Abstract

Hypertension and Diabetes Mellitus (DM) are leading causes of global mortality with continually rising incidences, impacting individuals' quality of life and imposing a significant burden on the healthcare system due to the high frequency of referrals to Advanced Level Health Facilities. The Chronic Disease Management Program (Prolanis) was initiated to address this issue by increasing awareness, adherence, and preventive activities through education and routine monitoring, which is expected to reduce referrals. This cross-sectional study aimed to analyze the relationship between participation in Prolanis activities and the referral of DM and Hypertension patients to Toto Kabila Regional General Hospital. The study sample involved 99 respondents who were DM and Hypertension patients registered as Prolanis participants and had been referred to the hospital, selected using an accidental sampling technique. Data were collected through a Prolanis participation questionnaire and secondary data from medical records for referrals. All instruments demonstrated acceptable psychometric properties, with significance values below 0.05 and Cronbach’s alpha coefficients exceeding 0.70. The analysis utilized the Chi-Square test and logistic regression. The statistical results indicate a highly significant relationship between patient referrals and participation in various Prolanis activities, including medical consultation (p = 0.002), reminders (p = 0.000), health status monitoring (p = 0.004), patient visits (p = 0.012), and Prolanis exercise (p = 0.000). The variable of Prolanis exercise had the greatest influence in reducing referrals, with an Odds Ratio of 5.37. Conclusion: Active participation in Prolanis activities, particularly physical activity, is strongly associated with a reduction in DM and Hypertension patient referrals. Health facilities are advised to prioritize and strengthen routine physical activities such as Prolanis exercise as the core of the program, while also improving the intensity and quality of reminder and monitoring activities to maximize patient adherence and the effectiveness of referral prevention.

Abstract

Dental caries is a chronic disease characterized by the destruction of hard dental tissues, including enamel, dentin, and cementum, resulting from bacterial activity within dental plaque that produces acids from food residues, particularly sugars and carbohydrates, adhering to tooth surfaces. These acids cause enamel demineralization, which, if it progresses, leads to the formation of cavities. This study aimed to analyze the risk factors associated with dental caries occurrence based on the def-t index among children aged 6–7 years in Tolinggula District, North Gorontalo Regency. This cross-sectional study involved 74 children aged 6–7 years selected using purposive sampling. Data were collected through structured questionnaires and clinical examinations, including def-t index assessment and salivary pH measurement. The questionnaire had undergone expert content validation and demonstrated acceptable reliability (Cronbach’s alpha > 0.70). Data were analyzed using the chi-square test and multiple linear regression. The findings revealed significant associations between dental caries occurrence based on the def-t index and the consumption behavior of cariogenic foods and beverages (p = 0.000), toothbrushing frequency (p = 0.000), and salivary pH (p = 0.000). Among these variables, salivary pH was identified as the most dominant factor associated with dental caries occurrence (p = 0.000). Several risk factors were found to be significantly associated with dental caries occurrence based on the def-t index among children aged 6–7 years in Tolinggula District, North Gorontalo Regency, with salivary pH being the strongest contributing factor.

Abstract

Tuberculosis (TB) remains a major public health burden, particularly in high-incidence countries such as Indonesia. The World Health Organization–recommended Directly Observed Treatment Short-course (DOTS) strategy plays a crucial role in improving treatment success; however, patient adherence during the intensive phase of treatment remains a significant challenge. This study aimed to analyze the relationship between the implementation of the DOTS strategy and adherence to anti-tuberculosis medication during the intensive phase among pulmonary TB patients in Bone Bolango Regency. This study employed a quantitative analytical design with a cross-sectional approach. A total of 75 respondents were recruited using total sampling. Data were collected through a structured questionnaire and analyzed using Chi-square tests and logistic regression analysis. The findings demonstrated significant associations between all DOTS components and treatment adherence, including government commitment (p = 0.006), microscopic examination (p = 0.003), treatment supervision (p = 0.000), drug availability (p = 0.002), and recording and reporting systems (p = 0.004). Treatment supervision was identified as the most dominant factor influencing adherence, with a p-value of 0.001 and an odds ratio (OR) of 0.133 (95% CI: 0.039–0.459). The implementation of the DOTS strategy is strongly associated with adherence to anti-tuberculosis medication during the intensive phase of treatment. Strengthening the role of treatment supervisors, ensuring consistent drug availability, and improving recording and reporting systems are essential to enhance treatment success.

