Abstract

Latar Belakang: Skoliosis merupakan suatu kelainan bentuk tulang belakang tiga dimensi yang abnormal. Skoliosis didiagnosis dengan mengukur besarnya deviasi tulang belakang pada radiografi di bidang koronal. Penegakan diagnosis secara klinis apabila telah menyentuh ambang batas sudut Cobb ≥ 10°. Skoliosis memiliki kumpulan permasalahan kompleks yang biasanya disertai dengan pola kelainan pernapasan yang terbatas pada hampir dua pertiga penderita dengan kurva skoliosis yang cukup besar. Metode Penelitian: Metode penelitian yang digunakan dalam penelitian ini yaitu case report dengan single subject research yang dimana penelitian ini melibatkan seorang perempuan usia 32 tahun yang bekerja sebagai bidan di salah satu rumah sakit di daerah Boyolali, Jawa Tengah. Diagnosis medis untuk pasien Nn. M yaitu Skoliosis Idiopatik. Intervensi kepada pasien diberikan menggunakan beberapa modalitas yaitu Microwave Diathermy dan metode Schroth yang diberikan dalam 4 sesi selama 4 minggu (1 kali per minggu) dengan dilakukan pemantauan pada tiap 1 minggu sekali oleh fisioterapis bersangkutan. Hasil Penelitian: Hasil pada penelitian ini diukur setiap sesi pemberian intervensi selesai. Berdasarkan gambar 1 dan gambar 2, didapatkan bahwa terdapat perubahan pada kapasitas inspirasi paru pada pasien yang semula pada angka 1500/2600 menjadi 2200/2600 dan perubahan angle of trunk rotation (ATR) walau tidak signifikan pengukuran menggunakan scoliometri menunjukkan nilai yang semula 5° pada apex thoracal menjadi 4° dan pada apex lumbal yang semula 4° menjadi 3°. Kesimpulan: Kesimpulan pada penelitian ini adalah bahwa penggunaan metode schroth dinilai memberikan efek yang positif dalam peningkatan kapasitas vital paru. Efek yang diberikan terapi latihan menggunakan metode Schroth juga memberikan efek lainnya berupa penurunan ATR pada pasien sehingga menurunkan tingkat risiko deformitas dari skoliosis.

Abstract

Introduction: Sekitar 30% penderita stroke lacunar mengalami gangguan fungsional dalam periode tindak lanjut 5 tahun. Penurunan kekuatan otot pasca stroke lacunar adalah masalah utama yang membatasi fungsi ekstremitas. Gangguan keseimbangan juga merupakan tantangan besar bagi pasien pasca stroke. Gejalanya meliputi asimetri tubuh, waktu tumpuan singkat, dan kecepatan berjalan yang lambat, semuanya meningkatkan risiko jatuh. Case Presentation: Seorang perempuan berusia 61 tahun mengunjungi poli fisioterapi di RS Soerojo Magelang dengan keluhan berat pada paha depan kanan dan kiri saat berjalan dan beraktivitas sehari-hari. Pasien didiagnosis pasca stroke hemiparese kanan. Keluhan berat pada kaki pertama kali dirasakan secara tiba-tiba setelah salat subuh di masjid pada bulan Oktober 2023, sehingga membutuhkan bantuan untuk pulang. Management and Outcome: Pasien menjalani fisioterapi mingguan selama empat minggu, dengan sesi satu setengah jam yang meliputi modalitas alat, latihan penguatan, dan latihan keseimbangan. Discussion: Peningkatan terjadi pada skor manual muscle testing pada hip meliputi grup otot fleksor (2), ekstensor (2), adduktor (3), dan abduktor (3) menjadi fleksor (4), ekstensor (3), adduktor (4), dan abduktor (4). Pada knee dan ankle juga terdapat peningkatan meliputi grup otot fleksor (3), ekstensor (3), plantar fleksor (3), dan dorsi fleksor (3) menjadi fleksor (4), ekstensor (4), plantar fleksor (4), dan dorsi fleksor (4) pada pertemuan ke-4. Tidak terjadi peningkatan pada lingkar segmen ekstremitas bawah. Peningkatan skor single leg stance terjadi dari (0 detik) untuk kaki tumpuan kanan dan (2 detik) untuk tumpuan kiri menjadi (5 detik) dan (8 detik). Tercatat juga peningkatan waktu tempuh time up and go test dari (37,26 detik) pada pertemuan ke-1 menjadi (30,13 detik) pada pertemuan ke-4. Akan tetapi peningkatan skor single leg stance dan time up and go test masih di bawah nilai normatif. Conclusion: Latihan penguatan dan keseimbangan dilaporkan bermanfaat. Studi ini menunjukkan peningkatan pada kekuatan otot dan keseimbangan, meskipun tidak signifikan. Studi selanjutnya sebaiknya meningkatkan pengawasan dan jumlah pertemuan.

