Abstract

Type 2 diabetes mellitus (T2D) is a chronic metabolic disorder characterized by reduced insulin sensitivity and inadequate insulin secretion, leading to elevated blood glucose levels. Standard treatment relies on synthetic drugs, which can cause adverse effects in some patients. Rhodomyrtus tomentosa, a plant native to Indonesia and widely used in traditional medicine, is being explored as a natural alternative for diabetes management due to its potential antidiabetic properties. This review compiles recent findings on its therapeutic potential. A systematic literature search was conducted in August 2024 in PubMed, Wiley Online Library, ScienceDirect, Cochrane Library, ProQuest, and Google Scholar using the keywords “Rhodomyrtus tomentosa” and “antidiabetic.” Of 774 screened articles, five met the inclusion criteria (2014–2024). The studies employed in vitro, in vivo, and in silico approaches, focusing mainly on fruits and leaves. Results indicate that R. tomentosa may help control blood glucose by inhibiting α-glucosidase and α-amylase, enzymes critical in carbohydrate digestion. Bioactive compounds such as phloroglucinols and meroterpenes were frequently identified. However, most evidence comes from laboratory models, with limited animal research and no clinical trials. The risk of bias was moderate, often due to incomplete reporting. While findings suggest R. tomentosa is a promising natural candidate for diabetes treatment, its therapeutic value remains unconfirmed. More robust in vivo studies and human clinical trials are essential to clarify its mechanisms, safety, pharmacokinetics, and efficacy. This review serves as a basis for future research and highlights the need for rigorous studies to validate its clinical potential.

Abstract

Introduction: Mitochondrial Disorders (1/4,000 patients) are rare and caused by dysfunctional mitochondria. Anesthetic consideration in patients with Mitochondrial Disorders involves careful preoperative and perioperative observations. Objective: To provide a brief insight into how general anesthetics interfere with mitochondrial energy formation pathways and help form precautions for anesthesiologists when managing patients with Mitochondrial Disorder. Review: Mitochondrial Disorder patients would experience various health problems, such as damaged cardiac functions, neurology systems, and musculoskeletal functions due to energy production disruptions by dysfunctional mitochondrial processes. Moreover, patients with Mitochondrial Disorders exhibit hyperreactivity to volatile anesthetics. Conclusion: No anesthetic strategies are found to be safe in patients with Mitochondrial Disorder yet. Therefore, anesthesiologists should remain alert when monitoring fluid choices and managing patient temperature with Mitochondrial Disorders.

Abstract

Abstract

The inflammatory condition, marked by pain, swelling, and elevated TNF-α levels, prompted the exploration of cherry leaves' flavonoids, known for their anti-inflammatory properties. An in vivo experimental study with a pre-and post-test control group design was conducted to compare the efficacy of cherry leaf extract (Muntingia calabura L.) in the Wistar rat model of acute gout arthritis. Five treatment groups (negative control (Na-CMC 0,5%), positive control (Colchicine), cherry leaf extract dose 200mg/kgBW, cherry leaf extract dose 400mg/kgBW, and cherry leaf extract dose 800mg/kgBW) were administered to Wistar rats to compare their ability in reducing TNF-α levels. Results revealed the extract's significant efficacy in reducing TNF-α levels at all doses compared to negative control. There is no significant difference among the three different doses compared to colchicine as a positive control. In conclusion, cherry leaf extract demonstrated significant efficacy as an anti-inflammatory agent by reducing TNF-α levels in acute gout arthritis rat models with the most efficient dose of 200mg/kg BW.

Abstract

Gout arthritis is a joint inflammation caused by increased uric acid levels, leading to the formation of monosodium urate (MSU) crystals. The study aimed to deepen the understanding of the role of MSU crystals in triggering both acute and chronic inflammatory responses, as well as the formation of tophi, which damage joint and bone tissues. The method used is the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). The reviewed articles were obtained from the Science Direct, Semantic Scholar, and PubMed databases using the keywords “Gout Arthritis,” “Histopathology,” “Inflammation,” and “Tophus.” The publication period was limited to 2019-2024. Out of 745 articles found, 12 were selected based on inclusion criteria, which included experimental, observational, and case study reports. The results show that MSU crystals induce inflammation through the infiltration of inflammatory cells such as neutrophils and macrophages. In chronic Gout, the formation of tophi, granulomatous lesions with a crystalline core, leads to bone and joint destruction. Understanding the histopathological aspects is crucial for more accurate diagnosis and more effective management of Gout arthritis. The progression of the disease is related to acute and chronic inflammation, as well as the destructive effects of tophi on joints and bones. In conclusion, MSU crystals play a significant role in triggering inflammation and tissue damage in Gout arthritis, and understanding histopathology can improve diagnostic accuracy and therapeutic efficacy.