Abstract

Malaria remains a major public health problem in mining areas due to environmental conditions that favor mosquito breeding and sustained human–vector contact. Mining workers are particularly vulnerable because of prolonged exposure to high-risk environments. This study aimed to analyze the factors associated with malaria incidence and the presence of vector breeding sites among mining workers in Hulawa Village, Buntulia Subdistrict, Pohuwato Regency. This observational analytic study employed a cross-sectional design involving 125 mining workers. Data were collected through structured interviews using a standardized questionnaire and environmental observations. The questionnaire was developed based on previous empirical studies and underwent validity and reliability testing prior to data collection; item validity was assessed using item–total correlation analysis, while internal consistency was confirmed with Cronbach’s alpha coefficients ≥0.70. Malaria incidence was determined using a Rapid Diagnostic Test (RDT). The independent variables included knowledge, nighttime outdoor activities, history of malaria, risk perception, self-efficacy, length of residence, presence of potential breeding sites, and presence of malaria vectors. Bivariate associations were examined using the chi-square test, and multivariate analysis was conducted using logistic regression. The results showed that several factors were significantly associated with malaria incidence. Knowledge (p = 0.000; OR = 0.018) and history of malaria (p = 0.014; OR = 0.068) acted as protective factors, whereas length of residence in the mining area (p = 0.001; OR = 31.179) and the presence of malaria vectors (p = 0.020; OR = 55.195) significantly increased the risk of malaria. In contrast, self-efficacy and the presence of breeding sites were not significantly associated with malaria incidence. The most dominant factor influencing malaria incidence was the presence of malaria vectors. Malaria incidence among mining workers in Hulawa Village is influenced by a combination of individual and environmental factors, with vector presence and prolonged exposure in mining areas playing a dominant role. These findings highlight the critical importance of prioritizing vector control and environmental management in mining areas as key strategies to reduce malaria incidence.

Abstract

Teo tuber flour (Tacca leontopetaloides) is a local carbohydrate source with potential to be developed as a functional food. However, information about its starch composition and glycemic load is still limited, even though these two parameters are important in determining blood glucose response after consumption as well as the carbohydrate quality of a food. This study aims to determine the amylose and amylopectin content of teo tuber flour and evaluate its glycemic load using male white rats (Rattus norvegicus). The study used a descriptive method with a quantitative approach and a laboratory experimental design. Analysis of amylose and amylopectin content was done using spectrophotometry with the DNS method and an amylose standard curve. Glycemic load was determined by measuring the rats’ blood glucose response after giving them teo tuber flour porridge and pure glucose, then calculated using the Incremental Area Under the Curve (iAUC) method. Research results show that teo yam flour contains 15.727% amylose and 64.389% amylopectin, meaning its starch composition is dominated by amylopectin. Rats fed with teo yam flour porridge showed lower and more stable blood glucose responses compared to rats given pure glucose. The glycemic load value of teo yam flour is 17, which falls into the medium category. These findings suggest that teo yam flour has the potential to be developed as a functional food that can help maintain stable blood glucose levels. Further research is needed to explore starch modification techniques that can reduce glycemic response and enhance its health benefits.

Abstract

Anemia is a condition when the hemoglobin (Hb) level in the blood is lower than the normal limit. Adolescent girls are at high risk of experiencing anemia due to regular blood loss during menstruation, and iron (Fe) deficiency is the main factor causing iron deficiency anemia, which is a condition resulting from insufficient iron for hemoglobin synthesis in erythrocytes that function to transport oxygen. The aim of this study is to determine the serum iron (Fe) levels and the number of erythrocytes in anemic adolescent girls who receive a free nutritious meal program. The research method used is an analytical observational approach with a cross-sectional design. The population in this study consisted of 80 anemic femal students, with a sample size of 40 anemic female students. The research results show that the iron levels in anemic adolescent girls who received the MBG program averaged 97.14 ± 91.67, and the average erythrocyte count was 4.87 ± 0.54. The results indicate that the average of both variables is still within the normal range. Meanwhile, the results in the group that has not received free nutrition show an average erythrocyte count of 4.73 ± 0.31, while the average iron level is at 47.50 ± 19.39.