Abstract

Introduction: Post Partum Spontan Induction is a normal birthing process that refers to iatrogenic stimulation of uterine contractions before the start of spontaneous labor. The prevalence of induced labor increased significantly at the 41st week of pregnancy from 9.7% to 25.9%. The condition of pregnant women with the risk of chronic energy deficiency is at risk of decreasing muscle strength that helps the birth process, which can result in prolonged labor and postpartum bleeding. Case Presentation: This article was a case study of a patient at Rumah Sakit Khusus Ibu dan Anak Ummi Khasanah, Bantul, Yogyakarta (Mrs NA, 26 years old) after giving birth spontaneously induction due to chronic energy deficiency. The patient is given breathing exercises, active limb movement, transfer and ambulation exercises, pelvic floor exercises. Management and Outcome: Pelvic floor exercise is capable of strengthening the pelvic floor muscles and stimulating the recovery of the urogenital organs to their physiological function in post partum mothers. Discussion: The training provided is: pelvic floor exercise which aims to ensure that postpartum mothers can carry out functional activities well without any discomfort to the body parts that work during the birth process. Conclusion: Physiotherapy in the rehabilitation process for postpartum mothers is carried out 3 times over 2 days with administration pelvic floor exercise effective in reducing pain relief and increasing the patient's functional ability.

Abstract

Introduction: Congenital Heart Disease (CHD) is a congenital disease since the child was born due to the formation of a less than perfect heart. Atrial Septal Defect (ASD) is one of the common CHD. ASD occurs because the partition separating the right and left atriums is damaged, which causes blood rich in oxygen and carbon dioxide mixed. Valve repair surgery in ASD is recomendation and safe treatment procedure with low mortality rate. Bed rest after surgery can lead to prolonged immobilization according to the recovery period of each individual. The usual treatment for physiotherapy is early mobilization to reduce the disorder.Case Presentation: A 6-year-old child was referred to RSUP Semarang with complaints of discomfort and dyspnea during activities. In December 2019 the family took the child to one of the doctors in Salatiga, and the doctor diagnosed that the child had congenital heart disease and was advised to have an examination. The results of catheterization and echocardio examination showed that the child had ASD II.Management and Outcome: Treatments provided such as mobility in bed, range of motion (ROM) exercises, transfers, ambulation and walking exercises. The measuring instrument used is 6 Minute Walk Test (6MWT) which is done when the patient is able to walk and use how long the patient stays in the hospital.Discussion: Early mobilization following surgery in Intensive Care Unit (ICU) is effective to prevent the risk of bed rest and prolonged stay in ICU as well as prevent deep vein thrombosis, pneumonia, preasure ulcers (decubitus) and can improve functional mobility. An important element for the recovery of patients post-cardiac surgery, is not the type of early mobilization but rather interventions that start from the best time according to the patient's condition to prevent the harmful effects of a long bed rest.Conclusion : Early mobilization given postoperatively can prevent the risk of worsening due to long bed rest, long stay in the hospital and can improve functional mobility. Early mobilization and breathing exercise can produce positive effects for patients post cardiac surgery.

