamin D level.

Abstract

Kusta merupakan penyakit yang menular dan menyebabkan masalah yang sangat kompleks di Indonesia, dimana mempengaruhi masalah medis, sosial, ekonomi, budaya, dan keamanan nasional. Tujuan: laporan kasus ini bertujuan untuk mengetahui perkembangan kasus Morbus Hansen tipe Lepramatosa dengan Eritema Nodosum Leprosum barta dan Cacat Kusta Tingkat Dua pada pasien laki-laki usia 40 tahun. Metode: Laporan kasus ini menggunakan metode kualitatif dengan pendekatan observasi dengan pemeriksaan fisik serta penunjang.Hasil: Diagnosis ditegakkan berdasarkan anamnesis adanya bercak-bercak berwarna putih pada hampir seluruh tubuh. Pada pemeriksaan fisik didapatkan benjolan kemerahan pada seluruh tubuh, disertai demam dan nyeri persendian. Status dermatologis pada regio fasialis tampak wajah yang berkerut dan berlipat dengan gambaran fasies leonina, madarosis pada alis mata kanan dan kiri, adanya infiltrat pada kedua cuping telinga, dan didapatkan efloresensi nodul eritema-hiperpigmentasi multipel, bentuk bulat, ukuran diameter 0,8 – 1,2 cm, tersebar diskret, konsistensi padat dan lunak, immobile, permukaan rata, nyeri tekan (+). Tatalaksana yang diberikan kepada pasien adalah metilprednisolon 16 miligram tablet tiap 8 jam intraoral, parasetamol 1000 miligram tiap 8 jam intraoral, vitamin B1B6B12 1 tablet tiap 24 jam intraoral, kompres NaCl 0,9% selama 10-15 menit tiap 8 jam topikal pada ulkus, dan natrium fusidat 2% krim tiap 12 jam topikal pada ulkus.

Abstract

Introduction : Leprosy is one of the most common causes of non-traumatic peripheral neuropathy in the world. There are many factors causing neuropathic pain in multibacillary (MB) type leprosy, one of which is an increase in serum levels of glial fibrillary acidic protein (GFAP). Aim: To prove high serum GFAP levels is a risk factor for neuropathic pain in MB type leprosy in Sanglah General Hospital Denpasar. Methods: This study was an observational analytic study with a case control design conducted at the Dermato-Venerology Department outpatient and in Pathological Clinic Laboratory in Sanglah General Hospital from November 2019 to January 2020. The sample of this study is all multibacillary leprosy patients with neuropathic pain who met inclusion criteria. A total of 60 patient were willing to participate in this study, the pain scales were measured using DN4 questionnaire and GFAP serum levels were examined. Chi-square test was performed to prove high serum GFAP levels is a risk factor of neuropathic pain in multibacillary leprosy. Results: High serum GFAP levels increase the incidence of neuropathic pain of MB type leprosy 5 times higher than controls (OR 5.72; p = 0.003), anxiety (OR 2.1; p = 0.24), and leprosy reaction (OR 2.2; p = 0.052). Discussion: High serum levels of GFAP is a risk factor of neuropathic pain in MB type leprosy in Sanglah General Hospital Denpasar. Keywords: GFAP, MB type leprosy, neuropathic pain
