Abstract

Many problems arose during the puerperium, including the production of little breast milk, milk that was not smooth, and the dam of breast milk. The hormones prolactin and oxytocin secretion impact how well breast milk is produced. The mother's mental health impacts how these two hormones are secreted. When a mother is under psychological strain (stress), the hypothalamus's work function is suppressed, preventing the pituitary gland from secreting prolactin and oxytocin. This study aimed to find the latest scientific evidence on the benefits of Oketani massage to overcome breast milk problems. The PICO Framework and PRISMA-ScR Checklist were used in this scoping review. The literature search in this study used 4 databases: Pubmed, Science Direct, Research Rabbit, and ResearchGate. The research used the Joana Briggs Institute (JBI) Appraisal Tool. There are 392 potentially relevant articles, and 11 eligible articles were selected. Article inclusion criteria were articles published in 2017–2023 in English or Indonesian, and articles focused on Oketani massage to address breastfeeding problems, namely the smoothness of breast milk, milk production, and milk dam. These 11 articles explain and prove scientifically that the Oketani massage technique and the Oketani massage combination will affect milk production, smooth milk, and breast milk dam. There is a conclusion that Oketani Massage can be used to overcome breast milk problems in postpartum mothers. Oketani massage can be done with 8 manual massage techniques and lasts 30-45 minutes, i.e., 15-20 minutes each for each breast.  And also, health workers are advised to learn Oketani massage techniques as a therapy to overcome breast milk problems, which is expected to overcome breast milk problems.

Abstract

Exclusive breastfeeding in developing countries is still low, especially among working mothers. This study aims to determine the relationship between family support and exclusive breastfeeding for working mothers in hospitals in the Lamongan district. The method in this study used a cross-sectional analytical research design, the sample was working mothers in the Lamongan district hospital who had babies aged 6-24 months and met the inclusion criteria, totaling 120 people using purposive sampling. Data collection was carried out in August–September 2022 using questionnaires, analyzed univariately, bivariate (chi-square), and multivariate (logistic regression). The results showed that family support was significantly related to exclusive breastfeeding (p 0.000; OR 13.8; 95% CI: 4.819-39.381). Maternal characteristics that were significantly associated with exclusive breastfeeding were parity (p 0.034; OR 2.999; 95% CI: 1.086-8.038) and employment (p 0.003; OR 3.76; 95% CI 1.513-9.357). Family support is the most dominant factor related to exclusive breastfeeding for working mothers in hospitals. Mothers who receive good family support have a 13.8 times chance of providing exclusive breastfeeding compared to working mothers who receive less family support. It is recommended for health workers to provide IEC to families to provide optimal support for working breastfeeding mothers.

Abstract

This systematic literature review explores the integration of learning-style-based differentiated instruction with active learning models, particularly Problem-Based Learning (PBL) and Project-Based Learning (PjBL), in secondary chemistry education. The purpose of the review is to identify, synthesize, and critically analyze how a differentiated learning approach based on learning styles can be effectively activated with active learning models such as PBL and PjBL in the context of secondary chemistry education. Drawing on eleven peer-reviewed studies published between 2015 and 2025, the review identifies how differentiation strategies, when aligned with students' visual, auditory, and kinesthetic preferences, can enhance conceptual understanding, motivation, and critical thinking. The review reveals consistent positive outcomes across various learning domains, with differentiation applied through content, process, and product adjustments. However, it also uncovers persistent implementation challenges, including limited teacher training, resource constraints, and lack of institutional support. The situation analysis finds these findings within the frameworks of constructivist pedagogy and responsive teaching, emphasizing the importance of adaptable, student-centered instruction in chemistry education. The study concludes by recommending stronger systemic support, longitudinal research, and professional development to ensure the sustainable application of differentiated instruction in diverse chemistry classrooms.