Abstract

Introduction: Diabetic Neuropathy has a negative impact on leg and ankle function (Strength and flexibility) which in itself affect the patients’s physical activity and quality of life. Physiotherapy protocols aimed at increasing intrinsic and extrinsic leg muscle strenght maybe a promosing approach to improve lower extremity function , prevent further complications and improve the patient's activities of dayli living.. Case Presentation: A 65-years old patients with diagnosis of Diabetic N europathy ,came to the Medical Rehabilitation Installation at RSUD Dr Hardjono Ponorogo with complains of burning sensation,tingling and numbness in both leg. Genu varus was seen in both legs,decreased right leg muscle strength,sensory los on feeling of touch,pain and temperature in both legs from the toes to the lower border of the knee. Management and Outcome: Aerobic Exercise, flexibility exercise, strengthening and balance exercise as well as gait training are given to patient with Diabetic Neuropathy. Exercise is given 3 times a week for 4 week. The measuring instrument uses Time Up And Go Test where it is reported that after undergoing comprehensive exercise therapy there is decreased in the TUG score from a score 22 second to score 19 second which means there is an increase in the patients static and dinamic’s balance. Discussion: Ankle exercises theraphy has also shown good results for improving ankle Range Of Motion and Diabetic Neuropathy symptoms as well as for redistributing plantar pressure during movement. Interventions in the form of strengthening, stretching, balance and gait training are very useful in preventing foot ulcers and amputations, reducing the risk of falls, increasing daily physical activity and quality of life, all of which can be beneficial in reducing mortality and comorbidities.. Conclusion: Combining Exercise therapy in the form of aerobic exercise, flexibility exercise, strengthening and balance exercise for the lower limbs, and gait training is beneficial in increasing muscle strength, balance and gait so that it can reduce the risk off falling in diabetic neuropathy patient.

Abstract

Introduction : This case report shows the application of the exercise therapy method with the New Bobath Concept Approach in cases of Traumatic Intracerebral Hemorrhage (TICH) and reports the results of the exercise. Case presentation: Mr. AR, a 16-year-old student with a clinical diagnosis of Traumatic Intracerebral Hemorrhage (TICH) received an exercise therapy program with a New Bobath Concept Approach consisting of abdominal exercise, facilitate foreward & backward pelvic, bridging exercise, sensibility exercise, sitting and sitting balance exercise, mobilization and facilitation of the scapula and arm, core stability exercise, standing exercise, facilitation of walking patterns and walking exercise. Management and outcome: Exercise was carried out for 12 weeks with a frequency of twice a week and a duration of 60 minutes per training session. Discussion: After giving exercise therapy with the New Bobath Concept Approach for 12 weeks, the results showed an increase MMT value in muscles shoulder, elbow, wrist, fingers, hip and knee from 0 to 2 and 3. Meanwhile for the Barthel index from a value of 2 (total dependence) to 10 (moderate dependence). Conclusion: Mr. AR is able to sit, stand independently and walk 100 meters with assistance.

Abstract

This case report shows a motor control exercise program as a treatment for walking patterns in children with idiopathic tip toe walking. A 5 year old child with talipes equiness walking pattern, during treatment received motor control with stair-step standing, standing balance, stooping and transition activities, control stepping, sensory and intrinsic foot activities, age-appropriate motivation method, information for parents, and home activities to facilitate a more upright standing and walking posture to strengthen surface reaction forces relative to the ankle axis. This program is conducted for 1 hour per session twice a week. Prior to intervention the patient had a Functional Gait Assessment (FGA) score of 8 with the highest score of 30. After 2 weeks of treatment, the Functional Gait Assessment (FGA) score increased by 17 with the highest score of 30. Our results suggest that a functional rehabilitation program may be beneficial in functional recovery. walking patients with idiopathic tip toe walking in children.

Abstract

Introduction: Osteoarthritis (OA) is known as degenerative joint disease where there is joint abnormality causing inflammation involving cartilage and surrounding tissue which result in morbidity and functional limitations, especially for older adults. Method: The aim of the study was to determine the benefits of performing Transcutaneous Electrical Nerve Stimulation (TENS), Patellar Mobilization, and Quadriceps Setting in Bilateral Knee Osteoarthritis Grade 2 on Elderly. The assessment used was Numerical Rating Scale Pain Assessment (NRS), Manual Muscle Testing (MMT), and Western Ontario and McMaster Universities Osteoarthritis (WOMAC). Discussion: The giving of Patellar Mobilization is able to increase function in older adults and also decrease knee joint stiffness and pain, meanwhile the giving of Quadriceps Setting is also able to increase Quadriceps muscle strength, and the application of Transcutaneous Electrical Nerve Stimulation (TENS) is able to decrease pain caused by OA and increase physical performance in patient with Knee OA. Conclusion: After doing physiotherapy as many as 4 times using Transcutaneous Electrical Nerve Stimulation (TENS), Patellar Mobilization, and Quadriceps Setting, it is gained the decrease of pressure and movement pain, the increase of right and left leg muscle strength, and also the increase of WOMAC functional Test result after being given Transcutaneous Electrical Nerve Stimulation (TENS), Patellar Mobilization, and Quadriceps Setting.

Abstract

Introduction : Hernia Nucleus Pulposus (HNP) is a condition that occurs due to biochemical changes in the spinal disc tissue. This change will cause rupture of the annulus fibrosus which makes the nucleus pulposus bulging (protrudes) towards the spinal canal which will put pressure on the nerve roots, causing symptoms in the form of neurological pain. Case Presentation : Mrs. E is a 37-year-old housewife with low back pain due to disc herniation with a history of post chemotherapy for breast cancer. Pain is felt when the patient is undergoing treatment, pain is like being stabbed and radiates from the lower back to both legs and especially the right leg, numbness and weakness in the right leg Management and Outcome : Treatments provided such us infrered, TENS, stretching, and core stability exercise performed 2 times a week for 4 weeks. The measuring instrument used is NRS, MMT, and Oswestry Disability Index. Discussion : One type of exercise that can be done for patients with lumbar HNP is core stability exercises in the form of abdominal hollowing exercises, prone lying, prone on elbow, prone press-up, prone press up with overpressure which can be selected by taking into account the ability of the patient. Core stability exercise is an exercise that functions as a synergistic activation that contracts the trunk and pelvic stabilizer muscles, to improve the control of the neuromuscular system and the strength of the trunk muscles and to maintain spinal stability which functions to maintain body posture and optimize the patient to control the trunk position. Core stability performed by the patient is able to reduce pressure on the nucleus pulposus during the spine extension position, so that nerve clamping can be reduced and can reduce pain symptoms, Conclusion : It can be concluded that in this case, the provision of physiotherapy programs in the form of infrared, TENS, stretching hamstrings, and core exercises performed 2 times a week for 4 weeks was able to reduce pain and improve functional ability in patients.

Abstract

Intoduction: Osteoarthritis is a degenerative disease characterized by the deterioration of joint cartilage which causes bones to rub against each other and triggers pain, stiffness and movement disorders. Osteoarthritis comes from the Greek word osteo which means bone. arthro which means joint, itis which means inflammation. Case Presentation : A 87 years old patient with Knee osteoarthritis come to PFDA Pekalongan Clinic. The patient had pain in both knees since 1 year ago. The patient feels pain in the knee when sitting to standing position, the patient complains of aches and pains in both knees when walking long distances. Pain is felt during activity. The intensity of pain increases at night. The patient has been to the doctor and has been injected 4 times. Management and Outcome : IR, TENS and strengthening exercises using theraband with hip extension standing position with elastic band and knee extension sitting position with elastic band was performed for 30 minutes per session, 3 times a week for 4 weeks. Before the intervention, the patient still felt pain and impaired functional activities with a NRS value of 6 and a WOMAC score of 28 . However, after a 4-week intervention, there was a decrease in pain (NRS) to 2 and functional disability increased with a WOMAC score of 23. Discussion : Exercise is carried out repeatedly according to a predetermined dose, This is where there is a decrease in subchondral inflammation resulting in decreased pain, and increased stability. To increase functional activity, theraband exercise provides direct stimulation to the skin receptors and which affects proprioception and maximizes sensation around the knee joint thereby increasing the performance of daily activities Conclusion : It can be concluded that in this case, the provision of a physiotherapy intervention program in the form of: IR, TENS and Theraband Exercises performed 3 times a week for 4 weeks was able to reduce pain, increase muscle strength and increase functional activity.
